Background: Health Behaviour in School-aged Children (HBSC) is one of the largest international studies on child and adolescent health and cooperates with the World Health Organization (WHO). In Germany, adolescents aged 11, 13 and 15 are surveyed every four years about their health, health behaviour and social conditions. This article describes the HBSC study and in particular the methodology of the current 2022 survey and prior surveys conducted between 2009/10 and 2017/18. Method: 174 schools with a total of 6,475 students participated in the 2022 survey. The survey was conducted using questionnaires and covered a wide range of topics (including mental health, physical activity, bullying experiences, social determinants of health and experiences related to COVID-19). The 2022 survey was complemented by a school principal survey (N = 160). In addition to the current sample, the samples of the three previous surveys with representative data for Germany are presented: 2009/10 (N = 5,005), 2013/14 (N = 5,961) and 2017/18 (N = 4,347).: Discussion The health of children and adolescents is of great public health importance. The HBSC study makes a substantial contribution by providing internationally comparable results, analysing trends, and providing stakeholders with comprehensive and representative health monitoring data.

Krebs ist eine der häufigsten Todesursachen weltweit und wird im Jahr 2020 für fast 10 Millionen Todesfälle verantwortlich sein. Im selben Jahr wurden weltweit etwa 19 Millionen neue Krebsfälle diagnostiziert [1]. Die Inzidenz aller Krebsarten ist zwischen 1998 und 2012 in den meisten Ländern und in allen Altersgruppen weltweit gestiegen [2]. Zumindest in Europa, den USA und einem Teil Asiens entsprach dieser Trend jedoch den demografischen Veränderungen und der Alterung der Bevölkerung [3,4]. Das Krebsrisiko nimmt mit dem Alter zu, und mehr als 60% der Krebserkrankungen werden bei Menschen im Alter von 65 Jahren oder älter diagnostiziert [5]. Jüngste Studien deuten jedoch auf einen starken Anstieg der Inzidenz von Krebserkrankungen bei Erwachsenen unter 50 Jahren [6-15]. Bei jungen Menschen, bei denen Krebs diagnostiziert wird, ist die Krankheit zum Zeitpunkt der Diagnose oft weiter fortgeschritten als bei älteren Menschen, was zum Teil auf eine verzögerte Diagnose zurückzuführen ist [16].

6. Participation of the adult population in preventive measures for non-communicable diseases during the COVID-19 pandemic in 2020/2021

Background: In 2020/2021, the COVID-19 pandemic and the protective measures associated therewith severely limited the opportunity to participate in prevention and health promotion measures. The article examines the utilisation of the measures and possible factors that are associated with a lower participation during these pandemic years. Methods: It is based on data acquired between March and August 2021 from the study ‘COVID-19 vaccination rate monitoring in Germany’ (COVIMO), a cross-sectional telephone survey. The data was used to examine the participation in preventive measures in the last 12 months in terms of sociodemographic factors and to analyse a decreased participation with regard to pandemic-related factors. The analysis sample includes individuals aged 18 years and over (n=3,998). Results: 63% of participants generally did not use these programmes, 7% indicated an unchanged participation, 28% reported having participated in fewer measures, and 2% in more measures. Men reported significantly more often than women that they generally do not participate in prevention and health promotion measures. A relevant pandemic-related factor for decreased participation of men was the less clearly perceived comprehensibility of the regulations against the spread of SARS-CoV-2. Conclusions: Prevention and health promotion should be part of the contingency planning in epidemically significant situations to prevent a decreased participation and to promote health and gender-related equal opportunities even in a crisis.

The SARS-CoV-2 coronavirus has spread rapidly across Germany. Infections are likely to be under-recorded in the notification data from local health authorities on laboratory-confirmed cases since SARS-CoV-2 infections can proceed with few symptoms and then often remain undetected. Seroepidemiological studies allow the estimation of the proportion in the population that has been infected with SARS-CoV-2 (seroprevalence) as well as the extent of undetected infections. The ‘CORONA-MONITORING bundesweit’ study (RKI-SOEP study) collects biospecimens and interview data in a nationwide population sample drawn from the German Socio-Economic Panel (SOEP). Participants are sent materials to self-collect a dry blood sample of capillary blood from their finger and a swab sample from their mouth and nose, as well as a questionnaire. The samples returned are tested for SARS-CoV-2 IgG antibodies and SARS-CoV-2 RNA to identify past or present infections. The methods applied enable the identification of SARS-CoV-2 infections, including those that previously went undetected. In addition, by linking the data collected with available SOEP data, the study has the potential to investigate social and health-related differences in infection status. Thus, the study contributes to an improved understanding of the extent of the epidemic in Germany, as well as identification of target groups for infection protection.

