The article by Thomas Zimmermann examines the function of decorative plates made of bone or stone in the pre-ceramic Neolithic period in Mesopotamia and the Near East. These plates are referred to as arm protection plates and have a symbolic meaning in relation to hunting. The article also deals with the function of arm protection plates from the late Neolithic period in Europe and finds that they are considered as protection or symbolic jewelry. During this time of transition from hunter-gatherer communities to settled lifestyles, central places with ritual installations played an important role. Hunting had a metaphysical dimension and body ornaments were used to create identities. The text also emphasizes the active role of women in hunting and archery. It is noted that the exact purpose of some archaeological finds remains unclear, but some objects could be interpreted as possible wrist guards. [Extracted from the article]

Ulnar wrist pain is a frequent symptom with many possible traumatic and non-traumatic causes. The complex anatomy and biomechanics of the wrist with the ulnocarpal complex including the triangular fibrocartilaginous complex (TFCC), the distal radio-ulnar joint (DRUJ) and the ulnar carpal bones make the differentiation between the possible causes difficult. A precise clinical investigation and appropriate imaging studies are essential for detecting the most important differential diagnoses, the first therapeutical steps and an early and appropriate referral to the hand surgeon. 8. [Posttraumatic growth disturbances of the distal radius in childhood].

Background: Fractures of the distal radius in childhood are frequent but rarely lead to relevant growth disturbances., Objective: The experiences of a pediatric trauma center over 15 years are exemplarily presented and discussed., Material and Methods: Osteotomies of the distal radius were selected and the clinical and radiological findings were recorded and analyzed., Results: Posttraumatic growth disturbances were corrected in 10 cases with a median age of 13 ½ years. In six cases an iatrogenic cause was also possible. In two cases callus distraction with secondary palmar plate osteosynthesis was used for consolidation. In eight cases primary ad hoc corrections were carried out using palmar plate osteosynthesis (six with iliac crest and two with allogeneic grafts). In five cases the distal ulnar growth plate was also addressed. During follow-up breakage of one plate was registered. After an average of 31 months postoperatively, 9 children had full range of motion (ROM) and in 1 case there was an extension deficit of 10
o ., Conclusion: It is important to evaluate the cause of the mostly partially inhibiting growth disturbances to draw preventive conclusions. From a clinical perspective the visible deformity is given priority and the pain and functional limitations are less relevant. The indications for corrective surgery must include the age of the patient, remaining growth potential, size and localization of the growth plate bridge, the deformity and the individual wishes of patients and parents. In most cases a full correction is possible with palmar plate osteosynthesis and an iliac crest graft. External fixation and callus distraction is an alternative in cases where the correction is more extensive. The distal ulnar growth plate should be controlled concerning further growth potential before hardware removal., (© 2024. Supporting phonological awareness is an important part of the early reading and writing acquisition especially for children with speech and language disorders. One popular method of teaching focuses on hand gestures although there is no evidence for their application. The aim of the bachelor thesis was to explore the efficacy of hand gestures in the context of phonological awareness. Moreover, the need and practical implementation should have been evaluated. Therefore, an experiment was conducted within a first grade of a school focusing on children with speech and language disorders (n=11). The class was divided into an experimental and a control group. Whereas the experimental group used additional hand gestures, the control group only received support on phonological awareness. A pre- and post-test as well as a competency grid were used to assess proficiency development in phonological awareness and the usage of hand gestures. The results suggests that hand gestures are effective to support the phonological awareness and that children improve their skills by using hand gestures. Nevertheless, it became apparent that the implementation of hand gestures in this form does not seem to be sufficient and, accordingly, it should be revived in the systematic learning of letters. Objectives: Buckle fractures of the distal forearm are a common fracture entity in children treated conservatively. Diagnostics primarily include radiographs in 2 planes. Inadequate images may occur in the mostly very young patients. Therefore, additional lateral radiographs are often obtained to assess a possible angular tilt. The aim of this study is to investigate whether a strictly lateral x-ray image has an influence on fracture management., Methods: Seventy-three children with buckle fractures of the distal forearm were included in this retrospective analysis. All cases were analyzed by quality of radiographs, necessity to obtain an additional lateral radiograph, and what