- Abstract
This paper considers the historical context when Luther uttered the proverbial words «Here I stand». They were expressed at the Worms Diet in 1521 and for the first time they affirmed the right to a free conscience invoking God's word the Agostinian monk felt prisoner of. This restraint leads him to an interior freedom responding only to God. This act and that appeal to conscience became an icon, a reference point for all those fighting for freedom and it was represented by the posterity in prints, paintings, sculptures, cinematography. - Abstract
Which are the foundations of ethical consciousness in Mediterranean antiquity according to the Hebrew and Greek roots outside the Bible and the biblical corpora? This work intends to outline a few general lines. Our attention is focused on the term syneidêsis in both biblical and extra-biblical Greek contexts without neglecting the comparison with the notion of heart in the biblical scriptures. The paper conclusion concerns the meaning of syneidêsis in the New Testament and exemplifies the eloquent case of Corinthians 8 and 10. Finally, some hermeneutical observations are proposed on the existential values implied in the notion of conscience ranging from the New Testament to contemporary life and culture. - Abstract
In the last two decades, progress in the areas of learning and fear extinction have led to the formulation of a new conceptual model, based on exposure therapy, according to which the extinction of fear (which represents its clinical application) would occur through the creation of a new inhibiting memory rather than through the habituation process. This has led to the development of specific changes in the procedure, which aim to maximise exposure therapy, particularly in terms of diminished fear return. To date, the new «modus operandi» of therapy is being applied to patients by exposing them directly to what they fear, in other words, «in vivo». Virtual Reality environments (VR) are excellent for implementing the potential of inhibitory expository learning, although they are in need of a fully implemented protocol. To this purpose, we are rethinking current technology, modelled towards the process of habituation, in favour of a tool capable of facilitating the learning process through inhibitory learning, and even using the Flow mechanism. This paper makes some considerations on these innovations and proposes the hardware and software changes needed to shape innovative virtual reality psychotherapy. [ABSTRACT FROM AUTHOR]

- Abstract
Indirect methods are data mining techniques for establishing or verifying reference intervals. They present many advantages over the traditional direct approach, including reduced costs, closeness of preanalytical and analytical conditions to routine conditions and usefulness in specific age groups or with uncommon sample types. After discussing advantages and limitations of the indirect approach, in this paper the main statistical algorithms, and their implementation in open-source software as well, are briefly introduced by a practical example. Moreover, key aspects of data selection and preparation are discussed. The indirect methods represent a powerful and valuable approach for the definition or validation of reference intervals, whose knowledge and correct use should be part of the laboratorian's toolbox. The paper aims to illustrate how, between the 19th and 20th centuries, the practice of archival inventory was established even in Italian municipalities, in relation to the calls of the Ministry of the Interior, the indications of a developing discipline and professional manuals exemplifying practices and models. Through the use of diagrams, we trace the development of the inventory in the local context, which initially has a patrimonial connotation, but gradually takes on a cultural function. In the field of archival publishing, the transition phase in which the relationship between print and digital was in contrast is considered closed and it’s time to move towards a new model of integration. It would be time to start a more mature reflection, in the perspective of imagining a dialectical relationship, not of contrast but capable of overcoming indifference in view of a fruitful integration. For this purpose, this paper provides some ideas and some proposals for work, in the hope that the academic world, Anai and Directorate General Archives will start in collaboration shared work. This review deals with a widely discussed topic and its yet unknown features. High Pressure Narcosis Syndrome (HPNS) or 'deepsea nervous syndrome' is an important condition with almost unknown causes and fascinating features, to say the least. It affects those who dive at great depths while breathing in certain gas mixtures. The purpose of the following paper is first of all to summarise the state of the art and to draw more detailed and precise conclusions on the basis of the various studies in the current scientific literature. In the course of the analysis, this syndrome will be differentiated from the better known nitrogen narcosis with a few hints. Another objective is to mention the preventive measures that allow one to understand the onset of potentially very dangerous symptoms in an extreme environment such as diving. We briefly discuss saturation dives and their usefulness, emphasising their use in the Navy. The drug therapy solution discussed here emphasises the compressive or decompressive differences, in particular with respect to the Navy tables and the more recent tables published by the U.S. Navy. - Abstract