At a regional and local level, the COVID-19 pandemic has not spread out uniformly and some German municipalities have been particularly affected. The seroepidemiological data from these areas helps estimate the proportion of the population that has been infected with SARS-CoV-2 (seroprevalence), as well as the number of undetected infections and asymptomatic cases. In four municipalities which were especially affected, 2,000 participants will be tested for an active SARS-CoV-2 infection (oropharyngeal swab) or a past infection (blood specimen IgG antibody test). Participants will also be asked to fill out a short written questionnaire at study centres and complete a follow-up questionnaire either online or by telephone, including information on issues such as possible exposure, susceptability, symptoms and medical history. The CORONA-MONITORING lokal study will allow to determine the proportion of the population with SARS-CoV-2 antibodies in four particularly affected locations. This study will increase the accuracy of estimates regarding the scope of the epidemic, help determine risk and protective factors for an infection and therefore also identify especially exposed groups and, as such, it will be crucial towards planning of prevention measures.

Background: Physicians are frequently not aware that patients may not be familiar with the meaning of medical terms or have limited knowledge about the location of organs. These aspects of functional health competence require particular attention when designing communication curricula for undergraduate medical students. The aim of our study was to evaluate the knowledge of laypersons about the anatomical locations of organs and the definitions of commonly used medical terms as relevant aspects of health literacy. Furthermore, we wished to compare it with the knowledge of a historical patient cohort who performed this study 50 years ago.Methods: In this replication study, multiple-choice questionnaires with simple anatomy and common medical terms which were published in 1970 were distributed among a convenience sample of lay volunteers (n=537) from the streets of Hamburg, Germany. Sociodemographic data including sex, age, highest educational school achievement, occupation in a field associated with medicine, and German as first language were also collected. The percentage of laypersons’ correct answers was compared to the percentage of correct answers of a historical patient cohort (n=234) published in 1970 to identify the development of health literacy as basis for curricular planning. Background/aim: Evidence-based practice (EBP) provides an important basis for improving both the quality of care and patient safety. Formulating a research question, searching the literature, and critical appraisal are crucial to developing evidence-based practice. The aim of this survey was to provide an overview of how these topics are integrated into bachelor's degree programs in nursing in Austria, Germany, and the German-speaking part of Switzerland. We also aimed to show how teachers implement these subjects and how they experience and assess the implementation., Method: We conducted an exploratory cross-sectional study using an online survey sent out to program directors and teaching staff of all 58 bachelor's degree programs in nursing in Austria, Germany and the German-speaking part of Switzerland. For data collection, a questionnaire was developed containing items on general teaching conditions, contents, and methods of evidence-based nursing practice, as well as on the estimated thematic interest of students. The data were analysed descriptively., Results: The program directors returned 24 questionnaires (41%). Of 75 questionnaires forwarded to the faculty, 17 (23%) were received from nine programs. On average, 5.6 teaching units (SD 2.6) are used for formulating a research question, 10 teaching units (SD 4.1) for literature review, and 11.3 teaching units (SD 6.9) for critical appraisal. Half of the teaching staff indicated that linkages between education and nursing care practice have been established. The traditional teaching method of frontal teaching is used predominantly. Student interest in topics was rated as moderate by most teachers., Conclusions: Topics on evidence-based practice are an integral part of bachelor's degree programs in nursing in German-speaking countries. An increase in teaching units, active learning methods and the growing interconnection between education and practice could improve the acquisition of competencies and attitudes of students regarding EBP and further advance its implementation in practice., (Copyright © 2024. Status of the implementation of simulation-based education in Germany: A cross-sectional study Abstract. Background: The importance of simulation-based learning in nursing education is growing. Its success depends on the qualification of facilitators and the use of standards. Up to now, there has been a lack of data on the state of implementation of simulation-based learning in nursing education in Germany. Research question: Which qualification level do simulation facilitators show in nursing education in Germany and which concepts and standards are used? Methods: A cross-sectional survey was conducted among simulation facilitators in three different educational areas. The questionnaire included socio-demographic and professional characteristics as well as qualification measures completed, and concepts and standards applied. Data were analysed descriptively by determining arithmetic means plus standard deviation (SD) or absolute and relative frequencies. Results: A total of 156 simulation facilitators (mean age 41.5 (SD 9.8) years, 74.2% female) participated. 95.4% of participants reported being (very) highly motivated for simulation-based learning. Specific simulation pedagogy training was completed by 16.8% of participants. The application of evidence-based concepts is not occurring across the board. Conclusions: Against the background of international standards, qualification needs for simulation facilitators in Germany become apparent. Increased specific, standardized continuing education and training as well as the establishment of evidence-based concepts are important for high quality in simulation facilitation.

