The initiative for the development of national treatment guidelines, dates back to the '90s. In Greece, however, National Clinical Guidelines for the management of schizophrenia were first formulated in 2014 when a Working Group was set up for this purpose by the Greek Ministry of Health. The objective of this Working Group was to provide evidence-based recommendations covering the pharmacological and psychosocial treatment of schizophrenia as well as the development of appropriate treatment services. The Working Group utilized the NICE Guideline (National Institute of Clinical Excellence, 2010, 2014) for the management of Psychosis & Schizophrenia as the main guide to develop the Greek National Guidelines , and in addition the American Psychiatric Association (APA) Practice Guideline for the Treatment of Patients with Schizophrenia (APA 2004), the Royal Australian and New Zealand College of Psychiatrists clinical practice guidelines for the treatment of schizophrenia and related disorders (Royal Australian and New Zealand College of Psychiatrists 2005) , as well as other relevant sources. The Working Group also took into consideration the available Greek bibliography as well as the external evaluations of the Greek psychiatric reform programs. A special effort was made to adapt the international experience to the current Greek landscape with the constraints resulting from the uneven dispersion of mental health services, the lack of coordination between services, the incomplete sectorization of mental health services provided as part of the National Health System, the still underdeveloped Primary Care Health Service, and last but not least the difficult economic situation in Greece. After the preparation of the draft guidelines, a thorough consultation followed with the relevant stakeholders, including mental health professionals, user associations and representatives of the Greek Ombudsman. Additionally, the Hellenic Psychiatric Association established an Expert Committee in Spring 2016, that contributed with its observations to the final exercise. This article will be followed by two further publications (incorporating the Hellenic Psychiatric Association comments) which include: (1) the summary recommendations regarding the pharmacological, psychological and psychosocial interventions in the treatment of schizophrenia, and (2) the summary recommendations regarding: (a) the role of primary health care service and the role of the family physician in the treatment of schizophrenia (b) the summary recommendations regarding continuity of care and c) the summary recommendations regarding community interventions with documented effectiveness in the treatment of schizophrenia.

Για τη διερεύνηση του στόχου, επιλέχθηκε το θεωρητικό και εμπειρικό πλαίσιο της ανάλυσης Εισροών-Εκροών, η οποία έχει το πλεονέκτημα να συμπεριλαμβάνει στις εξεταζόμενες μεταβλητές, την τεχνολογία παραγωγής ενός οικονομικού συστήματος, όπως αυτή εκφράζεται από τις διακλαδικές συναλλαγές του. Παράλληλα, διερευνώνται η διάρθρωση της παραγωγής και της απασχόλησης της οικονομίας, στα πλαίσια του ίδιου υποδείγματος. Στην κατεύθυνση αυτή, παρουσιάζεται αρχικά το θεωρητικό και αναλυτικό πλαίσιο του υποδείγματος εισροών-εκροών, καθώς και η εμπειρική του εξειδίκευση, μέσω της οποίας διερευνώνται οι διαρθρωτικές σχέσεις παραγωγής και απασχόλησης των κλάδων της ελληνικής οικονομίας. Στη συνέχεια, για τη μέτρηση της παραγωγικότητας της εργασίας και των διαρθρωτικών της χαρακτηριστικών προτείνεται ένα μοντέλο, στο πλαίσιο της ανάλυσης εισροών-εκροών, με την εφαρμογή του οποίου εκτιμάται το μέγεθός της σε σχέση και με την τεχνολογία παραγωγής της οικονομίας, όπως αυτή εκφράζεται από την αντίστροφη μήτρα του Leontief. Ενώ, παράλληλα, στη βάση του εν λόγω πλαισίου, προσαρμόζεται και επεκτείνεται κατάλληλα η μέθοδος του Δομικού Διαχωρισμού Παραγόντων προκειμένου να εκτιμηθεί και να αξιολογηθεί η συμβολή των προσδιοριστικών παραγόντων της παραγωγικότητας της εργασίας στη διαμόρφωσή της. Η διερεύνηση γίνεται για την ελληνική οικονομία, την περίοδο 1995-2010., The objective of the present dissertation is the investigation of labour productivity and its impact factors, in light of the structural relations developed within an economy. For the investigation, the theoretical and empirical framework of Input-Output Analysis is selected, which has the advantage to include in the examined variables, the production technology of the economic system, as expressed by its intersectoral transaction. The economic structure (and more specific the structure of output and employment) is investigated in the same framework. In the latter direction, at first the theoretical and analytical framework of input-output analysis and its empirical specialization are presented. Then, for the measurement of labour productivity and its structural characteristics a model, in the context of input-output methodologies, is suggested. The model estimates the relation of labour productivity with the production technology of the economy, as the last is expressed by Leontief’s inversed matrix. Finally, through the expansion and adaptation of Structural Decomposition Analysis, the contribution of the determinants of labour productivity is assessing and evaluating. The specific methodologies are applied for the Greek economy, over the period 1995-2010.

