Although intensive, assertive treatment options in child and adoledcent psychiatry have been successfully evaluated and established in several countries, this development is just beginning in Austria. As part of the so called home-treatment, care is provided by a multiprofessional team to patients with severe mental disorders, who would otherwise be treated in an inpatiet setting at their own home. This opens up the opportunity for a better generalizability of therapeutic progress and an intensive work with the family system. Alternative models for inpatient care like home-treatment, show the potential for intensive therapeutic options independent of building structures. Further development of this therapeutic option based on evaluated model projects is desirable within the Austrian health care system.Während intensive, aufsuchende Behandlungsformen in der Kinder- und Jugendpsychiatrie in vielen Ländern bereits erfolgreich evaluiert und etabliert wurden, ist die Entwicklung in diesem Bereich in Österreich am Beginn stehend. Im Zuge des sogenannten home-treatments erfolgt eine Behandlung von Patientinnen und Patienten, die aufgrund der Schwere ihrer Erkrankung ansonsten im stationären Setting behandelt werden würden durch ein multiprofessionelles Team im eigenen Haushalt. Das erlaubt unter anderem eine bessere Generalisierbarkeit therapeutischer Fortschritte und eine intensive Zusammenarbeit mit dem Familiensystem. Stationsersetzende Maßnahmen wie das home-treatment haben das Potenzial unabhängig von baulichen Strukturen eine intensive therapeutische Option darzustellen. Ein Ausbau dieser Behandlungsform basierend auf evaluierten Modellprojekten ist auch im österreichischen Gesundheitssystem wünschenswert.

Mental Disorders among Refugees

Adverse childhood experiences (ACEs) tend to co-occur. The more forms of childhood adverse are experienced, the higher are the psychological and physical impairments in adulthood. The present study therefore examines the extent to which the experience of parental separation is related to the risk of other forms of ACEs and the extent to which the accumulation of ACEs is related to impairment in adulthood.A cross-sectional analysis of a representative sample of the German population from the age of 18 onwards was performed (N=2466; mean age=49.5 years (18-93); f: 1368 (55.5%); m: 1098 (44.5%)). The demographic data were assessed by interview, the remaining data were collected by questionnaires. Current depressive and anxiety symptoms self-perceived somatic health and life quality were assessed, while ACEs were assessed retrospectively by the Adverse Childhood Experiences (ACE)-Questionnaire.Child maltreatment and mental illness of household members, substance abuse of household members, incarceration of household members and violence against the mother were more frequent when participants reported separation of parents. Parental separation was not associated with an increased rate of psychological impairment and, in women, physical impairment when no other forms of ACEs were present.Children and adolescents whose parents have separated more often experience other forms of ACEs than their peers. It seems to be this accumulation of ACEs that is associated with impairments in adulthood.Belastende Kindheitserlebnisse treten oft gehäuft auf. Je mehr Formen von Kindheitsbelastungen erlebt werden, desto höher sind die psychischen und physischen Beeinträchtigungen im Erwachsenenalter. Die vorliegende Studie untersucht daher, inwieweit das Erleben der elterlichen Trennung mit dem Risiko für andere Formen von Kindheitsbelastungen zusammenhängt und inwieweit die Kumulation von Belastungen in der Kindheit mit Beeinträchtigungen im Erwachsenenalter zusammenhängt.Es erfolgte eine querschnittliche Analyse einer repräsentativen Stichprobe der deutschen Bevölkerung ab dem 18. Lebensjahr (N=2466; mittleres Alter=49,5 Jahre (18–93); f: 1368 (55,5%); m: 1098 (44,5%)). Die Erfassung der demografischen Daten erfolgte durch ein Interview, die restlichen Daten wurden mittels Fragebögen erhoben. Die Einschätzung der depressiven und ängstlichen Symptome und der selbsteingeschätzten somatischen Gesundheit bezog sich auf den Untersuchungszeitpunkt, belastende Kindheitserfahrungen wurden retrospektiv mit dem Adverse Childhood Experiences (ACE)-Fragebogen erfasst.Kindesmisshandlung sowie psychische Erkrankung von Haushaltsmitgliedern, Substanzmittelabusus von Haushaltsmitgliedern und Gefängnisaufenthalt von Haushaltmitgliedern sowie Gewalt gegen die Mutter waren häufiger, wenn die Teilnehmenden von der Trennung der Eltern berichteten. Die Trennung der Eltern war nicht mit einer erhöhten Rate psychischer Beeinträchtigungen und bei Frauen auch für körperliche Beeinträchtigungen assoziiert, wenn keine anderen Formen von Kindheitsbelastungen vorlagen.Kinder und Jugendliche, deren Eltern sich getrennt haben, erleben häufiger auch weitere Formen von Kindheitsbelastungen als Altersgenossen. Gerade diese Kumulation von belastenden Kindheitserlebnissen scheint es zu sein, die mit Beeinträchtigungen im Erwachsenenalter zusammenhängen.

