The patient was a 67-year-old man. At 22 years of age, he underwent aortic valve replacement with Starr-Edwards ball valve. At 67 years of age, he complained of fatigue of the New York Heart Association (NYHA) class Ⅲ condition. He was diagnosed with mitral regurgitation, tricuspid regurgitation, ascending aortic aneurysm and chronic atrial fibrillation. Transthoracic and transesophageal echocardiograms showed a mobile, elongated echogenic mass attached to the valve cage and floating downstream. We performed Bentall procedure, ascending aortic replacement, mitral valve replacement, tricuspid annuloplasty and left atrial appendage closure. Explanted ball valve showed extensive cloth destruction and partial cloth tear. Mild pannus formation was observed beneath the valve. Despite 45 years after initial operation, significant valve dysfunction was not observed.

A 69-year-old man presented to our hospital with chief complaints of epigastral pain and nausea, was diagnosed with intestinal obstruction after gastric surgery. Abdominal CT performed on the admission showed the tumor located on the terminal ileum. On colonoscopy, type 1 cancer was found near the Bauhin valve in the ileum, and suspected primary ileal carcinoma. Laparoscopic ileocecal resection was performed. The pathological diagnosis was moderately differentiated adenocarcinoma, and the pathological stage was T3(SS), N1(3/16), M0, Stage ⅢA. Although superficial surgical site infection was occurred, the patient was discharged 11 days after surgery. He hoped to adopt without adjuvant chemotherapy, so he has been followed as outpatient. Twenty one months since the surgery, there has been no evidence of cancer recurrence.

Cardiac metastasis from renal cell carcinoma (RCC) is relatively rare. It generally initiates from the right atrium with or without involvement of the inferior vena cava (IVC). Here, we report a case of metastasis of isolated clear cell RCC to the left ventricular( LV) papillary muscle. An 84-year-old male who had undergone renal resection due to RCC 19 years before, presented to our hospital with suspected LV tumor by echocardiography. The tumor was located near the posterior papillary muscle, with a size of 15×12 mm. The patient underwent resection of the LV tumor. Pathologically, the tumor was diagnosed as clear cell RCC. The postoperative course was stable, and he did not require any chemotherapy. Although there is no definite evidence of the mechanisms of metastasis, the tumor was supposed to metastasize lymphogeneously.

In this series, we describe the techniques of atrial, ventricular incision and closure. Right atriotomy is a surgical procedure necessary to expose congenital abnormalities of the tricuspid valve and other congenital heart diseases such as atrial and ventricular septal defects. Whereas, left atriotomy( for example left side atrial, transseptal or superior transseptal incisions) is a technique needed to expose mitral valve disease, frequently used during the maze procedure. And finally, left ventriculostomy is a surgical approach applied during resection of scar tissue or thrombus of ventricular aneurysm surgery, repair of the ventricular septal rupture and endoaneurysmorrhaphy. These are known techniques that have already been described in many manuscripts and textbooks. Regarding atriotomy and ventriculostomy, the most important thing to know is how to expose the tricuspid, mitral valves and adjacent sections of the heart. Ventriculostomy uses strips of teflon felt to strengthen the suture line and enhance hemostasis.

Prosthetic valve endocarditis (PVE) is associated with high mortality and reoperation rates despite diagnostic and therapeutic improvements. We retrospectively analyzed the data of 35 patients who had undergone reoperative cardiac surgery for PVE in our hospital between January 1, 2005 and December 31, 2014. The mean age of the patients was 68 ± 12 years, and 15 (42%) of those patients were women. Early PVE was defined as PVE that occurred within 1 year after the surgery; 12 (34%) patients showed early PVE. The aortic valve was affected in 25; the mitral valve, in 18;and the tricuspid valve, in 2 patients. Streptococcal, staphylococcal, and other infections were observed in 4, 20, and 7 patients, respectively. Aortic valve replacement, mitral valve replacement, double valve replacement, and aortic root reconstruction was performed in 9, 10, 6, and 10, patients, respectively. The 30-day operative and hospital mortality occurred in 4 (11%) patients. All patients who died had developed early PVE and staphylococci infection. The overall 7-year survival and freedom from valve-related reoperation were 63% and 78%, respectively. Surgical management of PVE remains a challenge and is associated with remarkably high morbidity and mortality rates. In our series, early PVE and staphylococcal infection were associated with very poor prognosis.