Remaining in the nursing profession depends on vocational training and continuing education: A cross-sectional study Abstract. Background: The public discussion about nurses leaving the profession has shown increasing dynamics for years. Burdens on the caregivers, political and (vocational) educational policy mistakes and financial as well as structural framework conditions are given as reasons. However, little is known about the influence of the educational biography on remaining in the nursing profession. Aim: This study examines data on formal and alternative sources of education, the timing of educational measures and the changes of nurses' interests in education over time and thus focuses on the group of nursing staff remaining in the profession. Methods: Nurses with many years of experience in their profession were surveyed (N = 200). Results: The results show that 77.6% of long-term nurses have the minimum required school leaving certificate with average final grades. 65.3% of the nurses have attended specialized training and 74.5% vocational training, which was completed over the entire period of employment. The training topics and the sources of education change over the course of the working life. Conclusions: It turns out that the group of nursing staff who have remained in the profession for a long time is characterized by a high willingness to undertake vocational training, with and without relevance to remuneration.

The implementation of clinical assessment by nurses in Switzerland: A cross-sectional study from the nursing perspective Abstract: Background: Clinical assessment is a core competency of BSc and MSc prepared nurses in Switzerland. However, influencing factors of its implementation and the experience in the interprofessional team has been studied little so far. Objective: We used an exploratory, descriptive research design employing a cross-sectional study of the convenience sample of BScN and MScN graduates in Switzerland. 264 questionnaires were analysed quantitatively and 149 written statements as part of the survey qualitatively. Method: We used an exploratory, descriptive research design employing a cross-sectional study of the convenience sample of BScN and MScN graduates in Switzerland. 264 questionnaires were analysed quantitatively and 149 written statements as part of the survey qualitatively. Results: Respondents indicated that they routinely only use 18.9% of the 53 assessment skills they were trained in. The "respiratory system", "abdomen", "skin" and "mental status" were used most frequently. MScN graduates use respiratory assessments and BScN graduates use abdominal assessments more frequently. Lack of time and interruptions, lack of influence on patient care and specialty specific skills are hindering factors for the implementation. Better conditions at the institutional level and appreciation within the interprofessional team can make an encouraging contribution in future. Conclusion: This study illustrates, that the theory-practice transfer of clinical assessment in Switzerland needs to be further promoted.

Background: In order to preserve health and thus social participation, it is important for older people to make health-related decisions, such as those regarding the use of a secondary prevention service like cancer screening. National and international studies show that various predictors determine cancer screening participation. The aim of this study is to determine the cancer screening utilization of older people in a structurally weak region., Methods: In 2021, a cross-sectional study in each of two urban and rural communities in Saxony-Anhalt surveyed individuals aged 55 years and older about determinants, reasons, and barriers to preventive service use (n = 954). Binary logistic regression analysis is used to analyze determinants of cancer screening use., Results: Three quarters of the study population (76.6%) self-reported participating in a cancer screening service at least once. The multivariable analyses demonstrate factors that influence the utilization of cancer screening. Age, partial knowledge on cancer screening, cancer screening as a benefit offered by a statutory health insurances' bonus program, experience with cancer in the immediate environment, thoughts about one's own health, and the feeling of security that participation gives are factors that significantly influence the use of cancer screening. Descriptively, the physician's recommendation is the strongest factor for participation., Conclusion: The analyses show that cancer screenings are generally well received by older people in Saxony-Anhalt, but participation in them is not related to health literacy. In keeping with the National Cancer Plan, older people should generally be supported in making an informed decision, for example, through target-group-specific physician education., (© 2023. Objective Guidelines recommend task-oriented training and the use of standardized assessments to improve stroke-related mobility deficits. However, the German outpatient physical therapy prescription catalogue does not include these recommendations resulting in a possible gap between guideline recommendations and clinical practice. Therefore, the purpose of this study was to describe physical therapy practice patterns of stroke-related mobility deficits in the outpatient setting exemplified by the states Baden-Württemberg and Thuringia. Methods Using an online survey, physical therapists treating people with stroke in outpatient settings in Baden-Württemberg and Thuringia were recruited. The questionnaire was developed using a multi-step procedure. Using a case vignette and open-ended questions, preferred evaluation and treatment methods were assessed. Data were analyzed using content analysis and descriptive statistics. Results Data from 63 physical therapists were included in the analysis. Answers to the open questions showed a wide range of different citations. Of 186 citations on evaluation methods, 28 (15,1 %) were classified as “standardized assessments”, while 158 citations (84,9 %) were classified as “basic physical therapy evaluation (non-standardized)”. Standardized assessments were cited by 25% of participants. Of 182 citations on treatment methods, 69 (35,2%) were classified as “traditional therapies”. These traditional therapies were cited by 81 % of participants. Task-oriented training was not cited. Discussion Despite the small sample size our data confirm the insufficient guideline use in German physical therapy and contribute results on practice patterns in outpatient stroke settings. These results will be used to initiate theory-based implementation strategies aiming to optimize physical therapy for people with stroke.