This paper aims to deepen, on the basis of studies personally conducted, further facets of a little-known late Byzantine epyllion (the Theanó by Michael Choniates), and yet extremely interesting both from a literary and a more broadly cultural point of view. In particular, we will focus on two sections of the poem, that is the proem and the inspired revelation of the (neo)pythagorean Theanó, announced from the very beginning of the poem. For the purpose of facilitating the consultation of the text and its first translation to a modern language, the lines examined are reproduced as annex to the following contribution. - Abstract
The essay addresses the issue of decent work, analysing it through the lens of working time. A universal and even foundational topic for labour law, it proves capable of responding both to the more traditional needs of protecting workers' health and to the more modern ones, which today can be traced back to the articulated and complex principle of decent work (work-life balance, gender equality, employment inclusion of the disabled, and so on). In this context, the paper seeks to direct the reader's attention also beyond the present circumstances, to ask whether, in the combination of the different perspectives involved, there is not the risk of losing sight of the values that have always inspired the genetic heritage of (labour) law and which, it is hoped, will continue to denote its future developments: the importance of work for the fulfilment of the person who performs it and for the progress of society, well beyond the quantitative data of the time dedicated to it and the remuneration that may derive from it. - Abstract
The paper takes cue from the current changes occurring in the two emerging sectors named "the green (ecology) and the blue (technology)" by the scholar Floridi on one hand, and from the increasing diffusion of remote communication instruments linked to growing manifestations of psychopathological events of social withdrawal, on the other. While the proliferation of research focused on the increasing incidence of psychological and somatic diseases derived from solitude and loneliness is there for all to see, at the same time it is undeniable that relationships are started and carried on using new media which accentuate both the sense of isolation and omnipotence. These considerations have led the authors to the necessity of contemplating "new analytical practices". They think that one of the most productive ways of dealing with the exponential growth of serious psychic discomfort - especially in young people - is implementing permanently, in their modus agendi, the complex paradigm of a clinical-empirical research approach on oneiric narration. It is known from Jung indeed, that dreams are not only "the essence of the psyche" but also represent essential instruments to comprehend and treat psychic suffering. Such an approach, first presented in IAAP Conference held in LUMSA in 2022 Narratives in Current Analytical Psychology. Studies about Mind-Body Relations will be described here in full detail. - Abstract
This paper considers space and architecture as a mirror of the life of the Church and as an area of evidential value for its historical profile. Therefore, it will try to observe, in space and in its organization, the development of participation, recognized by both precursors and more recent documents as the constitutive dimension of the Church. Among the precursors, the Bolognese Laboratory will be particularly considered to illustrate the reasons that made the archdiocese led by Giacomo Lercaro become a point of international reference on the path of the liturgical reform. Then, considering the Bologna experience, useful criteria can be drawn for an assessment of the current situation, and to measure how, how much and whether participation to the altars has matured into a wider participation of all the people of God in the life of the Church and in the management of its spaces (liturgical, pastoral and educational). - Abstract
The paper deals with examine social citizenship in the jurisprudence of the Court of Justice of the European Union. The study aims to understand whether social citizenship is capable of reproducing at the supranational level the rules of reciprocity and solidarity typical of national welfare systems. Indeed, the case-law of the Court of Justice wavering between opening and closing spaces of solidarity makes it difficult to access welfare benefits for European citizens who make use of free movement in the EU, especially those who are economically inactive. As a result, this jurisprudence risks forming, in the EU legal system, a logic of sharing social solidarity subordinated to the individual's contribution to the production of economic wealth in the host Member State. - Abstract
This paper analyses the multilevel protection of rights from two perspectives. On the one hand, taking into consideration the legal and institutional contexts of each level in relation to the configuration in the constitutional state of a symmetry between democracy and rights, i.e. between a certain structure of public power and the protection of rights. The asymmetry inherent in the systemic and institutional contexts of the supranational levels must be understood from the perspective that, in multilevel protection, the interaction between the different levels is as important as the way in which rights are protected at each level. On the other hand, the work takes into account the impact that globalisation and European supranational integration are having on the multilevel protection of rights, which tends to detach the protection of rights from their constitutional substance. A loss of substance of rights that is also partially compensated by increased effectiveness, insofar as the sphere of rights protection is closer to the structural conditions of globalisation, such as the market, and to the rights on which conflicts are concentrated, consumer rights and the right to personal data protection. - Abstract