influence on fracture management resulted. Follow-up was performed 2 to 4 weeks after immobilization., Results: Thirty-five girls and 38 boys with a mean age of 7.16 years were included; 40 had fractured right and 33 had fractured left arms, respectively. Isolated distal radius fractures occurred in 48 cases, isolated distal ulna fractures in 6 cases, and both bones in 19 cases. Initial radiographic images were evaluated as inadequate in 25 cases. In each of those cases, an additional lateral image was obtained by fluoroscopy without resulting in an alternated fracture management protocol, which was chosen conservatively in each case and resulted in excellent clinical outcome at follow-up examination., Conclusions: Based on our results, the acquisition of additional lateral radiographs seems to be unnecessary in the diagnostic procedure of buckle fractures of the distal forearm, in case a possible palmar or dorsal angulation cannot be entirely assessed on the initial set of radiographs. Arthrosis and autoimmune arthritis frequently lead to major impairment of hand function. Primary therapy consists of well- tried conservative treatments and joint preserving surgical options for special indications. In advanced stages silicon spacers remain the gold standard surgical option for replacement of proximal interphalangeal and metacarpophalgeal joints of the fingers. Alternatively, surface replacement prostheses can restore the biomechanical properties of these joints more porperly. In case of the carpometacarpal (CMC) joint of the thumb, arthroplasty is gaining popularity as modern implants show excellent mid-term outcome. Although current forth generation implants for wrist replacement are promising, total wrist arthroplasty is currently reserved for exceptional indications., (© 2023. Summary: The fossil record forConfuciusornis sanctus is more extensive than that for most other fossil birds. The condition of preservation shows that the specimens died suddenly and were embedded in the sediment (influence of volcanic gases and dust) very quickly. The birds lived socially near freshwater lakes and ponds. They were probably able to swim but might have nested on trees. Their anatomy reveals a stunning mixture of primitive and derived features. This paper focusses mainly on structures affecting the capacity of flight, e.g. the bony sternum with a small keel; the metacarpalia and carpalia forming a proximally fused carpometacarpus; the structure of the joint between the humerus and the forearm indicating a mechanical arrangement synchronizing flexions and extensions at the elbow and wrist joints; the long remiges. The large wings leave no room for doubt thatConfuciusornis could fly; soaring was probably its preferred mode of flight. The three fingers could be moved independently of each other. The mobility of the second (big) finger was apparently restricted. This finger bore the distal primaries, its claw is small. The functions of the first and third fingers are not clear. The third finger has a very short basal phalange and was highly flexible. It probably served as a sort of landing flap when it was stretched out caudally, thus improving the flight properties of the wing, all the more as the alula of the first finger was lacking. The function of the enormous claws on the first and third finger is unknown. The assumption that these claws were climbing devices is implausible. The long remiges would have been quite obstructive in a stem-climbingConfuciusornis. Apart from that, there is no reason why a good flyer should have climbed. The anatomy of the hind limbs agrees with a rather generalized type of avian foot. The configuration of the toes is far from that of typical perching birds. Introduction: About one fifth of all sports accidents involve the hand. Many injuries can be diagnosed and treated easily. However, some of them such as fractures of the scaphoid or the hamate of the hamulus are difficult to detect with conventional radiographs and are often missed without a CT scan. Ligament injuries such as the skier's thumb must be recognized and treated properly, often surgically. There are also sport-specific injuries, such as the closed rupture of a flexor tendon pulley in climbers, which otherwise occur rarely and are little known. BACKGROUND AND OBJECTIVES: In complex hand traumas nerves and vessels are often destructed without the possibility for primary repair. For bridging defects of nerves, veins and arteries grafts are necessary. Commonly nerve and vein grafts from adjacent donor sites as the wrist, forearm or cubital region are harvested. METHODS: This study is a retrospective cohort study. Between 2017 and 2019, 10 patients with complex hand injuries were treated. There were 8 males and 2 females, with an average age of 39 years (range 8–63 years). In all cases grafts were used of the dorsum of the foot for reconstructing of the severed digital nerves and arteries. All donor sites could be closed primarily. RESULTS: In 100% of cases nerves and veins of the dorsum of the foot showed a good size match as well as adequate length for a sufficient repair. The overall Hand Injury Severity Score (HISS) was determined with a median of 86 (range 57 to 286). In the area of the donor site no relevant complications were seen. CONCLUSIONS: In complex hand injuries the dorsum of the foot is a favorable donor site for nerve and vein graft harvest.

