We report a case of osteochondral autogenous transfer (OAT) for spontaneous osteonecrosis of the knee in an elderly soccer player. A 61-year male suffered contusion of his right knee while playing soccer. He received conservative therapy at a local clinic, but the pain in his right knee did not improve. An MRI revealed osteonecrosis of the weight-bearing area of the medial condyle of his right femur, involving a volume of 2 x 1 x 0.5cm. We performed OAT 25 months after the injury. The osteochondral lesion was identified and improved after capsulotomy by mini-open methods, and tranfer of two osteochondral plugs of 8 mm in diameter that were harvested from the lateral trochlear ridge using a press-fit technique. Postoperative rehabilitation included no weight bearing for these weeks, partial weight bearing for 4 weeks, and full weight bearing by 7 weeks after the surgery. Five months after OAT, he started running. Eight months after the surgery, he started playing soccer again. The JOA score was 100 points and the Lysholm Knee Scoring Scale score was 100 points at 9 months after the surgery. OAT is normally performed for chondral defects and osteochondritis dissecans besides osteonecrosis. Several studies reported the inclusion of an age limitation on the surgery. If the chondral lesion is localized and the knee does not have mal-alignment that needs re-alignment surgery such as HTO, then OAT would be an effective option in active middle-aged and elderly people. Introduction : To detect early change of the cartilage matrix in osteoarthritis (OA) is technically challenging. Fourier transform infrared (FTIR) imaging is now a powerful tool used to investigate the collagen content in the cartilage. Ultrasound, which can be used clinically, has the potential to detect the superficial change of cartilage. The objectives of this study are to investigate the early change of the collagen content in OA cartilage with quantitative FTIR imaging and to determine the relation between the biomechanical factor detected by ultrasound and biochemical factor detected by FTIR imaging. Methods : Human OA articular cartilage was obtained at total knee arthroplasty. The cartilage was classified macroscopically by ICRS grading, and was examined by ultrasound. In ultrasound evaluation, the maximum magnitude of the reflected wave was defined as signal intensity. A frozen section of the cartilage was evaluated microscopically with modified Mankin grading. The frozen section was also evaluated with FTIR imaging, where the integrated area of collagen amide I and II was defined as the collagen content. Cartilage of the lateral femoral condyle was used for quantitative evaluation. The correlation was investigated between the FTIR collagen content and ultrasound signal intensity. Results : FTIR imaging revealed that the collagen content of superficial layers decreased in accordance with the cartilage denegation, and with the decrease of signal intensity in ultrasound with ICRS grade 1 compared to ICRS grade 0. Ultrasound signal intensity was significantly correlated with the collagen content of the superficial layer detected by FTIR imaging (R=0.80). Conclusions : Change in the superficial layers reflected initial loss of collagen. Reduction in the ultrasound signal was observed in full depth cartilage being mostly pronounced in the superficial cartilage where it was significantly correlated with the FTIR determined collagen content. Pharmacological chaperone therapy (PCT) structurally stabilizes mutant enzyme proteins and increases their activity. Although ease of oral administration and effectiveness in patients with central nervous system disorders serve as advantages, PCT is effective only for patients with amenable mutations because its efficacy depends on gene mutations. PCT, which prevents progression of Fabry cardiomyopathy and nephropathy, was approved in Japan in 2018. It is expected that PCT will also be developed for lysosomal diseases that cause central nervous system disorders in the future.

A 73-year-old man was referred to our hospital with a right pleural effusion. Chest computed tomography( CT) showed multifocal pleural effusion, and chest drainage was performed. Actinomyces meyeri was detected in the pleural fluid culture. Despite antibiotic treatment, the patient's condition did not improve, and a curettage was performed for empyema. The chest tube was removed on postoperative day 7 and the patient was discharged home uneventfully on day 21. Intravenous antibiotics were given for 16 days, followed by oral antibiotics for 6 months. Actinomycosis empyema is a rare disease with no established duration of treatment, but a total of 6 to 12 months of treatment is generally recommended.

A 87-year-old female was pointed out wall thickness in the upper part of gastric body for examination of anemia. The mass had a contrast effect, some of it protruded outside the wall, and the surrounding lymph nodes were enlarged. Upper endoscopy showed irregular ulcerative lesion with submucosal volume from posterior wall to the greater curvature in the upper part of gastric body. Biopsy was performed, and GIST of stomach was diagnosed. Surgery was performed for the GIST of the stomach. During open surgery, invasion of pancreatic tail was observed, therefore proximal gastrectomy with D1 lymph node dissection and distal pancreatectomy were performed. Pathologically, the tumor measured 95×78×65 mm with mitotic figures(38/50 high-power fields). Immunohistochemical analysis revealed that tumor cells expressed positive results for c-kit, α-SMA and CD34, and negative results for S-100 and desmin on the basis of the histology and immunostaining profile, the tumor was diagnosed as a GIST. The patient was classed as high risk according to Fletcher's risk classification. Tumor invades pancreatic tail, and lymph node metastasis was observed. She was discharged on the postoperative day 27 and alive without tumor recurrence at 6 months after surgery, not undergoing adjuvant chemotherapy.

