Inflammation of the spinal cord and the adjacent structures can be caused by viruses, bacteria, fungi and parasites. Viruses predominantly infect the spinal cord and the nerve roots directly or trigger a secondary immune response, whereas bacteria, fungi and parasites tend to form abscesses, granulomas and cysts and can lead to a secondary compression of the spinal cord, similar to a destructive osteomyelitis. The etiological clarification of an acute or subacute spinal process is carried out based on the clinical presentation, the time course of the development of symptoms, the immune status, neuroimaging and microbial and/or molecular biological examinations of cerebrospinal fluid and serum. The tropism of individual pathogens to certain fiber structures and cellular clusters in the spinal cord in synopsis with the clinical presentation, neuroimaging and a history of exposure, can often quickly lead to a focused clarification and diagnosis. This article deals with important pathogens of spinal and paraspinal infections, the geographical distribution, the clinical and neuroimaging presentation with special consideration of the anatomical and topographical localization and recent epidemiological developments. History and Clinical Findings:  We report on a 34-year-old female patient and a 50-year-old male patient, both of whom were admitted to our emergency department with severe septic conditions., Medical Examinations:  Both patients were resuscitated and exhibited clinical as well as laboratory evidence of a severe bacterial infection., Diagnosis:  Both patients had an invasive infection with Group A Streptococcus. The female patient had a Streptococcal sepsis with severe pneumonia, while the male patient had a Streptococcus-induced necrotizing fasciitis of the upper extremity., Therapy and Course:  While the female patient unfortunately died in the emergency department`s resuscitation room despite all intensive medical treatments, the male patient survived after prompt surgical therapy and an extended stay in the intensive care unit., Conclusion:  Patients with invasive infections caused by Group A Streptococcus can deteriorate rapidly clinically. Prompt diagnosis and initiation of often interdisciplinary treatment are important. The paper summarises present knowledge concerning the possible role of a virus which has in the meantime been classified as belonging to the arteriviridae and which causes PRRS (Porcine Reproductive and Respiratory Syndrome), in the pathogenesis of respiratory diseases four years after first isolation of the virus. Although the fact that PRRSV-infection produces an immunity which protects swine from repeated bouts of PRRSV is common knowledge by now, reinfection with the virus might be possible. An immunosuppression, which has been suggested by many investigators, and which was thought to be the result of destruction of alveolar macrophages during virus replication, was not found as yet. The significance of the extent and the duration of the decrease in the number of alveolar macrophages, which belong to the unspecific immunity, for the total immune system of swine infected with PRRSV remains unclear. A general impairment of specific immunity through PRRSV-infection could not be shown. The present role of PRRS for the pathogenesis of respiratory disease is seen very differently. The significance of PRRSV as primary cause of an "influenza-like" illness which is principally followed by severe bacterial infection of the respiratory tract, is questioned by the results of studies which identify PRRSV-infection as a mainly subclinical disease. In these studies, clinical cases are the exemption. Attempts at experimental reproduction of a clinically manifest, respiratory disease was not successful as yet. The paper describes factors which might be responsible for these variable results. Control of PRRS is difficult in areas with a high density of the swine population, since the spreading of the virus with the wind seems to be important besides the recruitment of new, infected animals. A vaccine has not yet been registered in Germany. First experimental experiences with vaccination are available from Denmark and the USA.

One case of malignant external otitis in a non-diabetic patient is presented. Malignant external otitis is a rare but severe bacterial infection, caused by Pseudomonas aeruginosa, predominantly in elderly diabetic patients. The process starts in the external auditory canal and after crossing the cartilaginous-osseus junction invades the connective tissue, cartilage, bone, and nerves of the temporal bone and the surrounding parts of the base of the skull. The clinical syndrome is a severe, painful inflammation of the external auditory canal with edematous obstruction of the external canal and typical granulation tissue in the canal wall. Diagnostic criteria are pain, edema, exudate, granulations, microabscess, diabetes, old age, identification of Pseudomonas aeruginosa, and even cranial nerve involvement. Further important criteria are failure of local treatment and a positive Tc-99 scan demonstrating osteomyelitis of the temporal bone as a sign of connective tissue, cartilage, and bone invasion. In most cases there is also a positive radiograph or HR-CT of the base of the skull. Except for sequestrations, surgical treatment in malignant external otitis is impractical because of the deep penetration into the base of the skull and the lack of demarcation lines in this diffuse pathological process. Nowadays long term i.v. antibiotic therapy is preferred. In our case we applied Ceftazidim (Fortum) 2 g for 12 weeks twice a day. After this period the patient was completely cured. Our case demonstrates that malignant external otitis should be considered even in non-diabetic patients.

Subsequent to orthopedic surgery 23 patients were studied by leukocyte scintiscanning using 99mTc-HMPAO to detect inflammation. 12 patients had all signs of bacterial infection. In 11 patients infection of the wound area was suspected. There was one false negative and no false positive scintigraphic finding. 2 patients with an unclear postoperative course after internal fracture fixation showed intensive uptake. Several days after the scan both patients developed severe bacterial infection. From that experience we conclude that the method may be of higher sensitivity than clinical symptoms. Further investigations are necessary for an accurate assessment of the method.

49 patients were given cefoxitin in combination with azlocillin, and 15 were given an aminoglycoside as well when severe bacterial infection was suspected. In 39 cases a prompt amelioration of the clinical signs of infection was observed. Of the 17 patients who died, a post-mortem was performed in seven; only in four were signs of bacterial infection present; one had tuberculosis and one had a cytomegalovirus infection. The combination therapy was well tolerated and side-effects were rare; no bleeding complications were seen. Clinical signs of possible antagonism between the two beta-lactam antibiotics were not observed.
