Objective: Patellar dislocations are a common occurrence in orthopedic practice, often accompanied by osteochondral fractures of the retropatellar cartilage surface, known as flake fractures, in up to 58% of cases. The parachute technique represents a simple and cost-effective surgical option aimed at restoring osteochondral integration and preserving native cartilage., Indications: Flake fracture of the patella with osteochondral fragments., Contraindications: Patella fracture., Surgical Technique: By utilizing transpatellar, absorbable sutures, a stable osteochondral interface is achieved without penetrating the fragment itself., Postoperative Management: Postoperative treatment involves partial weight-bearing with a maximum of 20 kg for 6 weeks in full knee extension. In addition, the range of motion of knee flexion is limited to 30° and is increased by 30° every 2 weeks., Results: To examine the short- to medium-term clinical outcomes, all patients with acute patellar dislocation treated using the parachute technique between 01/2012 and 11/2022 were included. Clinical outcomes were assessed using the visual analog scale (VAS), Tegner Activity Scale (TAS), Kujala Score, Knee Injury and Osteoarthritis Outcome Score (KOOS), and International Knee Documentation Committee (IKDC). Out of 20 patients, 19 (10 men, 11 right-sided, 95% follow-up rate) could be recruited for postoperative evaluation. The average follow-up period was 62.5 ± 20.5 months. The clinical outcome scores yielded the following results: VAS 0.5 ± 1.6, TAS 5.8 ± 2.2, Kujala 89.4 ± 12.5, KOOS 87.8 ± 14.1, and IKDC 86.7 ± 14.3. Overall, 18 patients (90.0%) expressed willingness to undergo the procedure again. At the time of follow-up, 19 patients (95.0%) were satisfied with the surgical outcome. One patient (23-year-old man) required revision. None of the included patients suffered from the recurrence of patellar dislocation. Osteochondritis dissecans (OD) is a rare condition with an incidence of 30/100,000. It especially affects male patients aged 10-20 years old. During the staged progression the osteochondral fragments can detach from their base. These can damage the adjacent articular cartilage, which can lead to premature osteoarthritis. Most commonly affected are the knee, ankle and elbow joints. The exact pathogenesis of OD has so far not been clearly confirmed. Several risk factors that can lead to the development of OD are discussed. These include repeated microtrauma and vascularization disorders that can lead to ischemia of the subchondral bone and to a separation of the fragments close to the joint and therefore to the development of free joint bodies. For an adequate clarification patients should undergo a thorough radiological evaluation including X‑ray imaging followed by magnetic resonance imaging (MRI) to assess the integrity of the cartilage-bone formation with determination of the OD stage. The assessment is based on criteria of the International Cartilage Repair Society (ICRS). The instability of the cartilage-bone fragment increases with higher stages. Stages I and II with stable cartilage-bone interconnection can be treated conservatively. For stages III and IV, i.e., instability of the OD fragment or the presence of free fragments, surgical treatment should be performed. Primarily, refixation of a free joint body should be carried out depending on the size and vitality of the fragment. In cases of unsuccessful conservative treatment or fixation, a debridement, if necessary in combination with a bone marrow stimulating procedure, can be employed corresponding to the size of the defect. For larger cartilage defects, an osteochondral graft transplantation should be considered. Overall, OD lesions in stages I and II show a good healing tendency under conservative treatment. In recent years biomechanical and clinical studies have shown that the three-dimensional bony alignment of the lower extremities has a relevant influence on the ligamentous structures of the knee and consequently on the stability of the knee joint. Therefore, in the case of pathological ligamentous damage of the knee joint, a possible malalignment must always be thoroughly evaluated and if necessary, included in the treatment planning. Varus malalignment plays an important role especially with respect to the cruciate ligaments as well as the posterolateral ligamentous structures and has been identified as a significant risk factor for failure after surgical reconstruction of these ligamentous structures. Similar data have also been published for valgus malalignment particularly with respect to its negative influence on the anterior cruciate ligament and the medial capsuloligamentous complex. Alignment deviations in the sagittal plane, especially the inclination of the tibial articular surface (slope), have been extensively investigated in several recent studies. It has been demonstrated that the tibial slope has a relevant influence on the anteroposterior stability of the knee joint and hence on the cruciate ligaments. First clinical studies on the surgical correction of the axis in selected patients showed very promising results with the potential of protecting ligament reconstructions against repeated failure; however, further data especially regarding the importance and the exact indications for an additional alignment correction are necessary., (© 2023. The Working Group of the German Orthopedic and Trauma Society (DGOU) on Tissue Regeneration has published recommendations on the indication of different surgical approaches for treatment of full-thickness cartilage defects in the knee joint in 2004, 2013 and 2016. Based upon new scientific knowledge and new developments, this recommendation is an update based upon the best clinical evidence available. In addition to prospective randomised controlled clinical trials, this also includes studies with a lower level of evidence. In the absence of evidence, the decision is based on a consensus process within the members of the working group.The principle of making decision dependent on defect size has not been changed in the new recommendation either. The indication for arthroscopic microfracturing has been reduced up to a defect size of 2 cm 2 maximum, while autologous chondrocyte implantation is the method of choice for larger cartilage defects. Additionally, matrix-augmented bone marrow stimulation (mBMS) has been included in the recommendation for defects ranging from 1 to 4.5 cm 2 . For the treatment of smaller osteochondral defects, in addition to osteochondral transplantation (OCT), mBMS is also recommended. Background: Commonly used cartilage repair procedures have been established for focal cartilage lesions; however, degenerative lesions with accompanying changes of other intraarticular structures are much more common in clinical practice. This stage, in which classic radiological signs of osteoarthritis are absent, is called early osteoarthritis and is characterized by impaired joint homeostasis with biomechanical and biochemical changes that can have a negative effect on regenerative cartilage therapy procedures., Indication: Cartilage repair procedures are indicated for symptomatic focal early osteoarthritis, defined as cartilage degeneration ICRS grades I or II around a focal cartilage defect ICRS grades III or IV. In more advanced osteoarthritis with significant narrowing of the joint space, cartilage repair procedures are generally contraindicated., Therapy: The most studied cartilage repair procedure for early osteoarthritis is autologous chondrocyte implantation, which has shown acceptable results in case series, although higher failure rates are to be expected compared to focal, traumatic cartilage lesions. The use of bone marrow-stimulating techniques seems to be limited in early osteoarthritis and should only be used in cases of lesion < 2 cm 2 and very little surrounding cartilage degeneration. Concomitant surgical procedures, especially unloading osteotomies, are very important.