We herein present a case of bilateral chronic subdural hematoma(bilateral CSDH)caused by spontaneous intracranial hypotension(SIH)without orthostatic headache. A 67-year-old male patient with mild head injury suffered from a chronic, non-postural headache. Computed tomography of the brain showed bilateral CSDH. The hematomas were surgically evacuated using a closed drainage method. His headache immediately disappeared, but it recurred after 2 weeks. We further performed hematoma irrigation;however, his neurological status deteriorated with the development of pneumocephalus. Magnetic resonance imaging with gadolinium-DTPA showed diffuse pachymeningeal enhancement. His cerebrospinal fluid pressure was 3cmH 2 O, and radioisotope cisternography revealed cerebrospinal fluid leakage at the level of the cervicothoracic transition. No recurrence was found after 6 weeks of horizontal bed rest. The possibility of SIH should be considered in patients with CSDH, especially bilateral CSDH, even in elderly patients with a history of mild head trauma, and absence of orthostatic headache. Closed drainage method may be preferable in cases where surgical treatment is used.

A 55-year-old man underwent rectal amputation for rectal cancer in August 2005. A tiny thin-walled cavity lesion in his left S1+2 was found on computed tomography (CT) of the chest in November 2008. The cavity lesion in the left S1+2 gradually increased in size over 3 months and positron emission tomography (PET) with 18F-fluorodeoxyglucose (FDG) showed FDG accumulation at the lesion. Videoassisted thoracoscopic (VATS) wedge resection was performed to make a definite diagnosis in March 2009. The pathological findings revealed a metastatic lung tumor from the rectal cancer. It is necessary to consider the possibility of metastatic lung tumors in a case with the cavity lesions especially in patients with a history of colon cancer.

We report a long-term survival case treated by chemotherapy with new anticancer drugs such as S-1, CPT-11 and docetaxel after bilateral overiectomy for bilateral ovarian metastases of progressive gastric cancer. A 68-year-old female, who had undergone total gastrectomy with D2 lymphadenectomy for scirrhous gastric cancer of Stage IIIA, was admitted because of ovarian metastasis. Laparoscopic bilateral ovariectomy was performed for ovarian metastases of the gastric cancer (Krukenberg tumor) 6 months after gastrectomy. Seven months after gastrectomy, 11-course of S-1 treatment (80 mg/m2) for bilateral ovarian metastases of the gastric cancer was administered. CT scan revealed mediastinal lymphadenopathy 2 years and 5 months after gastrectomy. Then, the drug was changed to docetaxel (60 mg/m2). After 9-course of docetaxel treatment, the mediastinal lymphadenopathy disappeared. For a treatment of grade 3 neuropathy, a 12-course CPT-11 was started after gastrectomy 3 years and 6 months ago. CT scan and PET-CT showed no new metastasis in 4 years and 10 months after gastrectomy.

A 74-year-old man was admitted to our hospital in order to treat a mediastinal mass and 2 ground-glass attenuations in the right upper lobe detected by chest X-ray and computed tomography (CT). Partial resection of right lung and thymectomy were performed. The mediastinal mass and 2 ground-glass attenuations in the right upper lobe proved to be thymoma and bronchioloalveolar carcinomas, respectively by pathology.

An 79-year-old man admitted our hospital for abdominal mass. Computed tomography showed a tumor measuring about 10 cm in diameter without any metastasis lesion and any sings of local infiltration. Gastroduodenal endoscopy revealed the presence of a submucosal tumor in the third portion of the duodenum, and biopsy revealed tumor cells stained positive for c-kit. These findings were consistent with a GIST and we performed a partial resection of the duodenum sparing the pancreas. Gastrointestinal stromal tumors (GIST) were mainly located in the stomach and the small intestine. Duodenal localization is rare. Surgical approach for GISTs should basically be a partial resection. However, for GISTs located in the duodenum, the partial resection was sometimes difficult and pancreaticoduodenectomy (PD) may be needed, depending on the tumor size and the location of the tumor close to the papilla Vater. Since GIST grew expansively, rarely involving lymph nodes, PD may be an excessive procedure to treat the disease. For this reason pancreas-sparing partial duodenectomy has been introduced for the treatment of duodenal GIST.

