Hintergrund: Bei Patientinnen und Patienten nach einem Myokardinfarkt ist eine regelmäßige Einnahme der verschriebenen Medikation von großer Bedeutung. Dies kann nicht immer ausreichend sichergestellt werden. Durch Telenursing besteht die Möglichkeit, die Patientinnen und Patienten in diesem Setting zu unterstützen und die Medikamententreue der Betroffenen zu verbessern.Forschungsfrage: Welchen Einfluss hat die telefonische Nachbetreuung durch diplomiertes Gesundheits- und Krankenpflegepersonal, bei Patientinnen und Patienten nach einem Myokardinfarkt auf die Medikamententreue?Methode: Zur Beantwortung der Forschungsfrage wurde eine strukturierte Literaturrecherche in den internationalen Datenbanken PubMed, CINAHL und Cochrane, sowie eine Handsuche in Google Scholar und in Referenzlisten durchgeführt. Die identifizierte Literatur wurde mithilfe eines geeigneten Bewertungsbogen von Behrens und Langer (2010) kritisch bewertet.Ergebnis: Durch Telenursing konnte die Medikamententreue verbessert werden. Zudem bietet telefonische Nachbetreuung Chancen, Reinfarkte, sowie erneute Hospitalisierungen zu reduzieren und die Lebensqualität der Betroffenen zu steigern. Zudem hatte die telefonische Nachbetreuung positive Auswirkungen auf die Selbstwirksamkeit, sowie das Ernährungsverhalten und die körperliche Aktivität.Schlussfolgerung: Einheitliche Messmethoden bei der Evaluierung der Medikamententreue sowie gleiche internationale Standards könnten in Zukunft die Forschungsarbeiten erleichtern. Eine Beobachtung der Patientinnen und Patienten über einen längeren Zeitraum sowie weitere Forschungen auf diesem Gebiet sind nötig, um noch genauere Aussagen hinsichtlich der Medikamententreue zu treffen. Background: For patients, who previously suffered a myocardial infarction, the regular intake of the prescribed medication is essential. However, this cannot always be sufficiently ensured. Telenursing offers the possibility to support patients in this situation and to ensure the regular and correct intake of the prescribed medication.Research question: What is the impact of nurse-led telephone follow-up on medication adherence among patients who had suffered a myocardial infarction?Method: To answer the research question, a structured literature search was conducted in the international databases PubMed, CINAHL and Cochrane, as well as a hand search in Google Scholar and various reference lists. A critical assessment of the identified literature was carried out by the evaluation tool of Behrens und Langer (2010).Results: With the support of telenursing, medication adherence was improved. In addition, telephone follow-up offers opportunities to reduce reinfarctions, as well as re-hospitalizations and to improve the quality of life for the patients after a myocardial infarction. Fur-thermore nurse-led telephone follow-up had positive effects on self-efficacy, nutritional behaviour and physical activity.Conclusion: Consistent measurement methods in the evaluation of medication adherence as well as equal international standards could facilitate future research. Observation of patients over a longer period of time as well as further research in this field are necessary to be able to make more accurate statements regarding medication adherence. Ziel der vorliegenden Arbeit war es, das Adhärenzprofil der mit dem oralen Medikament Tecfidera® behandelten Patienten innerhalb des ersten Therapiejahres in der ambulanten klinischen Routine („real life“) zu untersuchen und aus diesen Erkenntnissen potentiell prädiktive Faktoren für Adhärenz zu identifizieren. Auch galt es, die Gründe für den Therapieabbruch zu finden und diese, sowie auch die Adhärenz im Longitudinalverlauf zu beobachten. Außerdem sollte die Entwicklung der kognitiven und psychobehavioralen Faktoren bei mit Tecfidera® behandelten Multiple Sklerose (MS) -Patienten im ersten Therapiejahr analysiert werden. Subgruppenanalysen sollten die Frage nach Unterschieden zwischen vorbehandelten und therapienaiven Patienten, sowie zwischen Patienten, die direkt von einer anderen MS-Therapie zu Tecfidera® wechselten und denjenigen, die zuvor sechs Monate unbehandelt, aber nicht therapienaiv waren, klären. Der Beobachtungszeitraum betrug insgesamt ein Jahr, in welchem die Patienten zu Therapiebeginn (Baselinevisite, T1) sowie zum Follow-Up nach drei Monaten (T2), nach sechs Monaten (T3), nach neun Monaten (T4) und nach zwölf Monaten (T5) Datenassessments zu klinischen, kognitiven und psychobehavioralen Faktroen durchführten. Die Studie schloss 543 Patienten im durchschnittlichen Alter von 41,3 Jahren (±10,4) ein. 155 Patienten waren nicht vorbehandelt („De-Novo-Patienten“), 388 hatten bereits eine oder mehrere vorausgegangene Therapien erhalten („Switcher“). „Switcher“ wurden weiter in die Gruppe der „Fast Switcher“ (Patienten die in den letzten sechs Monaten vor dem Medikamentenwechsel zu Tecfidera® ein für MS zugelassenes Medikament eingenommen hatten) und die Gruppe „Returning Quitters“ (Patienten die innerhalb der letzten sechs Monate vor dem Wechsel zu Tecfidera® kein Medikament zur Therapie erhalten hatten) unterteilt. Innerhalb eines Jahres blieben 77,16% aller in die Studie aufgenommener Patienten adhärent. Dieser Wert ist vergleichbar mit Daten zu den Injektionspräparaten. 22,84% der Patienten brachen die Therapie mit Tecfidera® im Laufe des Beobachtungsjahres ab. Hauptgrund für den Abbruch waren über das gesamte Jahr gesehen physische (50,82%) und dabei insbesondere gastrointestinale Nebenwirkungen (34,87%). Im zweiten Beobachtungshalbjahr zeigte sich eine Zunahme „compliancebezogener Gründe“ als Abbruchgrund. Die führende physische Nebenwirkung im zweiten Halbjahr war die Blutbildveränderung (vor allem Lymphozytenabfall). Polypharmacia is a frequent challenge in modern medicine due to increasing complexity of both patients' aspects in an ageing society - multimorbidity - and progress of therapeutic approaches. Although risks are clearly increasing with number of simultaneously prescribed drugs, neither analysis of the polypharmacia-problem solely by number nor focusing on deprescribing strategies by number alone is sufficient. The optimum of an overall risk-benefit ratio in the multimorbidity situation has to be described and put into practice by an individualized approach. Pattern analysis may help this, by identifying both the most vulnerable patients and polypharmacia-pattern with pronounced risk burden. Patient vulnerability depends significantly on functional abilities and prioritizing therapeutic goals in a comprehensive deprescribing process needs to implement stage of life and disease.