Wie bei Erwachsenen sollte auch jedem Kind und Jugendlichen mit einer PTBS eine traumafokussierte Psychotherapie angeboten werden. Die traumafokussierte kognitive Verhaltenstherapie stellt hierbei aufgrund der breiten Evidenz die Behandlung erster Wahl dar. Eine Psychopharmakotherapie, insbesondere der Einsatz von Benzodiazepinen, ist aufgrund ihres Suchtpotenzials zu vermeiden. Der Einbezug von Eltern und / oder Bezugspersonen wird in altersangemessener Weise empfohlen. Fur eine komplexe PTBS sollte auch bei Kindern und Jugendlichen die Behandlung mit einer Kombination traumafokussierter Techniken erfolgen, bei der Schwerpunkte auf der Verarbeitung der Erinnerung und / oder ihrer Bedeutung liegen sowie auf Techniken zur Emotionsregulation und zur Verbesserung von Bindungsproblemen. Bei Behandlungsbeginn sollte die aktuelle Gefahrdung des Kindes bzw. des Jugendlichen abgeklart und bei anhaltender Bedrohung geeignete Masnahmen zur Sicherung des Kindeswohles ergriffen werden. Bei schwerwiegenden komorbiden Symptomen und akuter Suizidalitat konnen vor dem Einsatz traumafokussierter Techniken geeignete Interventionen durchgefuhrt werden. Zusatzliche Probleme wie z. B. das Risiko weiterer Viktimisierung, Aggressivitat, Trauerprozesse und Selbstwertdestabilisierung sollten abgeklart und berucksichtigt werden. The newly developed German guidelines for Posttraumatic Stress Disorder (PTSD) – treatment in children and adolescents As recommended by the guidelines for adults, children or adolescents suffering from PTSD should also be offered trauma-focused treatment. Based on robust findings trauma-focused cognitive-behavioral therapy is the recommended first-line-treatment. Psychopharmacotherapy and specifically benzodiazepines should not be given due to the high risk for substance dependence. It is recommended to include parents and / or significant others in an age-appropriate way in the treatment. In the case of complex PTSD the treatment should be a combination of trauma-focused techniques aiming at the processing of memories of the traumatic events and / or their respective meanings as well as emotion-regulation techniques. Problems regarding attachment should be addressed and improved. At the beginning of treatment, possible imminent threat for the respective child / adolescent needs to be addressed and in case of continuing danger, safety measures need to be installed with a high priority. In case of severe comorbid symptoms and acute suicidality, adequate interventions may be provided before starting trauma-focused therapy. Further problems such as the danger of revictimization, aggression, grief processes as well as negative self-concept should be addressed.

