Background: The COVID-19 pandemic caused by SARS-CoV-2 has greatly modified outpatient follow-ups. The aim of this retrospective study was to evaluate the organizational modalities and clinical effects of rearrangements of pacemaker (PM) and implantable cardioverter-defibrillator (ICD) outpatient visits performed in our centers at Ravenna and Lugo Hospitals, Italy, during the pandemic outbreak in 2020., Methods: All scheduled in-person device follow-up visits in March-December 2020 have been considered. On the basis of documented past functioning of each device and of remote monitoring (RM) capabilities, in-person visits were either performed or postponed at variable times. The characteristics of the follow-ups and the device-related clinically relevant events were analyzed, the latter being further divided into serious malfunction and problems to be corrected by device reprogramming., Results: Overall, 27% of in-person visits were postponed (n = 576) (36% of ICDs and 25% of PMs), peaking 62% in March-May 2020. RM compensated nearly all hold-ups in ICDs and just 63% of postponements in PMs. The postponement-caused delay between in-person visits was 5.6 ± 1.1 months for ICDs and 4.7 ± 1.2 months for PMs; in 24% of ICDs the time interval between in-person visits was ≥18 months. Clinically relevant events were 56 (18 [4.4%] in ICDs, 38 [2.1%] in PMs), with no deaths and 21 serious malfunctions (4 [1%] in ICDs, 15 [0.8%] in PMs). RM identified all ICD malfunctions, while it was not available in the affected PMs. In comparison with the year 2019, serious malfunctions increased, though the difference was not significant. Monthly RM transmissions increased by 2.3 fold., Conclusions: In our single-center experience during the COVID-19 pandemic, numerous in-person PM/ICD follow-up visits were postponed, and delays were well beyond the previously recommended time limits. However, device-related malfunctions did not increase, notably, when RM capabilities were used.

In recent years, due to the need to elaborate the amount of information available from the scientific literature, the meta-analyses and systematic reviews have become very numerous. The meta-analyses are carried out to evaluate the association between two events when single researches haven't provided comprehensive data. On the other hand, a good meta-analysis must satisfy certain criteria, from the selection of the studies until the evaluation of the outcomes; to this purpose, the application of methods for quality assessment is a crucial point to obtain data of adequate reliability. The aim of this review is to give some introductory tools for a critical approach to meta-analyses and systematic reviews, which have become useful instruments also in occupational medicine.

Objective: To study the urinary excretion of As, Cr, Mn, Co, Ni, Cu, Zn, Cd, Sn, Ba, Hg, Pb, Sb in workers at the Taranto integrated-cycle steel foundry and in subjects from the general population of Taranto, to assess the health risk posed by occupational exposure and environmental exposure, respectively, to these metals., Materials and Methods: The study included 49 steel foundry workers (exposed), working in the minerals and agglomerates pools, steel processing plants 1 and 2 and maritime plants, and 50 subjects belonging to the general population of Taranto resident at various distances from the factory (controls), randomly selected from the exposed subjects and controls enrolled in previous research conducted in 2005. A questionnaire was administered to all participants, enquiring into general characteristics, lifestyle, diet, and any medical conditions. Informed written consent to take part in the study was obtained from all subjects before enrolment. The results of environmental monitoring performed in 2005 in the workers' sectors, consisting of determining As, Cr, Mn, Ni, Cu, Zn, Cd and Pb in the respirable dust, revealed by both samplers applied in fixed positions and personal samplers, were considered. Urine samples were obtained from all participants on a Friday, to determine As and Cr by AAS and all the other metal elements by a multielement technique with ICP-MS. Urinary creatinine was also determined to make any necessary adjustments. All urine analyses were performed in 2005 within one month of urine collection., Results: In the respirable dust, As and Cd were always within the LOD, whereas Cr, Mn, Ni, Cu and Pb were 1-2 orders of magnitude below the respective TLV-TWA of the ACGIH. Mn was the only metal element that presented significantly higher urinary concentrations in exposed subjects as compared to controls, although the values in both groups were in any case within the Italian reference range. Co, Cu, Zn, Sn and Sb showed significantly higher urinary concentrations in controls than in the exposed subjects, while there were no differences in As, Cr, Cd, Ba, Hg and Pb between the two groups. Ni was within the LOD in the 60% of the subjects in the two groups. Multiple stepwise regression showed a dependence of Cr on age, of As on the number of days since the last meal of mollusks and crustaceans, of Zn on the consumption of mollusks, and of Cd and Pb on smoking expressed as the number of pack/year., Discussion: The research did not reveal higher urinary excretion of the metal elements in the steelworkers than in the subjects from the general population of Taranto resident at different distances from the factory, except for Mn, that was anyway within the reference range. This allows us to consider that there is no relevant health risk posed by occupational exposure to metal elements in the steelworkers, nor by environmental exposure to the same elements in the residents of the city of Taranto. In particular, no pollution of the living environment by metal elements of industrial origin was demonstrated, whereas other non occupational factors seem to condition the intake of these metal elements.