A 71-year-old man with dysuria was referred to our hospital. The level of serum prostate specific antigen was slightly elevated (4.66 ng/ml), and digital rectal examination revealed a stony hard prostate mass. We performed a transrectal prostate biopsy because malignancy was suspected. Histological examination revealed leukemia-like cells, and bone-marrow examination (aspiration) was performed to determine the location of the original lesion. However, no leukemia-like cells or any other form of malignant cells were identified. Clinical imaging confirmed the absence of any other lesions, and granulocytic sarcoma of the prostate was subsequently diagnosed. We treated the cancer with radiotherapy at a dose of 40 Gy and dysuria improved. Furthermore, significant reduction in prostate volume was confirmed. Four months after initial presentation, the patient developed acute myeloid leukemia [M2 by French-American-British classification]. Induction chemotherapy was initiated, and the patient was successfully induced to complete remission. Twenty months later, the patient showed relapse. Despite salvage chemotherapy, he died of brain hemorrhage twenty-four months after complete remission.

In this series, we describe the method of atriotomy, ventriculotomy and their repairs. Right atriotomy is needed for expose tricuspid valve, atrial and ventricular septal defect. Left atriotomy is needed for expose mitral valve disease and technique for, maze or pulmonary vein isolation. Left ventriculotomy is needed for resection of scar and/of thrombus of ventricular aneurysm, repair of ventricular septal rupture and endoaneurysmorrhphy. Recently, this method is also used to repair for ischemic mitral valve regurgitation. These techniques have already been described in many textbooks. Regarding atriotomy and ventriculotomy, the most important point is how to fully expose the tricuspid and mitral valves and others. The repair of ventriculotomy requires strips of teflon felt to strengthen the suture line and to enhance hemostasis. This method of repair is considered standard by many surgeons.

A 72-year-old woman was reffered to our hospital for further examination of a tumor shadow in the left upper lung field which was detected in a mass screening chest X-ray. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed a chest wall tumor located in the left 4th intercostal space. The lesion was suspected to be neurogenic tumor and CT-guided needle biopsy was performed. The tumor was consisted of spindle-shaped cells, but immunohistochemistry demonstrated no evidence of neurogenic tumor. As a possibility of malignant tumor could not denied, we performed tumor resection under video-assisted surgery. The lesion was not originated from nerves, but adhered to the intercostal muscle. Histologically, the tumor was consisted of spidle-shaped cells without atypia which sparsely proliferate in the myxoid stroma adjacent to intercostal muscle. In immunohistochemistry, tumor cells were positive for vimentin, and negative for desmin, S-100 protein, smooth muscle actin, CD34 and factor VIII. It was diagnosed as intramuscular myxoma. This histology in the intercostal muscle is extremely rare.

A 14-year-old girl was admitted to our hospital with a history of syncope and chest pain on exercise. Multi-slice coronary computed tomography (CT) demonstrated ostial stenosis of the left main trunk as well as wall thickening in the ascending aorta. Magnetic resonance imaging (MRI) also showed occlusion of the left carotid artery. She was diagnosed as having angina pectoris caused by aortitis syndrome. We performed off-pump coronary artery bypass grafting (OPCAB) with the left internal thoracic artery to the left anterior descending artery. The postoperative course was uneventful and coronary CT demonstrated good graft patency. We herein report successful OPCAB in a patient with left coronary main trunk ostial stenosis with aortitis syndrome.

A 42-year-old woman was admitted to our hospital because of an abnormal shadow on chest X-ray. Chest computed tomography scan revealed a tumor 4.0 cm in diameter in the right segment S8. We resected the right lower lobe because of the possibility of lung cancer. Historical finding of the resected specimen revealed epithelial-myoepithelial carcinoma of the lung. The patient has remained disease-free for a year and 3 months postoperatively.

A 59-year-old man was admitted to our hospital for abdominal mass and found to have a gastric cancer with peritoneal dissemination. Three courses of neoadjuvant chemotherapy combined with S-1 and CDDP were performed. This chemotherapy showed a substantial reduction of the size of primary tumor and peritoneal dissemination by CT examination. Surgical resection consisted of distal gasterectomy and D2 lymph node dissection was performed, and an ip catheter was placed through the douglas pouch, and the catheter was attached to the subcutaneous portal delivery system for ip chemotherapy. Operative cytology of ascites proved positive and remnant neoplasm cells were identified in the peritoneum. The pathological stage was determined as T3 N2 H0 P1 CY1 M0, pStage IV. Following surgery, we selected the ip administration of paclitaxel at a dose of 100 mg per body. Finally, the peritoneal dissemination was re-grown. However, we continued the ip chemotherapy for twenty-five times on ambulant basis. Most gastric cancer patients with peritoneal dissemination die within a few months, and there is no standard treatment for peritoneal dissemination from gastric cancer. In conclusion, a condition of no progression has been achieved and maintained for more than three years by intraperitoneal administration of paclitaxel for patient with peritoneal dissemination due to advanced gastric cancer.