Zusammenfassung Nichtsuizidales selbstverletzendes Verhalten (NSSV) stellt eine häufige Symptomatik im Jugendalter dar. Trotz der Existenz wirksamer psychotherapeutischer Interventionen berichtet die Mehrheit der betroffenen Jugendlichen und jungen Erwachsenen, keine Behandlung erhalten zu haben. Sowohl strukturelle (z. B. keine spezifischen Behandlungen, limitierte Ressourcen, mangelnde Zugänglichkeit) als auch individuelle Barrieren (z. B. gering ausgeprägtes Hilfesuchverhalten) verhindern die adäquate Versorgung des NSSV im Jugendalter. Online-Interventionen bieten die Möglichkeit, spezifische Interventionen unabhängig vom Standort und der Versorgungsstruktur zu vermitteln. Aufgrund des niederschwelligen Zugangs werden auch Jugendliche mit gering ausgeprägten Hilfesuchtendenzen angesprochen. Online-Interventionen wurden bereits erfolgreich bei verschiedenen psychischen Störungen eingesetzt. Hinsichtlich NSSV existiert noch keine wirksame Online-Intervention. Daher wurde im Rahmen des deutschlandweiten STAR-Konsortiums (STAR: Self-Injury – Treatment, Assessment, Recovery) am Standort Heidelberg eine onlinebasierte Intervention, basierend auf einem bereits evaluierten Kurzzeittherapieprogramm, speziell für Jugendliche und junge Erwachsene mit NSSV entwickelt. Im Kontext einer randomisiert-kontrollierten Studie wird die Intervention aktuell hinsichtlich ihrer Wirksamkeit evaluiert.

Pharmacotherapy for children and adolescents with sleep disorders: an overview Abstract. Sleep difficulties are a relevant symptom in childhood and adolescence with a prevalence of approximately 20 %. Because of the reduced psychosocial functioning and the burden of suffering associated with insomnia, effective treatment strategies are needed. If interventions such as the implementation of an adequate sleep hygiene and other non-drug treatment approaches are not sufficient, pharmacotherapeutic treatment is often considered. The present work provides a literature overview on the use of different substance classes in hypnotic indications. The use of melatonin in children and adolescents with autism spectrum disorder has been well studied and is associated with a positive effect on sleep in this patient group. To date, there is little evidence regarding the efficacy and tolerability of other medication in primary insomnia or sleep disorders in the context of other psychiatric disorders in minors. For this reason, non-drug treatment strategies are preferred and pharmacotherapy is only to be considered secondarily after critical examination.Zusammenfassung. Ein- und Durchschlafstörungen sind mit einer Prävalenz von etwa 20 % eine relevante Symptomatik im Kindes- und Jugendalter. Aufgrund des mit Insomnien oftmals verbundenen verminderten psychosozialen Funktionsniveaus und des sich einstellenden Leidensdrucks ist eine suffiziente Behandlung notwendig. Zeigen sich Maßnahmen wie eine angemessene Schlafhygiene und andere nichtmedikamentöse Therapieansätze als nicht ausreichend, wird oftmals eine pharmakotherapeutische Behandlung in Betracht gezogen. Die vorliegende Arbeit bietet eine Literaturübersicht zum Einsatz verschiedener Substanzklassen in hypnotischer Indikation. Der Einsatz von Melatonin bei Kindern und Jugendlichen mit Autismus-Spektrum-Störungen ist zwischenzeitlich gut untersucht und ist mit einem positiven Effekt auf den Schlaf bei dieser Patientengruppe assoziiert. Hinsichtlich der Wirksamkeit und Verträglichkeit anderer Wirkstoffe bei primären Insomnien bzw. Schlafstörungen im Kontext anderer psychiatrischer Erkrankungen bei Minderjährigen besteht bislang eine geringe Evidenz. Daher sind zunächst nichtmedikamentöse Behandlungsstrategien vorzuziehen und eine Pharmakotherapie erst sekundär und nach kritischer Abwägung zu erwägen.

