Abstract

Renal tubular acidosis (RTA) refers to a group of disorders of multiple etiology characterised by a hyperchloremic, normal anion-gap metabolic acidosis caused by different defects affecting several mechanisms of urinary acidification: an impaired secretion of protons by collecting duct in distal hypokalemic RTA (type 1), impaired resorption of bicarbonate in proximal RTA (type 2), a mixed proximal and distal defect in RTA type 3 and an impaired ammoniagenesis due to an absolute or functional hypoaldosteronism in distal hyperkalemic RTA (type 4). Etiology is heterogeneous and ranges from primitive genetic forms to secondary RTA within a multi-system disorder such as autoimmune disease, paraproteinemia and calcium-phosphate disorders. Drug-induced RTA has been increasingly reported over the last decade due to widespread use of some molecules (e.g. antiviral and oncologic agents, inhibitors of renin-angiotensin system). The diagnosis of RTA is based on assessment of serum and urine anion-gap, whereas differential diagnosis between RTA forms requires the analysis of urine pH or osmolality and of serum and urine electrolytes. An acidification test should be performed to confirm the diagnosis. Clinical setting can also provide important clues in orienting towards a type of RTA: nephrolithiasis and nephrocalcinosis are associated with type 1 RTA, ostemalacia and osteoporosis are more frequent in type 2 RTA, osteopetrosis is typically linked to type 3 RTA and hyperkalemia is the predominant manifestation of type 4 RTA. L'acidosi tubulare tipo IV (RTA di tipo IV) è una forma di acidosi metabolica ipercloremica causata da un ipoaldosteronismo assoluto o funzionale, che determina un deficit dell'acidificazione distale attraverso un'inibizione dell'ammoniogenesi e dell'escrezione di H+ a livello del dotto collettore. L'eziologia è spesso multifattoriale e include disordini che determinano una riduzione dei livelli di aldosterone o della sensibilità del dotto collettore alle azioni dell'ormone. I farmaci che inibiscono il sistema renina-angiotensina (RAS) a qualunque livello aumentano il rischio di sviluppare una RTA di tipo IV, soprattutto quando sono impiegati in associazione in pazienti anziani affetti da diabete mellito, scompenso cardiaco e insufficienza renale cronica oppure portatori di trapianto renale. La diagnosi si basa sul riscontro di un'iperkaliemia sproporzionata alla funzione renale associata a un'acidosi metabolica con anion gap sierico normale e anion gap urinario positivo; il pH urinario può essere inferiore a 5.5. Vari elementi laboratoristici consentono la diagnosi differenziale con l'acidosi tubulare distale di tipo 1. L'impatto clinico della RTA di tipo IV sta diventando sempre più rilevante a causa della diffusione di farmaci bloccanti il RAS in pazienti anziani già affetti da condizioni a rischio per questa complicanza, contribuendo a una rivalutazione critica delle indicazioni ad alcune terapie di associazione. Lo sviluppo di iperkaliemia pone in questi contesti un dilemma terapeutico, in quanto proprio i pazienti a maggior rischio per RTA di tipo IV sono quelli che possono trarre il massimo vantaggio cardiovascolare dal blocco farmacologico del RAS. È, quindi, essenziale trovare un punto di equilibrio nel rapporto rischio/beneficio nel singolo paziente, prevenendo la RTA di tipo IV con opportune misure.

Abstract

Chronic kidney disease (CKD) is a clinical condition associated with a high risk of cardiovascular (CV) events, mortality and progression to most severe stage of the disease, also known as kidney failure (KF). CKD is characterized by a wide variability of progression, which depends, in part, on the variability of individual response to nephroprotective treatments. Thus, a consistent proportion of patients have an elevated residual risk both CV and renal events, confirmed by the evidence that about 70% of CKD patients followed by the nephrologist have residual proteinuria. Among the new therapeutic strategies, which have been developed precisely with the aim of minimizing this residual risk, a class of particular interest is represented by the new non-steroidal mineralocorticoid receptor antagonists (non-steroidal MRA). These drugs exert an important anti-fibrotic and anti-proteinuric effect and, unlike steroid MRAs, are associated with a much lower incidence of adverse effects. The non-steroidal MRA molecule for which the most data is available, which is finerenone, is potent and extremely selective, and this partly explains the differences in efficacy and safety compared to steroid MRAs. In clinical trials, finerenone has been shown to significantly reduce the risk of progression to KF. Furthermore, there is also evidence that the combination of non-steroidal MRAs together with SGLT2 inhibitors may represent a valid alternative to reduce the residual risk in CKD patients. Given this evidence, non-steroidal MRAs are gaining momentum in the care, and particularly in individualized care, of CKD patients., (Copyright by Società Italiana di Nefrologia SIN, Rome, Italy.)