A 47-year-old man with a history of surgery for gastric cancer had enlarging right hilar tumor. Primary or metastatic malignant tumor was suggested and the right upper lobectomy with systematic nodal dissection was performed. Pathological diagnosis was lymph node metastasis of the squamous cell carcinoma. He received no additional treatment, and is well without recurrence 5 years after resection.

A 52-year-old woman was admitted to our hospital because of an abnormal shadow on chest X-ray. Chest computed tomography scan and magnetic resonance imaging demonstrated an anterior mediastinal tumor. The tumor was extirpated completely with combined partial resection of the left lung through a median sternotomy. Microscopically, the tumor was diagnosed thymic cancer, basaloid carcinoma. The patient was treated with combination chemotherapy and radiation, postoperatively. We reported a case of thymic carcinoma.

A 52-year-old man was admitted to our hospital because of an abnormal shadow on chest X-ray. Chest computed tomography scan and broncho-fiberscopic examination revealed a tumor at the left main bronchus. The tumor was removed completely by partial resection of the left main bronchus with thoracotomy. Historical finding of the resected specimen revealed a benign pleomorphic adenoma of the bronchus. The postoperative course was un-eventful for 9 years. We reported a rare case of pleomorphic adenoma of the bronchus.

Mucosa-associated lymphoid tissue (MALT) lymphoma of the lung is rare. We present a case of MALT lymphoma of the lung. A 64-year-old man was admitted to our hospital because of an abnormal shadow on chest X-ray. He was performed transbronchial lung biopsy (TBLB) in our hospital, but the results was not diagnostic. We performed surgical biopsy using thoracoscopy due to the possibility of malignancy. The pathological diagnosis was MALT lymphoma. He underwent an chemotherapy after the operation and has been followed up at an outpatient for 8 years.

Small bowel metastasis from lung cancer is rare. We present 2 cases of small bowel metastasis from lung cancer. A 69-year-old man with postoperative lung cancer was admitted to our hospital because of the development of anemia secondary to melena A 56-year-old man with postoperative lung cancer was admitted to our hospital because of weight loss and fever. Both patients were operated on with suspicion of small intestine tumor after some examinations. The metastatic tumors were found in the small intestines. Resection of metastasis to the small bowel of lung cancer is benefical for better prognosis.

Osteogenesis imperfecta is a disease in which fragile bones readily cause fracture. Valvular disease concurrently develops. However, the surgery-related mortality rate is approximately 30%. In this study, we report 2 patients with osteogenesis imperfecta who underwent valvular heart surgery. Patient 1 was a 31-year-old male. He had previously been diagnosed as having osteogenesis imperfecta. Echocardiography suggested aortic valve insufficiency, and aortic valve replacement was performed. Patient 2 was a 59-year-old male. During admission, osteogenesis imperfecta was diagnosed. Echocardiography suggested mitral valve insufficiency, and mitral valve plasty was performed. In the 2 patients, intraoperative hemorrhage was marked. However, there were no fatal complications. We also reviewed the literature.

We have recently encountered a rare case of anterior cerebral artery dissecting aneurysm, presenting cerebral infarction and subarachnoid hemorrhage, associated with fibromuscular dysplasia (FMD). A 50-year-old woman was admitted to our hospital with complaints of headache and hemiparesis. A right carotid angiogram obtained 9 hours after the onset revealed stenosis and slight dilatation in the A1 portion of the anterior cerebral artery. At 12 hours after the onset, the patient fell into a coma. CT scan showed diffuse subarachnoid hemorrhage. An angiogram had revealed aneurysmal dilatation and double lumen in the A1 portion, but, 34 days after the onset, angiogram showed growth of the aneurysm in the A1 portion and stenosis in the A2 and A3 portions. At 38 days after the onset, the trapping of the dilated aneurysm to prevent rerupture was performed. Pathological diagnosis was concluded to be FMD. We concluded that FMD caused stenosis as well as the formation and rupture of the dissecting aneurysm in the anterior cerebral artery.