Transitional and Adolescent Psychiatry in Austria: A Pilot Study on the Attitudes of Experts Abstract.This pilot study establishes baseline data on the current situation of transitional processes in Austrian psychiatry. We sought to gather information on the specific problems and needs occurring during the transition of mentally ill adolescents from child and adolescent to adult psychiatry.Professionals (psychiatrists, psychologists, psychotherapists) working in child and adolescent psychiatry and adult psychiatry (N = 86) were assessed by means of an online survey concerning their experience with the transition of young adults.Almost all of the subjects queried (98.8 %) considered the current system inappropriate, and the majority (70.9 %) thought that patients would not tolerate transition from youth to adult psychiatric institutions very well. Only 16.3 % reported having a structured transition protocol at their workplace. Further 83.7 % expressed the need to improve collaboration between adolescent and adult psychiatry.This sample of Austrian healthcare professionals describes large deficits regarding transitional issues in psychiatry. There is an urgent need to establish structured protocols for transition, especially since experiences occurring during this vulnerable period are crucial to the mental health of young people later in life.Zusammenfassung. Fragestellung: Ziel der vorliegenden Pilotstudie war die Erhebung der Ausgangslage zur aktuellen Situation der Transitionspsychiatrie in Österreich sowie zu den spezifischen Fragen, Problemen und Veränderungswünschen des Fachpersonals im Zusammenhang mit dem Übergang psychisch erkrankter Adoleszenter in den Behandlungsbereich der Erwachsenen. Methodik: Im Rahmen einer Online-Fragebogenuntersuchung wurden fachkundige KlinikerInnen (N = 86) der Arbeitsfelder Psychiatrie, Kinder- und Jugendpsychiatrie, Klinische Psychologie und Psychotherapie zu ihren Erfahrungen die Transition von jungen Erwachsenen betreffend befragt. Ergebnisse: Das aktuelle System der Transition wird von 98.8 % der Befragten als ungünstig beurteilt. Ein Großteil (70.9 %) der Stichprobe ist der Meinung, dass der Übergang vom Jugend- zum Erwachsenenbereich von den PatientInnen nicht gut verkraftet werde. Nur 16.3 % geben an, dass an ihrem Arbeitsplatz ein geplantes und geregeltes Prozedere des Überganges vom jugendpsychiatrischen in den Erwachsenenbereich bestehe. 83.7 % plädieren für eine Verbesserung der Zusammenarbeit an der Schnittstelle zwischen der Kinder- und Jugendpsychiatrie und der Erwachsenenpsychiatrie. Schlussfolgerungen: Aus Sicht der hier befragten BehandlerInnen finden sich in Österreich, die Transitionspsychiatrie betreffend, deutliche Defizite. Nicht zuletzt aufgrund der Tatsache, dass gerade im Transitionsalter die Weichenstellung für die psychische Konstellation im weiteren Leben erfolgt, besteht die dringende Notwendigkeit, Strukturen zur geregelten Transition im Bereich der Psychiatrie zu etablieren.

Nonsuicidal self-injury (NSSI) is a frequent, clinically relevant phenomenon occurring in adolescence, which can be observed parallel to various mental disorders. Within Europe, Germany has one of the highest prevalence rates in youths, with the lifetime prevalence ranging between 25–35 %. To date a guideline on the treatment of self-injury and stereotypic motoric disorders has been available, the last having been updated in 2006 and expired in 2011.The guideline development group consisted of members of 11 medical, psychological, or psychotherapeutic professional associations and two members from patient and prevention groups. The guideline was developed in two meetings followed up by consecutive literature searches; it was adopted in a final consensus conference. The algorithm on the treatment of NSSI was approved in three rounds of a Delphi process.Although there is solid evidence of NSSI from the fields of epidemiology and diagnostics, few studies report data on the longitudinal course as well as on the psychotherapeutic and adjuvant psychopharmacological treatment of NSSI in adolescence. The stepped care regimens in cases demanding somatic treatment were addressed interdisciplinarily.In accordance with the heterogeneous evidence, important points for the treatment of NSSI in adolescence were determined in a consensus conference. There is still a lack of knowledge on prevention as well as clinical interventions, both of which need to be addressed by further research.

