Invasive actions are closely related to psychological problems, special feelings of anxiety. Uncertain health conditions during and after procedures that can occur during surgery are decided for the patient's recovery. Need special methods from nurses to help patients cope with anxiety, such as caring nurses caring. The purpose of this study is to know the effect of the caring behavior of nurses on preoperative patient's anxiety levels. This research uses a quasi-experimental approach l design with a pretest-posttest one group design. This study was conducted in June 2018 in the Surgical Inpatient Room RSUD Dr.H Abdul Moeloek Lampung Province. The technique of sampling using nonprobability by purposive sampling, got a sample counted 38 respondents. Bivariate analysis using paired sample t-test. The result showed that the average score of the respondent anxiety index before caring behavior of nurse was 40,50 and the score after caring behavior was 34,63. The difference in the mean score of anxiety index score before and after caring behavior was 5.87 with the p-value of 0.000. Based on the conclusion of the research, the writer suggests that the interaction of nurses to the patient is improved and the Standard Operating Procedure of the frequency and duration of the nurse contact to the patient especially the preoperative patient can develop the use of the Caring Dimensions Inventory which covers biological, psychological, social and spiritual aspects.

The most common response by preoperative patients is anxiety. The results of research on the incidence of anxiety in preoperative patients in RS PKU Yogyakarta mentioned that 60% of patients experience anxiety from mild to severe level (Wuryani, 2015). There are several ways to reduce anxiety, one of them is by giving music therapy. Based on the result of interviews with five patients who were going to have an operation, the patients expressed that they were anxious because they were afraid of the operation and that was worried if the operation may fail. To determine whether three is an influence of listening to “Langgam Jawa” music genre on the level of anxiety of preoperative patients at “Kasuari” room of RSPAU, dr. S. Hardjolikito. This is a quasi-experiment research which used a pre-test and post-test non-equivalent control group design. There were 30 respondents involved as samples. The samples were selected using a systematic random sampling technique. The instrument used in this research waZung’s self-rating anxiety scale (SAS). The statistical test used a Wilcoxon signed rank test with an error level of 0.05. In the control group, which was not given “langgam jawa” music, during pre-test most patients experienced medium level anxiety (26,7%), during post-test most patients experienced medium level anxiety (60%); in the intervention group, before “langgam jawa” music treatment was given, most patients experienced medium level of anxiety (26,7%), after the music treatment was given, most patients experienced a medium level of anxiety (56,7%). Based on the research result, there is a significant influence from “langgam jawa: music therapy with a p-value=0.007. There is an influence of listening to “langgam jawa” music genre on the level of anxiety of preoperative patients at “Kasuari” Ward, dr. S. Hardjolikito Hospital.

All of the nursing intervention in hospital was not certainly be positively accepted by patients. People ability has different, so that can arise the stress condition or anxiety. The prevalence of anxiety in America were more than 28%. The age who get anxiety were 9-17 years, 13% age of 18-54 years, 16% age of 55 and 11.4% of elderly. Women have risk to anxiety, two times than man. This study was to know the factors that related to anxiety level in Preoperative patient in Mitra Husada Hospital, Pringsewu, in 2016. The method of this study was a correlation with using cross sectional approach. The population were 58 respondents. Technique of sampling was using accidental sampling. Data were analyzed by univariat and bivariat using Chi square. The result showed that there were a relationship of gender, age, and economic status (p-value0.05) with the anxiety level of Preoperative patient in Mitra Husada Hospital, Pringsewu.

Fraktur adalah kerusakan pada kontinuitas tulang, penanganannya dengan operasi yang dapat menimbulkan kecemasan. Ketika pasien mengalami kecemasan maka akan mengalami gangguan pada beberapa sistem tubuh diantaranya tekanan darah meningkat, gelisah dan sulit tidur. Jika hal tersebut terjadi ketika menjelang operasi dapat menyebabkan kegagalan operasi. Penelitian ini bertujuan untuk mengetahui gambaran tingkat kecemasan preoperative pada pasien fraktur di Ruang Marjan Atas RSUD dr. Slamet Garut. Jenis penelitian ini menggunakan metode deskriptif dengan pendekatan cross sectional. Populasi dalam penelitian ini adalah pasien preoperative dengan fraktur dengan jumlah sampel sebanyak 32 responden, dengan teknik total sampling. Data diambil dengan menggunakan instrumen Hamilton Anxiety Rating Scale (HARS), dianalisis secara deskriptif kuantitatif dan diolah dengan menggunakan software komputer, kemudian disajikan dalam bentuk distribusi frekuensi. Hasil penelitian menunjukkan sebagian besar responden mengalami kecemasan ringan (65,6%). Kecemasan ringan terjadi pada orang yang baru pertama kali melakukan operasi karena ketakutan dan kurangnya pengetahuan, sedangkan pasien yang tidak cemas sudah mempunyai pengalaman operasi sebelumnya dan ketakutan menjadi berkurang. Simpulannya adalah tingkat kecemasan pada pasien preoperative dengan fraktur di Ruang Marjan Atas RSUD dr. Slamet Garut mayoritas cemas ringan. Dapat disarankan intervensi preoperative teaching kepada pasien sehingga dapat meminimalisir terjadinya kecemasan preoperative.

6. The Influence of Aromatheraphy on the Client’s Anxiety Level Prior To Spinal Anesthesia Operative Procedure

A quasy-experimental research with one group without control group design was applied to 40 samples who planned to be operated by a spinal anesthesia using the HRS-A (Hamilton Rating Scale for Anxiety). Data of the research was analyzed by using Wilcoxon rank signed. Research results showed that 40% samples experienced severe anxiety level before receiving aromatherapy, and 42.5% samples experienced moderate anxiety level after intervention. Statistical analysis showed that there was a significant effect of aromatherapy on the client’s anxiety level prior to spinal anesthesia operative procedure (p = 0.00). It is recommended to give lavender to clients before getting an operative procedure as a part of a standard operating procedure.

The symptoms of anxiety relate to medical condition are often found, though there are various incidences for each condition. The surgery is a potential threat that may rise stress reaction either physically or psychologically. One factor that may decrease the level of anxiety of patients is nursing therpeutic communication. The purposes of this study were to know the relationship between nursing therapeutic communication and anxiety level of preoperative patients in central surgical instalation room at RSUD Saras Husada Purworejo. This study was used an observational with a cross sectional design. The population in this study was preoperative patients in central surgical instalation room at RSUD Saras Husada Purworejo. Samples were obtained by accidental sampling technique, which consisted of 81 respondent. Data analysis was done by Kendal Tau test. The results showed that nursing therapeutic communication was 37.0% in a good category. Anxiety level of preoperative patients was 44.4% in average level.
