La perizia di stima da sempre ha quale obiettivo prioritario quello di informare il committente del valore di un bene. Rispetto al passato, questo elaborato oggi deve conformarsi agli standard della due diligence. In particolare, è richiesto che esso fornisca informazioni aggiuntive circa i caratteri del bene in stima e degli altri aspetti ad esso correlati. Inoltre, l’estensore è sollecitato ad avere la consapevolezza che la perizia deve comunicare a terzi, che non hanno competenze specifiche, aspetti sostanziali e tecnici. Il presente studio è stato sviluppato in riferimento alla perizia di determinazione del valore del soprassuolo da abbattere. Dopo aver brevemente introdotto gli aspetti teorici di base, è stato effettuata l’analisi di un campione di 22 perizie. Tre sono gli aspetti principali sotto esame: gli aspetti formali, il processo estimativo e la capacità di comunicazione dell’elaborato. Dall’analisi sono emersi molti aspetti critici, alcuni dei quali possono essere spiegati dall’istituzionalizzazione del processo del valore di macchiatico da parte del Corpo Forestale dello Stato, oggi non più operativo. Nelle conclusioni si evidenziano i punti critici delle perizie esaminate e vengono proposti alcuni suggerimenti per l’estimatore forestale al fine di produrre un report coerente con gli standard propri della due diligence. Inoltre, si propone l’uso delle matrici utilizzate nel presente studio come strumenti per la validazione di parte seconda della perizia.

Home dialysis, and mainly peritoneal dialysis, is indicated as the optimal choice as far as the comfort and lifestyle of uremic patients is concerned. Despite this, home treatments show a lack of growth. The reasons are mainly linked to the patients’ cognitive, psychosocial, familiar and physical barriers due to aging and morbidity. To overcome these barriers, we analyzed all the available institutional aids: civil disability, not-self-sufficiency funds, home, social and nursing assistance, expenses refunds. The assessment of the patients’ needs is performed through validated instruments such as multidimensional evaluation (VMD) and equivalent economic index (ISEE). Overall, economic relief is limited to low income patients, and those in serious distress. Some Italian regions have issued specific measures dedicated to home dialysis. Our review shows a great heterogeneity of measures, centered in some cases on economic aids and on home assistance in others. Moreover, some Italian dialysis centers directly provide caregivers for home dialysis. The international literature describes many experiences relating to home dialysis assistance. Their common message is that, in developed countries, economic help is generally sustainable despite the heterogeneity of health care systems. Home support and economic aids for dialysis, in fact, are made possible by the overall savings enabled by home treatments and by the careful redistributions of the funds., (Copyright by Società Italiana di Nefrologia SIN, Rome, Italy.)

Primary percutaneous coronary intervention is the standard treatment in patients with ST-segment elevation myocardial infarction achieving a TIMI 3 flow in more than 90% of patients. However, despite a brisk epicardial coronary flow in the infarct-related artery, frequently post-ischemic microvascular damage limits the efficacy of primary PCI. Recent studies suggest that thrombectomy during primary PCI, in patients with intracoronary angiographically visible thrombus, represents a useful adjunct to pharmacotherapy able to prevent distal embolization.

Lactose breath test have been performed in healthy newborns, in whom breath hydrogen excretion was preliminarily demonstrated by lactulose breath test. 27.5% of newborns showed positive lactose breath test, with no differences related to sex and gestational age, while a higher percentage of positive test has been found in < 2500 g and formula-fed neonates compared, respectively, to > or = 2500 g and breast-fed ones. In healthy newborns, without any clinical signs of lactose intolerance, it is possible to demonstrate hydrogen in expired air; this is not equivalent of lactose malabsorption, but, instead, depends on the phenomenon of colonic fermentation that allows energy and nutrient absorption.