The paper analyzes the relationship between interpretation and construction, underlining the conjectural value of constructions. The author traces the Freudian conceptualization of interpretation and construction in relation to dreams, transference and resistance, which must be taken into consideration as a priority. The vision of the Kleinian school is also critically explored, specifically regarding the emphasis on the need to provide systematic transference interpretations, which tends to excessively personalize the transference, overriding its value of repetition of the past. Starting from the vision of the French school and Winnicott, it is underlined how the main task of the analytical work is to facilitate the transformation of instinctual impulses into representations to subsequently make them accessible to interpretation. Starting from the difference between interpretation and construction, emphasis is also placed on their different use in neurotic and non-neurotic structures. [ABSTRACT FROM AUTHOR]

- Abstract
Formulations regarding the patient’s history have not only played an important part in understanding the patient, but interpretations explicitly linking the present with the past have been seen as central to the therapeutic process. In this paper the author considers the role of historical reconstruction in bringing about psychic change. He emphasizes the therapeutic value that lies in the exploration of the way the patient’s history is embodied in his internal object relationships, becoming manifested in the transference-countertransference relationship. The author presents clinical material which he suggests allowed the analyst to follow the way the patient’s internal object relations, coloured by her history, became expressed and played out in the sessions. He suggests that, when these processes can be followed and addressed in the present, this may lead to a diminution in the underlying anxieties. This can thus promote psychic change by freeing the patient’s capacity to achieve a sense of connection with her history, and to tolerate the meaning of what emerges, which illuminates both the present and the past. - Abstract
The paper illustrates an evolving theoretical-clinical study about the relationship between psychoanalysis and psychodynamic psychotherapy. It comes from the observation of some recurrent phenomena in the context of daily clinical experience. Small variations, apparently accidental, that occur in the setting, and that are an expression of the complex dynamics of the unconscious intersubjective field. The psychoanalytic exploration of the experiences of affective interaction in the setting has brought out an increasingly real dimension of the implicit and its relationship with the unconscious. This pushes the analysis of the therapeutic process and therapeutic action towards a growing consideration of the silent and fruitful work that takes place beyond the symbolic-verbal support, where is a different way to dialogue, to communicate and influence each other. The approach presented aims to demonstrate that it is possible to take into account the growing evidence in the field of affective neuroscience, epigenetics and infant research, preserving the foundations of the psychoanalytic clinical approach developed in the relational context. An empathic, intersubjective and affective approach based on the phenomenological and holistic experience of mother-infant dyadic interaction. The clinical-technical foundation of this approach is the experience of regression in analysis, a condition not only for resurfacing and processing of trauma, but also for interactive repair. On the occasion of the release of Anima, corpo, relazioni. Storia della filosofia da una prospettiva antropologica, edited by proff. Marianelli, Mauro, Moschini and D’Anna, the paper delves into the strengths and the limits of the anthropological approach in history of philosophy. The article underlines that an authentic anthropological perspective in philosophical issues can only arise by dropping an anthropocentric standpoint: adequate answers to the question of humanity’s position in the world can be found only decentering the human, a process articulated in the textbook through a metaxological framework. - Abstract
The paper describes the transformative process that is generated by inhabiting the place of “between”, outlining the roots of a philosophy of vulnerability that emphasizes the paideutic and prophetic role of the exiled and fragile. It is through the metaphor of the seaweed that philosopher María Zambrano and philosopher Michel Serres narrate the unconventional condition of the fragile and the exiled: to be out-of-place enough to be able to move to see, opening oneself to the transformation this entails. The two philosophers' thinking is inspired by a poetic and maternal sensibility, that prefers perception to rational thought, listening and observing the world from the bowels, and that accredits the dimension of vulnerability as favored by the philosophical approach. This paper studies the concept of fraternity in the light of the Magisterium of Pope Francis. In the first part, it focuses on some of its meanings that are reductive and insufficient. Then, it presents the characteristics that Pope Francis assigns to it (2nd part) and how he applies it to our relationship with Sister Mother Earth (3rd part). Contradicting the current “undifferentiated and one-dimensional paradigm”, the Pope teaches that fraternity is unity in diversity. The model is the “many-faceted polyhedron whose different sides form a variegated unity”. [ABSTRACT FROM AUTHOR]