Abstract

Primary aldosteronism (PA) is the single most common cause of secondary hypertension and is associated with increased target organ injury. It can be can either surgically cured or treated with targeted pharmacotherapy. PA is frequently undiagnosed and untreated, leading to aldosterone-specific cardiovascular morbidity and nephrotoxicity. Thus, clinicians should perform case detection testing for PA at least once in all patients with hypertension. The diagnostic work-up of PA is a sequence of three phases comprising screening tests, confirmatory tests and the differentiation of unilateral from bilateral forms. With appropriate surgical expertise, laparoscopic unilateral adrenalectomy is safe, efficient and curative in patients with unilateral adrenal disease. In patients who have bilateral aldosterone hypersecretion, the optimal management is a low sodium diet and lifelong treatment with a mineralocorticoid receptor antagonist administered at a dosage to maintain a high-normal serum potassium concentration without the aid of oral potassium supplements. In patients with PA, specific treatment provides prognostic benefit over optimal antihypertensive therapy and is therefore crucial to reduce mortality and morbidity in this subgroup of patients with hypertension.

Abstract

Cardiovascular and renal diseases are one of the main health problems in all industrialized countries. Their incidence is constantly increasing due to the aging of the population and the greater prevalence of obesity and type 2 diabetes. Clinical evidence suggests that aldosterone and the activation of mineralocorticoid receptors (MR) have a role in the pathophysiology of cardiovascular and renal diseases. Moreover, clinical studies demonstrate the benefits of mineralocorticoid receptor antagonists (MRAs) on mortality and progression of heart and kidney disease. In addition to renal effects on body fluid homeostasis, aldosterone has multiple extrarenal effects including the induction of inflammation, vascular rigidity, collagen formation and stimulation of fibrosis. Given the fundamental role of MR activation in renal and cardiac fibrosis, effective and selective blocking of the signal with MRAs can be used in the clinical practice to prevent or slow down the progression of heart and kidney diseases. The aim of the present work is to review the role of MRAs in light of the new evidence as well as its potential use as an antifibrotic in chronic kidney disease (CKD). The initial clinical results suggest that MRAs are potentially useful in treating patients with chronic kidney disease, particularly in cases of diabetic nephropathy. Abstract

Studi sperimentali e clinici hanno indotto ad ipotizzare una interazione fisiopatologica tra aldosterone (PAC) e paratormone (PTH) nello sviluppo dell’ipertensione arteriosa e delle alterazioni cardiovascolari . Scopo di questo studio è stato di valutare il rapporto tra PAC e PTH nell’ipertensione essenziale (EH) e le eventuali interazioni con FGF23, proteina modulatrice della secrezione di PTH. In 105 H ed in 50 controlli sani sono stati analizzati i livelli plasmatici di PAC, PTH, FGF23 Vitamina D. L’analisi di correlazione multipla condotta considerando PAC come variabile dipendente ha mostrato una correlazione di PAC con FGF23 (β = 0.337; p = 0.0021) e con PTH (β = 0.241; p = 0.039) indipendente da pressione arteriosa 24h, Vitamina D, funzione renale. I nostri dati suggeriscono che sia PTH che FGF23 siano coinvolti nella fisiopatologia dell’ipertensione arteriosa, anche in assenza di malattia renale cronica

Abstract

L’iperaldosteronismo primario è una patologia considerata rara nel gatto ed è causata da un’eccessiva secrezione di aldosterone da parte della corteccia surrenalica. L’ipersecrezione può essere causata o da un’iperplasia bilaterale della zona glomerulosa o da una neoplasia aldosterone-secernente, detta aldosteronoma. Due gatti di sedici anni sono stati presentati per debolezza, atassia e ventroflessione del collo. Un protocollo diagnostico completo ha permesso di evidenziare in entrambi i soggetti ipokalemia e un notevole aumento della creatinchinasi. In entrambi i gatti l’ecografia addominale ha evidenziato una massa a carico di una ghiandola surrenale; tale reperto, associato alla misurazione dell’aldosteronemia, ha permesso di raggiungere un forte sospetto di aldosteronoma. Il protocollo terapeutico nel primo caso ha previsto l’asportazione chirurgica della ghiandola surrenale, mentre nel secondo caso si è optato per una terapia medica con integrazione di potassio gluconato e spironolattone. In entrambi i casi è stata ottenuta una completa remissione della sintomatologia.