Fractures of the upper limbs are frequent and often need operative treatment in addition to a well-indicated conservative management. This depends on the maturity of the nearest growth plate and the local remodeling potential, which is dependent on age. Following conventional x-ray imaging an individual prognostic growth analysis leads to the therapeutic decision. Around the elbow and the shaft of the forearm, criteria are stricter than near the shoulder or wrist. Gilchrist or Desault bandages as well as braces are adequate for most subcapital and humeral shaft fractures. The short forearm cast is the method of choice at the wrist. In the case of osteosynthesis the elastic stable intramedullary nailing (ESIN) method is the first choice for subcapital humerus, radial neck and shaft fractures. Screw fixation is usually carried out for epicondylar and condylar fractures and for supracondylar and wrist fractures K-wires represent the standard procedure. [ABSTRACT FROM AUTHOR]

Radiocarpal dislocations and fracture dislocations are rare but always severe and complex injuries. They occur frequently in young and active patients as a result of high energy accidents. A detailed clinical and imaging examination and an accurate classification leads to a suitable and mostly surgical treatment strategy. The strategy should consider the most important components of the injury, the bony, the ligamentous and the intracarpal lesions. Delayed sequelae, residual pain and functional impairment are frequent after these severe injuries, but with adequate treatment, good, even long-term functional results are possible., (© 2023. Introduction: A lesion of the median nerve due to nerve compression comparable to carpal tunnel syndrome is very common, with an incidence of 3.3 to 3.45 cases per 1000 inhabitants and year. In the present case series, we report compressions of the -median nerve due to wearing a smartwatch, which to our knowledge has not been previously reported in the literature. We -describe five patients with typical carpal tunnel symptoms and pathological sensory neurography who had been wearing a smartwatch more than 24 hours/day for several weeks to detect atrial fibrillation, and whose symptoms and electrophysiology improved after they stopped wearing it. Smartwatches can be an interesting additional gadget for diagnosing diseases such as cardiac arrhythmias. However, the tight wearing that is necessary can possibly also compress the median nerve and lead to symptoms such as the carpal tunnel syndrome. According to the manufacturer, it is important to choose the right strap size when buying and only leave the watch very tight when you need the sensors (but without feeling uncomfortable) and otherwise to tie the strap loosely, to take it off or alternate the wrist in order to avoid pain and injury., Competing Interests: Es bestehen keine Interessenskonflikte., (© 2023 Aerzteverlag medinfo AG.)

Background: Even today malunions still not infrequently remain after conservative or operative therapy of the most common fracture in humans, multiform fractures of the distal radius, which raises the question of a correction. Objective, material and methods: This article presents the techniques of corrective osteotomy for extra-articular and especially of intra-articular malunions of the distal radius based on the literature and own cases. The indications and possible complications of this demanding intervention are elaborated. Results and discussion: The results of the many publications since 1935 have shown a significant improvement in wrist mobility and rotation, the grip force of the hand and pain relief after extra-articular, mostly three dimensional restoration of the distal radius in shape and length. The intra-articular corrective osteotomy has also proven to be safe and effective. As a result of improved postoperative joint congruence, a beneficial effect on the formation of osteoarthritis can be expected. [ABSTRACT FROM AUTHOR]

Osteomyelitis of the hand and wrist is uncommon compared to the infections of the long bones but not rare. There is poor evidence on many answers to questions concerning this disease. This includes careful consideration of the prevalence, pathogenesis, microbiology, diagnostic methods and the conservative, medical and surgical treatment. The emergence of the disease can subdivided in post-traumatic, postoperative, spread from contiguous infections and hematogenous. The individual patient-specific risk must be considered. Early diagnosis and correct management are essential to preserve bony structures and the articular function of the hand. A two-step surgical procedure is usually necessary, but uncertainties about the optimal treatment are still existing. Satisfactory control rates of the infection and successful bone reconstructions can be achieved. But complications, a limited range of motion and amputations of finger parts can still not be avoided in all cases.Die Osteomyelitiden im Bereich von Handgelenk und Hand sind weniger häufig als die Knocheninfektionen der langen Röhrenknochen, aber nicht selten. Viele Fragen zur Klassifikation, Pathogenese, Diagnostik, zum Therapiemanagement sowie den erreichbaren Behandlungsergebnissen sind bisher nur wenig ausreichend wissenschaftlich kontrolliert. Die Pathogenese der Erkrankung kann in posttraumatische, postoperative, fortgeleitete und hämatogene Osteomyelitiden, unter Erfassung der patientenindividuellen Risiken, eingeteilt werden. Eine frühe Diagnose und eine adäquate Therapie sind notwendig, um die betroffenen knöchernen Strukturen sowie die Funktion der angrenzenden Gelenke zu erhalten. Ein mehrzeitiges operatives Vorgehen ist dabei häufig notwendig. Zufriedenstellende Raten der Infektionskontrolle und erfolgreiche knöcherne Rekonstruktionen können meist erreicht werden. Komplikationen, verbleibende Defizite und partielle Amputationen, auch bei angemessener Therapie der Osteomyelitiden, sind auf Grund der Schwere der Erkrankung aber nicht immer zu verhindern.