Surgical principles for treatment of full-thickness cartilage defects of the knee include bone marrow stimulation techniques (i.e. arthroscopic microfracturing) and transplantation techniques (i.e. autologous chondrocyte implantation and osteochondral transplantation). On the basis of increasing scientific evidence, indications for these established therapeutical concepts have been specified and clear recommendations for practical use have been given. Within recent years, matrix-augmented bone marrow stimulation has been established as a new treatment concept for chondral lesions. To date, scientific evidence is limited and specific indications are still unclear. The present paper gives an overview of available products as well as preclinical and clinical scientific evidence. On the basis of the present evidence and an expert consensus from the "Working Group on Tissue Regeneration" of the German Orthopaedic and Trauma Society (DGOU), indications are specified and recommendations for the use of matrix-augmented bone marrow stimulation are given. In principle, it can be stated that the various products offered in this field differ considerably in terms of the number and quality of related studies (evidence level). Against the background of the current data situation, their application is currently seen in the border area between cell transplantation and bone marrow stimulation techniques, but also as an improvement on traditional bone marrow stimulation within the indication range of microfracturing. Objective: Replacement of the anterior cruciate ligament (ACL) with an autologous tendon together with a high tibial osteotomy (HTO) in one operation., Indication: Simultaneous symptomatic ACL insufficiency and symptomatic varus osteoarthritis., Contraindications: Risk of a higher complication rate for a one-stage procedure, e.g., in loss of motion due to soft tissue contracture, loss of motion due to insufficiency of a existent ACL replacement with tunnel malplacement, tunnel widening of an existent ACL replacement with the risk of tunnel confluence, infection in a former operation. Varus osteoarthritis with a hollow posteromedial tibial plateau (knee abuser). Insertion of the graft and fixation with a button-wire construct at the femur and with a bioabsorbable interference screw and a lag screw at the tibia., Postoperative Management: Postoperative management relating to weight bearing rehabilitation follows osteotomy rules, while range of motion rehabilitation follows the ACL protocol.

What is presently observed is not only a rapid increase in the number of publications, but also a major change in the theoretical principles of behavior therapy. The former onesidedness of a form of behavior therapy theoretically substantiated by classical conditioning only may be regarded as having been completely overcome. For example, this is evident from new approaches to behavior diagnosis as an essential prerequisite of differentiated therapy planning. Recent concepts of the theoretical substantiation of the effectiveness of behavior therapy lead to an increasing consideration of both cognitive and motivational aspects and also take into account the complexity of the patient's social relations. This, in turn, results in the availability of a wide range of indication-specific methods of behavior therapy.

A report is given on dispensary investigations of professional divers. In 32 of a control group consisting of altogether 41 scaphander divers radiologically were found changes in the sense of aseptic bone necroses. 18 of the examined persons showed polyostotic affection and 14 had a monostotic localisation of the foci. Divers with no extensive findings could further work in their profession with a limitation of depth up to 13 depth of water. On the other hand examined persons with severe changes had temporarily to leave their profession. Relevant problems of the clinical symptomatology and the radiological differential diagnostics of aseptic bone necroses are discussed.

Lack of assertiveness is considered, in the sense of the theoretical concept of Ullrich de Muynck and Ullrich, to be due to such components as 1. abnormal attitude toward oneself, 2. fear of criticism and failure, and 3. inadequately developed social skills; after this, the term 'assertiveness' is defined. Previous therapeutical approaches to the remedying of unassertiveness and the underlying theoretical assumptions are critically examined and discussed with particular reference to their effectiveness. These include the training programs developed by Salter and Wolpe which are characterized by certain onesidedness as well as the concept of Ullrich de Muynck and Ullrich, which has a sound theoretical basis and a rather differentiated or specialized structure.
