Soft tissue defects of the foot are very common sequelae after trauma and require an individual reconstructive approach.Recommendations for the treatment of soft tissue injuries to the foot are given.The criteria of soft tissue reconstruction, postoperative follow-up and complications are first discussed before the therapeutic approach is explained depending on the reconstruction site. Case examples are given for illustration.Decision making for soft tissue reconstruction of the foot is based on the location, the 3‑dimensional extent of the defect, the patient requirements and concomitant diseases. Standardized treatment algorithms are usually applied that need to be adapted according to individual patient factors. Randomized and local pedicled flaps can be applied for foot reconstruction; however, these options involve a significant risk of complications. Consequently, free flaps are frequently indicated after appropriate preoperative diagnostics of the perfusion of the foot. Due to the vast variety of donor sites, free flaps allow an individualized reconstruction, which is adapted to local and patient requirements.Precise preoperative reconstructive planning and analysis of the vascularization form the foundation for a successful soft tissue reconstruction of the foot. The aims of the individualized approach to soft tissue reconstruction of the foot are stable soft tissue coverage, resistance to weight bearing of the sole of the foot, the ability to wear normal shoes and maintenance of sensibility.HINTERGRUND: Weichteilverletzungen am Fuß nach Traumata sind sehr häufig und bedingen eine individuelle Rekonstruktion.Es werden Empfehlungen zur Therapie von Weichteilverletzungen des Fußes formuliert.Zunächst werden die Kriterien, postoperative Nachbehandlung und Komplikationen der Hautweichteilrekonstruktion des Fußes erörtert, bevor die therapeutische Strategie entsprechend der Lokalisation erläutert und mit exemplarischen Beispielen aus dem eigenen Patientengut verdeutlicht wird.Entsprechend der Lokalisation, der dreidimensionalen Ausdehnung des Defekts, dem Anforderungsprofil des Patienten und den vorhandenen Begleiterkrankungen wird das Vorgehen anhand standardisierter rekonstruktiver Prinzipien geplant und den individuellen Patientenfaktoren angepasst. Lokale und gestielte Lappenplastiken können am Fuß eingesetzt werden, haben aber eine vergleichsweise erhöhte Komplikationsrate, sodass nach entsprechender präoperativer Abklärung der Gefäßsituation am Fuß häufig freie Lappenplastiken angewandt werden, die aufgrund ihrer Versatilität sehr individuell an die lokalen Anforderungen und die Patientenbedürfnisse adaptierfähig sind. Freie Lappenplastiken erfordern postoperativ ein Lappentraining.Eine präzise präoperative Planung der Rekonstruktion und Abklärung der Vaskularisation bilden die Grundlagen für eine erfolgreiche Weichteilrekonstruktion des Fußes, deren Ziele das Erreichen von stabilen Hautweichteilen, einer belastbaren Fußsohle ohne Scherkraftwirkung, das Ermöglichen der Schuhversorgung und der Erhalt der Sensibilität sind.

Nurses working in oncology use a wide range of naturopathic interventions in their daily practice to alleviate symptoms and improve the quality of life of oncological patients. However, there is no external evidence for many of these interventions. Due to a lack of scientific studies in the field, the aim of the project described here is to develop a standardized procedure to generate evidence on naturopathic interventions, on the basis of which recommendations may be derived for nursing practice.The systematic procedure presented here was developed by the working group (WG) Integrative Nursing in Oncology over a period of four years in an iterative process. This process is based on the expert panel members' experience with the development of guidelines and/or quality instruments such as practice standards.The systematic methodological approach presented here consists of three successive steps where internal and external evidence have been combined: a scoping review, a structured consensus process with oncology nurses to collect and evaluate naturopathic interventions, and finally a further supplementary literature review based on additional findings of the consensus conference. The procedure was successfully carried out for mucositis, insomnia, fatigue, hand-foot syndrome and chemotherapy-induced polyneuropathy.Through the step-by-step synthesis of internal evidence and the best available external evidence, the evidence base for naturopathic nursing interventions in oncology can be generated and practice recommendations derived. The procedure represents a successful theory-practice transfer through structured cooperation between (nursing) scientists and nursing practitioners. In terms of the AWMF classification, the practice recommendations developed in the process described fall somewhere in between an S1 guideline (informal consensus of an expert group) and an S2e guideline (evidence-based).