Due to the clinical relevance of suicidal risks, suicide attempts, and suicides in adolescence consensus-based guidelines with clinical recommendations were updated and summarized in this article. It should be considered that each indication has to be taken serious. Suicidality is an essential component of the psychopathological report and should be explored for the short- and long term risk. The clinical assessment of the acute suicidality results from a trustful anamnestic conversation, assessment of risk factors, mental disorders, and the use of alcohol and drugs. Acute suicidality is an indication for an inpatient treatment that for the protection of the patient has to be implemented also against his will. An adequate documentation is inevitable. After a suicide attempt, in addition to the initial medical treatment it has to be paid attention that the patient cannot harm himself any further and a rapid consultation in the responsible clinic takes place. First therapeutic goal is the reduction of suicidality and if necessary the re-achievement of the ability to negotiate a non-suicide agreement. For recurrent suicide thoughts an emergency plan has to be created. In addition to offer conversations, for a further relief a temporary sedated psychopharmacology can be necessary. In case of a suicide in a clinic, recommendations should be present that regulates responsibilities and procedures. Effective prevention methods are multiplier training, public education, restricted access to methods, and complying with media guidelines.

Postvention is the term used to describe a package of interventions designed for people affected by suicide. Inpatient suicides in Departments of Child and Adolescent Psychiatry are rare, but nevertheless constitute an immense burden to family members, friends, and fellow patients, as well as to medical staff members. Besides including an effective management of the acute crisis situation with well-defined communication rules, appropriate interaction with the bereaved, and prevention of copycat suicides in other patients, reliable guidelines for dealing with inpatient suicide must also contain a precise procedure on how to handle mental stress in physicians, psychotherapists, and other staff members. Furthermore, postvention should be an important part of the professional training of physicians as well as psychotherapists who work in child and adolescent psychiatry departments or outpatient clinics.

The German care system faces a growing number of unaccompanied refugee minors (URM). URM show high levels of traumatization, a variety of psychological symptoms and lack important resilience factors. Therefore an early and valid psychological assessment is important for intervention and service planning. Yet, no systematic review on validated instruments for the assessment of this group exists. Literature search revealed one study about translators in the assessment of URM and five validated instruments for proxy and self-report. These instruments are available in several languages and showed good psychometric properties. It has to be critically stated that all instruments have been validated by a single work group within a single population. Especially with regards to changing definitions of Posttraumatic Stress Disorder within the new (and upcoming) classification systems ICD-11 and DSM-5, increased awareness for diagnostic procedures is necessary. Additionally, more validated instruments for specific psychological disorders in multiple languages are needed. Under an economic perspective the use of open access questionnaires that are available in different languages seems useful, even if they are not especially validated for URM.

The desire for power and social dominance plays a significant role in the development of social aggression. Although the construct of Machiavellianism reflects these aspects well, it has hardly been recognized in Germany. In this review article the construct of Machiavellianism will be presented. Previous research on the relationship between Machiavellianism and particularly aggressive behavior in children and adolescents are critically discussed and implications for future studies are derived.