The median nerve is an important sensory and motor nerve of the arm and is not infrequently associated with diagnostic and therapeutic misjudgements. A bifid median nerve as a coincidental finding in traumatology is another rarity and is mostly found during various elective procedures, such as carpal tunnel splitting. We report on a patient with a bifid median nerve who sustained a laceration wound near the carpal tunnel due to a domestic accident.Der N. medianus ist ein wichtiger motorischer und sensibler Nerv des Arms und wird nicht selten mit diagnostischen und therapeutischen Fehleinschätzungen assoziiert. Ein bifider N. medianus als Zufallsbefund in der Traumatologie stellt bisher eine Seltenheit dar und ist meist im Rahmen elektiver Eingriffe wie bei der Karpaltunnelspaltung wiederzufinden. Der vorliegende Fall berichtet über einen Patienten mit einem bifiden N. medianus, der sich im Rahmen eines häuslichen Unfalls eine Schnittwunde in der Nähe des Karpaltunnels zugezogen hatte.

The carpal joint is one of the most complex joints in the body comprising multiple bones that allow flexibility while simultaneously providing stability. Variations in osseous structures that may be either cause or result of pathological changes may make radiological reporting challenging. Only the knowledge of important osseous variations allows a reliable assessment of carpal imaging studies.The standard imaging technique for evaluation of osseous carpal structures is conventional radiography, which is followed by computed tomography (CT) and-under special circumstances-magnetic resonance imaging (MRI). Other imaging methods such as sonography or nuclear medicine studies do not play a significant role in clinical routine.Apart from continuous reduction in effective radiation dose, there have been no significant methodical improvements in the past decade regarding imaging of osseous carpal structures in clinical routine.As the initial diagnostic procedure, conventional radiography usually allows a safe and reliable diagnosis of osseous structures. Unclear or discrepant imaging findings between clinical and imaging assessment should initiate further imaging, preferably with CT. Only for certain questions or to reduce effective radiation dose in children MRI studies should be performed in clinical routine.KLINISCHES/METHODISCHES PROBLEM: Das Handgelenk stellt mit zahlreichen an ihm beteiligten Knochen eines der komplexesten Gelenke des Körpers dar. Es besitzt eine hohe Flexibilität bei ausgezeichneter Stabilität. Die Variationsbreite der knöchernen Strukturen, welche zugleich Ursache oder Differenzialdiagnose von pathologischen Veränderungen sein kann, stellt den befundenden Radiologen vor differenzialdiagnostische Schwierigkeiten. Nur die Kenntnis der wichtigsten ossären Normvarianten erlaubt eine fundierte Befundung des Handgelenks.Als radiologisches Standardverfahren zur Untersuchung der knöchernen Strukturen des Handgelenks ist in erster Linie die konventionelle Röntgendiagnostik zu nennen, gefolgt von der Computertomographie (CT) und im Zuge dezidierter Fragestellungen ggf. auch der Magnetresonanztomographie (MRT). Andere Verfahren, wie z. B. die Sonographie oder Szintigraphie, haben in der Routinediagnostik der ossären Strukturen keinen Stellenwert.Neben der Reduktion der effektiven Strahlendosis haben sich seit der Einführung der Mehrzeilenspiral-CT keine wesentlichen Innovationen in der klinischen Routinediagnostik zur Beurteilung der knöchernen Strukturen des Handgelenks durchgesetzt. EMPFEHLUNG FüR DIE PRAXIS: Als initiales diagnostisches Verfahren erlaubt meist das konventionelle Röntgen bereits eine sichere Diagnose im Bereich der knöchernen Strukturen; bestehen jedoch bildmorphologische Unklarheiten oder eine Diskrepanz zwischen klinischem Befund und konventionell radiologischer Bildgebung, sollte zeitnah eine weitere Abklärung mittels CT erfolgen. Nur bei speziellen Fragestellungen oder zur Reduktion der Strahlenexposition bei Kindern ist eine MRT in der klinischen Routine indiziert.