The Italian Atlas of mortality at municipality level is the result of a research project coordinated by the Emilia-Romagna region and supported by the Health Ministry. To the realisation of this project have collaborated the Institute of Medical Statistics of Milano and the CILEA under the supervision of Prof. Cesare Cislaghi. Compared to previous equivalent products, this Atlas contains a number of methodological and content innovations. This was a consequence of the development of new statistical methods and the need of achieving different aims. First of all, the Atlas was aimed at describing the mortality at small area level, which is obtained via Kernel estimates; secondly, the objective was to identify suspect clusters of deaths which may suggest the existence of high risk areas. The Mortality Atlas is formed by 31 tables, one for each of the analysed cause of death; each table has a circular shape of 100 hundred kilometers radius and contains a variable number of municipalities; each municipality may be present in more than one circle. The Atlas is available on magnetic support and for each cause of death are provided several statistical analyses and indicators included in different files. One of these files can be directly used to build high quality maps using the graphical package MAPINFO.

The Authors make an analysis of the complex interactions occurring between peripheral and central factors in the regulation of anginal pain. As for the role of the endogenous opioid system, results obtained in a study using a particular methodological approach have demonstrated that patients with asymptomatic ischemic heart disease have a higher endogenous opioid tone at the level of the central nervous system.

Although heart rate variability is a very important prognostic factor in patients with myocardial infarction, the mechanism for the reduction in the vagal cardiac activity is unknown: myocardial infarction can cause local areas of sympathetic or parasympathetic denervation that leads to catecholamine hypersensitivity; the destruction of local ventricular receptors of the autonomic system may alter feedback to the higher centres, impairing autonomic regulation; the mechanical distortion consequent to the infarction can give rise to sympathetic overactivity. Effective thrombolysis in myocardial infarction could reduce myocardial necrosis and, therefore, cause less reduction of vagal activity and/or less sympathetic stimulation. To determine whether systemic thrombolysis has some effect on heart rate variability we studied 40 patients with a first transmural myocardial infarction. Enrollment criteria were very strict: all the patients with conditions potentially influencing heart rate variability were excluded from this study. Between day 15 and day 25 from the infarction patients had an Holter monitoring. A program we developed provided for the automatic identification of R wave, for the calculation of RR interval, and for the recognition of pause, extrasystolic and post-extrasystolic beats, that could be thus excluded from the analysis. Patients were divided in 2 groups: 17 patients treated with thrombolysis and 23 subjects treated traditionally. There were no difference between the 2 groups with regard to the age and cardiac function, except for the less echo score in the fibrinolytic treated group.(ABSTRACT TRUNCATED AT 250 WORDS)

Smoking habit has been evaluated in 108 patients with gastric or duodenal ulcer diagnosed by endoscopy and in a control group of 147 endoscopically normal subjects. We found a positive association for both the gastric and duodenal ulcer (relative risk of 3.5). Furthermore, an increasing risk was associated both with the duration of smoking and with the amount of cigarettes smoked daily. Conversely, no significant association was demonstrated between peptic ulcer and the more os less precocious age at starting smoking or with the nicotine and tar content of the cigarettes usually smoked. Among ex-smokers, an increasing risk (relative risk of 2.0) was observed for gastric ulcer only.

Isoelectrofocusing on polyacrilamide gel in the analysis of cerebrospinal fluid proteins is particularly suitable for investigations of gamma-globuline changes in the course of various neurological conditions. This technique makes it possible to recognise three patterns besides the normal one. The first is characterized by a diffuse increase in gamma-globulines with a great number of high density fractions showing uniform distribution in the alkaline pH area. It was observed in a case of Guillain-Barrè syndrome and in two cases of intramedullary tumours. It seems to be due to an alteration in the permeability of the blood-brain barrier. The second pattern shows a diffuse increase in the number of fractions and appears uneven owing to a certain amount of higher density bands which stand out clearly. This was observed in one case of tubercolous meningo-encephalytis, in one case of chickenpox encephalytis and one case of cryptococcal meningitis in a patient affected by lymphogranuloma. This pattern is probably due to an alteration of blood-brain as well as of immunological systems. The third pattern presents a clear fractionation of gamma-globulins gathered in a certain number of highly coloured bands, mainly in the most alkaline area of gel. This was observed in all the ten cases of multiple sclerosis and in the only case of subacute sclerosing panencephalitis. This may be attributed to the intrathecal production of oligoclonal gamma-globulines (Ig.G.).