Tin which manner is the ministry of diacons able to interconnect with the service of social justice in the ecclesial environment within the frame of implementation of social justice in a society that is more and more characterised as non-religious? The article pays attention to history of diaconate in the 20th century, in particular to the developments of diaconate in the context of search of social justice. It describes the paradigms within which the diaconate is comprehended and - in its final part - it outlines a perspective for practical implementation. CHIODI, Maurizio
- Abstract
The paper moves from a theoretical premise, concerning the "fruitful connection" between theology and pastoral practice. Between these two moments, the theological and properly theoretical one, and the pastoral, meaning the practical one, we can see a nexus of virtuous circularity that entails to think the one together with the other. In the context of the historical and exegetical complexity concerning the theoretical topic and the practice of the "divorced and remarried", the paper aims to highlight the basic structure of the nexus between accompaning, discerning and integrating as it can be thought from Amoris laetitia, the theological reception of the VIII chapter about the topic of "divorced and remarried", the reception of Amoris laetitia in some interesting pastoral papers written by Italian Bishops and lastly, a final consideration which intends to show how Amoris laetitia can be a precious occasion for a general renewing and recalibrating of pastoral practice as regards to marriage. In the reform of the career guidance system suggested by the National Recovery and Resilience Plan, new guidelines have been issued. Moving from a definition of career guidance, they indicate to schools how to renew themselves in supporting students and their families in the demanding task of choosing educational paths, in function, above all, of adequate and conscious placements in the world of work. This paper aims to identify the theoretical references from which the guidelines appear to be inspired and, using TalTac2 software and a textual analysis, to identify the most recurrent themes and constructs. In addition to highlighting the presence of some innovative elements, the paper will also point out the absence of some essential aspects that, if present, would have helped schools make a significant turn toward career education activities aimed at a quality future for all. A bogeyman lurks around the world: the fear that intelligent technological beings will substitute human beings in (almost) all their activities, starting from work. In this sense, AI is often perceived as a magical helper, a high technology (maybe the highest one) potentially capable of performing any task. Guided by Gilbert Simondon's takes on technical alienation, this paper argues that automation is actually the lowest form of human-machine interaction. Recalling some milestones in the history of AI, it makes some suggestions on why, regardless of their human design, the so-called 'intelligent' digital machines based on data reduction algorithms, tends to incorporate ideological assumptions, beliefs and values. And by analysing human interactions with search engines, it shows that automated systems implies human-machine mutual conditioning: the more humans address these machines in a mechanically, simple and unambiguous way, the more the latter act 'automatically' and appear 'intelligent'. Drawing on this analysis this paper finally discusses the exploitation of that mutual conditioning mechanics by a myriad of for-profit digital micro-activities. "Trained" machines, in this context, are far away from the ideal of the Simondonian "open machine". [ABSTRACT FROM AUTHOR]

This paper discusses "polemical triangulation," a common rhetorical and polemical device in Antiquity, through which an author attacks a person or a group of persons who are not the explicit addressees of a text or speech. It looks at its use and diversity with examples from Melito of Sardis and Ignatius, in the context of polemics against the Jews and the Docetists. In all cases, these models imply the existence of a marked anti-Judaism in Ignatius and his addressees. Some well-known methodological questions are addressed, such as the author's knowledge of the persons attacked, his or her relations with them, and other more or less consciously used sources of information. Finally, the paper draws conclusions about the relevance of the concerned statements for a historical study of the adversaries, their thinking or the arguments attributed to them. - Abstract
This paper takes as its focus the massive increase in children and young people (CYPs) seeking gender transition. It discusses the factors that have contributed to this both at the individual level and at the socio-cultural level. At the individual level there are many pathways to gender dysphoria (such as internalized homophobia, complex comorbid problems, histories of family trauma and so on). The author considers that a very large number of children have been seriously harmed through the penetration of the "trans" ideology into child and adolescent services leading to their complex problems being seen only through the prism of gender, thus leading to these problems being neglected. The paper also looks at the broader socio-cultural processes that have contributed to this phenomenon - such as the commodification of health care, misogyny, relativization of truth, all contributing to what the author terms "a peculiar mode of thinking" that has come to dominate this debate. The Gruppo di Parola for