Rational pharmacotherapy is a challenging task in child and adolescent psychiatry. Increasing prescription numbers contrast with the uncertainties of safety and efficacy issues. The lack of clinical (authorization) trials often implies a non- age-specific use of drugs. However, young patients show particular metabolic conditions and a higher vulnerability for adverse drug reactions. Thus it seems mandatory to create age-specific pharmacological data about efficacy and safety of psychotropic drug use in minors. Legislation authorities became aware of this situation and introduced European and national scientific pharmacovigilance regulations and programmes accordingly in order to continuously evaluate the benefit-risk-ratio, detect, collect, minimize, and prevent adverse effects of drugs by appropriate measures, e.g., therapeutic drug monitoring. In this paper the principles and needs of pharmacovigilance in child and adolescent psychiatry are discussed. Furthermore a large multicenter clinical trial («TDM-VIGIL»), funded by the German Federal Institute for Drugs and Medical Devices, is presented, which appeals to collect epidemiological prescription and safety data of psychotropic drugs in children and adolescents using an internet-based data infrastructure (patient registry).Die rationale Therapie mit Arzneimitteln stellt eine Herausforderung in der Kinder- und Jugendpsychiatrie dar. Steigende Verordnungszahlen stehen im Kontrast zu erheblichen Unsicherheiten in Bezug auf die Arzneimittelsicherheit und Wirksamkeit. Der Mangel an (Zulassungs-)Studien führt häufig zu einem nicht altersspezifischen Gebrauch der Medikamente. Junge Patienten weisen jedoch besondere Stoffwechselbedingungen sowie eine höhere Vulnerabilität für unerwünschte Arzneimittelwirkungen (UAW) auf. Es ist daher notwendig, eigene Daten zur Pharmakokinetik, Wirkung und Sicherheit von Psychopharmaka bei Minderjährigen zu generieren. Auch die Gesetzgebung hat diese Situation erkannt und auf nationaler sowie europäischer Ebene entsprechende Vorschriften sowie wissenschaftliche Programme eingeführt. Ziel der unter dem Begriff der Pharmakovigilanz zusammengefassten Maßnahmen ist es, das Nutzen-Risiko-Verhältnis eines Arzneimittels kontinuierlich zu evaluieren, UAWs so schnell wie möglich zu entdecken und deren Risiken durch geeignete Maßnahmen, z. B. konsequentes Therapeutisches Drug Monitoring (TDM), zu minimieren. Der folgende Artikel erläutert die Notwendigkeit sowie Grundlagen der Pharmakovigilanz in der Kinder- und Jugendpsychiatrie und stellt ein vom Bundesministerium für Arzneimittel und Medizinprodukte (BfArM) gefördertes multizentrisches epidemiologisches Forschungsprojekt («TDM-VIGIL») vor, an dem zahlreiche kinder- und jugendpsychiatrische Zentren sowie Facharztpraxen aus dem deutschsprachigen Raum beteiligt sind. Ziel ist die systematischen Erfassung von Informationen zum Verordnungsverhalten und der Sicherheit von Psychopharmaka im Kindes- und Jugendalter mittels eines internetbasierten Patientenregisters.

Non-Suicidal Self-Injury (NSSI) and Suicidal Behavior Disorder (SBD) were included as diagnostic categories in Section 3 of the 5th edition of the Diagnostic and Statistical Manual (DSM-5) of the American Psychiatric Association (APA). Thus, these diagnostic entities were not recognized as formal clinical diagnoses, but rather for the first time clearly defined in a classificatory system to standardize further research in this field. This paper introduces both concepts and addresses the discussion about NSSI and suicidal behavior disorder based on a selective review of the literature. First studies using the new definitions are introduced. In Germany the prevalence of NSSI is estimated to lie at about 4 %, of SBD at about 9 %. It can be expected that in the future the new definitions will lead to a better comparability of study outcomes with regards to NSSI and suicidal behavior disorder.Nicht-suizidales selbstverletzendes Verhalten (NSSV) und die Suizidale Verhaltensstörung (SVS) wurden in der 5. Auflage des «Diagnostic and Statistical Manual» (DSM-5) der amerikanischen psychiatrischen Gesellschaft (APA) in der «Sektion 3» erstmals als diagnostische Kategorien aufgenommen. Damit werden diese beiden Störungsbilder zwar nicht als formale Diagnosen im neuen Klassifikationssystem anerkannt, jedoch erstmals eindeutig im Rahmen eines Klassifikationssystems definiert. Dies soll weitere Forschungsarbeiten in diesem Bereich in Zukunft vereinheitlichen und erleichtern. Die vorliegende Arbeit stellt diese beiden neuen Störungsbilder vor und bietet anhand eines selektiven Literaturreviews einen Überblick über die Diskussion über NSSV und SVS als eigenständige Störungsbilder. Erste vorliegende Arbeiten, die auf diese Definitionen zurückgreifen, werden vorgestellt. In Deutschland dürfte die Ein-Jahres Prävalenz von NSSV im Jugendalter ca. 4 % betragen, für SVS ca. 9 %. Es kann davon ausgegangen werden, dass die neuen Definitionen zur besseren Vergleichbarkeit von künftigen Studienergebnissen führen werden.

