1985年より群馬県及びその近郊の病・医院における未治療前立腺癌の登録作業を開始し, その資料をもとに, 1985～2000年の群馬県における前立腺癌の罹患数, 粗罹患率, 年齢調整罹患率, 年齢別罹患率, 臨床病期, 組織学的分化度の推移について検討した.群馬県における前立腺癌の罹患数及び粗罹患率(/10万人/年)の推移は, 各々1985年には114例, 12.0であったものが, 2000年には539例, 53.9と急激に増加した.年齢調整罹患率(/10万人/年)の推移について, 世界人口と1985年モデル人口の両方で年齢調整を行ったところ, 世界人口では, 1985年に8.3であったものが2000年には24.2に, 1985年モデル人口では, 各々13.1であったものが36.8と約3倍に増加した.80年代後半に対しての90年代前半, 90年代後半の年齢別罹患率の増加率については, 年代が進むにつれていずれの年齢層においても罹患率は増加した.各年齢における増加率は, 60歳代から70歳代前半の中間の年齢層において高い値を示した.臨床病期では, 年代共に病期A, Dが減り, 病期B, Cが増える傾向であった.組織学的分化度では, 年代によって明らかな分化度別割合の変化はみられなかった, Prostate cancer is the most common urogenital cancer, and is increasing rapidly. We performed an epidemiological study on prostate cancer in Gunma Prefecture, Japan. Registration of prostate cancer patients diagnosed at clinics and hospitals in and around Gunma Prefecture was started at Gunma University in 1985. The epidemiological characteristics of prostate cancer patients in Gunma Prefecture were analyzed by these data. The incidence and crude incidence rates have increased five-fold, from 114 and 12.0 in 1985 to 539 and 53.9 in 2000, respectively. The age-adjusted incidence rate (adjusted to the world population) was increased three-fold, from 8.3 in 1985 to 24.2 in 2000. The age-specific incidence rate showed an increase with age. The cancers in clinical stages A and D decreased, while those in stages B and C increased. No change in distribution was observed in pathological differentiation. Prostate cancer has increased rapidly during these 16 years in Gunma Prefecture. It is important to perform PSA testing aggressively in males age 50 or older, and detect prostate cancer in an early stage.

1986年4月からの9年2ヵ月間に前立腺癌と診断された169例の治療結果を示し, 予後因子と患者の年齢がそれらに与える影響について検討した.予後因子として臨床病期と組織学的分化度が重要であるが, 他因死が多いことからは年齢を考慮して治療法を選択することが必要である.今後は早期診断の為のスクリーニングが必要であろう, At Matsumoto National Hospital, 169 patients with prostate cancer were diagnosed between April 1986 and May 1994. The prostate cancer incidence was the highest in the latter half of the seventies, with an average age of 74.3 years. The clinical stage was defined as A1, A2, B, C, and D2 in 24 (14.2%), 38 (22.5%), 39 (23.1%), 23 (13.6%) and 45 (26.6%) patients, respectively. The clinical stage was not correlated with the patient's age. Incidental carcinoma was discovered in 5.8% of the patients who underwent prostatectomy for benign prostatic hypertrophy (BPH). At initial diagnosis, the tumor was well, moderately, and poorly differentiated adenocarcinoma in 71 (42.0%), 64 (37.9%), and 34 (20.1%) patients, respectively. The median follow-up period was 38.7 months. The over all five-year crude survival rate was 54.8%, while the cause-specific five-year survival rate was 80.0%. The five-year crude survival rate was 58.4, 82.0, 55.2, 42.5 and 37.4%, for patients with cancer at stage A1, A2, B, C, and D. The survival rate was higher for patients with cancer at clinical stage A and shorter for those with cancer at clinical stage D than in other stages. Prognosis was also worse in patients with moderately or poorly differentiated adenocarcinoma than in those with well-differentiated adenocarcinoma. The clinical stage and the pathological grade are important as prognostic factors, although the high incidence of death from other diseases shows that the patients' age should be considered to choose the modality of therapy. These findings indicate that intensive treatment of the patients in clinical stage A2, B and C prostate cancer in combination with screening for the men between 50 and 75 years old for early cancer detection is required.

1)腎細胞癌例を対象として, 術前にIFN-γを投与した投与群と無投与群で比べると, 投与群でCD3, CD8, CD11b, ICAM-1が有意に多く認められた. 2)High stageの例で無投与群に比べ, CD3, CD8, CD11b, ICAM-1が投与群に有意に多く認められた, We studied the relationship between clinical stage and tumor infiltrating lymphocytes (TIL) in 26 cases of renal cell carcinoma. In 10 patients, interferon-gamma (IFN-gamma) was administered preoperatively (administration group); 5 patients had low stage (Robson or = II) tumors. The other 16 patients underwent nephrectomy alone without preoperative IFN administration (control group); 11 patients had low stage, and the remaining 5 patients had high stage tumors. Immunohistochemical studies of tumor infiltrating lymphocytes in renal cell carcinoma showed a significantly high incidence of CD3, CD8, CD11b and ICAM-1 in the administration group, while CD4, LFA-1 and Ber-MAC3 were increased without significance (p < 0.05). Concerning clinical stage (Robson), a significant increase in CD3, CD8, CD11b and ICAM-1 was observed in the patients with high-stage tumors in the administration group compared to those in the control group (p < 0.05). This suggested that TIL could be changed by preoperative administration of IFN-gamma.