Einleitung: Eine Nierentransplantation stellt für Patient*innen mit Nierenversagen im Endstadium die bestmögliche Behandlung dar. Die Vermeidung mehrmals wöchentlicher Dialyse ermöglicht vor allem für junge Menschen einen großen Gewinn an Lebensqualität. Allerdings erfordert die transplantierte Niere eine lebenslange exakte Einnahme von Immunsuppressiva, um Abstoßungsreaktionen zu verhindern. Somit wird ein hohes Maß an Medikamentenadhärenz von den Patient*innen erfordert. Diese Medikamente haben auch unangenehme Nebenwirkungen wie Schmerzen, Bluthochdruck etc., sodass die regelmäßige Einnahme von Betroffenen oftmals vernachlässigt wird. Methodik: Mittels Literaturrecherche in den Datenbanken „CINAHL“, „Science direct“, „PubMed“, „Cochrane“ und „Academic Search Elite“ wurden 13 Studien zur Bearbeitung der Forschungsfrage ermittelt. Zwölf quantitative und eine qualitative Studie entsprachen dem EMED-Format. Ergebnisse: Die Medikamentenadhärenz wird von vielen Faktoren auf unterschiedlichen Ebenen beeinflusst. Während zum Großteil soziale Unterstützung eine positive Wirkung auf die Medikamentenadhärenz hat, konnte in den vorliegenden Studien keine Verbindung zu Alter, Geschlecht, Bildungsstand sowie Arbeitsverhältnis gefunden werden. Es konnte nicht eindeutig festgestellt werden ob die vergangene Zeit nach der Nierentransplantation eine Auswirkung auf die Medikamentenadhärenz besitzt. Ein signifikanter Zusammenhang besteht zwischen fehlendem Vertrauen an die Medikamentenwirksamkeit bzw. psychiatrischen Erkrankungen und fehlender Medikamentenadhärenz. Interventionen wie Informationsvideos zur Transplantsituation von Gesundheits- und Krankenpfleger*innen oder das Abschließen von Verhaltensverträgen zeigten einen positiven Effekt auf die Medikamentenadhärenz. Demgegenüber lässt der Einsatz von elektronischen Erinnerungen und Pillenboxen in Kombination mit regelmäßiger Betreuung durch Gesundheits- und Krankenpfleger*innen keine eindeutigen Ergebnisse bezüglich einer Wirkung auf die Medikamentenadhärenz erkennen. Schlussfolgerung: Eine effektive und andauernde Medikamentenadhärenz bei Nierentransplantationspatient*innen wird gewährleistet, wenn ein ganzheitlicher Interventionsansatz angewendet wird. Die Implementierung in die Praxis stellt sich aufgrund der Individualität der Patient*innen und der vielfachen Einflussfaktoren als komplex dar. Für zukünftige Forschung bedarf es nicht nur eines Einbezugs der möglichen negativen bzw. positiven Einflussfaktoren, sondern auch die Beachtung der verschiedenen Phasen der Patient*innenedukation. Introduction: Especially for patients who are young kidney transplantation is the best possible treatment for endstage renal failure. This can be attributed to the avoidance of dialysis, which takes place several times a week. To avoid adverse reactions, patients must take immunosuppressants for their whole life. In order to take the medication correctly, a high degree of medication adherence is stipulated from the patients. The intake can have some negative effects such as pain, hypertension etc., which causes the neglection of regular medicine intake. Methodology: This paper is based on a literature research. Databases such as „CINAHL“, „Science direct“, „PubMed“, „Cochrane“ and „Academic Search Elite“ have been used to determine 13 studies to answer the research-question. 12 of them were quantitative and one of them was a qualitative study. They were structured after the EMED-format. Results: Because of the complexity of medication adherence, different factors have a positive or negative impact. Despite mostly social support having a positive impact, no connection to age, gender, education and working conditions was found. Additionally, varying results to the influence of past time after the transplantation on medication adherence were present. Nonetheless, a significant connection between missing trust or mental illness and absent medication adherence was detected. Regarding interventions, it was detected that showing informational videos and the arranging of behavioural contracts had a possible impact on the medication adherence. Compared with this, the use of electronic reminders, pillboxes and additionally support from nurses yielded different results in regard of their positive or lack of effect of medication adherence. Conclusion: While an effective and enduring drug adherence is given by kidney transplant patients, if a holistic intervention strategy is used, the implementation into the practice displays complicated condition due to the patient’s individuality and different factors. For future research not only possible negative or positive factors, but also the different phases of patient education must be considered.