Deep Infiltrating Endometriosis - a Systematic Approach to Diagnostics and Surgery Planning

Die im Rahmen dieser Arbeit durchgeführten Studien zeigen, dass Produktentwickler den Einsatz von Vorausschau als äußerst relevant ansehen, jedoch diese nicht ausreichend systematisch zur Findung und Validierung von Produktprofilen und Produktideen einsetzen. Dies ist unter anderem mit einem fehlenden Vertrauen in die generierten Vorausschauergebnisse begründet, sowie dadurch, dass diese häufig zu komplex sind und weniger auf konkrete Suchfelder mit Innovationspotential aufmerksam machen. In dieser Arbeit wird deshalb eine Systematik vorgestellt, die den Menschen im Mittelpunkt der Produktentwicklung (Albers, 2010), als kreativen Problemlöser der Technik zur Identifikation von Produktprofilen mit hohem Innovationspotential durch Vorausschau befähigt. Die Systematik folgt dabei dem Verständnis nach Dörner (1979), dass das Ziel einer jeden Produktentwicklung ist, einen unerwünschten Istzustand in einen erwünschten, zukünftigen Sollzustand zu überführen. Weiterhin folgt die Systematik basierend auf dem Verständnis des Modells der PGE – Produktgenerationsentwicklung nach Albers, dass jede Entwicklung auf Basis von Referenzen erfolgt. Das erste Modul der Systematik ist die Analyse des Istzustands. Dazu werden am Markt befindliche Referenzprodukte bezüglich deren heute relevanten Produkteigenschaften analysiert. Das zweite Modul ist die Synthese des Sollzustands durch Vorausschau zur Identifikation zukünftig relevanter Produkteigenschaften. Die Delta-Analyse ist das dritte Modul, bei dem ein Abgleich zwischen Ist- und Sollzustand erfolgt und eine Roadmap zur zielgerichteten Überführung vom Ist- in den Sollzustand abgeleitet sowie Suchfelder mit hohem Innovationspotential identifiziert werden. Das Nutzenversprechen der Systematik ist das Generieren von Wissen um zukünftig relevante Produkteigenschaften, das Richten der Kreativität der Produktentwickler auf Suchfelder mit hohem Innovationspotential, eine Priorisierung von Entwicklungsumfängen sowie eine strukturierte, dokumentierte und dadurch nachvollziehbare Entscheidungsfindung. Die durchgeführten Evaluationsstudien zur Anwendung der Systematik zeigen, dass Produktentwickler durch Anwendung der Systematik besser in der Lage sind, Vorausschauergebnisse für die Produktentwicklung zu nutzen. Dies zeigt sich insbesondere in den quantitativen Evaluationsnachweisen in der zweiten deskriptiven Studie: Zum einen steigt mit zeitlicher Exponierung der Probanden mit der Systematik der Erfüllungsgrad der identifizierten Erfolgsfaktoren. Zum anderen kann gezeigt werden, dass das lnnovationspotential der mit der Systematik generierten Produktprofile höher ist, als durch den bisher eher unstrukturierten Einsatz von Methoden der Vorausschau in der Produktentwicklung.

Recently, there has been a significant change in the management of iatrogenic gastrointestinal perforation from surgery towards primary endoscopic therapy. All perforations occurring in all consecutive endoscopies from 1/1/2014 to 12/31/2017 in our hospital (Klinikum St. Marien Amberg, Germany) were recorded, evaluated and followed up prospectively. In-house SOPs were designed and communicated with all physicians within our hospital. Endoscopic closure of the perforation was primarily attempted, always in consent with the abdominal surgeon. In total, we observed 24 perforations in 18 627 consecutive endoscopies (0.13 %). There were also 24 cases of free extraluminal gas without perforation (12 post-polypectomy-syndromes und 12 post-ERCP with papillotomy). Diagnosis of perforation could be established within 12 hours in 95.8 % (23/24) (in 20 cases during endoscopy). Initial therapeutic approach was surgical in 3 cases, conservative in 3 cases and interventional-endoscopic closure of perforation in 17 cases (4 × Clips, 10 × OTSC, 3 × SEMS). In 1 case, no therapy was performed. Mortality was 4.2 % (1/24). In 3 cases, the patient had to be operated on secondary to endoscopic therapy. In summary, surgical therapy was necessary in 6 of 24 cases (25 %). Interventional-endoscopic therapy was successful technically in 94.1 % (16/17) and clinically in 87.5 % (14/16). Primary interventional-endoscopic closure of iatrogenic gastrointestinal perforation is a safe and successful option in the everyday practice of a secondary referral hospital. The most important factor is prevention of delay until closure of perforation. Also, interdisciplinary consensus between endoscopist and surgeon is essential. Based on our own data, we developed and introduced a system for documentation and management of all endoscopic complications in endoscopy called "KEMS", which could be successfully integrated in our IT-system. Zurzeit ist ein Paradigmenwechsel bei der Therapie der iatrogenen gastrointestinalen Perforation hin zu einer primär endoskopischen Versorgung festzustellen. Alle Perforationen im Rahmen aller konsekutiven gastrointestinalen Endoskopien von 01.01.2014 bis 31.12.2017 im Klinikum St. Marien Amberg wurden erfasst, dokumentiert und evaluiert. Hausinterne SOPs zum Prozedere wurden erstellt und kommuniziert. Grundsätzlich wurde der primäre interventionell-endoskopische Perforationsverschluss angestrebt, stets im interdisziplinären Konsens mit der Viszeralchirurgie. Es kam im Beobachtungszeitraum zu 24 Perforationen bei 18 627 konsekutiven Endoskopien (0,13 %). Fälle mit bildgebendem Nachweis freier Luft ohne erkennbare Perforation waren gleich häufig wie Perforationen (12 Post-Polypektomiesyndrome und 12 Ereignisse nach ERCP mit EPT). Die Diagnose der Perforation erfolgte zu 95,8 % (23/24) innerhalb 12 h (davon 20 bereits bei der Endoskopie). Die initiale Therapiemodalität war in jeweils 3 Fällen primär operativ bzw. primär konservativ und in 17 Fällen primär endoskopisch (4 × Clips, 10 × OTSC, 3 × SEMS). In 1 Fall wurde keine Therapie durchgeführt. Es war eine Letalität von 4,2 % (1/24) zu verzeichnen. In 3 Fällen war sekundär ein operatives Vorgehen erforderlich, sodass insgesamt in 25 % (6/24) eine chirurgische Therapie erfolgte. Die interventionell-endoskopische Therapie war technisch erfolgreich zu 94,1 % (16/17) bei einem klinischen Erfolg nach Perforationsverschluss von 87,5 % (14/16). Das Konzept der interventionell-endoskopischen Therapie iatrogener Perforationen ist im klinischen Alltag sicher und erfolgreich umsetzbar. Entscheidende Erfolgsfaktoren sind die kurze Zeitspanne bis zum Verschluss der Perforation und der stets anzustrebende interdisziplinäre Konsens im Vorfeld der Therapie. Auf der Basis der Ergebnisse wurde ein umfassendes Komplikationserfassungs- und Managementsystem (KEMS) für die Endoskopie ab 2016 in unserer Klinik etabliert (integriert in das Befunderstellungssystem und das Klinikinformationssystem).