children having separated parents is a short, preventive and support intervention aiming at promoting the resources and the well-being of participants both in individual terms as well as in the relational and familiar ones. In the scientific literature few contributions analyze this intervention from the process' point of view. This paper focuses on a specific phase of the path during the first meeting - called "opening phase". The aim is to identify some common elements in the interactive exchanges that allow the researcher to identify patterns of interaction. Three different Gruppi di Parola were considered, conducted in the same context (Service for families) and by the same operators (two females). This paper illustrates in details the coding system developed with the aid of the Transana software. The analyses confirm the presence of interactive configurations common to the three Gruppi di Parola. In particular, the intervention is characterized by an involvement of participants through an individual level as well as a group level (the group becomes the subject). Some elements of processual rituality and micro-rituality are also identified. [ABSTRACT FROM AUTHOR]

Among other things, the role of intuition and the ability of making predictions in terms of probability are discussed, as well as the relationship between clinical and empirical research. (This paper is the transcription of the audiorecording of a talk given at the Fourth training course of the "Milan Group for the Advancement of Psychotherapy" - which since the 1970s took the name of Psicoterapia e Scienze Umane - entitled "Theoretical problems and clinical cases", held in Milan on May 1-4, 1964; after the paper the discussion that followed this talk is published, with interventions by Silvano Arieti, Enzo Codignola, Franco Fornari, Luigi Frighi, Pier Francesco Galli, Mario Moreno, Diego Napolitani, and Enzo Spaltro). The functional category of general extenders (e.g. and so on, or something, etcetera) has been mainly analysed in previous work with a focus on spoken language. The aim of this paper is to further explore its use in standard written language, through the analysis of a corpus of Italian texts. The paper shows that general extenders in written language display several specific features that need to be considered within a general account of the class: (i) from a quantitative point of view, their frequency of use is lower than that of their spoken counterparts; (ii) forms like the abbreviation ecc./etc. and the ellipsis <...> are exclusive of written language; (iii) they are often employed in lists that exemplify a co-occurring referring expression, thus reinforcing text cohesion without the proper online categorization that regularly takes place in spoken language; (iv) their frequency of use is higher in formal texts, whereas in spoken language it increases in interactive and informal discourse. The paper presents the study and research project “Arti e Periferie”, promoted by the Department of Human Sciences for Education “R. Massa” (University of Milano-Bicocca), aimed at networking different art and performance experiences with one another, that occurred in public, contemporary, urban spaces. Specifically, the project crosses and enhances experiences located outside the usual urban routes, in order to look at the intrinsic potential of artistic and performative languages to interweave places and people and to connect them with each other, and to develop ties even in contexts considered as peripheral and marginal. In such contexts, it is the artistic occasion that can play as transformative trigger, stimulating innovative connections with local frameworks of sociality and cultural contexts. Pedagogically, the different cultural witnessings confirmed how artistic languages can be opportunities for lifelong learning, generating attention on the transformative micro-processes that decline art as a generative renewal relationship between the subject and her or his context. Advertisements play a fundamental role in the process of constructing social reality and, thanks to easy availability and widespread dissemination, they represent an unlimited source of linguistic, rhetorical-stylistic, and cultural elements that can be used to develop knowledge and skills in a foreign language and culture. As authentic texts, the result of an ingenious interaction between different forms of verbal, para-verbal and nonverbal input, they allow different levels of linguistic analysis and enable the acquisition and memorisation of expressions that are often related to spoken language. This paper aims to highlight the linguistic, stylistic and cultural potential of printed advertising in German, promoting its use in classrooms through a contrastive reading and possible translation to foster intercultural reflection. ETHICS , *MORAL education , *VIRTUE - Abstract
In these years there has been a growing interest, in the philosophical debate, for practical philosophy, with a particular attention to the theme of virtues, starting from the now "classic" works of Alasdair MacIntyre. At the same time, especially in the Anglo-Saxon world, a renewed interest in the theme of character education has been consolidated. This paper offers a proposal of pedagogical foundation of reflection on the virtues, in educational perspective. Our theoretical reference point is the moral philosophy of Thomas Aquinas, which is already very rich in pedagogical suggestions, which are developed explicitly by Jacques Maritain and Gino Corallo. The work critically explores a recently-emerged strategy of territorial regeneration for small towns at risk of depopulation in Italy, "One-euro houses". Situated at the intersection between rural gentrification and the researches on real estate capitalism, the analysis is based on a place-based case-study in Troina, Sicily, to understand if the initiative supports socio-economic regeneration in small villages and/or marginal areas or, on the contrary, it fosters commodification/financialization of the real estate market as well as extractive dynamics. So, the paper scrutinizes how the local coalition of public and private actors mobilizes the initiative to deal with the challenges of the local development, thus highlighting the mismatch between their perception and the critical views of the academic and cultural debate. ECONOMIC geography , *GLOBAL production networks , *ENVIRONMENTAL justice , *ATLASES , *REGIONAL development - Abstract