This cross-sectional study was aimed at evaluating in a large sample of male foundry workers the current exposure levels to carcinogenic compounds, including metallic elements [arsenic (As), beryllium (Be), cadmium (Cd), chromium (Cr), nickel (Ni)] and aromatic polycyclic hydrocarbons (PAH) by a biological monitoring approach, using validated biomarkers of exposure. Workers were recruited from 15 aluminium, copper alloy, electric steel and cast iron foundries and provided an end-of-shift urine sample to determine urinary concentrations of As, Be, Cd, Cr, Ni and 1-hydroxypyrene (1-OHP). Metallic elements were determined either by inductively coupled plasma mass spectrometry (Be, Cd and Cr) or by atomic absorption spectrometry (As, Ni), whereas 1-OHP was determined by high pressure liquid chromatography with fluorimetric detection. Most of the determinations fell within the laboratory's reference values. Age and lifestyle habits (smoking, alcohol, diet) played a significant interfering role.

The aim of this study was to evaluate the hypothesis of an increased exposure to metallic elements through the biological monitoring (urine and hair) in a group of children living near an electric steel plant and in a control group. The concentrations were also compared with the reference values of the literature. The study included the measure of porphyrins in urine (total and homologue), by some authors considered as effect indicators of xenobiotics such as heavy metals and some persistent organochlorine compounds (polychlorinated biphenyls, polychlorinated dibenzofurans, tetrachlorodibenzo-p-dioxin). The comparison between the groups and with the reference values shows that the metallic elements in the hair and urine and urinary porphyrins does not indicates significant differences thus excluding specific health risk. In both groups the values of some metallic elements exceeded the upper limit of reference values. Following a control carried out two months later values returned in reference range probable due to different food intake and individual behavior. As a whole our survey demonstrated that there is no exposure to metallic elements representing a significant risk for this particular groups of general population possibly exposed to environmental pollution.

