The number of regional lymph nodes relevant for lymphadenectomy in gastric cancer was investigated in an autopsy study. In group N1-16 as described by JRSGC on the average 43 lymph nodes were found (range 25 to 64 lymph nodes). For group N1-11 (R1 and R2 resection, compartments I and II respectively) 27 lymph nodes were found on the average (range 17 to 44 lymph nodes). The results differed considerably in total number as well as within each single group and in the groups N1-16/N1-11. These results can be taken as statistical standard values.

A postmortem study by ductography and histology was carried out in 50 macroscopically normal human pancreata. The alterations of the pancreatic duct system, their relations to the parenchyma, and their changes depending on age were studied. The most frequent changes of the ducts are epithelial hyperplasia (88%) and periductal fibrosis (74%). In the ducts they produce cystic widening (62%) and intraluminal protein precipitates (40%), within the parenchyma an intralobular fibrosis up to total fibrosis of lobules (88%). All these alterations increase with aging both in frequency and intensity. But they are already visible before the age of 50 and sometimes before the age of 30. The pathogenetic connexions between the changes of the duct system and the alterations of parenchyma were studied by serial sections and ceroplasty. This investigation revealed that the small branches of the larger ducts, which drain small periductally situated lobules, play an important role in the development of the parenchymal alterations. No connections were found between the pancreatic alterations and diseases of the bile duct system.

In 383 patients, suffering from Crohn's disease, ileum was studied both endoscopically and histologically. In patients without surgical interventions (n = 207), endoscopic lesions were observed in 69%, histologic lesions in 74%. Most numerous endoscopic lesions were redness (51%), vulnerability (29%) and ulcer (27%), most frequent histologic lesions discontinuous infiltration (30%) and ulcer (30%). In patients, operated on, endoscopic and histologic lesions were found more often in the ileal region, near to anastomosis than in the more proximal ileum. Overall, the endoscopic and histologic appearance of the distal segment of the ileum widely corresponded to that of the terminal ileum in patients without surgery. Several endoscopic parameters were correlated to histologic variables, but overall the correlation was low (maximal correlation coefficient 0.34). Granulomas were present in 7.4% of the biopsies and 9.1% of the patients. The occurrence was significantly correlated to the endoscopic variables cobblestone appearance and ulcer as well as to the histologic variables density of infiltration, ulcer, and activity of inflammation. In conservatively treated patients, the incidence of mucosal vulnerability, and cobblestone appearance significantly declined in long standing disease, the incidence of stenosis increased. In patients operated on, aphthous lesions and ulcers increased in longstanding disease.

Viral-hepatitis-like hepatic damage ("aprindin hepatitis") developed in a 50-year-old woman after taking aprindin, the cause of the jaundice being confirmed by the same signs occurring when the drug was taken again after initial withdrawal.

Four years after mastectomy for a scirrhous carcinoma a 71-year-old woman developed diarrhoea. Crohn's disease was suspected. At endoscopy a stenosis of the sigmoid colon was found which could not be passed: the mucosa was normal looking. Gastrointestinal radiography revealed segmental subtotal stenoses of the colon with linitis plastica, typical for tumour-caused infiltration, as well as indentations in the small intestine by mesenteric metastases. The diagnosis was confirmed by computed tomography and, finally, operation. Chemotherapy failed to produce any regression of the colon stenoses, and the patient died from mechanical ileus. In case of a similar history and colon stenoses of uncertain aetiology the possibility of intestinal metastases should be considered in the differential diagnosis.

The study was performed in 64 post mortem ductograms of pancreata, missing any inflammatory and/or neoplastic alterations in extensive histological investigations by whole organ sections. The pancreatic duct was measured in the radiograms. From these measured values and by mean of a computer program of a cubic spline function the straightened pancreatic ducts were plotted. The number, the length and the amplitude of ductal deviations were measured in these plots. Luminal dilatations were found in all organs, ranging from 0.5 mm to 6 mm and more. Their frequency was significantly lowered in young people (11-30 years).

Ureteroscopy is used not only to clarify tumorous processes in the area of the ureter but also for removal or ultrasonic destruction of distal ureteral stones. Instruments have been developed that allow application of an optically controlled ultrasonic or metal loop. With our instrument we removed distal ureteral stones in 12 cases applying ultrasound or a Dormia loop. The different techniques and strategies are presented. In animal experiments we investigated the effect of ultrasound on the urothelium. We found that short time application (less than 30 s) does not leave irreversible damages. Removal of ureteral stones by ureteroscopy is indicated in stones which do not pass spontaneously or after unsuccessful efforts to remove the stones by means of a Zeissloop that is in cases where hitherto the only way of action possible was to carry out an ureterolithotomy.

