Abstract

The post-operative analgesic therapy represents a target of quality and saving for a Hospital because it ensures a positive outcome for surgery-treated patients. From this point of view, it is important to use validated protocols in each specific clinical context and to pay attention to all the factors involved in the genesis of pain...In this study the analgesic benefit offered by different protocols of treatment of post-operative pain in 88 patients who underwent an operation of transurethral prostatectomy (TURP) has been evaluated. In addition to verifying the efficacy of different pharmacological therapies, we wanted to estimate the impact of providing the patients with detailed information before surgery on the following response to the analgesic therapy. Our present results demonstrate the validity of a therapeutic protocol based on the use of an opiate administered by continuous intravenous infusion and point out the importance of a multidisciplinary approach to the patient with post-operative pain.

Abstract

In this study we report the case of an acute form of ATL in a HTLV-I-infected Nigeria-born 27-year-old female prostitute living in Italy from February, 2001. The presence of HTLV-I infection was demonstrated by the detection of serum antibody to HTLV-I by immunoenzymatic assay and western blot analysis. In addition, the presence of HTLV-I proviral DNA was confirmed by a hemi-nested PCR in a sample of peripheral blood mononuclear cells. From an epidemiological point of view, it is important to report new cases of imported ATL, as it may explain the otherwise untraceable origin of some rare and apparently autochthonous cases of ATL in non-endemic areas.

Abstract

Adjuvant therapy represents the gold standard treatment for radically resected pancreatic cancer. Results from randomized clinical trials confirmed the efficacy of adjuvant therapy for pancreatic cancer but did not define what is the "right choice" in terms of type of antiblastic drug (among gemcitabine, 5-fluorouracil or other drugs), role of polychemotherapy and chemoradiotherapy. The objective of our study was to evaluate the efficacy and toxicity of adjuvant chemotherapy and chemoradiotherapy for radically resected pancreatic cancer through a systematic review of literature data, emphasizing the benefits regarding overall survival, disease-free survival and toxicity.

Abstract

In a 60 years old patient affected by primary gastric MALT lymphoma stage IE, we observed a complete regression after aspecific treatment with ranitidine 150 mg x 2/day; after regression we treated the patient with anti-Helicobacter pylori therapy for 30 days. Nowadays, after 3 years of clinical and endoscopical follow-up we observe no relapse of disease. The elimination of antigenic stimulation of Helicobacter pylori, stopping T-lymphocytes' effect on B-lymphocytes, stopped neoplastic proliferation, causing the complete regression of neoplastic lesion. This clinical case, according to biological data about primary gastric lymphomas, suggests different ways of treatment for primary gastric lymphomas at the lowest stage of disease.