Self-efficacy expectancy in adolescents with anorexia nervosa in the outpatient setting: A cross-sectional study Abstract: Introduction: The population involved in this study are individuals with anorexia nervosa (AN). AN is a disease with sometimes life-threatening underweight. Self-esteem problems, anxiety and social problems accompany those affected. AN is often chronic. Hospitalized patients quickly gain weight, but often the underlying eating problem persists, which leads to rehospitalization. Studies on the self-efficacy expectation (SWE) of those affected are largely lacking. Aim: As part of this cross-sectional study, those affected were interviewed in the post-hospital, psychosomatic setting using a standardized questionnaire "General SWE". The data were evaluated descriptively and by means of inferential statistics. Method: As part of this cross-sectional study, those affected were interviewed in the post-hospital, psychosomatic setting using a standardized questionnaire "General SWE". The data were evaluated descriptively and by means of inferential statistics. Result: 85 young people were interviewed. In the sample, the SWE score averaged = 28.53 with up to and including three hospitalizations and = 24.68 with four or more hospitalizations. If the inpatient stay was only recently, the SWE score shows a lower value (= 26.08) than for those with a longer stay (= 29.00). Conclusion: The results indicate that SWE can influence the course of the disease in those affected. People with higher SWE values show a more favorable course of the disease with fewer rehospitalizations. The strengthening of the SWE is therefore of great importance in the inpatient and outpatient setting through targeted interventions by specialists.