Preceding studies demonstrated a high agreement between self-report and clinician's assessment of depression. The concordance on the level of sum scores, subscales, and single items, however, has yet to be investigated in a psychiatric adolescent sample. Also, the influence of additional variables such as age, sex, and IQ has been insufficiently studied in adolescents.Scores on the BDI-II and CDRS-R, assessed within 1 week, were collected from 105 adolescents (mean age = 15.94 years). Analyses of correlation were done on levels of sum scores, subscales, and single items.There was a high correlation between self-report and clinician's assessment (r = .67). At the level of subscales, items assessing somatic contents demonstrated no higher agreement than did items assessing cognitive and affective contents. The highest agreement at the symptom level was shown for the item assessing suicidal ideations. Additional variables had no significant influence on concordance. Adolescents with a high IQ and outpatient adolescents tended to overestimate their symptoms.The overall correlation was high and did not differ from results of comparable studies of correlations. The highest congruence was shown for the item assessing suicidal ideations, which underlines the accurate assessment of suicidality by clinicians as well. In summary, questionnaires can provide information about the existence of a depressive disorder, although one diagnostic instrument should not be replaced by the other despite a high correlation.Fragestellung: Vorhergehende Studien zeigen eine hohe Übereinstimmung von Selbst- und Klinikerurteil bei der Erfassung von Depressivität. Die Übereinstimmung auf Summen-, Subskalen- und Einzelitemebene wurde aber bisher nicht bei einer psychiatrischen jugendlichen Stichprobe untersucht. Der Einfluss von Drittvariablen, wie Alter, Geschlecht, IQ und Behandlungssetting wurde bei Jugendlichen bisher unzureichend überprüft. Methodik: Untersucht wurden 105 depressive Jugendliche (mittleres Alter = 15.94) bei welchen innerhalb einer Woche das BDI-II und CDRS-R durchgeführt wurde. Korrelationsanalysen erfolgten auf Ebene der Summenwerte, Subskalen und Einzelitems. Ergebnisse: Es zeigte sich eine hohe Korrelation zwischen Selbst- und Klinikerurteil (r = .67). Auf Subskalen-Ebene zeigten Items, die somatische Inhalte erfassen, keine höhere Konkordanz als Subskalen, die kognitive oder affektive Inhalte messen. Auf Symptom-Ebene zeigte sich die höchste Übereinstimmung bei dem Item Selbstmordgedanken. Drittvariablen hatten keinen signifikanten Einfluss auf die Höhe der Korrelation aber Jugendliche mit einem eher hohen IQ und ambulant Behandelte neigten zur relativen Überschätzung ihrer Symptomatik. Schlussfolgerungen: Die Gesamtkorrelation war hoch und unterschied sich nicht von Ergebnissen vergleichbarer Korrelationsstudien. Die höchste Übereinstimmung bei dem Item «Selbstmordgedanken» spricht für die gute Erfassbarkeit von Suizidalität auch im Klinikerurteil. Der Fragebogen kann somit deutliche Hinweise auf das Vorliegen einer Depression liefern, jedoch sollte trotz hoher Korrelationen nicht ein Verfahren durch ein anderes ersetzt werden.

Although adolescents in juvenile residential facilities are at high risk for mental disorders, not all of these adolescents develop psychiatric symptoms. The aim of our study was to define traits protecting these adolescents. A total of 314 adolescents of 20 juvenile residential facilities were examined using standardized assessment instruments (Child Behavior Checklist (CBCL), Junior Temperament and Character Inventory (JTCI)). Educators of the facilities filled out a sociodemographic questionnaire for each participant. Adolescents were divided into two groups (resilient vs. mental problems) by CBCL-Cut-off-Scores. Analyses were based on these two groups. Resilient youth differed from youth with psychiatric symptoms in temperament- and character-traits (e. g. self-directedness F = 28.4, p.001, d = 0.64) with small to moderate effect sizes (d = 0.36 to 0.64). Resilient adolescents lived in larger facilities (U = 9080, p = .025). No associations were found between gender and resilience, as well as age at placement in the facility and duration of the stay in the facility and resilience. Significant differences in temperament- and character-traits between resilient adolescents and adolescents with mental problems were identified. Especially character-traits could be a basis for developing strategies for promoting mental health and further resilience factors, as social support.