The medication adherence of people with chronic diseases is often deficient. The present study investigated facilitators and barriers of the implementation of prescribed medication among rehabilitants with cardiological diseases in medical rehabilitation.In total, 22 rehabilitants with cardiological diseases in medical rehabilitation were interviewed by means of a guided interview. All interviews were digitally recorded and literally translated. The analysis of the interviews was software-based (MAXQDA 12) according to the comprehensive content-analytic approach of Mayring. The main categories (barriers/facilitators) were deductively and the subcategories inductively developed.In total, 698 passages were coded in 22 documents, on average 32 per interview (SD=13.2, range 12-65). The 370 entries in the main category barriers are divided into 13 subcategories (e. g. carelessness, lack of information, reservations, burdens, forgetfulness). The 328 statements given in the main category facilitators could be allocated to 14 subcategories (e. g. individual strategies, knowledge, aids, patient insight into the necessity of treatment, communication).The findings suggest that a lack of medication adherence can be improved, in particular through individualized multi-level barrier management.Die Medikamentenadhärenz von Menschen mit chronischen Erkrankungen ist häufig defizitär. Die vorliegende Studie untersuchte Förderfaktoren und Barrieren bezüglich der Umsetzung von ärztlich verordneter Medikation bei Rehabilitanden mit kardiologischen Erkrankungen in der medizinischen Rehabilitation.Es wurden N=22 Rehabilitanden mit kardiologischen Erkrankungen in der medizinischen Rehabilitation mithilfe eines leitfadengestützten Interviews befragt. Alle Interviews wurden digital aufgezeichnet und wörtlich transkribiert. Die Analyse der Interviews erfolgte softwaregestützt (MAXQDA 12) nach dem zusammenfassenden inhaltsanalytischen Ansatz von Mayring. Die in der Hauptkategorie Barrieren vergebenen 370 Nennungen verteilen sich auf 13 Subkategorien (u. a. Sorglosigkeit, Mangelnde Informiertheit, Vorbehalte, Belastungen, Vergesslichkeit). Die in der Hauptkategorie Förderfaktoren vergebenen 328 Aussagen konnten 14 Subkategorien zugeordnet werden (u. a. Individuelle Strategien, Informiertheit, Hilfsmittel, Behandlungseinsicht, Kommunikation).Die Befunde legen nahe, dass eine mangelnde Medikamentenadhärenz v. a. durch ein individualisiertes Barrierenmanagement, das auf mehreren Ebenen ansetzen muss, verbessert werden kann.

