Die Trikuspidalklappeninsuffizienz (TI) hat eine zunehmende Prävalenz und kann mit einer erhöhten Mortalität einhergehen. Sie kann primär auftreten, ist aber überwiegend sekundäre Folge einer Erkrankung des linken Herzens. Dabei kommt es meistens zu einer Dilatation des Trikuspidalklappenanulus (sog. funktionelle TI). Neben konservativen stehen chirurgische Therapieansätze zur Option für die Behandlung der TI. Noch relativ neu sind dagegen minimalinvasive Verfahren. Diese sind vor allem für Patienten mit hohem perioperativem Risiko bei chirurgischen Eingriffen eine Alternative. Ein Ansatz ist dabei unter anderem die kathetergestützte Implantation von Klappenprothesen in die obere und untere Hohlvene. Eine Limitation ergibt sich dabei durch die vorgegebene maximale Größe der zum Zeitpunkt der Arbeit verfügbaren Klappenprothesen. Die Implantation von Klappenprothesen erfolgt an der 'Klinik mit Schwerpunkt Kardiologie und Angiologie' der 'Charité - Universitätsmedizin Berlin: Campus Charité Mitte' zum Zeitpunkt der Arbeit bis zu einem Diameter der Hohlvene von maximal 32 mm (entsprechend einem Umfang von 100,5 mm). Dabei war zunächst die Implantation von Stents erforderlich, unter anderem um das Lumen der Hohlvenen einzuengen. In dieser retrospektiven Arbeit wurde daher bei Patienten mit und ohne TI der Umfang der oberen und unteren Hohlvene in der Computertomographie untersucht mit dem Ziel, herauszufinden, in welchen Fällen aus anatomischen Gesichtspunkten dieser Therapieansatz eine Option ist. Zum anderen sollen Daten zur Verfügung gestellt werden, um die Entwicklung neuer Klappenprothesen zu ermöglichen. Es zeigte sich, dass mit zunehmendem Schweregrad der TI der Umfang der Hohlvenen zunimmt. Ab einer mittelschweren TI nimmt dabei der Anteil der Patienten zu, bei denen die Implantation von Klappenprothesen in die Hohlvenen aufgrund einer Dilatation der selbigen nicht mehr möglich ist. Die Implantation der Klappenprothesen sollte rechtzeitig erfolgen und nicht hinausgezögert werden. Eine Vergrößerung des Durchmessers der größten aktuell verfügbaren Klappenprothese um 6 mm (von 29 mm auf 35 mm) ermöglicht die Implantation bei 84 % der Patienten mit einer schweren TI (statt 61 %)., The tricuspid insufficiency (TI) has an increasing prevalence and can be associated with increased mortality. One approach among these is the catheter- supported implantation of valve prostheses into the Venae cavae. Limitation results from the predefined maximum size of the available valve prostheses at the time of this thesis. The implantation of valve prostheses takes place at the 'Clinic with focus cardiology and angiology' of the 'Charité - Universitätsmedizin Berlin: Campus Charité Mitte' up to a diameter of the Venae cavae of a maximum of 32 mm (equal to a circumference of 100,5 mm). However, the implantation of stents prior to the implantation of the valve prostheses was necessary, amongst others, to narrow the lumen of the Venae cavae. In this retrospective study, the circumference of the Vena cava superior et inferior was investigated in patients with and without TI using computer tomography with the aim of identifying patients, who, from an anatomical point of view, could receive this therapy approach. Furthermore, data should be made available to enable the development of new valve prostheses. It was found that with increasing severity of TI, the circumference of the Venae cavae increases. From a moderate TI the proportion of patients grows, in which the implantation of valve prostheses into the Venae cavae is no longer possible due to a dilatation of the same. Implantation of the valve prostheses should be done in time and not delayed. An enlargement of the diameter of the largest currently available valve prostheses by 6 mm (from 29 mm to 35 mm) allows implantation in 84 % of patients with severe TI (instead of 61 %).