Ο εν τω βάθει ηλεκτρικός εγκεφαλικός ερεθισμός (DBS) αποτελεί μία ευρέως αποδεκτή και πολύ αποτελεσματική θεραπευτική μέθοδο για ασθενείς με φαρμακοανθεκτική ν. Πάρκινσον. Διάφορες μελέτες έδειξαν ότι το DBS στον υποθαλάμιο πυρήνα (STN) οδηγεί σε αύξηση του σωματικού βάρους, ο μηχανισμός της οποίας παραμένει άγνωστος. Τα τελευταία χρόνια διατυπώθηκαν διάφορες θεωρίες για τον πιθανό μηχανισμό αυτής της αύξησης βάρους. Σύμφωνα με μία από τις πιο ενδιαφέρουσες θεωρίες, η αύξηση βάρους οφείλεται σε διαταραχή του μηχανισμού ρύθμισης της λήψης τροφής σε υποθαλαμικό επίπεδο. Είναι γνωστό ότι ο υποθάλαμος κατέχει κεντρικό ρόλο στη ρύθμιση της ομοιόστασης της ενέργειας: δέχεται, επεξεργάζεται και ερμηνεύει ορεξιογόνα και ανορεξιογόνα σήματα όπως η γκρελίνη, το ΝΡΥ και η λεπτίνη. Σκοπός της παρούσας μελέτης ήταν η διερεύνηση της πιθανής συμμετοχής του ηλεκτρικού ερεθισμού του υποθαλάμιου πυρήνα στη ρύθμιση της ομοιόστασης της ενέργειας, μέσω της διαταραχής των ορεξιογόνων και ανορεξιογόνων πεπτιδίων γκρελίνη, ΝΡΥ και λεπτίνη. Για το σκοπό αυτό μελετήθηκαν 23 από τους ασθενείς με ν.Πάρκινσον που υποβλήθηκαν σε STN DBS στην κλινική μας (15 άντρες-8 γυναίκες, ηλικία: 65,2 ± 8,9χρόνια, διάρκεια νόσου:12,7 ± 6χρόνια). Κάθε ασθενής εξετάστηκε σε 3 διαδοχικές χρονικές στιγμές: 3 μέρες πριν το χειρουργείο, 3 και 6 μήνες μετά το χειρουργείο και υπεβλήθη σε μέτρηση του σωματικού βάρους και του BMI, λιπομέτρηση και μέτρηση των επιπέδων γκρελίνης, λεπτίνης, NPY και κορτιζόλης ορού. Τρεις μέρες πριν και 6 μήνες μετά το χειρουργείο, πραγματοποιήθηκε κλινική εκτίμηση των ασθενών με τη χρήση των: Unified Parkinson’s Desease Rating Scale (UPDRS), Schwab and England Scale και Hoehn Yahr scale, καθώς και υπολογισμός της ημερήσιας δόσης ντοπαμίνης (LEDD). Τα αποτελέσματα της μελέτης μας συνοψίζονται ως εξής: 3 μήνες μετά τη χειρουργική επέμβαση διαπιστώθηκε σημαντική αύξηση βάρους των ασθενών μας: (3.09±5.00kg, P=0.007), χωρίς περαιτέρω αύξηση στους 6 μήνες. Τα επίπεδα του ΝΡΥ στο περιφερικό αίμα αυξήθηκαν σημαντικά 3 μήνες μετά το χειρουργείο (p=0.05), ενώ τα επίπεδα της γκρελίνης αυξήθηκαν σημαντικά στους 6 μήνες (p=0.001). Η αύξηση του σωματικού βάρους συσχετίστηκε σημαντικά με τη μεταβολή των επιπέδων της γκρελίνης και της λεπτίνης στους 3 και 6 μήνες αντίστοιχα. Συμπερασματικά μπορούμε να πούμε ότι το STN DBS φαίνεται να προκαλεί μία προσωρινή δυσλειτουργία της υποθαλάμιας έκκρισης ΝΡΥ και γκρελίνης. Η μεταβολή του σωματικού βάρους μπορεί να αποδοθεί στην αυξημένη έκκριση γκρελίνης και λεπτίνης. Περαιτέρω μελέτες με μεγαλύτερο αριθμό ασθενών απαιτούνται για να επιβεβαιωθεί ο ρόλος της πεπτιδιακής δυσλειτουργίας στην αύξηση βάρους μετά τη νευροδιέγερση και για να διερευνηθεί η πιθανή νευροπροστατευτική δράση που το DBS μπορεί να ασκήσει μέσω της αύξησης των επιπέδων της γκρελίνης. Deep brain stimulation (DBS) is a widely accepted and highly effective treatment method for patients with medically refractory idiopathic Parkinson's desease. Various studies have shown that DBS of the subthalamic nucleus (STN) results in increased body weight, the mechanism of which is still unknown. In recent years there were various theories as to the possible mechanism of this weight gain. According to the most interesting theory, weight gain is due to a disruption of the central mechanism that regulates food intake. It is known that the hypothalamus plays a central role in the regulation of energy homeostasis: it receives, processes and interprets orexigenic and anorexigenic signals such as