Problem: Undetected scapholunate ligament instability often leads to severe degenerative osteoarthritis of the wrist. Therapy: If the injury is diagnosed early, complete restitution is possible after ligament repair. In cases of delayed diagnosis there are various forms of damage to the wrist. Depending on the clinical findings there are reconstructive options to preserve function or salvage procedures to limit further osteoarthritic complications. There are no data ensuring a standardized landmark-based-technique for blocking sensitive nerves of the forearm.To identify locations were with use of good palpable bony landmarks and lines between them sensitive nerve blocks on the forearm can be done with great success.Dissection of the superficial branch of the radial nerve (SBRN), the dorsal branch of the ulnar nerve (DBUN), the lateral, medial and dorsal antebrachial cutaneous nerve (LACN, MACN, and DACN) as well as the palmar branch of the median nerve (PBMN) was performed on five upper limbs of five different Caucasian cadavers. With respect to radius and ulnar styloid, Lister's tubercle, and the medial and lateral epicondyle of the humerus as well as connecting lines between these bony landmarks locations were defined, where the mentioned nerves can be found and blocked.The six nerves can be safely blocked at the following sites: the SBRN 85 mm proximal to Lister's tubercle on a line drawn between the latter and the medial humeral epicondyle; the LACN 38 mm and the dorsal one 32 mm ulnar from the lateral epicondyle; the MACN 14 mm radial to the medial epicondyle; the DBUN 27 mm proximal to the ulnar styloid in direction to the lateral epicondyle; the PBMN 45 mm proximal to the radial styloid following a line between the latter and the medial epicondyle and 21 mm ulnarwards perpendicular to this line.Using superficial good palpable landmarks at the wrist and elbow as well as connecting lines between them the SBRN, DBUN, PBMN, LACN, MACN, and DACN can easily be located.Es gibt keine Daten, die eine standardisierte, auf Orientierungspunkten basierende Technik zur Blockierung sensibler Nerven des Unterarms gewährleisten. ZIEL: Ziel dieser Untersuchung war es, Stellen an Unterarm und Ellenbogen zu definieren, an denen unter Orientierung an gut tastbaren knöchernen Landmarken Nervenblockaden relativ sicher erfolgreich möglich sind.An fünf Leichenarmen wurden der Ramus superficialis des N. radialis (RSNR), der dorsale Ast des N. ulnaris (RDNU), der laterale, mediale und dorsale N. cutaneus antebrachii (NCAL, NCAM, NCAD) sowie der Ramus palmaris des N. medianus (RPNM) präpariert. In Bezug auf den Processus styloideus radii und ulnaris, das Listerʼsche Tuberkulum sowie den Epikondylus medialis und lateralis sowie Verbindungslinien zwischen diesen gut tastbaren Knochenvorsprüngen wurden Stellen lokalisiert, an denen die vorgenannten Nerven aufzufinden und zu blockieren sind.Die Nerven können an folgenden Stellen sicher blockiert werden: der RSNR 85 mm proximal des Listerʼschen Tuberkels auf einer Linie zwischen letzterem und dem Epicondylus humeri medialis; der NCAL 38 mm und der NCAD 32 mm ulnar vom Epicondylus humeri lateralis; der NCAM 14 mm radial zum medialen Epicondylus; der RDNU 27 mm proximal zum Ulnastyloid in Richtung des lateralen Epicondylus; der RPNM auf einer nach ulnar zielenden Senkrechten zur Verbindungslinie zwischen dem Processus styloideus radii (PSR) und dem medialen Epikondylus 45 mm proximal des PSR und 21 mm nach ulnar.Unter Verwendung gut tastbarer Knochenvorsprünge am distalen Unterarm und Ellenbogen und Verbindungslinien zwischen ihnen lassen sich der RSNR, RDNU, RPNM sowie die NNCAL/M und D sicher lokalisieren.IV.