Background and Objectives: The risk assessment for Upper Extremities Work Related Muscle skeletal Disorders (UEWMSDs) remains a complex and open question. For professionals involved in this analysis of fundamental importance appears the use of technical/good practice norms or the guide lines produced by scientific society or association, such as for our country the guide lines published on this theme by the Italian Society of Occupational Medicine and Industrial Hygiene (SIMLII). The objective of this research was to verify and validate the multiple step method suggested by above mentioned guide lines and to compare results obtained by six analysis methods adopted (Washington State Standard, OCRA, HAL, RULA, OREGE and STRAIN INDEX (SI))., Methods: 439 workstation in 17 manufactures employing 4166 workers for a total of 1396 analysis by different methods were considered, by adopting the following multiple step procedure: preliminary evaluation by Washington State method and OCRA checklist in all the working stations, RULA or HAL as first level evaluation, OREGE or SI as second level evaluation when complexity of work variables or contradictory results emerged. We have also collected data, provided by occupational physicians, about the pathologies of the upper limb. A representative sample of workers was also administered a INRS OREGE questionnaire that investigates the disorders of the upper limb, the subjective perception of risk, psychosocial factors and stress-related disturbs., Results: The preliminary evaluation resulted negative (risk absent) in the 87% of examined work stations and by using checklist OCRA optimal-acceptable condition was found in 57% (the percentage reaches more than 90% when classes of risk absent-very low-low risk classes of the method were considered all together), by HAL in 91% of analysis, by RULA in 90%, by OREGE in 58%; by SI in 67% of examined working position. Five methods have been contemporary used for 31 working position with the following results: 58% at different degree of risk by OCRA, 13% by HAL, 19% by RULA; 13% by OREGE; 10% by SI. Among the main components of the bio mechanical risk the high frequency of working task resulted at first place for 4/5 methods and for checklist OCRA also the evaluation of recovery times. Consistent with the findings from risk assessment, the majority of reports of occupational diseases by occupational physician is localized at the wrist and workers assigned to tasks at risk not always have had occupational diseases. The role of main psychosocial factors was demonstrated in about the 15-20% of working conditions and regarded in particular the working climate, the job maintenance perspective, the time pressure and errors possibility together with the difficulty of help by colleagues and managers and stress related disturbs., Discussion and Conclusion: The preliminary evaluation by State of Washington method appears to be an adequate instrument for identify the working condition at risk. The results of this method appears well correlated with OCRA checklist and HAL. The risk conditions were always confirmed by higher complexity methods (RULA, OREGE, SI). All the adopted methods were in a good agreement in two extreme situations: high risk or absent risk. In the degree of accordance varied on the basis of their rationale and of the role of their different components. A necessary integration of biomechanical analysis appears the evaluation of working conditions and musculoskeletal and psychological disorders collected directly from the workers and the results of health surveillance programs. The recommendations of SIMLII about the critical use of biomechanical methods and about the possible use of more than one of them considering the working characteristics have been confirmed.

Background: The aim of this study was to assess long-term results and adverse events in patients with ventricular tachycardia from different etiologies., Methods: The recurrence rate of tachycardia, consequent further therapies (other catheter ablation procedures, drug therapy and implantable defibrillator) and clinical events have been assessed in 60 consecutive patients undergoing ventricular tachycardia catheter ablation between January 2000 and December 2004., Results: During a median follow-up of 20 months (interquartile range 13-36 months), tachycardia recurred in 27 patients (45%) after a median of 3 months (interquartile range 1-12 months). A second procedure was performed in 11 patients; it was successful in 8 patients. Four patients underwent pharmacological therapy which was successful in all cases. Overall, after ablation (> or =1 procedures) and pharmacological therapy, tachycardia was cured in 75% of cases. All the 20 patients without structural heart disease were cured with ablation vs. 62% of patients with heart disease (p = 0.001). Patients with dilated cardiomyopathy reported worst results (33% success, p = 0.03). Recurrences were predicted by acute failure of procedure (p = 0.05), presence of heart disease (p = 0.006) and history of atrial arrhythmias (p = 0.02). On a multivariate analysis, only structural heart disease continued to be an independent predictor of ventricular tachycardia recurrence., Conclusions: Catheter ablation of ventricular tachycardia has a high percentage of recurrences in patients with heart disease, whereas is curative in subjects without structural heart disease.