Das Thema „Vegane Ernährung“ gewinnt aufgrund der steigenden Prävalenz immer mehr an Relevanz (Schulz, 2021). Geschätzt 2% der österreichischen Bevölkerung ernährt sich vegan (Stand September 2021). Die Supplementierung des kritischen Nährstoffes Vitamin B12 ist bei einer veganen Ernährung essenziel (Richter et al., 2016, S.96). Veganer*innen werden eine Ernährungsberatung, sowie regelmäßige ärztliche Untersuchungen auf Nährstoffmängel empfohlen. Eine Unter-, sowie Übersupplementierung kann teils irreversible gesundheitliche Schäden nach sich ziehen (Leitzmann & Keller, 2020, S.295). Momentan gibt es keine Daten zum Supplementierungsverhalten österreichischer Veganer*innen. Ziel dieser Arbeit war, diese Daten erstmals zu erheben, um herauszufinden ob die Empfehlungen der Fachgesellschaften umgesetzt werden. In einer Querschnittsstudie (n=1615) wurden die Supplementierungsgewohnheiten mittels Online-Fragebogen ermittelt und analysiert. Dabei wurden unter anderem Zusammenhänge zwischen den biologischen Geschlechtern untersucht. Die Ergebnisse zeigen, dass Veganer*innen verglichen mit der Allgemeinbevölkerung überwiegend jung, weiblich und gebildet sind und einen gesünderen Lebensstil pflegen. Gesamt nehmen 91,8% der Veganer*innen regelmäßig Nährstoffsupplemente ein. Am häufigsten wird der Nährstoff Vitamin B12 (93%) supplementiert. Vitamin B2 (31,6%) und Jod (35,5%) werden am seltensten supplementiert. Die Dosierung erfolgt bei der Hälfte (50,9%) nach Angabe des Produktes. Weiters zeigt die Auswertung, dass die Mehrheit der Befragten (61,2%), ihren Nährstoffstatus mindestens einmal jährlich bestimmen lässt. 89% kontrollieren ihren Vitamin B12 Status regelmäßig, wobei der Großteil (60,7%) nur einen relevanten Marker erheben lässt. Bezüglich der Supplementierung von Nährstoffen zeigten sich zwischen den Geschlechtern kaum signifikante Unterschiede. Die Ergebnisse dieser Beobachtungsstudie deuten darauf hin, dass die österreichische vegane Bevölkerung ein hohes Bewusstsein für die Vitamin B12 Supplementierung und dessen Laborkontrolle hat. Aufklärungsbedarf besteht jedoch bei der Bestimmung relevanter Blutparameter. Die Stichprobe dieser Arbeit kann nicht als repräsentativ angenommen werden. Weiterführende Untersuchungen der veganen Bevölkerung sind erforderlich, um gewonnene Ergebnisse zu festigen und neue Erkenntnisse zu gewinnen. Auf Basis dieser Ergebnisse ist die Erstellung eines Leitfadens bezüglich Supplementierung und Nährstoffstatusüberprüfung anzudenken. The topic "vegan nutrition" is becoming increasingly relevant due to its rising prevalence (Schulz, 2021). Approximately 2% of the Austrian population follows a vegan diet (September 2021). Supplementing the critical nutrient vitamin B12 is essential in a vegan diet (Richter et al., 2016, p.96). Nutritional counseling and regular medical examinations are recommended for vegans. Under- and over-supplementation can cause irreversible health damages (Leitzmann & Keller, 2020, p.295). Currently, there is no data on the supplementation behavior of Austrian vegans. This study aimed to collect this data for the first time to find out whether the recommendations of the professional societies are implemented. In a cross-sectional study (n=1615) the supplementation habits were determined and analyzed via online questionnaire. Among other things, correlations between biological sexes were investigated. Results show that compared to the general population, vegans are predominantly young, female, educated and have a healthier lifestyle. 91.8% of vegans regularly take nutrient supplements. Vitamin B12 (93%) is supplemented most frequently. Vitamin B2 (31.6%) and iodine (35.5%) are supplemented least frequently. Half of the respondents (50.9%) take the dosage according to the product. Furthermore, the evaluation shows that most of the respondents (61.2%) have their nutrient status determined at least once a year. 89% check their vitamin B12 status regularly, whereby the majority (60.7%) only have one relevant marker determined. Regarding nutrient supplementation, there were few significant differences between genders. This observational study indicates that the Austrian vegan population has a high awareness of vitamin B12 supplementation and its laboratory control. However, there is a need for clarification regarding the determination of relevant blood parameters. The sample of this study is not representative. Further studies are necessary to consolidate obtained results and to gain new insights. Based on these results, creating a guideline regarding supplementation and nutrient status monitoring should be considered.

Wundinfektionen nach chirurgischen Eingriffen zählen zu den häufigsten nosokomialen Infektionen und treten bei rund 1% aller operierten Patienten auf. Das Infektionsrisiko hängt dabei stark mit der Art des Eingriffs und den vorhandenen Risikofaktoren zusammen. Spezifische Infektionsraten aus der hepato-pankreato-biliären Chirurgie (Publikationszeitraum 2010 bis 2020) reichen von 9,1% bis 24,8%, was auch aufgrund unterschiedlich zusammengesetzter Studienpopulationen und abgebildeter Eingriffsspektren nur eingeschränkt Rückschlüsse zur tatsächlichen Häufigkeit von Wundinfektionen innerhalb dieses Fachbereichs zulässt. Neben einer Operationsstatistik, grundlegenden Erkenntnissen zur örtlichen Patienten- und Risikostruktur sowie der Krankenhausaufenthaltsdauer sollte deshalb vor allen Dingen die abteilungsinterne Prävalenz postoperativer Wundinfektionen nach hepato-pankreato-biliären Eingriffen ermittelt werden. Den Kern der vorliegenden Arbeit stellt dabei der erhobene Datensatz dar, der methodisch nach dem Vorbild dreier unterschiedlicher Forschungsgruppen ausgewertet wurde. Dies ermöglicht präzisere Rückschlüsse und eine erhöhte Vergleichbarkeit der Daten. Darüber hinaus werden Schwächen im Studiendesign inklusive entsprechender Verbesserungsvorschläge sowie zukünftige Forschungsfelder besprochen., Surgical site infections rank among the most frequent healthcare associated infections and affect approximately 1% of all surgical patients. The chance for infection though is strongly correlated to type of surgery and preexisting risk factors. Empirical data based on hepato-pancreato-biliary interventions from 2010 untill 2020 suggest a wide spread of infection rates between 9,1% and 24,8%, which makes it difficult to draw reliable conclusion in addition to different study populations and sometimes incomparable range of interventions. The main contribution of this paper is an empirical dataset, that is composed according to the study designs of 3 different research groups and therefore allows a more structured comparison of the frequency of surgical site infections and their potential risk factors. This work further presents conclusions regarding to the length of stay and discusses potential design weaknesses. In closing, potential study design improvements are proposed and further prospective research areas are identified.