In his work "The Liberal Archipelago" (2003) Chandran Kukathas designs a peacefull coexistence of different cultural communities. Three basic freedoms determine this liberal society: freedom of association, freedom of dissociation and mutual toleration of associations. To examine whether his specific conception of the "exit option" is also applicable for children and teens was the intention of my dissertation. I started my analysis from the perspective of political philosophy, bringing a mainly British-American discourse to Germany. I added a systematization of cases of children and youth trying to exit to illustrate the conflicts underlying exit and to show the obstacles of their exit. In correlation to the philosophical discussion of "private" and "public", the conclusion of my dissertation states that exit is only available if there exists a specifically designed "wider society". This is based on my concept of an "integrative multiculturalism" where minorities and wider community are linked by an interculturally opened public sphere.

Diffuse hair loss is a common complaint and cause of significant emotional distress particularly in women. The best way to alleviate the anxiety is to effectively treat the hair loss. It is paramount to address the symptom systematically. In addition to its psychological impact, hair loss may be a manifestation of a more general medical problem. The diagnosis can be established with a detailed patient history focussing on chronology of events, examination of the scalp and pattern of hair loss, a pull test with examination of bulbs of shed hairs, trichoscopy, and few pertinent screening blood tests. In selected cases a scalp biopsy may be required. The most important differential diagnoses include acute and chronic telogen effluvium, female pattern hair loss, and diffuse alopecia areata. Occasionally, patients seeking advice are not necessarily losing hair. In the absence of convincing evidence of hair loss, they are suffering of psychogenic pseudoeffluvium, and thought should be given to an underlying psychological disorder. Once the diagnosis is established, treatment appropriate for that diagnosis is likely to control the hair loss. Finally, appropriate psychological support and education about the basics of the hair cycle, and why considerable patience is required for effective cosmetic recovery, are essential to help limit patient anxiety.

Systematic reviews and effectiveness analyses in the context of evidence-based medicine are not free of personal or group related values and interests. Effectiveness analysis has to deal with various "value judgments" concerning questions like 'what are relevant study endpoints and adequate methodologies for context-related data analysis?' or 'which deviation from the desirable und feasible methodology in clinical research is still acceptable?' Value judgments regarding these questions have a considerable influence on the process of effectiveness analysis and the presentation of its results. A sound and responsible approach to the necessary projects of evidence-based medicine requires a methodology for identifying their implicit normativity and making it more explicit. This article presents a checklist for the identification of central value judgments in effectiveness analyses and describes their role within the ethics of evidence-based medicine. Furthermore, the need for a broader concept of transparency--as a consequence of the implicit value judgments--will be discussed.

Public health and nursing meet mainly in the fields of care managing processes and health promotion. Since there is a new development of Public Health in the German speaking countries also the role of nurses in health education and health promotion has become more important. But traditional fields and new conceptions of public health nursing are still unmethodical. I present a systematic approach of different fields and different strategies of health promotion in nursing which has to be discussed further on. Health promotion in nursing is differentiated in a behavioural approach (empowerment of patients on different levels of nursing) and an approach which focuses on the conditions for health, mainly those for better social support, for better health in the community and in other settings.

As there is no single, simple and reliable laboratory method to test the entire hemostasis the evaluation of patients with hemorrhagic complications remains a multistep process. A detailed history and clinical examination is central in the planning and interpretation of laboratory tests. The present work emphasizes on history taking and guides general practitioners in choosing and interpreting appropriate screening tests.