We evaluated the medium-term follow-up results, effectiveness, and suitability of arthroplasty for hemophilic arthropathy. We performed 26 total knee and 9 total hip arthroplasty operations on hemophilic patients between 1988 and 1998 under general anesthesia and appropriate hemostatic management. Postoperative treatment for hemophilic arthropathy is the same as that for osteoarthritis and rheumatoid arthritis. After their operations, patients experienced relief from pain and intra-articular bleeding in affected joints but only marginal improvement in the range of motion. In general, total joint arthroplasty is not indicated for young patients. However, arthropathy can have a severe impact on the active life of patients during their youth. For severe hemophilic arthritis, total knee and hip arthroplasty can lead to a pain-free and improved quality of life. We believe total knee and hip arthroplasty is a good solutions for hemophiliac arthropathy before severe deformity occurs. Although treated relatively young hemophilic patients, age was not considered to be a contraindication.

We report a case of constrictive pericarditis with atrial fibrillation after mitral valve repair and the Maze III procedure. A 66-year-old male underwent mitral valve repair and the Maze procedure for mitral valve regurgitation and chronic atrial fibrillation. About 4 months after discharge, he suffered from shortness of breath. Physical examination revealed a heart rate of 80 beats/min with irregular rhythm, external jugular venous dilatation and abdominal ascites. Electrocardiography revealed atrial fibrillation, and chest X-ray revealed moderate left pleural effusion. Computed tomographic images of the chest showed a that thickened pericardium. A distinct diastolic dip and plateau pattern were recognized on cardiac catheterization. The right atrial, right ventricular end-diastolic, and pulmonary wedge pressures were elevated. Idiopathic pericarditis and recurrent atrial fibrillation were diagnosed and pericardiectomy was performed through a median sternotomy incision. Intraoperatively, the atrial fibrillation converted spontaneously to sinus rhythm. The postoperative hemodynamics improved after pericardiectomy. Total pericardiectomy alone may not convert the rhythm to sinus rhythm in patients with constrictive pericarditis and chronic atrial fibrillation. In this case, atrial fibrillation converted to sinus rhythm during the procedure. This case report suggests that adequate unloading of atrial pressures is necessary for the maintenance of sinus rhythm in patients who have undergone the Maze procedure.

A 28-year-old Marfan's syndrome woman was referred to our hospital because of sudden chest and back pain. She had received replacement of the descending and ascending aorta for acute aortic dissection previously in other two institutions. CT scan revealed the massive mediastinal hematoma and hemothorax which suggested ruptured dissecting aneurysm of the transverse arch. Emergent total arch replacement, re-ascending replacement and aortic root plasty successfully weve performed. She recovered uneventfully although she had residual thoracoabdominal dissecting aneurysm. It was suggested that total arch replacement should be performed for Stanford A type dissection in Marfan's syndrome.

Aortic arch coarctation proximal to the left subclavian artery is a rare anomaly. It is about 1% of all coarctation of the aorta. This 16 days neonate with coarctation of aortic arch with severe subaortic stenosis and ventricular septal defect was underwent two staged repair. In the first-stage operation, coarctation of the aortic arch was reconstructed by extended end to end direct anastomosis through posterolateral thoracotomy. In the second stage, we performed intracardiac repair with use of a technique which included resection of superior margin of conus septum through VSD and patch closure of VSD. The postoperative pressure study showed no gradient between left ventricle and left radial artery. The postoperative course was satisfactory. This patient is doing well.

Outcome of 77 cases with ruptured intracranial aneurysms above 60 years of age was reported. 44 cases were treated conservatively and 33 cases surgically. 1. Among conservatively treated group, 10 cases died after initial hemorrhage and 14 cases after second hemorrhage. The mortality rate was 55%. 2. Among surgically treated group, 11 cases died and 4 cases had severe neurological deficits following the direct intracranial operation. The mortality and morbidity rate was 33% and 12% respectively. 3. These results indicate that the direct intracranial operation should be the first choice of treatment to the aged patients also. 4. Intracranial complications occured more frequently than the extracranial one during intra and postoperative course. Among them normal pressure hydrocephalus was the comonnest. 5. Occlusion of the main cerebral artery occured in 5 cases related to operative procedure. Outcome of 4 cases of them was poor. Microsurgical technique should be used in order to prevent these conditions which tend to occur in aged patients.

