The Position Paper (PP) on asbestos of the Italian Society of Occupational Medicine (SIML) aims at providing a tool to the occupational physician to address current diagnostic criteria and results of epidemiological studies, and their consequences in terms of preventive and evaluation actions for insurance, compensation and litigation. The PP was based on an extensive review of the scientific literature and was compiled by a Working Group comprising researchers who have contributed to the international literature on asbestos-related diseases, as well as occupational physicians with extensive experience in the evaluation of risks and the medical surveillance of workers currently and formerly exposed to asbestos. The PP was drafted and reviewed between 2017 and 2018; its final version was prepared according to the guidelines of AGREE Reporting Checklist. All the members of the Working Group subscribed to the document, which was eventually approved by SIML’s Executive Committee. The first section addresses industrial hygiene issues, such as methods for environmental monitoring, advantages and limitations of different microscopy techniques, the potential role of microfibers and approaches for retrospective assessment of exposure, in particular in epidemiological studies. The second section reviews the biological effects of asbestos with particular attention to the diagnostic aspects of asbestosis, pleural changes, mesothelioma and lung cancer. In the following section the criteria of causal attribution are discussed, together with different hypotheses on the form of the risk functions, with a comparison of the opinions prevalent in the literature. In particular, the models of the risk function for mesothelioma were examined, in the light of the hypothesis of an acceleration or anticipation of the events in relation to the dose. The last section discusses topics of immediate relevance for the occupational physician, such as health surveillance of former exposed and of workers currently exposed in remediation activities.

Il Position Paper SIML dedicato all’asbesto si propone di offrire uno strumento che consenta al Medico Competente un rapido orientamento sui criteri diagnostici attuali e sui risultati della ricerca epidemiologica con le conseguenti indicazioni per l’azione preventiva e di valutazione in contesti previdenziali, assicurativi e medico-legali. Par- tendo da una ampia revisione della letteratura scientifica in argomento e, nel corso delle riunioni del Gruppo di lavoro, costituito sia da ricercatori che hanno fornito un apporto di livello internazionale sul tema delle patologie asbesto-correlate sia da Medici del Lavoro con ampia esperienza nella valutazione del rischio e nella sorveglianza sanitaria di esposti ed ex-esposti ad amianto, sono stati discussi tra il 2017 ed il 2018 i diversi argomenti trattati con ampie e ripetute revisioni del testo. La versione definitiva è stata sottoposta alla valutazione abituale per le Linee Guida secondo lo standard metodologico AGREE. Il documento, su cui si è registrato l’unanime consenso del Gruppo di Lavoro ed è stato successivamente approvato dal Direttivo Nazionale della Società, si articola in alcune principali sezioni. La prima riguarda gli argomenti propri dell’Igiene Industriale quali le metodiche per il monitoraggio ambientale, i vantaggi e i limiti delle tecniche microscopiche disponibili, il potenziale ruolo delle mi- crofibre e gli approcci per la valutazione retrospettiva dell’esposizione, specialmente a fini epidemiologici. La secon- da gli effetti biologici da amianto con particolare attenzione agli aspetti diagnostici dell’asbestosi, delle alterazioni pleuriche, del mesotelioma e del tumore del polmone. La terza i criteri di attribuzione causale e discusse le diverse ipotesi sulla forma delle funzioni di rischio, vengono confrontate le principali posizioni al riguardo disponibili in letteratura. In particolare sono stati approfonditi i modelli della funzione di rischio per il mesotelioma ed affron- tato il tema dell’ipotesi di un effetto acceleratore e anticipazione della frequenza degli eventi in relazione alla dose. Infine sono stati trattati gli argomenti di attualità applicativa per il Medico Competente quali la sorveglianza sanitaria degli ex esposti ad amianto e di quella degli operatori attualmente esposti in attività di bonifica.

Questo documento è stato predisposto da un gruppo di lavoro del Collegio dei Docenti Universitari della Società Italiana di Medicina del Lavoro e Igiene Industriale, con lo scopo di definire il ruolo di questa disciplina nel Corso di Laurea in Tecniche della Prevenzione nell'Ambiente e nei Luoghi di Lavoro. Il Corso di Laurea in Tecniche della Prevenzione nell' Ambiente e nei Luoghi di Lavoro forma un laureato in grado di applicare i criteri valutativi, le misure preventive e gli strumenti legislativi per l'igiene e la sicurezza nei luoghi di vita e di lavoro l’igiene degli alimenti e delle bevande e l’igiene e la sanità pubblica e veterinaria. Il documento descrive l'organizzazione e la struttura del corso, gli obiettivi formativi generali e specifici, l'insegnamento di Medicina del Lavoro e alcuni altri aspetti organizzativi. Viene infine affrontato il problema della formazione dei Tecnici della Prevenzione nell'Ambiente e nei Luoghi di Lavoro non laureati in servizio presso le Aziende Sanitarie.