ghrelin, NPY and leptin. The aim of this study was to investigate the possible involvement of STN DBS in the regulation of energy homeostasis, through the disruption of orexigenic and anorexigenic peptides ghrelin, leptin and NPY. Twenty three patients with idiopathic Parkinson’s desease who underwent STN DBS in our clinic were included in our study (15 males - 8 females, age : 65,2 ± 8,9 years, disease duration : 12,7 ± 6chronia ). Each patient was examined at three consecutive time points: 3 days before surgery, 3 and 6 months after surgery. At each clinical appointment all patients underwent body composition measurements including body weight, body mass index (BMI) and fat mass, as well as blood sampling for the measurement of the circulating levels of ghrelin, leptin, NPY and cortisol. Three days before and 6 months after surgery patients were clinically evaluated with the use of the Unified Parkinson's Desease Rating Scale (UPDRS), Schwab and England Scale and Hoehn Yahr scale and the L-dopa daily dose (LEDD) was recorded. The results of our study are summarized as follows : 3 months after surgery there was a significant increase of body weight: (3.09 ± 5.00kg, P = 0.007), with no further increase at 6 months. NPY levels increased significantly 3 months after surgery (p = 0.05), while ghrelin levels increased significantly at 6 months (p = 0.001). Weight gain was significantly correlated with the change of ghrelin and leptin levels at 3 and 6 months respectively. In conclusion, STN DBS seems to temporarily dysregulate the hypothalamic secretion of NPY and ghrelin and weight gain can be attributed to the increased secretion of leptin and ghrelin. Further studies with a larger number of patients are required to confirm the role of peptide dysfunction on weight gain after neurostimulation and to investigate the possible neuroprotective role of DBS, exerted through the increase of ghrelin levels.

Σκοπός της παρούσας διδακτορικής διατριβής είναι η ανάπτυξη αλγορίθμων επεξεργασίας ιατρικής εικόνας για την ενσωμάτωση τους σε ιατρικές εφαρμογές ακτινοθεραπευτικού ενδιαφέροντος. Οι αλγόριθμοι αυτοί στηρίζονται στην αρχή λειτουργίας των χαρτών αυτο-οργάνωσης Kohonen και αξιοποιούν την πληροφορία που περιέχεται σε περιοχές των εικόνων γύρω από σημεία ενδιαφέροντος, ώστε να εντοπίσουν αυτόματα, με ακρίβεια και αξιοπιστία, αντιστοιχίες μεταξύ των εικόνων. Πιο συγκεκριμένα, ένας επαναληπτικός αλγόριθμος προτείνεται για την αυτόματη εύρεση αντίστοιχων σημείων σε ιατρικές εικόνες δύο διαστάσεων. Ο προτεινόμενος αλγόριθμος προϋποθέτει την εύρεση σημείων ενδιαφέροντος μόνο στη μια από τις δύο εικόνες και εντοπίζει τα αντίστοιχα σημεία στη δεύτερη εικόνα μέσα από μια επαναληπτική διαδικασία, η οποία προσομοιάζει τη φάση εκπαίδευσης του νευρωνικού δικτύου. Με βάση τα ζεύγη των αντίστοιχων σημείων, υπολογίζονται στη συνέχεια οι παράμετροι ενός μετασχηματισμού, κατάλληλου για να περιγράψει τη σχέση μεταξύ των δεδομένων εικόνων. Ο αλγόριθμος ευθυγράμμισης εφαρμόζεται σε δεδομένες εικόνες ηλεκτρονικής πυλαίας απεικόνισης (Electronic Portal Images), που λαμβάνονται πριν από κάθε συνεδρία της ακτινοθεραπείας, για τον υπολογισμό του σφάλματος τοποθέτησης του ασθενούς. Το ζήτημα της επαλήθευσης της θέσης του ασθενούς στην ακτινοθεραπεία αντιμετωπίζεται επίσης με τη βοήθεια μιας αυτόματης μεθόδου εύρεσης αντίστοιχων σημείων σε τρισδιάστατα δεδομένα, η οποία εφαρμόζεται για την ευθυγράμμιση της αξονικής τομογραφίας του σχεδιασμού της ακτινοθεραπείας και μιας αξονικής τομογραφίας