Abstract

Una corretta nutrizione costituisce un fattore imprescindibile per il raggiungimento dello stato di salute L’alterazione dell’equilibrio nutrizionale, nel senso di un eccesso o un difetto, conduce infatti inevitabilmente a uno stato di malnutrizione con conseguenze anche molto gravi sullo stato di salute stesso. In questo senso, la dietetica svolge un compito indispensabile soprattutto attraverso l’educazione nutrizionale, un intervento che, oggi, dovrebbe essere necessariamente rivolto ad una fascia della popolazione In un’ottica di prevenzione, diviene infatti fondamentale che gli stili di vita alimentari diffusi nella nostra società vengano attentamente rivalutati e sovvertiti E per fare ciò il ruolo di medici e esperti divien cruciale: al fine di ridurre morbilità e mortalità legate alla scorretta alimentazione. È indispensabile che tali figure offrano alla popolazione gli strumenti adeguati per aumentare la propria consapevolezza e per poter riconoscere iniziali stati di squilibrio In tal modo si potrà infatti creare una rete di intervento articolato ed efficiente della quale faranno parte non solo le figure professionali ma anche e soprattutto il cittadino che finalmente collaborerà attivamente del proprio stato di salute e benessere.

Abstract

Cushing's syndrome is presently divided into ACTH-dependent and ACTH-independent forms. Eighty percent of the former is caused by an ACTH-secreting pituitary adenoma (Cushing's disease), presently considered of monoclonal origin. Fifteen percent of the ACTH-dependent forms are due to ectopic secretion of ACTH and/or CRH by tumors. The ACTH-independent forms are due to adrenal tumors or are iatrogenic. A peculiar form of Cushing's syndrome is the nodular adrenal hyperplasia which may be ACTH-dependent or independent according to the stage of the disease. Definitely rare is the micronodular adrenal dysplasia, a form of ACTH-independent hypercortisolism which is part of a multi-tumoral syndrome (Carney's complex) and is attributed to circulating antibodies which bind to the ACTH receptors promoting adrenal growth and steroidogenesis. The diagnosis of Cushing's syndrome is firstly directed to ascertain the existence of hypercortisolism, then to establish its cause. However, the functional exploration of the hypothalamic-pituitary-adrenal axis on which the laboratory diagnosis is founded may provide misleading results in major depression, chronic ethylism, liver or renal failure, polycystic ovary syndrome as well as during therapy with antiepileptic drugs, rifampicin and estrogens. Furthermore, Cushing's syndrome may have an intermittent or cyclic pattern. Defining the cause of Cushing's syndrome may also be difficult due to the overlapping of clinical and laboratory findings in pituitary and ectopic ACTH hypersecretion.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract

Twenty hyperthyroid patients were investigated for growth hormone (GH) and immunoreactive insulin (IRI) secretion in response to insulin hypoglycaemia, arginine infusion and glucose-induced hyperglycaemia. GH response to either insulin hypoglycaemia or arginine infusion was significantly reduced in these patients compared with 20 normal subjects. Thyrotoxic patients also displayed an abnormal GH pattern after a 100 g oral glucose load: in fact, serum GH underwent a paradoxical increase in spite of abnormally high levels attained by blood glucose. IRI secretion was also clearly reduced in response to arginine infusion and moderately blunted after oral glucose. In a group of patients re-evaluated under euthyroid conditions, a fair increase of GH response to the provocative stimuli jointly with the restoration of a normal suppressibility of serum GH by glucose were noted; by contrast, no significant change of IRI response to arginine or glucose took place. Likewise, the impairment of glucose tolerance was not improved. These findings indicate that an impairment of GH and IRI secretion is present in hyperthyroidism. The possibility that a potentiation of the catecholamine effects caused by the thyroid hormones is involved in this alteration deserves consideration.