Scientific knowledge about epilepsies and their clinical ramifications is rapidly expanding. This becomes an even greater challenge for non-specialists to process. Clinical decision support systems (CDSS) can play an important role as an expert-driven diagnostic and therapeutic tool which gives automated and individualized advice. In addition, medical apps and telemedical procedures for diagnostics and treatment and assistance systems for seizure detection in epilepsy patients have become available.This article provides an overview on current tele-epileptological developments and the available telemedical applications.Based on personal experience and a review of the literature, current epilepsy-specific CDSS, medical apps and assistance systems as well as telemedical applications are characterized and the clinical fields of application are presented.Due to the chronic course and the complexity of the epilepsies and their sequelae, persons with epilepsy could profit from CDSS. Epilepsy-specific CDSS should be usable by medical professionals and patients themselves. Currently, medical apps for people with epilepsy are mostly used to document the clinical course, seizure frequency, medication compliance and side effects. Available seizure detection systems mainly detect generalized tonic-clonic seizures (GTCS). A clinical benefit of such devices is not yet sufficiently confirmed but appears to be likely, because these seizures are specifically associated with sudden unexpected death in epilepsy patients (SUDEP) and interventions are considered to be effective.HINTERGRUND: Die wissenschaftlichen Erkenntnisse über Epilepsien und deren klinische Implikationen nehmen rasant zu. Für Nichtexperten stellt sich die zunehmende Herausforderung, den Überblick hierüber zu bewahren. Hier setzen Clinical-decision-support-Systeme (CDSS) an, indem sie standard- und expertengetriggertes Wissen zur Diagnostik und Therapie individualisiert und automatisiert liefern. Zudem sind Medizin-Apps und telemedizinische Verfahren zur Diagnostik und Therapie sowie Assistenzsysteme zur Anfallsdetektion bei Epilepsien verfügbar.Es soll ein Überblick über die aktuellen Entwicklungen und Anwendungsmöglichkeiten verfügbarer tele-epileptologischer Methoden gegeben werden.Auf der Basis persönlicher Kenntnis und eines Literaturreviews werden epilepsiespezifische CDSS, Medizin-Apps, Assistenzsysteme sowie telemedizinische Anwendungen charakterisiert und deren klinische Einsatzmöglichkeiten dargestellt.Personen mit Epilepsie könnten aufgrund des chronischen Verlaufs und der Komplexität der Erkrankung und ihrer Folgen von CDSS profitieren. Es erscheint wünschenswert, dass epilepsiespezifische CDSS sowohl für die Behandelnden als auch für Patienten nutzbar werden. Apps für Menschen mit Epilepsie dienen derzeit meist der Verlaufsdokumentation von Anfallsfrequenz, Medikamentencompliance und Nebenwirkungen. Gegenwärtige Anfallsdetektionssysteme erkennen vor allem generalisiert tonisch-klonische Anfälle (GTKA). Ein klinischer Nutzen ist noch nicht hinreichend belegt, erscheint aber wahrscheinlich, insbesondere da GTKA mit dem Risiko eines plötzlichen Todes von Epilepsiepatienten assoziiert sind und Interventionen als wirksam gelten.

