- Abstract
COSYCONET 1 is the only German COPD cohort which is large enough to be internationally comparable. The recruitment, which started in 2010 and ended in December 2013, comprised 2741 patients with the diagnosis of COPD who were subsequently investigated in regular follow-up visits. All visits included a comprehensive functional and clinical characterisation. On the basis of this detailed data set, it was possible to address a large number of clinical questions. These questions ranged from the prescription of medication, the detailed analysis of comorbidities, in particular cardiovascular disease, and biomarker assessment to radiological and health-economic aspects. Currently, more than 60 publications of COSYCONET data are internationally available. The present overview provides a description of all the results that were obtained, focussing on the relationship between different clinical and functional aspects as well as their potential practical consequences. In addition, information on the follow-up study COSYCONET 2 is given.COSYCONET 1 ist die derzeit einzige international konkurrenzfähige deutsche COPD-Kohorte. Die Rekrutierung begann Ende 2010 und endete im Dezember 2013. Sie resultierte in 2741 Patienten mit der Diagnose einer COPD, die in der Folge in regelmäßigen Visiten nachuntersucht wurden. Diese Visiten umfassten eine umfangreiche funktionelle und klinische Charakterisierung. Auf der Basis dieses hochwertigen Datensatzes war es möglich, eine Vielzahl klinischer Fragen zu beantworten. Diese reichten vom Verschreibungsverhalten über die genaue Analyse verschiedener Komorbiditäten, v.a. kardiovaskulärer Art, bis zu Biomarkern, radiologischen und gesundheitsökonomischen Analysen. Inzwischen wurden mehr als 60 Publikationen zu COSYCONET international veröffentlicht. Die nachstehende Arbeit liefert eine Übersicht über alle bislang erhaltenen Ergebnisse, mit einem besonderen Fokus darauf, deren wechselseitigen Zusammenhang sowie die möglichen praktischen Konsequenzen herauszuarbeiten. Ferner werden einige Informationen über die Folgestudie COSYCONET 2 gegeben.

3. [Adherence to CPAP Three Months after Starting Therapy in 1078 Patients with Obstructive Sleep Apnea (OSA)]

Nocturnal Continuous Positive Airway Pressure (CPAP) is considered the gold standard treatment for obstructive sleep apnoea (OSA). The CPAP therapy is a long-term treatment but does come with few possible side effects. The adherence to the therapy is frequently suboptimal. In this paper, adherence to therapy was assessed and typical problems were classified. 1078 OSA patients received CPAP therapy after a diagnostic polygraphy (PG) or polysomnography (PSG). Adherence to therapy was followed up three months after treatment induction. The following therapy adherence groups were defined: 1. correctly calibrated CPAP and good adherence, 2. CPAP non-acceptance, after initial use, 3. CPAP intolerance use due to side effects, 4. discontinuation due to lack of motivation/low rates of symptoms. 5. mask intolerance, 6. CPAP failure due to a lack of perceived treatment effect, 7. Change to another non-invasive ventilation method, 8. No control carried out. Out of 1078 OSA patients a therapy control was performed in 830 patients (77 %). Of these, 450 patients (54.2 %) were placed in group 1, 216 patients (26 %) in group 2, 71 patients (8.5 %) in group 3, 35 patients (4.2 %) in group 4, 14 patients (1.7 %) in group 5, 3 patients (0.4 %) in group 6 and 41 patients (4.9 %) in group 7. A mild obstructive index, low CPAP pressure and, as a trend, a low Epworth- Sleepiness score were predictors of CPAP failure. No significant predictors could be shown for adherence to therapy. An effective treatment use of 54 % after 3 months is a suboptimal result. Predictors of CPAP failure were parameters that indicated that the patient was less symptomatic prior to therapy. Despite a large patient cohort, neither anthropometric nor PSG-data provided any significant CPAP adherence predictors. Rather, experiences in the first nights of use could be decisive. CPAP devices offer comfort settings that have to be personalised to patients' needs and wants. A large selection of different mask shapes requires experience and training in patient-centred mask fitting. A three-month follow-up appointment seems too long to discuss therapy problems with the patient in a timely manner. Telemedical options or short-term telephone appointments should be considered. Die nächtliche kontinuierliche positive Überdruckatmung (CPAP) gilt als therapeutischer Goldstandard einer obstruktiven Schlafapnoe (OSA). Die CPAP-Therapie stellt zumeist eine Dauertherapie mit geringen Nebenwirkungen dar. Trotzdem ist die Therapietreue unbefriedigend. In dieser Arbeit wurden die Therapietreue betrachtet und typische Probleme katalogisiert. 1078 OSA-Patienten wurden nach diagnostischer Polygrafie (PG) oder Polysomnografie (PSG) auf eine CPAP-Therapie eingestellt. Die Therapietreue wurde nach 3 Monaten nachverfolgt. Folgende Therapieadhärenz-Gruppen wurden festgelegt: 1. gute CPAP-Einstellung und Nutzung, 2. CPAP-Abbruch nach anfänglicher Nutzung, 3. mangelnde CPAP-Nutzung wegen Nebenwirkungen, 4. Abbruch aufgrund fehlender Motivation/geringem Leidensdruck, 5. Masken-Unverträglichkeit, 6. CPAP-Versagen aufgrund fehlenden Therapieeffekts, 7. Umstellung auf anderes Beatmungsverfahren, 8. keine Kontrolle erfolgt. Von 1078 CPAP-Patienten erschienen 830 Patienten (77 %) zur Therapiekontrolle. Hiervon waren 450 Patienten (54,2 %) in Gruppe 1, 216 Patienten (26 %) in Gruppe 2, 71 Patienten (8,5 %) in Gruppe 3, 35 Patienten (4,2 %) in Gruppe 4, 14 Patienten (1,7 %) in Gruppe 5, 3 Patienten (0,4 %) in Gruppe 6 und 41 Patienten (4,9 %) in Gruppe 7. Ein geringer obstruktiver Ereignisindex, niedriger CPAP-Druck und tendenziell auch ein geringer Epworth-Sleepiness-Score waren Prädiktoren für CPAP-Versagen. Für die Therapietreue konnten keine signifikanten Prädiktoren dargestellt werden. Eine effektive Therapienutzung von 54 % nach 3 Monaten ist ein suboptimales Ergebnis. Prädiktoren für CPAP-Versagen waren Parameter, die vor Therapie auf geringen Leidensdruck der Patienten deuteten. Trotz großer Patientenkohorte konnten keine signifikanten CPAP-Adhärenz-Prädiktoren durch anthropometrische oder PSG-Daten dargestellt werden. Vielmehr könnten Erfahrungen in den ersten Therapietagen ausschlaggebend sein. CPAP-Geräte bieten Komforteinstellungen, die kenntnisgerecht auf den Patienten individualisiert werden müssen. Eine große Auswahl an unterschiedlichen Maskenformen erfordert Erfahrung und Training bei der patientenorientierten Maskenanpassung. Eine 3-monatige Wiedervorstellung erscheint zu lang, um Therapieprobleme zeitnah mit dem Patienten zu besprechen. Telemedizinische Möglichkeiten oder kurzfristige telefonische Beratungsoptionen sollten in Betracht gezogen werden.