Introduction: During the COVID-19 pandemic, people in need of long-term care were among the most vulnerable population groups. Home-care services were under exceptional strain, especially at the beginning of the pandemic. The aim of this study is to examine the situation and problems of care services and the people in need of care during the first two waves of the pandemic in Germany., Methods: Two cross-sectional studies were conducted during the first two COVID-19 waves (first survey 28 April to 12 May 2020, second survey 12 January to 7 February 2021). In total, data from N = 1029 outpatient care services were included in the analysis. Descriptive measures were used for the analysis., Results: The clients of home-care services were severely burdened in the first two waves of the pandemic. This can be seen on the one hand in an increased risk of illness and increased mortality, and on the other in the loss of various care and support services. The latter also has negative effects on the psychosocial condition of those in need of care, for example. Care services were affected by high staff absenteeism and additional work due to protective measures., Discussion: The COVID-19 pandemic led to immense burdens for people in need of care and home-care services and to a reduction in care services. The deterioration of care provision met with an already tense situation. It has become clear that the provision of care for those in need of care by outpatient care services is not crisis-proof, and that additional challenges such as a pandemic can have dramatic consequences. INTRODUCTION: In Germany,1.2 million asylum seekers have entered the country in 2015-2016. More than a third of these asylum seekers were women. To understand the situation of asylum seekers' pregnancies, we examined the primary health care of this particular group to understand what, if anything, needs to improve to make the German health system more accessible to pregnant asylum seekers.; METHODOLOGY: we examined 960 cases of pregnant women who delivered in 2 large maternity clinics in Berlin-Charite between January 2016-August 2017. In our comparative cross-sectional study, we compared 480 asylum-seeking women with 480 local German women. For both patient populations we acquired data on various parameters.; RESULTS: We found only two significant differences: (1) The number of antenatal appointments attended by refugee women during their pregnancy was lower than that of the control group. The average number of antenatal appointments in the local control group was 10.6 (Max.: 27, SD 2.85, Min: 2); the refugee women attended on average 8.0 appointments (Max.: 20, SD 3.385, Min: 0) (p=0.000, adjusted relative risk 0.77 95% CI 0.74-0.81). (2) The mean Hb value in the local control group was 11.1 g/dl, while in the asylum-seeking women's group it was 10.6 g/dl (p= 0.00, adjusted difference 0.54 95% CI -0.77 to -0.34).; CONCLUSIONS: This study showed that the German health care system is capable of treating asylum-seeking women. Nevertheless, it is possible that our results do not necessarily reflect the ability of the health care system but stem from other selection parameters of the health condition of our subjects, such as the "healthy migrant effect." In Deutschland sind Kopfschmerzerkrankungen sowohl unter Frauen als auch unter Männern weit verbreitet und führen meist zu Einschränkungen der Lebensqualität. Die häufigsten Kopfschmerzerkrankungen sind Migräne und Spannungskopfschmerz. Zum Zweck einer validen Schätzung der Prävalenzen wurde zwischen Oktober 2019 und März 2020 eine telefonische Querschnittbefragung unter Erwachsenen in Deutschland (N = 5.009) durchgeführt. Anhand der diagnostischen Kriterien der International Classification of Headache Disorders wurden Häufigkeit, Dauer, Eigenschaften und Begleiterscheinungen der Kopfschmerzen erfragt. 57,5 % der Frauen und 44,4 % der Männer in Deutschland berichten, binnen eines Jahres mindestens einmal von Kopfschmerzen betroffen zu sein. 14,8 % der Frauen und 6,0 % der Männer erfüllen die kompletten Kriterien für Migräne. 10,3 % der Frauen und 6,5 % der Männer sind von Spannungskopfschmerzen betroffen. Migräne und Spannungskopfschmerzen treten vorwiegend im erwerbsfähigen Alter auf und nehmen im Altersverlauf stetig ab. Migräne geht häufig mit Begleiterkrankungen wie depressiven Symptomen und Angststörungen einher. Der Versorgungsgrad bei Kopfschmerzerkrankungen ist gering. Nur eine Minderheit der Betroffenen sucht binnen eines Jahres ärztliche Hilfe. Die Ergebnisse geben ein umfangreiches Bild zu den bevölkerungsbezogenen Beeinträchtigungen durch Kopfschmerzerkrankungen. Sie werden im Rahmen der Studie BURDEN 2020 genutzt, um zentrale Indikatoren der Krankheitslastrechnung zu quantifizieren.