Approximately 40% of patients with heart failure show cognitive deficits, such as concentration difficulties, attention deficits, and memory impairment. Affected patients have a higher rate of rehospitalization and an increased mortality. This could be due to low adherence mediated by a lack of disease-specific knowledge and strategies to deal with symptom exacerbation. The pathogenesis of cognitive deficits in heart failure is multifactorial. In this respect the heart failure itself as well as cardiovascular risk factors and comorbidities play a role. Orientating neuropsychological screening tests can show a first indication of the presence of cognitive impairment. When identified, the causes can be specifically treated and care can be optimized through certain measures, such as the involvement of caregivers and healthcare providers.

The current COVID-19 pandemic inherits an unprecedented challenge for the treating rheumatologists. On the one hand, antirheumatic drugs can increase the risk of infection and potentially deteriorate the course of an infection. On the other hand, an active inflammatory rheumatic disease can also increase the risk for an infection. In the recommendations of the German Society for Rheumatology (www.dgrh.de), it is recommended that our patients continue the antirheumatic therapy to maintain remission or low state of activity despite the pandemic. In this study, patients with inflammatory rheumatic disease were asked in the first weeks of the pandemic on their opinion of their immunomodulating therapy. The result shows that over 90% of the patients followed the recommendation of the rheumatologist to continue the antirheumatic therapy, and only a small percentage of the patients terminated the therapy on their own. This result was independent of the individual anti-rheumatic therapy. Taken together, the results of this study illustrate not only the trustful patient-physician partnership in a threatening situation but also the high impact of state-of-the art recommendations by the respective scientific society.

A 66-year-old women with chronic hepatitis-b and antiviral therapy was presented with icterus and sickness 2 weeks after cold symptoms. The treatment was stopped by patient 6 month before. The liver was enlarged, the abdomen and further body status unsuspicious.The chemical results showed elevated liver enzymes and bilirubin values. The hepatitis-b status revealed signs of chronical course (HBsAg positive), the virus amount was 21 Mio IU/ml. The sonographic abdominal status was normal.The antiviral therapy was re-established and liver enzymes improved. The virus amount was falling and after 22 weeks antiviral therapy seroconversion (HBsAg negative) and healing up of hepatitis-b was objectified.Acute exacerbation of chronic hepatitis-b followed stopping antiviral treatment can lead to healing by loss of HBsAg.Eine 66-jährige Patientin mit chronischer HBV-Infektion unter antiviraler Therapie stellte sich mit Ikterus und allgemeiner Schwäche 2 Wochen nach einem Infekt der oberen Atemwege vor. Die Therapie setzte die Patientin 6 Monate vor Vorstellung eigenständig ab. Die Leber war leicht vergrößert tastbar, das Abdomen sowie der weitere körperliche Untersuchungsstatus waren unauffällig.Das Labor zeigte deutlich erhöhte Lebertransaminasen sowie ein erhöhtes Bilirubin. Der Hepatitis-B-Infektionsstatus bestätigte das Bild des chronisch-aktiven Verlaufs (HBsAg positiv), die Virusquantifizierung ergab Werte 21 Mio. IU/ml. Sonografisch zeigten sich keine Auffälligkeiten.Die antivirale Therapie wurde wieder aufgenommen mit nachfolgender Besserung der Lebertransaminasen und Abfall der Virusmenge. Nach 22 Wochen Behandlung traten eine Serokonversion (HBsAg negativ) und Ausheilung der Hepatitis ein.Im Rahmen eines akuten Schubs einer chronischen Hepatitis B nach Beendigung der antiviralen Therapie ist eine Ausheilung mit Verlust des HBsAg möglich.