To assess tricuspid regurgitation (TR) in patients with permanent transvenous right ventricular (RV) pacing, we performed phonocardiographic, contrast and pulsed Doppler echocardiographic studies in 18 patients with transvenous leads for RV pacing. In addition, a pathological study was performed on 26 autopsy cases with transvenous leads for RV pacing. None of the patients had right-sided heart failure. The previous phonocardiograms revealed regurgitant murmurs of TR in one clinical case and five autopsy cases. In the clinical study, definite TR was diagnosed both by contrast and pulsed Doppler echocardiography in five cases (28%). Probable TR was diagnosed only by one technique in three cases (17%), and the absence of TR was confirmed by both techniques in 10 cases (55%) (non-TR group). The average right atrial dimension was 59 +/- 5.3 mm in the definite TR group and 39 +/- 2.4 mm in the non-TR group (p less than 0.01). The average inferior vena cava dimension was 19 +/- 1.7 mm in the definite TR group and 15 +/- 0.8 mm in the non-TR group (p less than 0.05). Right atrial and inferior vena cava dimensions showed a significantly positive correlation (r = 0.58, p less than 0.05). In the pathological study, the presence of TR, which was explained by the position of the pacemaker lead in relation to the valve structure, was confirmed in 11 cases (42%). Valve motion interference was classified as type I (two cases), in which the lead was suppressed and the leaflet immobilized, type II (4 cases), in which chordae tendineae were involved by a pacemaker lead, and type III (five cases), in which both mechanisms contributed to valvular regurgitation. In conclusion, TR may follow transvenous RV pacing in approximately half of the cases with RV pacing. Contrast and pulsed Doppler echocardiography are sensitive noninvasive techniques for detecting this valvular abnormality and they should be used in the follow-up of such pacemaker recipients.

It is well recognized that the communicating hydrocephalus following the rupture of intracranial aneurysms is one of the factors which cause the disturbance of consciousness in acute stage and the impairment of mental functions in chronic stage. In this report we analyzed the surgical experience with this complication and discussed the mechanisms which cause the ventricular dilatation. 1. The communicating hydrocephalus following the rupture of intracranial aneurysms is devided into the acute form and the chronic form. 2. Acute communicating hydrocephalus may develop with 24 hours after the onset and the incidence is about 10% (6 out of 66 cases). 3. Intraventicular pressure is high in acute communicating hydrocephalus and becomes normal in chronic stage. 4. The incidence of the chronic communicating hydrocephalus is about 10% (6 out of 66 cases). 5. There are two types of chronic communicating hydrocephalus. One develops from acute form and another develops gradually after onset. Among 6 chronic communicating hydrocephalus, 3 developed from acute form, 2 showed slowly progressive ventricular dilatation and 1 admitted with hydrocephalus at chronic stage resepctively. 6. The intraventricular pressure of the acute form is high and the chronic form shows high pressure in acute stage, intermittent high pressure wave lasting 20-30 minutes in subacute stage and normal pressure in chronic stage when the ventricle is dilated. 7. The mechanisms of ventricular dilatation in normal pressure hydrocephalus are discussed on the basis of the continuous monitoring of intraventricular pressure in cases with rupture of intracranial aneurysm. 8. Indication of shunting procedures whould be determined by combination of clinical pictures, ventriculocranial index, findings of cisternography and EEG. Our operative criteria are presented.

A case of giant intracranial aneurysm which almost completely occluded the internal carotid artery and was diagnosed angiographically by prolonged injection technique was reported. A 62-year-old house-wife was admitted to the Department of Neurosurgery of The Asahi General Hospital, complaining of headache, nausea, and left ptosis. Neurological examination revealed left IInd, IIIrd, IVth, and VIth cranial nerve palsy. Films and laminagrams of the skull demonstrated the double floor of the sella trucica. Plain CT scan showed an oval high density area without surrounding low density area in the left middle cranial fossa. A left carotid angiogram revealed almost complete occlusion of the internal carotid artery at the foramen lacerum. Good cross filling through the anterior communicating artery was seen in the right carotid angiogram. Pre-operative diagnosis was pituitary tumor which extended to the middle cranial fossa. Left fronto-temporal craniotomy was performed and the tumor was suspected to be a giant intracranial aneurysm. Post-operative carotid angiogram by prolonged injection technique also showed a blood channel in the aneurysm and a part of the aneurysm. Left carotid ligation was carried out following intracranial surgery and her symptoms were remarkably improved. Apart from left VIth cranial nerve palsy she showed no abnormal neurological findings at discharge. Usefulness of the prolonged injection technique for differential diagnosis of the parasellar tumor was stressed.

Palinopsia is defined as persistence or recurrence of a visual image when the stimulus is no longer present. The authors describe a female patient with a left tentorial meningioma who experienced recurrent visual images. Computed tomography revealed a well demarcated, high-density mass above the left side of the tentorium, but formal field testing revealed no visual field defect. The findings in this case suggest that the palinopsia may have been the result of disordered temporal synthesis of visual experiences.