19. Relazione tra obesità ed aldosteronemia nell'ipertensione essenziale: influenza del sesso

Abstract

Una relazione tra obesità e aldosteronemia (ALD), indipendente dall’attività reninica plasmatica (PRA), è stata ipotizzata sulla base di diverse evidenze sperimentali, ma non è stata confermata in tutti gli studi clinici effettuati sull’argomento. Inoltre, pochi dati esistono in letteratura sull’eventuale influenza del sesso su tale relazione. Scopo del nostro studio è stato quello di esaminare, prima globalmente e poi separatamente nei due sessi, l’eventuale associazione tra indice di massa corporea (BMI), circonferenza addominale (W) e ALD in un ampio gruppo di ipertesi essenziali non diabetici, afferenti al nostro Centro di Riferimento per lo studio dell’Ipertensione Arteriosa. Sono stati studiati 590 ipertesi non diabetici (età media 43.8 ± 12.5 anni; 61% maschi), con rapporto aldosterone/renina < 400 ed esenti da complicanze cardiovascolari e renali. Tutti i soggetti sono stati sottoposti, in assenza di trattamento farmacologico, oltre che a dosaggio dell’aldosteronemia e dell’attività reninica plasmatica dopo 1 ora di clinostatismo e degli elettroliti sierici ed urinari, a monitoraggio ambulatorio della pressione arteriosa delle 24 ore. Nell’intero gruppo esaminato sono state osservate delle correlazioni positive e statisticamente significative sia del BMI (r = 0.13; p = 0.002) che della W (r = 0.14; p = 0.001) con l’ALD, trasformata logaritmicamente per la sua distribuzione non gaussiana. Tali relazioni sono state confermate (p < 0.01) in modelli di regressione lineare multipla stepwise, in cui l’ALD è stato considerato variabile dipendente, mentre l’età, il sesso, la natriuria delle 24 ore, i valori pressori sistolici e diastolici (clinici o delle 24 ore), il logaritmo della PRA ed alternativamente il BMI o la W sono stati inclusi come variabili indipendenti. Benchè il fattore di interazione BMI (o W) x sesso non sia risultato significativo, quando aggiunto ai modelli di regressione multipla, abbiamo osservato che ripetendo le analisi multivariate separatamente nei due sessi, le relazioni del BMI e del W con l’ALD sono rimaste statisticamente significative solo nei soggetti di sesso maschile (rispettivamente, β = 0.11; p = 0.02 e β = 0.10; p = 0.03). Inoltre, la suddivisione della popolazione studiata in soggetti normopeso, sovrappeso ed obesi ha mostrato livelli di ALD crescenti nei tre gruppi, e statisticamente differenti, negli uomini, ma non nelle donne. I nostri risultati sembrano avvalorare l’ipotesi di una associazione positiva tra obesità e aldosterone plasmatico, anche se tale relazione sembra essere confermata solo nel sesso maschile.

Abstract

The purpose of this review is to summarize the current knowledge regarding metabolic syndrome prevalence and features in primary aldosteronism. We will also discuss the link between aldosterone and the different metabolic changes typical of the metabolic syndrome. Hypertensive patients have a high prevalence of obesity, dyslipidemia and hyperglycaemia. These are risk factors for the metabolic syndrome, and are associated with an increased cardiovascular risk profile. In particular, insulin resistance seems to be the major alteration in patients affected by primary aldosteronism. We will then describe the experimental and clinical evidences of the role of aldosterone in the pathogenesis of insulin resistance. Higher rates of cardiovascular events have been recently reported in primary aldosteronism: they could be partly due to the increased prevalence of the metabolic syndrome in this disorder.

24. [The early hemodynamic and hormonal changes in patients with left ventricular dysfunction]

Abstract

The aim of this study was to highlight a different hormonal and hemodynamic pattern in patients with mild cardiomyopathy. For this purpose, we studied subjects with mild heart failure (CHF; NYHA class I and II; post-ischemic and idiopathic) who underwent an isotonic saline load (SL) (0.22 ml/kg/min of 0.9% NaCl for 120 min). A second group of age- and sex-matched normal subjects (C) was studied as a control. Basal hormonal and hemodynamic values of the 2 groups differed only in atrial natriuretic factor (ANF), left ventricular end-diastolic diameter and ejection fraction (EF). There were, on the contrary, no differences in basal plasma renin activity (PRA) and plasma aldosterone (PA) values. After SL, in C, percent changes in EF, cardiac output and ANF values were significantly higher than in CHF while total peripheral resistances increased only in CHF but not in C. In both groups there were decrements of PRA and PA, but these responses were significantly higher in C than in CHF. In conclusion, our results show that hormonal, renal and hemodynamic responses to salt/volume load are compromised in the early asymptomatic phase of heart failure. These abnormalities may predict the progressive deterioration of cardiac function, and may indicate appropriate therapeutic interventions since the early phases of the disease.