We experienced a case of surgical aortic valve re-replacement due to structural valve deterioration caused by pannus formation 4 years after transcatheter aortic valve replacement( TAVR). The patient underwent surgery because the mean transvalvular pressure gradient increased to 48 mmHg on echocardiography. Contrast-enhanced computed tomography (CT) was useful for predicting the site of adhesion to surrounding tissue preoperatively and exploring the presence of the pannus. Intraoperative findings showed the TAVR valve was covered with neointima except around the origins of the left and right coronary arteries and was firmly adhered to the surrounding tissues. As residual pannus was present in the subvalvular tissues, it was carefully removed. The explanted TAVR valve functioned well with good opening and closure. The postoperative course was uneventful. Pannus formation can result from mechanical stress. TAVR valves put significantly greater stress on the left ventricular outflow tract than surgical valves and may be more likely to cause pannus formation.

We present a 58-year-old female patient who underwent resection of a leiomyosarcoma arising from the right ovarian vein. She was referred to our hospital because of lower abdominal pain that had been present for 1 month prior to the visit. Ultrasound examination revealed a well-defined, smooth, lobulated, highly vascular mass(57 mm)adjacent to the distal portion of the duodenum. Contrast-enhanced computed tomography revealed the contrast enhancement mass (60 mm)located surround the right ovarian vein. In abdominal magnetic resonance image examination, the mass exhibited isointense signal on T1-weighted images, high signal on T2-weighted images, and restricted diffusion on diffusion- weighted images. We suspected primary leiomyosarcoma of the ovarian vein and proceeded with surgical intervention. On intraoperative findings, the mass was in contact with the duodenum and the inferior vena cava but dissection was easily achieved. We excised the mass together with the right ovarian vein. Pathological findings showed the mass was composed of proliferating spindle-shaped cells arranged in bundles. Some areas showed polygonal nuclear atypia and abnormal mitotic figures. Additional immunostaining showed positive for α-SMA, caldesmon, calponin, and negative for desmin, CD34, CKA1/AE3, S100. Based on the intraoperative findings, we diagnosed it as leiomyosarcoma arising of the right ovarian vein.

Cisternography using heavy T2-weighted images from 3-Tesla magnetic resonance imaging(MRI)and three-dimensional time-of-flight MR angiography(3D TOF MRA)is useful for identifying conflicting vessels in primary hemifacial spasm(HFS). Cisternography provides high-signal images of the cerebrospinal fluid and low-signal images of the cranial nerves and cerebral blood vessels, whereas 3D TOF MRA provides high-signal images with only vascular information. The combination of these two methods increases the identification rate of conflicting vessels. The neurovascular conflict(NVC)site in HFS is where the facial nerve exits the brainstem. However, on MRI, the true NVC site is often more proximal than the facial nerve attachment to the brainstem. On preoperative MRI, it is important to not miss the blood vessels surrounding the proximal portion of the facial nerve. If multiple compression vessels or deep vessels are located in the supraolivary fossette, they may be missed. Coronal section imaging and multiplanar reconstruction(MPR)minimize the chances of missing a compression vessel. Preoperative MRI and CT can also provide various other information, such as volume of the cerebellum, presence of emissary veins, shape of the petrosal bone, and size of the flocculus.

Stereotactic radiosurgery(SRS)performed with Gamma Knife or CyberKnife has been reported to be effective in treating trigeminal neuralgia(TN). Microvascular decompression is the first choice of treatment for patients with trigeminal neuralgia who are difficult to treat with drugs because of its high efficacy, with a pain relief rate of 70%-80% after 5 years. The pain relief rate of TN treated with SRS is approximately 50%-60% after 5 years, which is less than that of MVD. SRS is also inferior to surgery, causing more frequent sensory disturbances in the trigeminal nerve area(6%-20%). However, the serious complications, severe morbidity and mortality, associated with SRS are quite rare. SRS is an extremely minimally invasive treatment that does not require general anesthesia and can be used to treat TN with short-term hospitalization or outpatient visits.

