Abstract

In this paper, the Italian Society of Cardiac Rehabilitation and Prevention (GICR) presents the third survey on the status of cardiac rehabilitation (CR) in Italy. The Italian SurveY on carDiac rEhabilitation 2008 (ISYDE 2008) is a multicenter, observational study aimed at identifying the number and characteristics of Italian CR facilities, both in terms of health operators and interventions. Clinical records of all patients consecutively discharged within the whole network--composed of up to 200 CR units--from January 28 to February 10, 2008 will also be reviewed for diagnosis of admission, comorbidities, rehabilitation programs, and drug therapy, in order to obtain a snapshot of current implementation strategies in daily clinical practice. The survey will adopt a web-based methodology for data provision and transmission. Preliminary results of the survey are expected in the late summer 2008.

Abstract

Nocardia is found worldwide in soils and dust. In immunocompromised patients the lungs appear to be the most common initial site of infection due to the inhalation of free living organisms. Nocardia asteroides is the most commonly isolated pathogenic Nocardia species. Pulmonary nocardiosis may mimic tuberculosis, staphylococcal or mycotic infections. Disseminated disease mainly occurs in immunocompromised patients with underlying illnesses such as chronic granulomatous disease, human immunodeficiency virus (HIV) infection, in patients undergoing cytotoxic chemotherapy, organ transplantation or prolonged glucocorticoid treatment. Subcutaneous infection occurs from trauma related inoculation of the organism. The Authors describe a clinical case regarding a patient with acquired haemophilia A, admitted to the hospital because of disseminated nocardiosis mimicking a neoplastic disease. He recovered completely after the antibiotic treatment.

Abstract

Infections contracted during hemodialysis are an important aspect of the issue of healthcare-associated infections (HCAIs). The dialyzed kidney patient has a very high risk of infections, which may be life-threatening. Only the strict application of proper nursing standards, in addition to surveillance and good management of the patient, will allow to lower the incidence of infections. We studied the hemodialysis-related infection risk by evaluating the types of patients, medical devices, and nursing practices involved. Patient outcome can be improved by the implementation of correct management practices. The analysis of variables including standards of good practice, hand washing, and surveillance of colonized patients will promote the development of effective measures to reduce the incidence of infections, particularly antibiotic-resistant ones.

Abstract

The authors describe a clinical case regarding a young female patient affected by sepsis due to methicillin-resistant Staphylococcus aureus (MRSA), associated to meningoencephalitis and cerebral abscess. The patient had no contact with hospitals in the months prior to illness and had always been healthy. She recovered thanks to linezolid therapy. The MRSA strain proved positive for Panton-Valentine leukocidin (PVL positive) and was therefore defined as community-acquired MRSA (CA-MRSA).

Abstract

In a retrospective study concerning the epidemiology of extended-spectrum beta-lactamase (ESBL) positive Enterobacteriaceae during 2007-2008 in the wards of the Carlo Poma hospital in Mantova, Mercurio surveillance software was used to detect alert microorganisms. Our objective was to link the epidemiological data with the type of patient and ward, and to assess the risk factors for such infections in particular nosocomial environments. The study enabled the change in the relative epidemiological data to be detected, and showed that such bacteria can be found almost throughout the hospital.

Abstract

The epidemiological impact of blood-borne infections (HCV, HBV and HIV) in dialysis is a major concern. In the past two decades the implementation of specific recommendations for prevention has been associated with a sharp reduction of the HBV and HCV incidence in this setting. Moreover, in recent years new therapeutic agents against these viruses were introduced, resulting in a marked improvement in clinical outcomes in subjects with normal kidney function. A collaborative effort between nephrologists and infectious disease specialists should be made to extend this improvement to chronic kidney disease patients.

Abstract

We compare epidemiological data from two twelve-month periods, before and after the introduction of preoperative antibiotic prophylaxis guidelines at Carlo Poma hospital in Mantova, Italy, in June 2003. Considering the results from the microbiology laboratory and the data from the pharmacy, concerning the consumption of some antimicrobials, we noted a significant decrease in the incidence of methicillin-resistant Staphylococcus aureus (MRSA) in surgical wards, where the incidence of MRSA had previously exceeded that in medical wards. At the same time, analysis of antibiotic consumption revealed a considerable decrease in third and fourth-generation cephalosporins and an increasing use of cephazolin, in compliance with prophylaxis protocol rules. This trend was confirmed by analysis of the same data regarding the first six months of 2006.