Abstract

Previous studies demonstrate that high doses of angiotensin II (Ang II) increase the release of ANF from atrial cells but it is not known whether this is a direct effect of Ang II or due to the induced hemodynamic changes. We report the effects of low doses of Ang II (1, 2.5, 5, 10 ng/kg/min) in anesthetized, instrumented dogs after volume load (2.5% body weight) and converting enzyme inhibition. During Ang II infusion we found an increase in mean blood pressure (from 147 +/- 3 to 160 +/- 3 mmHg, p less than 0.05) and arterial ANF (from 32 +/- 6 to 80 +/- 23 fmol/ml, p less than 0.05), while left and right atrial pressures did not change significantly. In a second group of dogs (n = 4) that underwent a similar protocol with the infusion of vehicle alone we failed to find any statistical difference in the above mentioned parameters. The Ang II induced ANF release was not related to the hemodynamic changes. Changes in plasma ANF levels were, in turn, related to the effects of Ang II on hormones and kidney, thus suggesting a role for endogenous ANF. In a separate study we found an increase of ANF production (+129 +/- 18, +176 +/- 46, +210 +/- 66% basal value) from isolated atrial minces exposed to Ang II concentration of 1, 10, and 100 nM, respectively.

28. [Parametric methods applied to the standardization of gestational reference intervals for blood renin activity, aldosterone and cortisol using the transversal and longitudinal approach]

Abstract

This investigation is devoted to define monthly-qualified gestational reference intervals for plasma renin, aldosterone and cortisol as derived by transverse or longitudinal approaches. Reference intervals refer to 95% confidence interval, 90% tolerance interval, the 95% predictive interval, in addition to the mean plus or minus the standard error and standard deviation. By applying both methodological approaches it has been found that plasma renin and aldosterone exhibit a progressive gestational increase. By contrast, cortisol shows an initial increase and thereafter stays on the same values. Statistical analysis revealed the transversally--and longitudinally--derived reference intervals to be not statistically different. Because of such a similarity, laboratory medicine can interchangeably use both kinds of reference intervals for estimating the normality of plasma renin, aldosterone and cortisol values in a single case of pregnancy. This interchangeability of values leads to simplify the methodology for measuring plasma renin and aldosterone in pregnancy since it is not necessary to standardize the gestational nomograms for renin-sodium and/or aldosterone-sodium relationship. This is a real way to reduce the costs and to optimize the compliance in the monitoring of pregnancy.

Abstract

The activity of bromocriptin has been clinically tested on 18 patients suffering from psoriasis. The plasma levels of Human Growth Hormone (HGH), Human Adrenocorticotropic Hormone (ACTH), Thyroid Stimulating Hormone (TSH), Prolactin (PL), Aldosterone and Cortisol were investigated in these 18 patients along with 35 untreated psoriatic patients and 19 normal subjects. Bromocriptin was shown to be effective in 13 of our 18 psoriatic patients. Plasma levels of HGH, ACTH, and Aldosterone, measured in all 53 psoriatic patients, were found to be higher than normal in 11, 26 and 40 patients respectively (HGH and Aldosterone: p less than 0,005; ACTH: p less than 0,001).

Abstract

Normal-renin essential mesor hypertensives are characterized by a consistent increase in erythrocyte membrane-bound Na/K-ATPase activity. Low-renin essential hypertensives exhibit, in contrast, a lower activity in Na/K-ATPase of cell membranes. This study documents a third disorder characterized by a temporal shift in the rhythmic activity of the erythrocyte membrane-bound Na/K-ATPase in normal-renin hypertensives. The disorder causes the synchronism with the aldosterone circadian rhythm to be invariably lost. The uncoupling phenomenon could be invoked to explain the inversion in the day-night sodium excretion rate found in essential hypertensives. In addition, it suggests that the circadian rhythm in Na/K-ATPase is under the control of cycling factors other than aldosterone.
