The cardiovascular disease is largely increased in chronic renal failure and the patients have a 10-20 times higher mortality respect normal population. Besides habitual risk-factors they add the mineral metabolism alterations, iperomocisteine and chronical vessel flogosis. In these patients the vascular disease is often lately diagnosed, but early diagnosis would be extremely important to establish appropriate pharmacologic or surgical treatment (PTA or by pass). The basic diagnostic methods are still digital angiography, angio-NMR or angio-CT. In our experience appears that dialysed patients present high total mortality and re-vascolarization (particularly for peripheral occlusive disease) gives less guarantee of success. During last years endovascular surgery procedures extremely improved short-term prognosis for these patients. When there is no space for the re-vascolarization and the situation is strongly compromised by the presence of extended gangrene or infected lesion, amputation is still indicated and can be considered the only possible solution.

Critical limb ischemia secondary to chronic peripheral occlusive disease is common in chronically dialysed patients, with an incidence rate of 25-30%. Atherosclerotic lesions are more frequent in the infrainguinal arteries and long infrapopliteal occlusions often occur. Due to diabetes, hypertension and ischemic cardiopathy, the surgical prognosis is very poor in these patients; medical treatment should always be attempted associated with analgesia, without an excessive delay in surgical therapy if needed. Both spinal stimulation and lumbar simpaticectomy often fail; open and endovascular surgery are the best options before major amputation, which has a high incidence in this patient subgroup. Between 2000 and 2003, 23 chronically dialysed patients underwent surgery. Nine open and 13 endovascular procedures were performed, associated with four immediate and five late minor amputations. Despite an immediate mortality rate of 8.6%, we obtained immediate patency and limb salvage in all cases. In a medium follow-up of 25 months (range 3-36), five thromboses were found in subinguinal procedures; not one in iliac procedures. The five patients underwent major amputation. Another two patients underwent amputation despite arterious patency. Seven patients died due to cardiovascular diseases during the follow-up. Our experience confirms that the association between POAD and dialysis is a prediction factor for medium-term death and that the surgical risk is highly increased. It is important to select patients undergoing surgical treatment to check for the lowest invasivity.

Purpose: Spontaneous dissection of the carotid and vertebral arteries represents a rare pathology. Its pathogenesis, probably multi-factorial, can be related to neck trauma and to a genetic basis, extensively demonstrated in the cases of association with type IV fibromuscular dysplasia, Ehlers-Danlos syndrome, Marfan's syndrome, pseudoxanthoma elasticum and osteogenesis imperfecta. Onset symptoms include neck, facial pain and diffuse headache. Cranial nerve palsy has been also described. The aim of this study was to evaluate MR angiography and MR findings in a population of patients with carotid and vertebral artery dissections., Materials and Methods: We reviewed the MR angiography and MR exams of 15 patients (10 males, 5 females, average age 42.0, age range 30-55) with carotid and/or vertebral artery dissection; the serial MR follow-up studies were also examined., Results: Twelve internal carotid arteries (ICA) showed the presence of an irregular stenosis; three of these showed complete occlusion. In two patients both ICA were involved. Six vertebral arteries (VA) showed irregular stenosis (only one patient suffered from both CI and VA dissection); in one case there was also a pseudoaneurysm of the VA. Follow-up studies showed vessel lumen re-opening in 5/11 ICA (one patient underwent vascular stent positioning) and in 3/6 VA., Conclusions: The present study confirms the usefulness of MR angiography and MR in the diagnosis and follow-up of patients with carotid and vertebral artery dissection.