The article investigates the economic-geography's category of Global Production Networks (GPNs), especially with reference to its ability to grasp the critical aspects of the relationship between multinational capital and regional development. After having briefly framed the GPNs in the literature on the global systems of production, we'll discuss some controversial aspects of this concept. Particular attention will be devoted to the capacity of GPNs category of dealing with the questions of the asymmetries of power and of the ecological relationships. To reiterate the urgency of reinforcing the study of these issues, we present a short analysis of the most relevant aspects of the environmental conflicts, as they appear in the international repository of the Atlas on Environmental Justice (Atlas EJ). REMOTE sensing , *PUBLIC spaces , *URBAN growth - Abstract
In the last few decades, the remarkable increase in the number of people living in urban areas around the world has posed new challenges for urban planning. The environmental, economic, and social issues of urban sprawl are well-known also in several metropolitan areas of Italy. In this paper, we consider a southern city of Italy (Campania Region), Caserta and its surroundings. We use several satellite images with different spatial and spectral resolutions (Landsat, Sentinel, and KH-9 Hexagon) within the Google Earth Engine platform, to map the main land use changes of Caserta conurbation from the 1980s till 2021. The results highlight the great land consumption and, at the same time, the relevant urban infill that occurred in this area in this short period, both on topographic and local scale (referring, in this last case, only to the town of Caserta). While at an early stage, the increase in artificial areas responds to a specific demand related to the increase in the resident population, the urban growth of the last two decades has more complex answers. Systematic delays in land use planning and a biased perspective (usually focusing only on the municipal scale) to solve the problems of a significantly larger area means insufficient services to the citizens in their daily lives (mainly in relation to local public transport and the actual availability of urban green areas). - Abstract
Insomnia symptoms are common during peripartum and are considered a critical risk factor for postpartum psychopathology. Sleep is particularly vulnerable during pregnancy and postnatal period and its alterations are associated with risks for mental health. Nonetheless, often insomnia disorder during pregnancy doesn't receive appropriate clinical attention, with consequent lack of proper treatment offer. Evaluation and treatment of insomnia during the prenatal period should be a priority in the clinical practice and should consider the whole family context. Cognitive-behavioral treatment is recognized as first line intervention for insomnia disorder from international guidelines. Many authors highlighted that during pregnancy and during the first months of child's life psychotherapy seems to be more adequate and more favorably accepted from patients than pharmacological treatment. This is also linked with possible negative consequences associated to the assumption of drugs, both for the fetus and the newborn. The aim of this paper is to examine in depth, through a narrative review, the importance of a correct evaluation of the insomnia disorder during the prenatal period and the characteristics of cognitive-behavioral therapy.. - Abstract
Eating disorders are characterised by dysfunctional eating behaviours and deeply negative beliefs about their weight and body shape. The literature on eating symptomatology is extensive and detailed, but systematic research about how sleep is characterised in such patients is currently lacking. The results that have emerged so far are contradictory and therefore do not allow a clear understanding of this topic. This paper aims to understand the current state of the art and which aspects need to be further investigated in the future. The results highlighted an actual sleep impairment in patients, although not always consistent. The most pronounced impairment seems to be in anorexia nervosa, moreover, this disorder has proved to be the most investigated. On the contrary, very few studies have evaluated sleep in patients suffering from binge eating disorder, especially using physiological measures. Few studies have also investigated circadian preference, thus leaving several questions unanswered. Future studies will not only aim at investigating sleep and circadian preference more rigorously and systematically, but they should especially try to relate specific aspects of eating symptomatology to specific sleep parameters. One of the advantages of such an approach would be to support clinical practice through sleep-centred treatments that could indirectly act on the eating symptomatology itself. [ABSTRACT FROM AUTHOR]

