Laparoscopic technique in elective cholecystectomy is the last step in an evolutive time to minimize the abdominal access. From 1st January 2004 to 31th December 2006 we analyzed 5515 cholecystectomy procedures: 4877 laparoscopic cholecystectomy, 635 open cholecystectomy. Complications and supplementary diagnosis have been identified in SDO Lombardia's country database. Morbidity occurred in 82 patients (12.9%) with open technique and 109 patients (2.23%) with laparoscopic technique; mortality occurred in 11 patients (1.73%) with open technique and 1 patient (0.02%) with laparoscopic technique. Mean hospital stay are 14.40 days with open technique and 4.75 with laparoscopic technique. Morbidity in open technique is 6 fold more than laparoscopia technique. The difference between the two technique is present in literature and it's the result of non invasive technique compared with the incision of the laparoscopia technique. This is the critical point in the difference of hospital stay between the two technique all to the good of laparoscopy. The high mortality ratio is due to the selective criteria in laparoscopic technique. First remark is the high quality of our hospital care, compared with hospital teaching in the word. In this hospital the laparoscopic cholecystectomy is the gold standard in cholelitiasis treatment. The second remark is the limit of the open technique in severe cholelitiasis with evidence in high ratio of hospital stay, morbidity and mortality.

Biliary lithiasis is a disease with a high incidence in the western world and a high social cost. To evaluate the impact of new technologies--mainly laparoscopic cholecystectomy and radiological and endoscopic techniques--in the treatment of biliary lithiasis, a statistical study was conducted on the surgical interventions performed over the past 15 years in Italy in the Lazio and Abruzzo regions. From 1985 to 1999, 150,000 cases of hepatobiliary lithiasis were hospitalized every year in Italy with a mean hospital stay of 7.8 days (13,000 in Lazio and 6,000 in Abruzzo). 100,000 cholecystectomies were performed every year in Italy in public hospitals, 30% of which by conventional laparotomy and 70% laparoscopically. Ten percent of these operations on average were performed in the Lazio region and 3% in the Abruzzo region. 10,615 cases of main bile duct stones were treated in 1999, 6,502 of which by ERCP and the others with open procedures and radiologically. The mortality was 0-0.1% for laparoscopic cholecystectomy as against 0-0.5% in open procedures.

Background: The aim of the study was to demonstrate the importance of early laparoscopic cholecystectomy for acute cholecystitis., Methods: From 1998 to 2000, 66 patients were submitted to laparoscopic cholecystectomy. All patients were submitted to US scans preoperatively and operated on by surgeon skilled in emergency laparoscopic operative technique., Results: Only one patient (1.5%) had conversion to open cholecystectomy. There was no mortality and no bile duct or major vascular injuries. The overall operative morbidity rate was 3%. The mean postoperative hospital stay was 3.1 days., Conclusions: Author's experience and results support the validity of early laparoscopic cholecystectomy in the treatment of acute cholecystitis, since it reduces the postoperative length of hospital stay and hospital costs. Early treatment is always helpful for inflamed and oedematous tissue which favours dissection.

Background: The many advantages and extreme versatility made laparoscopic cholecystectomy (L.C.) the gold standard for symptomatic cholelithiasis. The aim of this research is a retrospective analysis of personal experience with laparoscopic cholecystectomy in a peripheral hospital compared with the literature on the subject., Methods: Since June 1982, a total of 989 L.C. have been performed. The patients were: 691 (68.6%) women and 298 (29.4%) men with a mean age of 56 years (range 17-84). The indications were: 791 symptomatic cholelithiasis, 142 chronic cholecystitis, 45 empyema-hydrops and 11 adenomyomatosis., Results: No postoperative death have been observed and the conversion rate was of 24 cases (2.3%). The main complications were 3 cases of injury of the biliary tract and 2 cases of postoperative bleeding (1 from cystic artery and 1 from the umbilical wound). Minor complications observed were 12 cases (1.2%) of infections of the umbilical wound and 3 cases of umbilical hernia (0.3%). The elevation of stasis index was observed in 3 cases which solved spontaneously. The length of stay was 2 days in 957 cases (97%), 8 days in 24 cases and 4-5 days in 8 (0.7%)., Conclusions: No major trochar's lesions occurred, contrary to the percentages quoted in the literature (0.02%-0.9%). The selective use of the open technique and of the multiuse conic section trocar in the "closed" technique is suggested. The 3 cases of bile duct lesions did not occur during the training period, contrary to what quoted in the literature. In personal opinion, a careful surgical technique with a good Calot's triangle preparation, is necessary to prevent these inconveniences. Parietal complications (umbilical wound infections and laparocele) even if lesser than in the laparotomy technique, can be reduced by using the endobag and suturing the abdominal fascia of the 10 mm trocars. Moreover, the use of a systematic subhepatic drainage during the first 24 postoperative hours is suggested, since it can be useful to reveal possible bleeding.