Current occupationl exposure levels to benzene are reduced by three orders of magnitudo (from ppm to ppb) as compared to the past. As benzene toxicity is related to its biotransformation and bioactivation pathways seem to be more active at lower exposure levels, observed effects could be higher than expected. Although the genetic polymorphisms of relevant and functional metabolic enzymes are implied in the modulation of either the risk of adverse effects [myeloperoxidase and NAD(P)H: quinone oxidoreductase] or of the biomarkers of internal dose (glutathione S-transferases M1-1, T1-1, A1-1), they are not appliable as biomarkers of susceptibility. Among biomarkers of early effect, only the longitudinal monitoring of blood cell count seems suitable to be applied in health surveillance protocols, whereas the use of biomarkers of genotoxic effect at current exposure levels is at the present not supported by literature data.

BACKGROUND: Few data exist on the risk of musculoskeletal disorders in road paving workers, in addition methods for risk assessment of biomechanics overload of the lower limbs and the spine are also lacking. OBJECTIVES: The aim of this study was to assess the risk from biomechanics overload for the whole musculoskeletal system and manual handling involved in the activity of stone paving of roads. METHODS: In accordance with the guidelines of the Italian Society of Occupational Medicine and Industrial Hygiene various risk assessment methods were used. RESULTS: The Washington State checklist revealed the presence of biomechanics overload of the spine in flexion. The Turin checklist showed a risk score higher than the acceptable limit for the cervical and lumbar spine and limbs (risk level similar to that obtained from the OCRA checklist for the upper limbs). The assessment of risk from manual handling of loads carried out using the NIOSH method provided a synthetic risk index whereas evaluation conducted using the Washington State method was acceptable. CONCLUSIONS: Biomechanical risk assessment showed the presence of risk for use of force and awkward postures of the wrist and elbow, which agrees with literature data. The division of tasks among workers and the distribution of breaks helped to reduce the risk to an acceptable level for limbs. All methods however showed a risk for the spine. Health surveillance showed disorders of the spine in 28% of the workers and no disorders of the upper limbs, which agrees with the risk assessment.

La prima edizione delle Linee Guida SIMLII sul rischio associato alla movimentazione manuale di carichi è stata pubblicata nel maggio 2004. Da allora sia in campo scientifico che legislativo si sono verificate numerose novità che hanno imposto una nuova edizione delle Linee Guida stesse. La movimentazione manuale di carichi è un compito lavorativo (e non) che può essere associato a un rischio per la salute (a causa di un possibile sovraccarico biomeccanico) e di questo un medico del lavoro dovrebbe comunque occuparsi, indipendentemente dall’eventuale esistenza di specifici obblighi normativi.