196. Equazioni per la stima della velocità di filtrazione glomerulare: lontani da un punto di arrivo.

- Abstract
The recent position paper by the European Federation of Laboratory Medicine (EFLM) advises against following the US recommendations on the use of the new race-free version of the Chronic Kidney Disease-Epidemiology Consortium (CKD-EPI 2021) equation to estimate the glomerular filtration rate (eGFR). The main concern derives from a possible poor CKD estimation, causing an underestimation of renal impairment for the Caucasian population, and therefore for all Europeans. Considering the relevance of the EFLM indication, Biochimica Clinica published the Italian translation of the document and an accompanying editorial suggesting that the European position could be an end point for a better eGFR harmonization. However, the use of different equations for eGFR in US and Europe will entail rather a worse harmonisation: the eGFR values are used for patients enrolment in numerous clinical studies, for the admission to diagnostic procedures or therapies and for the evaluation of their complications and adverse effects. As a consequence, many clinical trials will not be comparable, guidelines and recommendations will be less easily transferable from one country to another and the "regionality" of the equations will not slow down the search for new equations, for different groups and different countries. With equally shared responsibility between US and Europe, this situation seems to be a paradigmatic example of how recommendations in laboratory medicine should not be developed. Nevertheless, the EFLM indication not to adopt the CKD-EPI 2021 race-free equation should be accepted for the moment in Europe, since the harmonization between neighbouring and regional sanitary structures is a priority in the patient care. It should be strengthened that the problem has not been solved yet, and that a greater harmonization action must be rapidly sought, with the aim of a homogeneous calculation and reporting of this widely diffused estimate. - Abstract
The revolutionary new cystic fibrosis (CF) drug combination of ivacaftor (IVA), tezacaftor (TEZ) and elexacaftor (ELX) directly modulates the activity and trafficking of the transmembrane conductance defect (CFTR) regulatory protein underlying CF disease. Limited information is currently available in the literature on plasmatic concentrations of IVA, TEZ and ELX. However, significant interindividual pharmacokinetic variability has been reported in patients receiving the IVA/TEZ/ELX combination, exacerbated by several previously documented drug-drug interactions. Therapeutic drug monitoring (TDM) could therefore be useful in optimizing treatments with CFTR modulators. The purpose of this paper is to provide an overview of currently available information on pharmacokinetics/pharmacodynamics (PK/PD) of the drugs and their relationships to better understand the basis for differences in treatment response. Relevant articles reporting PK/PD data on IVA, TEZ, ELX administered for the treatment of CF were obtained from the PubMed database. [ABSTRACT FROM AUTHOR]

Object: Foster parenting is when a person temporarily serves as a parent for a child who has lost or has been removed from their biological family. Method: A clinical case is presented relating to the fostering history of Stefania, an eight-year-old girl who has been placed in a residential community since the earliest years of her life. Results: The child's history is characterized by a series of life events in which abandonments constituted a risk actor in relation to dysfunctional aspects in several areas of development. A psychodynamic perspective as adopted both to assess Stefania's psychic functioning and psychopathological symptomatology, and to identify the processes of intergenerational transmission of mentalization failure. Conclusions: In this paper we present a reflection about the pervasiveness of the consequences of parental neglect, and offer clinical indications on the care of foster children. - Abstract
This paper deals with a topic that results still little discussed and which seems to be central to imagining what trasformations libraries will undergo in the near future. This is the concept of multifunctionality. Starting from the data analysis of the first three Istat Census’s editions, this concept was explored in relation to the characteristics of Italian libraries and thanks to a data driven approach a detailed geography is offered, reflecting variations that occurred between 2020 and 2022. The physiognomy of the libraries’s multifunctionality before and after the Covid-19 pandemic was therefore reconstructed. Thanks to the results of an ongoing survey, it was also possible to combine this study with a mapping of the so-called ‘new multidisciplinary and independent cultural centres’ which are widely spread nationwide and which seem to stand out precisely for their multifunctionality. By comparing different visions and analyses, the intention was to set up a comparative reflection betweenmultifunctional libraries and new cultural centers to discover the characteristics that make them contemporary places.