Abstract

Background: In the C. Poma Hospital of Mantua we have been using a system of continuous surveillance of nosocomial infections based on microbiological data for the past 4 years. This monitoring estimates the incidence of the microorganisms found in cultures, especially those at risk of causing nosocomial infections., Materials and Methods: Since June 2001 microbiological data have been registered using the Mercurio-Dianoema software and elaborated by means of Microsoft Excel in order to obtain information about isolated bacteria, especially those resistant to antibiotics., Results: Surveillance in "critical" wards revealed the presence of Pseudomonas aeruginosa, Staphylococcus aureus and Candida albicans in the intensive care unit in the period 2003-2005. The most frequent bacteria in hemodialysis have been coagulase-negative Staphylococci and Staphylococcus aureus, with variable methicillin resistance., Conclusion: The analysis of microbiological data has promoted effective measures to reduce the incidence of these bacteria (increased rules of good practice, hand washing, etc.). If nosocomial infections or high-risk microorganisms occur, assessments are carried out; monitoring of the antibiotic resistance of the bacteria is very important.

Abstract

For the period 2002-2005 we verified and compared the data of the prevalence and resistance of Pseudomonas aeruginosa (PA) isolated in Mantova Hospital (Italy) with the data from the international database. From the first six-month period of 2004 a significant increase was found (9% vs 28.8%) in the prevalence of multi-drug resistant PA (MDR-PA). The principal wards involved were the Intensive Care Unit and the Department of Respiratory Diseases. A significant increase in resistance rates was observed for all antimicrobials tested, in particular for aztreonam, ceftazidime, ciprofloxacin, gentamycin and imipenem. The lowest dual resistance rates were observed between amikacina with piperacillin/tazobactam, while the highest were for those that included ciprofloxacin and beta-lactams (aztreonam, cefepime). In this study we confirm the importance of continuous surveillance of laboratory data and tightening local control measures for nosocomial infections in order to prevent the spread and selection of MDR-PA.

The Aa discuss the evidence and recommendations for the use of personal protective equipment by health care practitioners in general care settings; the use of aprons, gowns, gloves, eye protections, face masks is valutated on the basis of an assessment of the risk of transmission of microrganisms to the patient or from the patient to health care practitioners. The primary uses of personal protective equipment are to protect staff and reduce opportunities for transmission of microrganisms in hospitals. A trend to eliminate the unnecessary wearing of aprons, gowns and masks in general care settings has evolved over the past twenty years due to the absence of evidence that they are effective.

Abstract

The number of supraventricular tachyarrhythmias recently arisen among patients admitted to the Coronary Care Unit in Piombino, and the results of therapeutical interventions, have been estimated by analysing, retrospectively, the cases pertinent to the period February '84- October '87. Out of 1451 hospitalized patients, the supraventricular tachyarrhythmias were 145; 85 of them (46 F and 39 M, average age 63 +/- 14) with an assured cardiopathy and 60 of them (34 F and 26 M, average age 58 +/- 13) with no assured cardiopathy. The employed drugs, as a first choice, were verapamil, amiodarone or flecainide on the ground of study protocols in order to value their compared effectiveness. When amiodarone was not employed, as a first choice, in conformity with a specific protocol, it was utilized as a second choice after verapamil or flecainide. A DC shock was carried out in the case of pharmacological failure. Among the patients with an assured cardiopathy, the pharmacologically cardioversed ones turn out 55.3%; the spontaneously ones 9.4%; the ones treated by DC shock 14.1%; the non cardioversed ones 21.2%. Among the patients with no assured cardiopathy, the pharmacologically cardioversed ones turn out 78.3%; the spontaneously ones 5%; the treated by DC shock ones 11.7%; the non-cardioversed ones 5%. Therefore, the supraventricular tachyarrhythmias of recent onset make up nearly 10% of the hospitalizations into a Coronary Care Unit; the percentage of cardioversions in a sinusal rhythm with a pharmacological treatment reaches 95% in those patients with no assured cardiopathy.(ABSTRACT TRUNCATED AT 250 WORDS)