RIASSUNTO. INTRODUZIONE. Studiare il contributo che tradizionali (acido t,t-muconico - t,t-MA ed S-fenilmercapturico - SPMA) e nuovi (benzene urinario) biomarcatori urinari di dose interna possono fornire per escludere l’origine occupazionale di esposizioni a concentrazioni molto molto basse di benzene, considerando altresì l’influenza che fonti di esposizione non occupazionale ad esso, quali fumo di sigaretta ed inquinamento delle aree urbane, possono avere sui livelli di questi indicatori. MATERIALI E METODI. Sono stati esaminati 6 lavoratori che operavano presso un impianto di depurazione di acqua di falda contaminata da benzene (esposti) e 6 impiegati amministrativi che lavoravano presso la stessa azienda (controlli); entrambi i gruppi comprendevano fumatori e non fumatori. Il monitoraggio ambientale (campionamenti fissi e personali della durata di 8 ore) ed il monitoraggio biologico (determinazione di t,t-MA, SPMA, benzene urinario, creatinina urinaria per la correzione dei biomarcatori) sono stati effettuati negli esposti per 10 giorni consecutivi, comprendenti anche i giorni di riposo (ambienti di vita), e nei controlli una sola volta. RISULTATI. L’esposizione ambientale a benzene è risultata sempre inferiore al limite di rilevabilità della metodica analitica in tutti i campionamenti, sia fissi che personali, eseguiti su esposti e controlli durante i giorni di lavoro, mentre le misurazioni personali effettuate sugli esposti durante i giorni di riposo hanno mostrato concentrazioni di benzene anche superiori a 5 μg/m3, che rappresenta il valore limite di qualità dell’aria degli ambienti di vita. Le concentrazioni di t,t-MA, SPMA e benzene urinario non hanno mostrato differenze tra esposti, indipendentemente se considerati nei giorni di lavoro o di riposo, e controlli e sono apparse sostanzialmente comprese nel range dei valori di riferimento per la popolazione italiana. Per tutti gli indicatori di dose interna studiati sono stati osservati valori significativamente più alti nei fumatori rispetto ai non fumatori. In questi ultimi, l’SPMA è risultato sempre inferiore al limite di rilevabilità mentre il benzene urinario è risultato dosabile nel 60.0% delle determinazioni effettuate nei giorni di lavoro e nell’87.5% di quelle effettuate nei giorni di riposo. CONCLUSIONI. In condizioni di esposizione occupazionale molto molto bassa o assente a benzene, per escludere l’esposizione occupazionale si rende indispensabile applicare un approccio integrato monitoraggio ambientale - monitoraggio biologico con determinazione di SPMA e/o benzene urinario, insieme ad un’attenta valutazione dei fattori che possono aver determinato un’esposizione non occupazionale al tossico. In queste particolari situazioni di esposizione occupazionale a benzene l’interpretazione dei risultati del monitoraggio ambientale e di quello biologico non deve tenere conto soltanto dei TLV o degli IBE, ma anche dei valori limite di qualità dell’aria degli ambienti di vita e rispettivamente dei valori di riferimento della popolazione generale, considerata la possibilità di comparsa degli effetti cancerogeni genotossici del benzene anche a concentrazioni di esposizione molto molto basse.

La cute come via per l'escrezione dei metalli

Starting from the experience of last five years, during which 24 guide liens about the most important aspects of Occupational Physician activity have been produced, the Italian Society of Occupational Medicine and Industrial Hygiene (SIMLII) delegated a specific working group for updating the methodology to be adopted for guide lines and other instruments for improving and standardizing the current activity in our professional field. SIMLII produced in the context of the specific Education and Accreditation Programme for occupational physicians prepared from 2002 25 guide lines or other informative instruments on the most important and controversial themes in which our discipline is involved. They were considered and treated to meet the need to improve and standardise activities and to modify the current approach of occupational physicians and aimed not only at improving the effectiveness of preventive actions but also at constantly adopting rigorous methodologies based where possible on evidence based or on consensus procedures. The Directive of SIMLII was firmly convinced about the opportunity-necessity to critically evaluate the experience carried out during the last years, at the light of the National Program for Guide Lines edited By Italian National Health Institute since 2002 and which concerns preparation, dissemination, updating, implementation of guide lines in Medicine. The guide lines were defined as rational critical effective aid addressed to professionals and patients for health services organization. Relevant was the new Framework Act for the occupational safety and health (Decreto legislativo 81/08) too signed by the President of the Italian Republic on April 9, 2008, which for the first time includes and defines in a legislative act the different possible instruments (technical normative, good practices, guide lines). In this paper we present the new methodology defined by our Society for producing the different kind of instruments such as guide lines, consensus conference reports, technology assessments, good practices statements guide lines focusing as the main aspects those related to definitions, argument choice, working group and coordinator identification, producing methods, evidence evaluation, grading, quality evaluation using AGREE method, dissemination procedure, the conflict of interest and the possible use for distance formation procedure focusing the recommendations that take a practical-applicative approach.

The SIMLII set up a program of continuous training and accreditation of postgraduate specialists in Occupational Health in 2000, before continuous medical training (ECM) was formally introduced. Therefore, especially over the past few years, SIMLII has involved its membership to an ever increasing extent, in an attempt to understand their expectations as regards the scientific community and as regards a continuously evolving discipline that is subject to continuous changes in educational requirements.The self-administered questionnaire in a survey carried out in 2005 by the National Institute of Occupational Safety and Prevention (ISPESL), in collaboration with SLMLII, provided data that, besides identifying the type of activity of the membership, attempted to assess members' compliance with the society's initiatives, with special reference to the Programme for continuous training and accreditation of postgraduates in Occupational Health and the issue of Guidelines.