Einleitung: Ungefähr fünf Prozent der Bevölkerung in Deutschland und Österreich leiden an bipolaren Störungen. Bei der Hälfte aller bipolaren Langzeitpatient*innen kommt es zu Adhärenzproblemen. Das Ziel dieser Bachelorarbeit ist es Faktoren ausfindig zu machen, welche Einfluss auf die Therapieadhärenz von Menschen mit bipolaren Störungen haben und Interventionen zu suchen, wie das Pflegepersonal diese verbessern kann. Methodik: Die systematische Literatur-Recherche erfolgte vorwiegend in den Datenbanken „Cinahl“, „Pubmed“, „SpringerLink“ und „Cochrane“. Es wurde Literatur der letzten sieben Jahre untersucht. Für die Bewertung der Literatur wurde der Fragebogen von Brandenburg, Panfil und Mayer (2007) verwendet. Ergebnisse: Es wurden insgesamt 14 Faktoren identifiziert, welche in drei Kategorien, den therapiebezogenen, individuellen und störungsbezogenen Faktoren zusammenfasst wurden. Die zwei am häufigsten genannten Interventionen, um die Therapieadhärenz zu verbessern, sind Psychoedukation und die Stärkung der therapeutischen Allianz. Interventionen mit telekommunikativen Aspekten zeigen Potenzial, um zukünftig mehr Anwendung im Gesundheitswesen zu finden. Schlussfolgerung: Mangelnde Adhärenz ist ein komplexes Phänomen, welches mehr Forschung erfordert um Patient*innen effektiv helfen zu können. Die Problematik in der Erhebung von wissenschaftlichen Daten über Adhärenz, ist die objektive Überprüfbarkeit über das Verhalten der Patient*innen. Pflegepersonen haben nicht die Möglichkeit Patient*innen außerhalb ihrer Institution effektiv zu unterstützen. Der Einsatz von telekommunikativen Technologien könnte Abhilfe für dieses Problem schaffen. Introduction: Roughly five percent of the German and Austrian population suffer from bipolar disorder. Almost half of the long-term patients develop poor adherence to treatment over the course of time. The aim of this bachelor thesis is to evaluate factors associated with poor adherence to treatment in bipolar disorder and to analyze interventions set by the nursing stuff with the aim to enhance adherence to treatment. Methodology: The systematic literature search was conducted in the databases „Cinahl“, „Pubmed“, „SpringerLink“ and „Cochrane“. Literature of the last seven years was analyzed. The studies were rated with the questionnaire of Brandenburg, Panfil und Mayer (2007). Results: 14 factors were identified, that were associated with poor adherence. The factors were split up in three categories, treatment related factors, individual factors and disorder related factors. The two most common interventions to improve adherence to treatment are psychoeducation and the improvement of therapeutic alliance. Interventions with mobile technologies display potential for broader implementation in the healthcare system. Conclusio: Poor adherence is a complex phenomenon which needs further research to be fully understood. Gathering scientific data about adherence is challenging, since it is not easy to monitor the behavior of patients in an outpatient setting. The use of mobile technologies could solve this problem, thus giving it a promising outlook.

Einleitung: Die Nierentransplantation ist bei einer chronischen Niereninsuffizienz die beste Behandlungsmethode. Um das Behandlungsziel zu erreichen, ist eine gewissenhafte Einnahme der immunsuppressiven Therapie essentiell. Die Immunsuppressiva bestimmen unter anderem die Funktionsdauer des Transplantats. Ziel dieser Arbeit ist herauszufinden, ob sich der Einsatz von speziellen Telenursingsystemen fördernd auf die Medikamentenadhärenz auswirkt und welche Voraussetzungen für eine Akzeptanz dieser benötigt werden. Methodik: Grundlage dieser Literaturarbeit ist eine systematische Literaturrecherche in unterschiedlichen wissenschaftlichen Datenbanken. Durch eine gezielte Selektion der recherchierten Literatur, konnten zwölf passende Studien zur Beantwortung der Forschungsfragen verwendet werden. Die Ergebnisse dieser Studien werden in Form einer Ergebnissynthese gegenübergestellt. Ergebnis: Es zeigt sich, dass durch die Anwendung solcher Telenursingssysteme die Medikamentenadhärenz von Patient*innen nach einer Nierentransplantation gesteigert werden kann. Der Einsatz von mobilen Unterstützungsmöglichkeiten wird von Patient*innen als positiv wahrgenommen und es würde die Kommunikation mit den Gesundheitsdienstleister*innen erleichtern. Durch die stetige Entwicklung der Technologie und der zunehmenden Digitalisierung im Gesundheitswesen sind Telenursing und die Telemedizin Instrumente der Zukunft. Introduction: A kidney transplantation is the best treatment for chronic kidney failures. Conscientious use of immunosuppressive therapy is essential to achieve the desired treatment goal. Among other things, the immunosuppressant determines the period for which the transplants function. The aim of this work is to find out whether the use of special telenursing systems promotes drug adherence and which conditions are required for the patient´s acceptance. Methods: This work is based on a systematic literature research in various scientific databases. Through targeted selection of the researched literature, twelve suitable studies could be used to answer the research questions. The results of these studies are compared in the form of a synthesis of results. Results: According to the studies, the use of such telenursing systems can increase the patient's adherence to medication after a kidney transplantation. Patients perceive the use of mobile support options as positive and it would facilitate the communication with health care providers. Discussion: The use of information and communication technologies not only requires the patient´s acceptance, but also the necessary knowledge and the essential instruments to use them. Due to the constant development of technology and the increasing digitization in healthcare, telenursing and tele-medicine are instruments for the future.