Introduction: Cardiovascular disease remains the leading cause of death in the world. Invasive coronary angiography is currently the only clinical method available to visualize the coronary arteries, with up to 20% of the procedures demonstrating no evidence of severe coronary artery stenoses. We investigated the role of two-dimensional (2D) coronary magnetic resonance angiography (MRA) in patients with suspected coronary arteries disease and to check the placement and the patency of previously placed coronary artery stents., Material and Methods: Eleven patients with suspected coronary artery disease who underwent elective cardiac catheterization with coronary angiography were examined with 2D coronary MRA to detect coronary artery stenoses. Other 11 patients with 13 stented coronary arteries (6 RCA, 5 LAD, 2 Lcx) were prospectively examined with MRA one day to 8 months after stent placement. Eighteen amagnetic stents were imaged. Imaging was performed with a 1.5 T MR unit (GE Signa Horizon Echo Speed) with a phased array multicoil. Segmented k-space fast GE sequences were acquired with and without fat suppression at several cardiac cycle phases within a single breath-hold. Correlation with coronary angiography was performed in all patients., Results: Thirteen significant stenoses were found at coronary angiography in 11 coronary arteries. The sensitivity and specificity of MR coronary angiography, as compared with conventional angiography, in correctly identifying the single vessels with > 50% angiographic stenoses were 73% and 94%, respectively. The corresponding positive and negative predictive values and accuracy were 88%; 84% and 86%, respectively. As far as the study of coronary artery stents is concerned, no MR-related adverse events were observed. The stents were visualized as signal loss areas. The length of the signal loss corresponded to the length of the stents in all 18 cases (r = 97). The patent blood flow distal to the stents appeared as a high-signal band distal to the signal void, corresponding to stent patency at coronary angiography., Conclusions: Although in an early stage of technical development, 2D coronary MRA can depict 73% of hemodynamically severe coronary artery stenoses. Moreover breath-hold coronary cine MRA is a safe technique to visualize coronary artery stents. Stent site and patency can be noninvasively studied with this technique.

Lyme borreliosis is a systemic disease. The etiologic agent is a tick-bite borne spirochete (Borrelia burgdorferi) of which men are casual hosts. Although the illness is reported worldwide, it is more frequent in the northern parts of Countries. Some isolated clinical pictures are reported in the past, but only in 1975 the illness was well described at Old Lyme (Connecticut, Usa) and then well defined in its pathogenesis and clinical course in three stages. Lyme disease, involving skin, heart, nervous system and joints, is frequently protean and misdiagnosed, so that it was called "the new great imitator". In Italy the first case is reported in 1985. The authors report three (3) new cases, mostly with neuro-articular signs and symptoms. Literature is partially reviewed, for a better definition and outline of some epidemiological, etiopathogenetic, historical and clinico-therapeutic aspects of Lyme disease.

The authors report a rare association: Hashimoto's thyroiditis, thrombocytopenic idiopathic purpura (Werlhof's disease) and Steinert's myotonic dystrophia. The authors outline the fingerprints of each disease, report particular syndromes and compare that association to data of partially reviewed literature. They suppose a suggestive pathogenetic connection among these diseases, due to the coexistence of altered genetic, endocrine and autoimmune factors and consequently impaired cell membrane permeability.

The paper reports the Authors' ten year experience on simple ligation in appendicectomy. After having reviewed the various surgical procedures, the Authors report their clinical experience and, considering the good postoperative results with this method, regard this procedure as an alternative to the technique of ligation with inversion of the stump in appendicectomy.

Two cases of acute caecal diverticulitis are reported. The Authors describe the clinical aspects, diagnostic procedures, histological pictures and surgical treatment. Particular emphasis is also placed on problem of a correct preoperative diagnosis.

The authors report on a case of solitary ulcer of the large bowel associated with mucosal melanosis, stressing the peculiar nature of this rare pathology and its differentiation from solitary ulcer of the rectum. The pathogenesis of the disease is still unknown and histological data from biopsies performed at various time intervals in chronic cases show, in this case as in other cases of solitary ulcer of the rectum, that histological findings vary over time; in the case reported on here, histology revealed a transition from a chronic inflammatory situation to one of mucosal melanosis after three months. Diagnosis is reached by exclusion and is based on colonoscopy plus biopsy. Useful techniques in complicated cases are selective angiography (for haemorrhage) or CAT (for perforations, abscesses). Treatment is conservative in uncomplicated cases, with suitable follow-up in the form of check colonoscopies and targeted biopsies. Surgical treatment is confined to those cases--by no means infrequent--complicated by haemorrhage, perforation and abscesses.