Background: To assess which factors determined conversion to laparotomy in patients undergoing laparoscopic elective cholecystectomy., Setting: department of General Surgery. University of Genoa. Italy., Methods: Two hundred sixty-four consecutive laparoscopic cholecystectomies were performed in our Department., Interventions: laparoscopic cholecystectomy was performed according to Dubois's technique. Duration of the procedure was not considered a reason for conversion., Results: 121 patients showed "difficult intraoperative situations" with further conversion risk factor. Conversion to laparotomy was necessary in 11 patients (4.16%). Five patients underwent conversion in the first 50 cases (10%), while six in the last 214 (2.8%). We had to convert to open cholecystectomy only in eleven patients, despite the high rate of technical difficulties and anatomic anomalies even in cases which, in the past, represented a contraindication to this kind of technique. The use of new instruments and new surgical techniques has reduced to only factors of increased risk in those situations that in the past were considered as contraindications to laparoscopic cholecystectomy., Conclusions: Conversion to open cholecystectomy is based on the surgeon's decision and the safety should be the main consideration in performing laparoscopic cholecystectomy. The use of a careful dissection could avoid the conversion in many patients.

Aim: Considering that laparoscopic procedure is associated with increased resistance to lower-limb venous return and subsequent stasis, with possible implications in terms of thromboembolic complications, the aim of our study was to investigate prospectively the coagulative-fibrinolytic profile, in laparoscopic and open cholecystectomy, in patients randomly alloted to receive or not preoperative heparin., Methods: We prospectively analyzed 36 patients (20 laparoscopic and 16 open) and we randomly divided the patients in two groups: Group-A (28 patients--16 laparoscopic and 12 open) didn't take any preoperative thromboprophylaxis, Group-B (8 patients--4 laparoscopic and 4 open) took preoperative subcutaneous heparin. We took blood venous samples before surgery, at time 0 and + 30 min., at the end and 1 and 24 hours postoperatively. The following parameters were assessed: prothrombin time, partial thromboplastin time, fibrinogen and D-dimer. We statistically analyzed the differences by ANOVA test., Results: In Group A, fibrinogen and D-dimer were significantly higher (p < 0.0001 and p = 0.0266) in open group as compared with laparoscopic one and we observed significant time-depending changes of fibrinogen's concentration (p = 0.0168). In Group B we obtained a higher fibrinogen's value in laparoscopic group than in conventional one, with a significant difference (p = 0.0283); also, the sampling-time affected the result in a very significant meaning (p = 0.0041). Comparing fibrinogen levels between Groups A and B, we observed lower values in heparin-treated group than in the other one (p < 0.0001), while in laparoscopic surgery there was not a significant difference between two groups of treatment., Conclusions: Our preliminary data suggest that, perioperatively (besides a smaller laparoscopic acute-phase response) the coagulative-fibrinolytic changes are lower in laparoscopic cholecystectomy than in open one and heparin treatment significantly reduces these changes in open surgery but doesn't seem to affect laparoscopic group. Our results seem to show another possible advantage of the laparoscopic surgical procedures over the traditional ones.

The authors report their experience with 850 consecutive laparoscopic cholecystectomies performed from May 1990 to July 1997. In this series, despite a sporadic use of intraoperative cholangiography (IOC), there were no lesions of main biliary tract and there was only a biliary leaking cystic duct resolved with conservative treatment. The authors are satisfied with selective use of endoscopic cholangiography and sphincterotomy for the management of choledocholithiasis, and conclude that the use of IOC is not so essential to avoid iatrogenic lesions of main biliary tract, that could be avoid by applying a meticulous operative technique.

