In the last decade, ''fertiloscopy'', a new mini-invasive diagnostic technique, is becoming more and more popular: it is a good alternative to the diagnostic laparoscopy, a standard procedure but surely not harmless, very often capable to discover pathologies in asymptomatic patients. Fertiloscopy allows the visualization of the posterior pelvis (posterior face of the uterus, ovaries, tubes and intestinal ansae with the rectum), with a technique of introducing an optical device in the pouch of Douglas, through the posterior vaginal fornix, under previous general or local anesthesia. When fertiloscopy is performed under local anesthesia, it can comfortably be carried out in out-patient departments and it is generally well tolerated by patients, who follow the whole procedure on the monitor. Moreover, it is possible to perform small interventions, such as adhesiolysis, ovarian drilling, coagulation of endometriosis spots and to perform chromosalpingoscopy and salpingoscopy, important investigations in the diagnostic iter of unexplained female infertility. With fertiloscopy, the patient, therefore, can avoid a real surgical intervention, such as diagnostic laparoscopy, and also uncomfortable examinations, such as hysterosalpingography.

One case of ovarian ectopic pregnancy associated with twin intrauterine viable pregnancy after IVF-ET, and one case of contralateral tubal ectopic pregnancy after GIFT performed with normal fallopian tubes, are reported. The possible pathogenesis and management are discussed.

A case of ovarian pregnancy in a patient with intrauterine device is reporter. The patient was hemodynamically instable and immediate laparotomy was indicated. The treatment was left oophorectomy. Ovarian pregnancy is a rare form of ectopic pregnancy in which the gestational sac is implanted in the ovary. The incidence appears to be of 1 to 3 per cent of all ectopic gestations with a frequency of one in 7000 to 40000 deliveries and it is now believed to occur four times more frequently than previously thought. It has been suggested that the increasing incidence is caused by the use of IUD. In contrast to patients with tubal pregnancy traditional risk factors such as pelvic inflammatory disease, prior surgical procedure upon the pelvis may not apply. The clinical signs are similar to those found in tubal pregnancies and in hemorrhagic corpus luteum cysts. The treatment is emergency laparatomy followed by oophorectomy in many instances. When the patient is hemodynamically stable laparoscopy and the ovarian wedge resection should be the treatment of choice.

Protein kinases in wild-type CHO cells have been characterized. Cells cultured on MEM were collected, homogenized and the extract assayed for protein kinase activity. DEAE cellulose chromatography of 30.000xg extract yields 2 peaks of protein kinases activity, PKI and PKII. The two peaks when analyzed for the binding of 8-N3-(32P)cAMP show two subunits RI and RII and a RI not associated with the enzymatic activity, named RF. This characterization allows us to discuss the meaning of protein kinases in the modulation of the growth regulating effects of cAMP.

An observed case of ovarian pregnancy after intraperitoneal insemination (IPI) triggers a description of this new technique and a suggestion as to the possible dynamics of the ensuing ectopic pregnancy.

In this paper we characterize the biochemical defect of a mutant (10248) of CHO cells, resistant to the cAMP treatment. Cells cultured on MEM were collected each three days, homogenized and centrifuged. The cell extract was assayed for protein kinases activity and the binding of 8-N3-(32P)cAMP. The same extract was also applied on to a DEAE cellulose column, eluted with a linear gradient and the fractions tested for the phosphotransferase activity and 8-N3-(32P)cAMP binding. Mutant 10248 shows a different profile of protein kinases activity as compared to 10001 control. Protein kinases II is absent whereas a normal RII binding activity is present. RI shows altered affinity for cAMP.

Bilateral breast cancer is examined in the light of the literature data and a personal series. Lobular, asynchronous forms, either in situ or infiltrating, are usually involved. The fact that in situ changes are often found in the second neoplasia, with metastases solely in its tributary lymph nodes, suggests that such carcinoma was already present at the time of the first operation, though not clinically detectable. Since bilateral forms are commonly hormone-dependent, multiple biopsies should be routine on the contralateral breast. Preventive mastectomy associated with ovariectomy shold be performed on the slightest sign of carcinomatous or even precancerous alterations.