The current COVID-19 pandemic inherits an unprecedented challenge for the treating rheumatologists. On the one hand, antirheumatic drugs can increase the risk of infection and potentially deteriorate the course of an infection. On the other hand, an active inflammatory rheumatic disease can also increase the risk for an infection. In the recommendations of the German Society for Rheumatology (www.dgrh.de), it is recommended that our patients continue the antirheumatic therapy to maintain remission or low state of activity despite the pandemic. In this study, patients with inflammatory rheumatic disease were asked in the first weeks of the pandemic on their opinion of their immunomodulating therapy. The result shows that over 90% of the patients followed the recommendation of the rheumatologist to continue the antirheumatic therapy, and only a small percentage of the patients terminated the therapy on their own. This result was independent of the individual anti-rheumatic therapy. Taken together, the results of this study illustrate not only the trustful patient-physician partnership in a threatening situation but also the high impact of state-of-the art recommendations by the respective scientific society.

Data on self-reported cardiovascular and metabolic diseases are available for the first 100,000 participants of the population-based German National Cohort (GNC, NAKO Gesundheitsstudie).To describe assessment methods and the frequency of self-reported cardiovascular and metabolic diseases in the German National Cohort.Using a computer-based, standardized personal interview, 101,806 participants (20-75 years, 46% men) from 18 nationwide study centres were asked to use a predefined list to report medical conditions ever diagnosed by a physician, including cardiovascular or metabolic diseases. For the latter, we calculated sex-stratified relative frequencies and compared these with reference data.With regard to cardiovascular diseases, 3.5% of men and 0.8% of women reported to have ever been diagnosed with a myocardial infarction, 4.8% and 1.5% with angina pectoris, 3.5% and 2.5% with heart failure, 10.1% and 10.4% with cardiac arrhythmia, 2.7% and 1.8% with claudicatio intermittens, and 34.6% and 27.0% with arterial hypertension. The frequencies of self-reported diagnosed metabolic diseases were 8.1% and 5.8% for diabetes mellitus, 28.6% and 24.5% for hyperlipidaemia, 7.9% and 2.4% for gout, and 10.1% and 34.3% for thyroid diseases. HintergrundErste empirische Befunde zur studentischen GK an den Hochschulen in Fulda und Bochum deuten darauf hin, dass mehr als jeder zweite Studierende Schwierigkeiten im Finden, Verstehen, Beurteilen und Anwenden von Gesundheitsinformationen hat. Soziale Unterschiede in der GK von Studierenden wurden in nationalen Studien kaum berücksichtigt.Ziel der ArbeitErfassung der GK von Studierenden und Ermittlung sozialer Hintergrundmerkmale, die mit der GK assoziiert sind.Material und MethodeDie standardisierte Online-Erhebung mit 996 Studierenden fand im Sommersemester 2018 an der Technischen Universität Dortmund statt. Die GK wurde mit der Kurzversion HLS-EU-Q16 erfasst. Zusätzlich wurden soziale Hintergrundmerkmale der Studierenden erhoben (u. a. Migrationshintergrund, finanzielle Situation, subjektiver Sozialstatus, soziale Unterstützung). Die GK wurde mittels uni-, bi- und multivariater (binär-logistischer Regressions‑)Analysen ausgewertet.ErgebnisseEine limitierte GK wurde von 58,5 % der Studierenden angegeben. Junge Studierende im Alter von 18 bis 20 Jahren, die ihre finanzielle Situation als nicht ausreichend bewerteten, einen niedrigen subjektiven Sozialstatus (SSS) und eine geringe soziale Unterstützung (SU) angaben, berichteten am häufigsten eine limitierte GK im Vergleich zu älteren Studierenden mit ausreichenden finanziellen Mitteln, einem hohen SSS und einer hohen SU.SchlussfolgerungenDie Studie verdeutlicht, dass Studierende aufgrund des hohen Anteils mit limitierter GK eine relevante Zielgruppe darstellen und daher in zukünftigen, repräsentativen Forschungen zur GK berücksichtigt werden sollten. Zudem besteht der Bedarf, geeignete Maßnahmen zur Gesundheitsförderung und Prävention, einschließlich sozialer Unterstützungsangebote im Hochschulsetting zu etablieren. Background: Preliminary empirical findings on student health literacy generated by universities in Fulda and Bochum indicate that more than one student in two experiences difficulties in accessing, understanding, appraising and using information relevant to health. Yet there are very few national studies taking account of social differences in university student health literacy. Objectives: Measurement of student’s health literacy and identification of social characteristics associated with health literacy. Materials and methods: The standardized survey including 996 students was conducted during the summer semester 2018 at the Technical University of Dortmund. Health literacy was measured by the health literacy short version HLS-EU-Q16. In addition, social characteristics were examined (e.g. migrant background, financial situation, subjective social status, social support). Health literacy was analysed by uni-, bi- and multivariate (binary logistic regression) analysis. Results: Of the surveyed students, 58.5% reported limited health literacy. Percentage frequency for limited health literacy was highest for young students aged 18 to 20 years, who considered their current financial situation as not sufficient, reported a low subjective social status (SSS) and low social support (SU) compared to older students with sufficient financial resources, high SSS and high SU. Conclusions: According to the results, students can be considered as a relevant target group due to the high proportion of limited health literacy and therefore ought to be sampled in future representative research on health literacy. There is also a need to establish appropriate measures for health promotion and prevention, including social support services in the university setting.