Development of inhaled therapies for COPD and asthma

After the decisions of the German Federal Constitutional Court in 2011 and the Federal Supreme Court in 2012, involuntary treatment was not approvable for a period of seven months in the State of Baden-Wuerttemberg. Previous analyses of routine data had demonstrated that at that time in a rather small group of patients, aggressive incidents and coercive interventions had significantly increased and then decreased to the previous level after the new legislation came into force. The changes concerned a relatively small group of involuntary patients. Based on an analysis of medical charts in 6 hospitals, this study aimed to investigate 1) whether refusal of prescribed medication became more frequent in that period and 2) how frequently antipsychotic medication was administered without coercion.We conducted a longitudinal intra-individual comparison and included all of the patients with schizophrenic and manic disorders who had been admitted in the period without the option of involuntary treatment and in a defined control period one year before as well (N = 174). Thus, study group and control group were identical.In the period without the option of involuntary treatment, patients remained involuntarily committed significantly more frequently (+ 26 %) but only insignificantly longer. Length of stay and number of mechanical restraints remained unchanged, number of seclusions doubled, and some patients could not leave the ward for long periods of time and had frequent readmissions. Persistent refusal of prescribed medication was significantly more frequent (+ 130 %, p.001). However, the percentage of patients who received an antipsychotic drug during their hospital stay did not differ (96.0 vs. 96.6 %). The dosage at discharge as calculated in chlorpromazine units tended to be even higher during the period without option of involuntary treatment (+ 7.9 %, p = .06). All differences concerned both voluntary and involuntary patients.Without the option of involuntary treatment, persistent refusal of medication and different forms of deprivation of liberty increased. Nevertheless, oral antipsychotic treatment was realized in nearly all cases until discharge.Nach den Entscheidungen des Bundesverfassungsgerichts von 2011 und des Bundesgerichtshofs von 2012 waren Zwangsbehandlungen in Baden-Württemberg vorübergehend für sieben Monate nicht genehmigungsfähig. Auswertungen aus Routinedaten hatten gezeigt, dass bei einer kleinen Gruppe von Patientinnen und Patienten aggressive Übergriffe, Fixierungen und Isolierungen signifikant zunahmen und danach wieder auf das vorige Niveau zurückgingen. Die vorliegende Studie sollte mittels der Analyse von Krankenakten aus sechs Klinikstandorten klären, ob Verweigerung der angeordneten Medikation häufiger war und wie häufig eine antipsychotische Medikation ohne die Möglichkeit des Zwangs realisiert wurde.In einem quasi-experimentellen intraindividuellen longitudinalen Vergleich bezogen wir alle Patientinnen und Patienten mit schizophrenen und manischen Störungen ein, die eine Behandlungsepisode sowohl in der Zeit ohne zulässige Zwangsmedikation als auch in einem Zeitraum gleicher Länge im Vorjahr aufwiesen (N = 174).In der Periode ohne Möglichkeit einer Zwangsbehandlung waren die Patientinnen und Patienten im Durchschnitt signifikant häufiger (+ 26 %), aber nur insignifikant länger untergebracht, Aufenthaltsdauer und Anzahl der Fixierungen blieben unverändert, Isolierungen verdoppelten sich, einige Patienten hatten sehr lange keinen Ausgang und wurden häufig wieder aufgenommen. Medikamentenverweigerung war signifikant häufiger (+130 %, p .001). Dennoch unterschied sich der Anteil der Patienten, die im Verlauf ein Antipsychotikum einnahmen, nicht (96,0 bzw. 96,6 %), die Dosis in Chlorpromazin-Einheiten bei Entlassung war in der Periode ohne Zwangsbehandlung tendenziell sogar höher (+7,9 %, p = 0.07). Die Unterschiede betrafen gleichermaßen freiwillig und unfreiwillig behandelte Patienten.Ohne die Möglichkeit der Genehmigung einer Zwangsbehandlung nahmen anhaltende Medikamentenverweigerung und Freiheitseinschränkungen verschiedener Art zu, eine orale antipsychotische Behandlung wurde in fast allen Fällen realisiert.