We report the case of a long-term-surviving adolescent and young adult patient with unresectable advanced gastric cancer for whom multidisciplinary treatment was effective. A 29-year-old woman was referred to our hospital for further examination following a diagnosis of gastric cancer by a local physician. Esophagogastroduodenoscopy showed a deep ulcerated lesion in the lower third of the stomach, and analysis of biopsy specimens revealed an adenocarcinoma. Abdominal contrast- enhanced computed tomography showed gastric wall thickening in the lower third of the stomach. The patient underwent distal gastrectomy with lymph node dissection, including resection of localized peritoneal metastases, followed by Roux-en- Y reconstruction. The pathological diagnosis was serosa-invading poorly differentiated adenocarcinoma with 1 lymph node metastasis measuring 6.0×5.5 cm in the posterior wall of the lower third of the stomach and negative immunohistochemical staining for human epidermal growth factor receptor 2. The patient received postoperative chemotherapy with S-1 and oxaliplatin. She developed bilateral ovarian metastases measuring 13.0 cm and 7.8 cm after 17 months. The patient presented with severe lower abdominal pain and underwent an emergency bilateral ovarian metastasectomy, which revealed torsion of the right ovarian tumor, which had twisted twice on its pedicle, and a left ovarian mass. After the operation, 41 courses of ramucirumab with nab-paclitaxel were administered as a second-line treatment, and she received systemic drug treatment. Sixty months after the gastrectomy, the patient developed left hydronephrosis due to peritoneal metastases and was treated with nivolumab after ureteral stent replacement. No effective treatment was proposed in cancer multigene panel testing, and she died 66 months after the initial treatment because of disease progression. Comprehensive multidisciplinary treatment, including surgical and local therapy for peritoneal dissemination based on drug therapy for unresectable advanced gastric cancer, may result in long-term survival. Further research and accumulation of such cases would lead to the development of novel treatments.

Objectives　This study aimed to develop scales for evaluating the level of implementation of community practices and explore the perception of public health nurses in municipalities regarding their community practices.Methods　Draft scales were developed based on a literature review and interviews with municipal public health nurses. Subsequently, a questionnaire survey was conducted with municipal public health nurses across Japan in response to these draft scales. Respondents were recruited based on the municipality population size. Questionnaires were distributed to 2,074 individuals from 52 municipalities ensuring representation from diverse municipalities. Exploratory factor analysis was performed to determine the number of factors, while confirmatory factor analysis was performed to determine the factor structure. The reliability of the scales was assessed by calculating Cronbach's alpha coefficients, and their validity was tested by examining correlations with existing scales, namely the Moral Competence Scale for Public Health Nurses and the Professional Identity Scale for Public Health Nurses, and years of experience as a public health nurse.Results　A total of 721 (34.8%) valid respondents were included in the analysis. Three scales, methods of community practices (three factors, nine items), perceptions of public health nurses toward community residents through community practices (three factors, 10 items), and organizational environment supporting community practices (two factors, 11 items) were developed as indicators of the implementation of public health nurses' community practices. Cronbach's alpha coefficients for the three scales were 0.896, 0.913, and 0.868, respectively. As hypothesized, each subfactor exhibited a positive correlation with the existing scales. However, certain subfactors did not demonstrate any correlation with years of experience.Conclusion　The three scales developed in this study were individually examined for reliability and validity. These scales can be used independently or in combination, allowing public health nurses to select the most suitable scale(s) based on their objective.　A notable contribution of this study is the establishment of concrete indicators for evaluating community practices, addressing the previously vague nature of this evaluation. By incorporating the scale items into daily health practices, we anticipate that these indicators can be employed to evaluate community practices at the organizational and individual levels.

We conducted a post-marketing surveillance of Zykadia® in patients with ALK-positive unresectable, advanced or recurrent non-small cell lung cancer. There were 573 patients included in the safety analysis, of which 54.62% were female, and their mean age was 59.9 years. Overall, 473 patients(82.55%)experienced side effects, which were mainly diarrhea in 43.46%, nausea in 34.38%, vomiting in 18.67%, and hepatic dysfunction in 16.40%. Side effects led to treatment discontinuation in 24.78%. Among 455 patients included in the efficacy analysis, the response rate was 30.99%. The median progression- free survival was approximately 4 months. The safety and efficacy were similar to those of previous reports, and this study confirmed that there are no problems requiring additional precautions in clinical use of Zykadia®.

The JSH Practical Guidelines for Hematological Malignancies, 2018 expanded edition, newly adopted brentuximab vedotin, doxorubicin, vinblastine, dacarbazine(A+AVD)protocol as a standard treatment for advanced-stage classical Hodgkin lymphoma(CHL). Therefore, this retrospective analysis compared 15 patients who received A+AVD therapy with 21 patients who received doxorubicin, bleomycin, vinblastine, dacarbazine(ABVD)therapy. All patients were newly diagnosed with CHL and received induction therapy between April 2015 and June 2022 in our hospital. All except 1 patient of the A+AVD group had advanced-stage CHL. The median age was 63(23-85)years. The estimated 2-year overall survival of the A+AVD group was better than that of the ABVD group which included 6 patients with clinical stage Ⅲ or higher CHL (100% vs 66.7%, p=0.047). In contrast, there was no significant difference in the complete response rate(53.8% vs 100%, p=0.109)between the 2 groups. The overall response rate after first-line treatment(69.2% vs 100%, p=0.255), and the estimated 2-year progression-free survival(70.1% vs 66.7%, p=0.321)between the A+AVD and the ABVD groups were similar.