A case of hepatocellular carcinoma following acute lymphoblastic leukemia (ALL) in childhood is described. After completion of the combined therapy of ALL, there was splenogenic thrombocytopenia which was successfully treated by splenectomy. Cirrhosis of the liver and HBs antigen-positive hepatitis was detected by biopsy. The cirrhosis progressed during the following year. After a further year a hepatocellular carcinoma was diagnosed. This gave rise to the death of the patient in complete hematological remission 5 years and 7 months after commencement and 2 years and 10 months after completion of the ALL therapy. Only two cases of hepatocellular carcinoma following ALL at this age have been reported by other authors.

Explorative laparotomies were carried out on 68 patients with Hodgkin's disease in the University Hospital of Marburg from 1969 through 1978. These laparotomies were preceded by clinical examination, abdominal sonography in 27 cases, lymphography in 55 cases, scintigraphy of liver and spleen in 58 cases, and radiographic examinations of the intestinal tract in 18 cases. Sonography revealed a greater accuracy (90%) for splenic involvement than scintigraphy (74%); the results of sonography and scintigraphy of the liver were comparable. For the detection of para-aortal lymphomas ultrasound and lymphography can be regarded as complementary methods. Our results are compared with findings in the literature on abdominal sonography (n = 50), scintigraphy of liver and spleen (n = 185), and lymphography (n = 465) carried out before explorative laparotomy for lymphogranulomatosis. There was a better correlation for the systemic symptoms of the 68 patients with the clinical stage than with the pathologic stage. The importance of diagnosing different groups, e.g. in stage III, is stressed.

Partial gastrectomy was performed in 23 patients for different reasons, and the specimens recovered were investigated histologically. Localization and size of inflammatory and atrophic changes were looked for especially. Inflammatory lesions of the fundus area are distributed primarily as spots and do not spread continuously; they extend from the aboral gastric region to the upper parts of the stomach. These alterations start as small and spot-like lesions, increase in size and join neighbouring lesions to form larger inflammatory areas. Atrophic alterations of the mucosa generally develop in the same way as inflammatory lesions occurring most frequently in the antral zone and in the adjacent areas of the fundus as well; however patterns of distribution of inflammatory and atrophic changes do not seem to correlate well in the individual case.

Disseminated fat necrosis can be produced by intraperitoneal injection of porcine pancreatic lipase. They get detecable by intravital staining with Phosphine 3R about 15 min after injection. The earliest fine structural findings are spotty destruction of the pinocytic invaginations and vesicles, combined with alterations of the cell membrane. Later there is a complete destruction and disintegration of the cytoplasm and its organels as well as the nucleus, whereas the cell membrane partly remains visible. At the same time the central lipid droplet shows cloudy disintegration and clumping as well as cristalline and granular structures. In the beginning the necrosis is limited to single cells. Later the adjacent fat cells also undergo necrosis, even when the applicated lipase has been removed after 30 min. In light microscopic studies fat necrosis are detectable 30 min after the application of lipase. During the first 48 hours they get demarcated by leucocytes. In the following days resorption and organisation take place. Lipolytic drugs facilitate the development of fat necrosis, whereas antilipolytic drugs inhibit it. In starvation the number of fat necrosis rises, after feeding it decreases. In the diurnal rhythm there is a maximum after midnight and a minimum in the early afternoon. The results support the hypothesis that pancreatic lipase only attacks fat cells, which are lipolytically active.

The case history of a boy suffering from acute lymphoblastic leukemia (ALL) and carcinoma of the liver as secondary malignoma is reported. After completion of the combined ALL-therapy, a splenogenous thrombocytopenia emerged which was successfully treated by splenectomy. In the biopsy material liver cirrhosis and HBsAg-positive hepatitis could be detected; these proved to be progressive in the course of the following year. At the end of another year, a hepatocellular carcinoma was diagnosed, causing the patient's death, who was still in complete hematological remission 5 7/12 years after the onset and 2 9/12 years after the end of the ALL-therapy. Only two cases of hepatocellular carcinoma following ALL in childhood have been described in the literature so far.

1. The influence of dyes with special affinity to amyloid on the resorption of experimental amyloidosis was studied in mice (A/J/Bom). 2. The amyloidosis was produced by injections of 5% casein over a period of seven weeks. After an interval of six weeks the animals received 0.5 ml of a 0.01% dye solution intraperitoneally (congo red, diamine green, Evans blue) or 0.9% NaCl (controls) every seventh day over a period of 22 weeks. 10, 17, and 22 weeks after the dye treatment was started 3-4 animals of each group were sacrificed and studied histologically. 3. In contrast to the in vitro findings of Shirahama et al. (1971), in vivo there is no acceleration of amyloid resorption by congo red. In the same way diamine green does not accelerate the resorption of amyloid. Evans blue produces a decrease in the amount of amyloid, but at the same time it causes wasting in the animals. Moreover, congo red and diamine green produce amyloidosis by themselves. 4. From our findings neither congo red nor the other amyloidaffine dyes studied, diamine green and Evans blue, are suitable for a therapy of amyloidosis.

A case of Whipple's disease is reported in detail. Pathological changes of duodenal mucosa are demonstrated and the diagnostic importance of duodenoscopy is stressed.