Nationalismus – ein Thema, das leider wieder in aller Munde ist, und das nicht nur, aber ausgerechnet auch in Deutschland. AfD, Pegida und Co., aber auch die Regierungen einiger Staaten der EU, reklamieren für sich einen positiven Nationalismus, einen ›gesunden Patriotismus‹, und greifen dafür doch tief in die unterste Schublade von Hass und Ausgrenzung. ›Nationalismus‹ ist dabei per se kein Kampfbegriff derer am rechten Rand, sondern ursprünglich geradezu das Gegenteil: eine völkerverbindende Bewegung im Zeichen von Freiheit, Gleichheit, Brüderlichkeit. Sudden cardiac death of suspected healthy young athletes is a rare, but deeply moving event. Usually, the affected person has been completely free of symptoms. Commonly, unrecognized inflammatory, hypertrophic or dilated cardiomyopathies are the most frequent causes. All therapeutic principles of angiotensin-converting-enzyme (ACE) inhibition, beta-blockade, and diuretics in heart failure aim to unload the heart. During physical activity increased sympathetic tonus and loading conditions for the heart point into the opposite direction. This raises the question to what extent physical activity in patients with myocarditis, dilated cardiomyopathy or heart failure in general is tolerable.Several experimental studies revealed disadvantages of physical exercise during acute myocarditis leading to an increase in mortality. On the other hand, several small trials in men demonstrate an improvement of physical fitness and quality of life attributed to controlled supervised exercise training in patients with heart failure without assessment of mortality. Dilated cardiomyopathy was diagnosed in one third of these patients. There was no biopsy confirmation of these conditions. The other two thirds of patients suffered from ischemic heart diseases.Since the borderline between inflammatory heart disease and noninflammatory or postinflammatory dilated cardiomyopathy is difficult to determine, abstention from physical training during and shortly after inflammatory heart disease is recommended, because it is known that viral persistence or autoimmune processes could last for several months.

Very good clinical long-term results of the Lubinus SP II hip prosthesis stem were reported in the literature. We therefore asked whether there is a relationship between these findings with biomechanical data of strain gauge measurements.14 strain gauges were applied at a femur being measured at 10 different load cases before and after implantation of the stem.After stem implantation a similar patterns of principal stress distributions was observed, however, the magnitude was markedly reduced. A striking reduction of the hoop stresses at the femoral calcar was seen in the case of a missing collar contact. Even in the case of a perfect collar contact the hoop stresses were diminished after strem implantation. The S-shaped physiological stem did not correspond with a specific stress pattern measured at the femoral surface.These results suggest that the stresses at the femoral calcar may be lower than the limits of bone growth while the other parts of the femur are more physiologically stressed. However, the prosthesis may tolerate a missing collar contact during a long follow-up period. The large experimental data file presented here could be used to validate future finite element analyses which could evaluate the stress distribution within internal parts of the bone and the cement layer.