Examination in times of Corona: A cross-sectional study on the use of digital media and the implementation of examinations in nursing education Abstract. Background: In the pandemic, performance assessments had to be carried out under new conditions during the school closures. The aim of vocational nursing education is to develop competence to act. So far, there are no empirical findings on learning assessments and final examinations under pandemic conditions. Aim: The study aims to record the mode of performance assessments under the changed conditions. Methods: Written online survey of teachers at nursing schools in Germany. The collected data were analysed descriptively and inferentially using Chi-Square tests, and subjected to an integrative content analysis. Results: 884 nursing schools were contacted, 430 questionnaires from 179 responding schools (school-related response rate: 20,2%) could be evaluated. PCs or laptops are mostly used to check learning levels (n = 222). 205 participants do not use digital media for this purpose. Digital media are also used for the oral (n = 108) and written (n = 116) parts of the final examination; various organisational and hygiene measures are taken for face-to-face examinations. The practical part is often conducted in new formats (OSCE, simulation). Conclusions: Based on the results, it remains unclear to what extent competence orientation of vocational nursing education is implemented with digital support in examinations. The practical examination in particular seems to pose a special challenge for all those involved.

Objective: Metabolic Syndrome (MetSyn) describes a cluster of metabolic disorders and is considered a risk factor for development of cardiovascular disease. Although a high prevalence is commonly assumed in Germany data about the degree of its occurrence in the population and in subgroups are still missing. The aim of this study was to assess the prevalence of the MetSyn according to the NCEP ATP-III (National Cholesterol Education Program Adult Treatment Panel III) criteria in persons aged ≥18 years attending a general practitioner in Germany. Here we describe in detail the methods used and the feasibility of determining the MetSyn in a primary health care setting. Research design and methods: The German-wide cross-sectional study was performed during two weeks in October 2005. Blood samples were analyzed in a central laboratory. Waist circumference and blood pressure were assessed, data on smoking, life style, fasting status, socio-demographic characteristics and core information from non-participants collected. Quality control procedures included telephone-monitoring and random on-site visits. In order to achieve a maximal number of fasting blood samples with a minimal need for follow-up appointments a stepwise approach was developed. Basic descriptive statistics were calculated, the Taylor expansion method used to estimate standard errors needed for calculation of confidence intervals for clustered observations. Results: In total, 1511 randomly selected general practices from 397 out of 438 German cities and administrative districts enrolled 35,869 patients (age range: 18-99, women 61.1%). More than 50,000 blood samples were taken. Fasting blood samples were available for 49% of the participants. Of the participating patients 99.3% returned questionnaires to the GP, only 12% were not filled out completely. The overall prevalence of the MetSyn (NCEP/ATP III 2001) was found to be 19.8%, with men showing higher prevalence rates than women (22.7% respective 18.0%). Conclusions: This study was designed to provide data as robust as possible within the confines of an epidemiological study. Judging by the low degree of missing data and the high data quality, the feasibility for this kind of a research setting (short evaluation period, practitioners as data assessment sites) was found to be very good. The results will help to gain a more comprehensive insight into the prevalence of MetSyn for patients in primary health care in Germany.
