Between 1990 and 2007, being the eighth successor leader following Ignác Semmelweis at the university department of obstetrics in Baross Street, what interested me mainly were the following: the lucky coincidences needed for the great discovery, the doctor's hopeless struggles, the circumstances of his death, his five burials and fourfold exhumations, the Hungarianization of his name and the deliberate or unintentional mistakes related to him. In the recollection, my impressions and experiences related to the great predecessor will be reviewed. Orv Hetil. 2018; 159(26): 1071-1078.

It is a regrettable deficiency in the Hungarian healthcare that the culture and the system of quality control of cure have not been formed (except for a few subspecialties, units or wards). If hospital wards do not have a national, professionally unified and modern information system presenting the most important quantity and quality indicators of their medicinal activity annually, a stable basis for definition of future tasks is absent. The author puts forward a proposal for the establishment of the information systems for different professional fields. On the basis of experience of perinatological information system operating for over 3 decades in Borsod-Abaúj-Zemplén county, he also proposes introduction of a nationally unified, Europeristat-compatible information system following Tauffer-statistics which may serve as a uniform quality control of obstetrics and perinatological care, as well as introduction of its base, the dataform "TePERA" (Form of Obstetrics and Perinatological Care Risk).

Cesarean section rates are increasing worldwide, which has been paralelled by an increase in primary cesarean delivery and decrease in vaginal birth after cesarean section. Behind the different frequencies there is a number of interrelated factors including advanced maternal age, increasing incidence of obesity, assisted reproductive technologies, and maternal request for non-medical reasons. The sub-optimal management of labor and the concerns about medical liability claims and litigations increase the number of abdominal deliveries. The author reviews the changing indications for cesarean deliveries in the last few decades and summarizes the effects on the obstetrical clinical practice.

Recently the most frequently applied surgical procedure worldwide has been Cesarean section. It is essential to perform the operation without any complication. In addition, a fast postoperative recovery must be provided, because all mothers must have the chance for being together with their newborn infant even on the first day. The maternal mortality rates of Cesarean section significantly decreased in the last decades due to the planned team work as well as the widely applied regional (spinal and epidural) anesthetic procedures. Apart from the obstetrician and neonatologist the anesthesiologist is the member of the perinatal team, too, who is responsible for the patient's perioperative care. To prevent complications and have an early successful treatment the anesthesiologist should be informed by the pregnant woman' s health status in time in order to be able to plan the perioperative management. The high-risk groups of pregnant women, the most common causes of maternal death and possibilities of prevention and treatment are discussed.

Introduction: During the last few years family centred obstetric practice became more widespread in Hungary as well. The opinion of the population, however, is hardly known., Objectives: The authors attempted to acquire a better knowledge of the expectations of the various population segments towards postpartum care., Methods: The opinion of 485 person was analyzed with a questionnaire survey. The questions asked about ideal visiting time at the postpartum ward, postpartum organisational structures and appropriate time for discharge., Results: The average age of the examined sample was 25.2 years; 32.4% were still students, 52.6% were state employees. A very early discharge (within 6-8 hours) was suggested only by 1.2% of the sample. The ratio of those who wanted to leave the hospital at less than 24 hours postpartum was only 7.8% as well. At the same time, only one third (35.9%) of the participants would like to be discharged after 4 days after delivery. 57.7% of the whole sample would prefer discharge between day 2 and 4 after delivery. The opinions about the time of discharge are independent from the size of the family, martial status, sex, education and monthly income of the interviewees. The patients' preferences concerning the appropriate time for discharge following an uncomplicated pregnancy and normal vaginal delivery is mainly influenced by the number of children (chi 2 = 62.260; df = 20; p = 0.000) and previous deliveries (chi 2 = 57.803; df = 20; p = 0.000). Early discharge was most commonly voted for by those who had no children or by those who had many., Conclusions: When organising postpartum care, the opinion and expectations of the population should be taken into account as well. Almost 62% of the sample had no children at the time of the interview. They will be in need of obstetric care in the future as well. Because of this, their opinion is especially important.
