Background: Over the past 20 years the importance of treatment of people with mental and neurological disorders has greatly increased. Parallel to this development it has become more difficult to attract young physicians to this field. The aim of this study was to examine the development of the number of physicians specialized in the care of patients suffering from neurological, mental and psychosomatic disorders with special consideration of the age structure. Material and methods: The analyses were based on the number of professionally active physicians and specialized physicians published by the German Medical Association for the years 2000–2020. Separate age groups were looked at for psychiatry and psychotherapy (PPT), psychosomatic medicine and psychotherapy (PMPT), Nervenheilkunde (formerly psychiatry and neurology together, NHK) and neurology. Results: In comparison to the year 2000 the number of specialized physicians working in PPT (4736 vs. 12,053), neurology (2226 vs. 8355) and PMPT (3543 vs. 4130) increased in 2020, while the number of specialists actively working in NHK decreased (5184 vs. 2301). Parallel to this the proportion of women increased. Dramatic changes occurred concerning the age structure. Currently, 77.7% of specialists working in NHK and 59.7% working in PMPT are over 60 years old. In 2020 there were 2988 specialists aged over 60 years in the discipline of PPT compared to only 1070 under 40 years, which is dramatically different from 20 years earlier when only 181 were over 60 years but 1491 were under 40 years old. Conclusion: The overaging of professional specialists and the shortage of junior physicians jeopardize modern and adequate provision of care for mentally ill patients. Possible solutions include a marked increase in medical school capacities as well as strategies to convince young physicians to work in the disciplines of PPT and PMPT. [ABSTRACT FROM AUTHOR]

Background: With increasing life expectancy, progressive demographic change and decreasing societal stigmatization of incontinence urologists and gynaecologists are increasingly faced with urogynaecological challenges. To date however, urogynaecology is a poorly standardized area of expertise in both disciplines. Therefore, the urogynaecology training, especially in Germany, is very heterogeneous and requires evaluation as well as improvement.Materials and Methods: The GeSRU-Academics research group "Functional urology and LUTS" evaluated this subject nationwide among urological and gynecological trainees and their chief physicians by using a comprehensive questionnaire (34/38 multiple-choice items) between April 2015 and May 2016.Results: 336 urological residents and 190 chief physicians as well as 171 gynaecological residents and 175 chief physicians participated in the survey. Of all trainees, 70.0 % stated a personal interest in urogynaecology, but 45.4 % (gynaecological residents) and 52.9 % (urological residents) mention not to receive a standardized training in their own department. The chief physicians' survey resulted in discrepancies concerning the same question, <10 % of all residents do not receive a standardized urogynaecological training from their point of view. However, standardized urogynaecological training is of importance for those chief physicians.Conclusions: There is a discrepancy between expectations and reality of urogynaecological education and training. To enable a well-structured and standardized urogynaecological education and training, it is compulsory to focus on an interdisciplinary cooperation and to promote multidisciplinary development. As in other medical specialties, there is currently an imbalance between the number of open positions and the number of applicants in the field of psychiatry and psychotherapy, despite an overall growing number of physicians. To assess the magnitude of this deficit, the German Association for Psychiatry and Psychotherapy (DGPPN) conducted a survey among heads of clinics for psychiatry and psychotherapy in Germany. There were several characteristic trends: there is high variability in the lack of applicants depending on the contextual conditions of the specific clinic. It increased significantly from 2008 to 2009, is more pronounced in the east rather than in the west, in smaller rather than in larger clinics, and for assistant doctors rather than for specialists. The lack of applicants is most likely also masked by compensatory measures such as the hiring of psychologists for the position of a physician. It is assumed that in addition to the general lack of physicians there is a specialty-specific component that plays a role: the disproportionate increase in the demand for psychiatric-psychotherapeutic services. Background: Practical contents are gaining in importance in medical curricula in Germany. In times of a lack of applicants, practical courses provide an increased level of interest for students in the respective disciplines. A practical appeal of ophthalmology is microsurgical procedures. A microsurgical suture wet lab can be an introduction for medical students., Objective: Assessment of the increased interest in ophthalmology through evaluation of a suture wet lab course, including suturing under a microscope., Material and Methods: The data were obtained from the practical course in ophthalmology during the sixth semester for medical students at the university medical center in Mainz in April 2019. In a questionnaire developed in collaboration with the Center for Quality Assurance and Development various statements had to be answered and evaluated using ordinal scales., Results: A total of 64 evaluation questionnaires from 8 groups each with 8 medical students supervised by different tutors were analyzed. The wet lab was rated with 1.24 ± 0.5 (mean ± SD) on a German school grade scale (1 = best, 6 = worst). There was agreement (1 = total agreement, 7 = total disagreement) concerning the desire for further microscopic wet labs (1.86 ± 1.28) as well as learning more ophthalmological surgical techniques (2.02 ± 1.13). The interest in ophthalmology (1 = very high interest, 7 = very low interest) increased from 3.66 ± 1.55 before the course to 2.52 ± 1.00., Conclusion: Interest in ophthalmology can be increased through a microsurgical wet lab. The interest of students can therefore be awakened, which can have advantageous effects in job application situations and in research work. In this way experience and practical skills in ophthalmology can already be acquired during medical studies.

Background: To safeguard scientific and clinical progress, German urology requires properly trained junior scientists. Before initiating or continuing actions aiming at quality improvement an analysis of the status quo is necessary., Objective: To assess the conditions to pursue research, research skills and research output of junior scientists in urology in Germany., Material Und Methods: A 16-item online questionnaire was sent to 95 junior scientists in urology within the research network GeSRU Academics. Primary outcomes were the conditions to pursue research in terms of research time, research skills and sources of learning and research output as measured by peer-reviewed publications. Subpopulations were compared with respect to the number of peer-reviewed publications., Results: Out of 78 junior scientists (82% response rate) 45% pursued research exclusively in their leisure time. Self-assessment of research skills varied from good (systematic literature search) to sufficient (grant acquisition). The main source of learning for research skills was self-study, followed by mentor, own department, courses and networks. Of the junior scientists 81% had peer-reviewed publications (median 4). The groups of junior scientists who pursued research (partially) during working hours, who had good skills and whose research skills were supported by a mentor/network had significantly more peer-reviewed publications than their counterparts., Conclusion: Junior scientists in urology in Germany lack protected time to pursue research and have varying research skills, which are predominantly acquired by self-study and demonstrate their first research output as peer-reviewed publications.