Depending on symptom severity, psychopharmacological treatment can be a valuable option in the treatment of depressive disorders in childhood and adolescence. This review provides recommendations for clinical treatment, focusing on suicidality and treatment-resistant patients. The quality of studies regarding the psychopharmacological therapy of depressive disorders in childhood and adolescence has improved since the «black box» warning of the FDA concerning the occurrence of suicidality under treatment with selective serotonin reuptake inhibitors (SSRIs). In Germany, there is proof for a trend toward a more evidence-based psychopharmacological treatment approach within recent years.

Standardized instruments are crucial for a valid assessment of suicidal behavior. In questionnaire assessments the wording of questions can influence the results. In this study we compared the congruence of the Self-Harm Behavior Questionnaire (SHBQ) and the Ottawa Self-Injury Inventory (OSI) as measures of prevalence of suicidal ideation in an adolescent sample (n = 665, mean age: 14.8, SD: 0.66). We found a life-time prevalence of 36.4 % in the SHBQ and a one-year-prevalence of 27.8 % in the OSI. There was good agreement between the two instruments in terms of the prevalence of suicidal ideation and the association with suicide attempts. The observed rates are comparable with other studies in adolescents; differences arise with varying study designs.

In both classificatory systems DSM-IV and ICD-10 self-injury is a symptom of borderline personality disorder (BPD). But it has been shown empirically that self-injury can also occur independent of BPD, for example, as a component of depressive states or even in adolescents without classifiable psychopathology. The scientific discussion about future diagnostic criteria recently led to a proposal to include Non-Suicidal Self-Injury as an independent disorder in the upcoming DSM-5 classification system. Based on recent epidemiological studies of adolescents in Germany, one may assume that approximately 4% of all youths in middle to late adolescence would fulfill the prevalence criterion (criterion A) of the proposed DSM-5 disorder (that is, at least five self-injury incidents within the previous 12 months). A precise classification of Non-Suicidal Self-Injury based on empirical research is needed to further the research, treatment, and prevention of this diagnosis.

Attention Deficit Hyperactivity Disorder (ADHD) is not only a childhood disorder but symptoms persist into adolescence and adulthood in approximately one third of the patients. Especially inattention and poor concentration impair driving performance in road traffic. Adolescents and young adults with ADHD are twice as likely to be involved in traffic accidents as people of the same age. This review sums up the legal situation in Germany and provides an overview of the current existing experimental studies on driving performance of adolescents and young adults with ADHD. Psychostimulant therapy seems to improve driving performance in ADHD patients. At the same time psychostimulants are prohibited, according to the road traffic act. Atomoxetine as a non-stimulant is not mentioned there. Therefore it could be unproblematically prescribed, however, the evidence for improved driving is not as unequivocal as for methylphenidate. The psychoeducation of adolescents and young adults with ADHD concerning their increased risk in road traffic often seems to be insufficient in clinical practice. Given the high number of traffic deaths in these young age groups consulting regarding this matter should be of high priority.

"Chemical restraint" (CR) is an important intervention for the management of aggressive behaviour in children and adolescents. The use of CR during in-patient treatment should be based on standardized intervention strategies. The principles of such an approach include transparent agreement with all participants on fast save and staff-independent reduction of aggressive behaviour without inducing retraumatisation. In clinical practice medication that is effective both for tranquilization and prevention of aggressive escalation is recommended. Current data suggest first line treatment using neuroleptics and benzodiazepines. Due to their potential use both for rapid sedation and prevention atypical neuroleptics are of particular importance. Further controlled studies are needed to evaluate pharmacological treatment of aggressive states with potential (self) harming behaviour in children and adolescents.