επαλήθευσης, που λαμβάνεται πριν την πρώτη συνεδρία της ακτινοθεραπείας. Ο προτεινόμενος αλγόριθμος εντοπίζει αντίστοιχα σημεία ενδιαφέροντος στις δεδομένες τομογραφικές εικόνες και υπολογίζει τις παραμέτρους ενός μη γραμμικού μετασχηματισμού ευθυγράμμισης. Μετά την ευθυγράμμιση των δύο τομογραφιών, υπολογίζεται η μετατόπιση του ισοκέντρου στην τομογραφία επαλήθευσης σε σχέση με τη θέση του ισοκέντρου που προβλέπεται στην αρχική τομογραφία του σχεδιασμού. Με την ενσωμάτωση αυτής της μεθόδου ευθυγράμμισης στη διαδικασία της ακτινοθεραπείας, ικανοποιούνται δύο ανάγκες της κλινικής πρακτικής. Αφενός, η μετατόπιση του ισοκέντρου, όπως υπολογίζεται από την προτεινόμενη μέθοδο, παρέχει μια αξιόπιστη ένδειξη για τη μετατόπιση του ασθενούς που απαιτείται πριν τη χορήγηση της ακτινοβολίας. Αφετέρου, επιχειρείται η καλύτερη αξιοποίηση των πόρων του τμήματος της ακτινοθεραπείας με τη διαδικασία της εύρεσης του ισοκέντρου της ακτινοθεραπείας να λαμβάνει χώρα στην αίθουσα του αξονικού τομογράφου και να μειώνεται συνεπώς ο χρόνος που απαιτείται για την προετοιμασία του ασθενούς στον γραμμικό επιταχυντή κατά την πρώτη συνεδρία της ακτινοθεραπείας. Aim of the present thesis is the development of image processing algorithms for radiotherapy applications. These algorithms are based on the principles of Kohonen Self Organizing Maps and exploit the information contained in image regions around distinctive points of interest, in order to determine image correspondences in an automatic, accurate and robust way. In particular, an iterative algorithm is proposed for automatic detection of point correspondences in two-dimensional medical images. The proposed algorithm requires the extraction of interest points only in one image and detects the homologous points in the second image through an iterative procedure, respective to the training phase of a neural network. Subsequently, the parameters of an appropriate registration transformation are computed to describe the mapping between the two images. The computation is based on the detected point correspondence. The proposed registration algorithm is applied to Electronic Portal Images, acquired prior to the radiotherapy treatment delivery, in order to estimate the setup error of the patient. The issue of patient position verification in radiotherapy is also addressed in the present thesis by developing an algorithm for automatic detection of point correspondences in three-dimensional medical data. The algorithm is used to register the CT data of radiotherapy planning to an additional verification CT, acquired prior to the first treatment fraction. The proposed algorithm detects corresponding points in the two CT images and computes the parameters of a non-rigid registration transformation. After the registration of the two CT images, the isocenter displacement of the verification CT is calculated with respect to the ideal isocenter position, defined in the planning CT. By integrating the proposed registration procedure in the clinical practice, two needs are met. Firstly, the isocenter displacement, calculated by the proposed method, provides a reliable indication of the patient shift, needed before the treatment delivery, for optimization of the dose delivery. Secondly, an improvement of the radiotherapy department efficiency is attempted by performing the procedure of isocenter marking in the CT scanner room and, consequently, reducing the time expenditure of the patient in the LINAC during the first radiotherapy fraction.