March 1991 through October 1992, in the Clinica Chirurgica II of the Bologna University, 59 patients were submitted to laparoscopic cholecystectomy; the age range was 25 to 76 years and the mean 50 years. In no patient stones bigger than 35 mm were observed and 31% of the subjects were treated with litholysis before surgery. Fifty-eight patients were affected with single or multiple cholelithiasis, 1 had adenomyomatosis and 4 patients had associated choledocholithiasis treated with preoperative ERCP. Both US and cholangiography were performed to detect absolute contraindications--e.g., acute cholecystitis, cholangitis, peritonitis and cirrhosis--or relative contraindications--e.g., choledocholithiasis, > 5 mm stones and short cystic duct. US proved to be more sensitive than cholangiography to assess the number of stones and gallbladder wall thickness and to diagnose acute cholecystitis or scleroatrophic gallbladder, but it appeared to be less reliable in case of choledocholithiasis, where cholangiography was the technique of choice, and in possible anatomical variations--e.g., short cystic duct--which must be detected before laparoscopic cholecystectomy. Cholangiography appeared to be rather inadequate to study cholelithiasis when associated with functional gallbladder exclusion (as it happened in 17% of our patients). Intraoperative cholangiography was performed on 2 patients only, because their obesity hindered the preoperative study. In conclusion, the need is stressed of combining US and cholangiography for the accurate preoperative evaluation of gallbladder stones patients.

The paper analyses the authors' initial experience of video laparoscopic cholecystectomy, outing the difficulties and incidence of morbidity in the preliminary group of 38 patients. The details of complications and their pathogenesis are also reported.

The videocelioscopic technique is becoming increasingly widely used in the treatment of cholelithiasis. In view of the results obtained, the authors underline the advantages of this method in geriatrics.

The management of calculous disease of the gallbladder has undergone significant change during the last decade. Recent years have seen the development of alternative methods for the treatment of biliary stones including dissolution therapy, endoscopic and percutaneous extraction, extracorporeal shock wave lithotripsy. Yet, despite these technologic advances, most surgeons have continued to consider cholecystectomy the safest, most effective and the only curative procedure. Open cholecystectomy, performed in the same fashion for more than a century has demonstrated low morbidity and minimal mortality rates. However, the operation does incur sizable expense in terms of hospitalization, cosmetic appearance and time lost from work. Laparoscopic cholecystectomy quickly emerged as an alternative to open cholecystectomy. The authors report their experience of laparoscopic cholecystectomy performed at the Surgical Department of Montecchio Maggiore Hospital (VI). Twenty-eight patients were operated on: in all but one laparoscopic cholecystectomy was successfully completed; in one case a fistula between gallbladder and the common bile duct led to T-tube drainage insertion via a laparotomy. In one patient laparoscopic cholecystectomy was performed after an endoscopic retrograde cholangio-sphincterotomy for associated stones of the common bile duct. Mean operative time was 69 minutes (range 46-210). Morbidity and mortality were 0%; maximal hospital stay was 48 hours. These data confirm that laparoscopic cholecystectomy may be considered the treatment of choice for stone disease of the gallbladder and it should enter into the cultural and technical background of a general surgeon.

The introduction of laparoscopic cholecystectomy (LC) has aroused enthusiasm on the part of the mass media and perplexity on the part of many medical workers. In an attempt to reconcile the two points of view, the video-laparoscopic activity in a surgical ambient was audited over 19 months. 450 LC were carried out during this period. The number of cholecystectomies performed by this method has risen significantly in the recent past thanks to the high esteem in which it is held by the public in general and by gastroenterologists in particular. The average age of patients is lower since young people accept more readily a type of surgery involving a lesser degree of traumatism, a relatively short convalescence and less aesthetic damage. After a brief initial period of scepticism, the method has also been received favourably by anaesthetist, and by operating theatre and ward nursing staff. Morbidity and mortality rates are not different from those noted after traditional cholecystectomy, which confirms the method as being safe and thus meriting the success it has widely achieved.