Case: 82-year-old female. In October 2020, the patient underwent transcatheter aortic valve implantation( TAVI)[Evolut PRO R 23 mm] via left subclavian artery approach for severe aortic valve stenosis. The patient was discharged home without any issues. However, nine months after surgery, the patient was hospitalized at another hospital for a right upper arm fracture and developed a fever. The patient was transferred to our hospital and was diagnosed with Enterococcal bacteremia while receiving antibiotic treatment. Echocardiography revealed leaflet thickening and cord-like structure on the artificial valve, and a diagnosis of prosthetic valve endocarditis (PVE) was made. The patient then underwent surgical removal of the valve. The patient had a good postoperative course and completed 4 weeks of antibiotic treatment before being transferred back to the referring hospital 31 days after surgery. TAVI made unprecendented revolution in the treatment of aortic valve stenosis. TAVI is often used for elderly patients with comorbidities because of high perioperative risk is for surgical aortic valve replacement. Although TAVI is widely promoted for its benefits, it is not without limitations. In cases with more than one year of follow-up, there are many complications and the risk of surgery is high. There have been few reports of cases from Japan requiring surgical removal of TAVI valve. In this case, PVE was diagnosed nine months after TAVI and the patient had a good outcome.

A 26-year-old man referred to our hospital because of dysphagia and a mediastinal tumor detected on chest computed tomography (CT). A contrast-enhanced CT revealed a 12 cm long cystic tumor along the right thoracic esophagus. An upper gastrointestinal endoscopy showed no abnormalities in the esophageal mucosa, and an unclear boundary between the tumor and the esophageal wall was observed by echography. In surgery, the tumor and the esophagus were in one lump, and esophagectomy was performed. On the fourth postoperative day, esophagogastric anastomosis was performed with poststernal reconstruction, and the patient was discharged home on the 38th postoperative day. Pathological examination revealed that the mass was a cystic lesion within the esophageal muscular layer, and the cyst wall was coated with airway-like multi-lineal hairy epithelium, which led to the diagnosis of a bronchogenic cyst. Even if the cyst is within the esophageal muscularis layer, bronchogenic cyst should be considered in the differential.

A 72-year-old man who underwent aortic valve replacement by a minimally invasive cardiac surgery (MICS) approach two years ago was visited our hospital complaining of swelling and pain in the right anterior chest. A chest computed tomography (CT) scan showed that the right upper lobe protruded beyond the right second intercostal space and outside the thorax. He was diagnosed as a right intercostal lung hernia and underwent chest wall reconstruction with a substitute method. Postoperative course was uneventful without any evidence of recurrence. Postoperative intercostal lung hernias in MICS may increase with the increment in MICS, and it is necessary to accumulate cases as one of the complications.

A 46-year-old man was diagnosed with chronic myeloid leukemia (CML) in chronic phase. He was treated with imatinib, nilotinib, and dasatinib, but failed to achieve a complete cytogenetic response (CCyR). After tyrosine kinase inhibitor therapy, F317L BCR-ABL1 kinase domain mutation was detected. At age 66, the patient started ponatinib (PON) at 45 mg/day, and achieved CCyR within three months. Subsequently, PON was tapered to 15 mg once weekly due to arterial-occlusive events. PON was discontinued after a 3-year deep molecular response (≥ MR 4.5 ). However, the patient lost MR 4.0 within two months, and PON (15 mg once weekly) was restarted. He achieved MR 4.0 again within one month, and then a deeper molecular response (MR 5.0 ) after starting dialysis therapy at the same PON dose. The trough value of PON (15 mg once weekly) was 5.8 ng/ml, which suppressed F317L mutation in the CML clone. Currently, the patient is 77 years old and is sustaining MR 5.0 . Chronic renal failure may cause hyperabsorption and metabolic retardation in patients receiving PON. Initiation of hemodialysis may improve homeostasis resulting in enhanced anti-tumor immunity against CML.

Tremor is defined as an involuntary, rhythmic oscillatory movement of a body part. Based on clinical symptoms, tremors are classified into rest, postural, simple kinetic, intention, and task-specific tremors. Pathological conditions associated with tremors include Parkinson's disease, essential tremor, cerebellar tremor, task-specific focal dystonia, peripheral neuropathy induced tremors, tremor secondary to poisoning/drugs, and physiological tremor. In this article, we discuss classifications and tremor etiologies, as well as diagnosis and treatments including surgical management.