Background: It is current opinion that concealed and manifest accessory pathways (APs) are indistinguishable with respect to their location and contribution to orthodromic reciprocating tachycardias. The aim of this study was to compare clinical and electrophysiological characteristics of two groups of patients., Methods: Between January 1999 and June 2000, 42 consecutive patients underwent radiofrequency catheter ablation for paroxysmal atrioventricular reciprocating tachycardia attributable to a concealed AP. Their clinical and electrophysiological characteristics were compared with a group of 48 consecutive patients with manifest AP and supraventricular tachyarrhythmias., Results: There were no differences regarding gender, the prevalence of heart disease and the age of onset of symptomatic tachycardias between the two groups. Compared to those with a manifest AP, the patients presenting with a tachyarrhythmia due to a concealed AP were older (48 +/- 15 vs 40 +/- 16 years, p < 0.05) and had a longer history of tachyarrhythmias (22 +/- 16 vs 13 +/- 13 years, p < 0.05). Atrial fibrillation was more frequent in patients with a manifest AP than in patients with a concealed AP (50 vs 9.5% respectively, p = 0.02). Atrioventricular reciprocating tachycardia was a cause of more hospitalizations (76 vs 35%, p = 0.01) and episodes of pre-syncope (47 vs 22%, p < 0.05) in the group of patients with a concealed AP. The anatomical site of concealed and manifest APs was significantly different: concealed APs were more frequently localized in the left side (93% left, 7% right), while manifest APs were seen in the left side in 64% of cases, in the right side in 29% and in the posteroseptal left + right region in 7% of cases. The retrograde electrophysiological properties and the inducibility of other types of reentrant arrhythmias were similar. Catheter ablation was similarly successful regardless of whether the AP was concealed or manifest, the rates of success being 91 and 88% respectively at the first attempt and with a similar number of energy applications (7 +/- 7 vs 10 +/- 9, p = NS). At a second attempt, the procedure was successful in 100 and 98% of cases respectively. Periprocedural complications occurred in 5% of patients with a concealed (1 ventricular fibrillation, 1 cerebral transient ischemic attack) and in 8% of patients with a manifest AP (2 pericardial effusion, 1 transient atrioventricular block, 1 anginal attack with spontaneous recovery) (p = NS). Complications occurred only for left-sided APs and were independent of the approach (transseptal or retrograde). Relapse of AP conduction was more frequent in the group of patients with a manifest than in those with a concealed AP (12 vs 5%), though not significantly. There were no late complications., Conclusions: Those patients presenting with a tachyarrhythmia due to a concealed AP, compared to those with a manifest AP, were older and had a longer history of tachyarrhythmia. Atrial fibrillation was more frequent in patients with manifest AP. Atrioventricular reciprocating tachycardia episodes were longer-lasting and caused more hospitalizations and more frequently pre-syncope in the group of patients with a concealed AP. Almost all concealed APs were localized in the left side. The retrograde electrophysiological properties were similar. The results of radiofrequency catheter ablation were comparable in both groups.

This study aimed determining the chronic effects of nifedipine (N) on left ventricular filling in 25 patients (mean age 55) with mild to moderate arterial hypertension. M-mode, B-mode and pulsed Doppler measurements were performed at baseline, after 30 min from administration of sublingual N (10 mg) and after 6 months of therapy with slow release N (max dose: 60 mg die). Acute and chronic N reduced significantly both systolic (p less than 0.001) and diastolic pressure (p less than 0.001). At the end of the treatment with slow release N, and septal wall thicknesses had a slight but significant decrease (p less than 0.01). Diastolic and systolic dimension were not modified. Left ventricular mass index decreased significantly from 141 +/- 34 to 130 +/- 31 g/m2 (p less than 0.05). The Doppler-derived diastolic filling indexes (peak E, ratio peak E/A,E deceleration) were abnormal at baseline, and had a significant increase after sublingual and chronic therapy. Changes in left ventricular mass index and diastolic filling indexes were not correlated. A significant correlation was found between peak E changes after acute and chronic N administration (r = 0.732 and p less than 0.001). The results of this study demonstrate that both acute and chronic administration of N modify transmitral flow velocity pattern, suggesting that, in hypertensive patients, left ventricular filling abnormalities are partly dynamic and reversible. Our findings also demonstrate that acute N effects may predict the chronic results.