Psychiatric disorders and psychological problems are common in patients with diabetes mellitus. There is a twofold increase in depression which is associated with suboptimal glycemic control and increased morbidity and mortality. Other psychiatric disorders with a higher incidence of diabetes mellitus are cognitive impairment, dementia, disturbed eating behavior, anxiety disorders, schizophrenia, bipolar disorders and borderline personality disorder. The coincidence of mental disorders and diabetes mellitus has unfavorable influences on metabolic control and micro- and macroangiopathic complications. Improvement of therapeutic outcome is a challenge in the modern health care system. The intentions behind this position paper are to rise awareness of this special set of problems, to intensify cooperation between involved health care providers and to reduce incidence of diabetes mellitus as well as morbidity and mortality from diabetes in this patient group.

Motivational interviewing (MI) has become established nowadays as an approach for a cooperative style of conversation to promote intrinsic motivation for change by exploring and resolving ambivalences. The change of addictive behavior is no longer sought by exerting pressure or lecturing/converting attempts of convincing or persuasion but by activating existing but "buried" or newly acquired self-motivation to change. The MI is now also used to change the treatment of other health-related behavior and chronic diseases, including schizophrenic disorders. Compared to the efficacy of MI in the addiction area, the data situation in schizophrenic patients is still insufficient. According to the available studies, MI can positively influence important aspects of disease-related impairments, such as medication adherence, the frequency and severity of psychotic relapses, the duration of hospitalization, the level of function, insight into the disease and cognitive rehabilitation. The practical implementation of MI requires a good knowledge of the method as well as changes in treatment principles and work processes.

The overall goal of all measures for medication safety is the optimization of drug treatment and the prevention of unnecessary risk that potentially endangers patients. Per definition, the goal of medication safety is a beneficial patient outcome. Particularly in primary care, the success of many medication safety strategies will depend on the active participation of patients, as in this healthcare sector the patient is responsible for many substeps of the medication process.From a healthcare systems perspective, a number of medication safety strategies have been developed that aim to support the patient in their active role in the medication process. These strategies intend to safeguard the processes of care including transfer of information, but also aim to prevent intentional and unintentional nonadherence. The prerequisites for successful implementation are targeted awareness-raising measures to sensitize all participants for potential risks in the medication process. Moreover, readily available medication safety strategies must be easily accessible and promoted both to patients and healthcare providers.

The abrupt discontinuation of lithium in patients with bipolar disease puts the patient at a high risk for a relapse. Lithium is a drug with a narrow therapeutic index. Intoxications might occur during the treatment. The individual risk-benefit-ratio should be discussed with the patient before starting lithium. Atypical antipsychotics are an alternative to lithium in the treatment of bipolar disease. Several guidelines recommend the use of atypical antipsychotics in a long acting (depot) formulation in bipolar patients (off label use). The long acting (depot) formulations offer the benefit that during a hypomanic or a beginning manic episode, the drug can not easily be discontinued. During the phase of early signs of a beginning manic episode the patients tend not to see the need to take any medication (lack of insight into illness) which leads to non-adherence that puts the patient at a high risk for a manic episode. If lithium is prescribed, special counselling is needed, especially about fluid intake and potential drug interactions (NSAIDS, ACE-inhibitors, diuretics).

This article describes the pharmaceutical care of an adolescent with type 1 diabetes mellitus, who took part in the DIADEMA study. Diabetes was diagnosed for three years and his baseline HbA1c-value was 9.3 %. In the DIADEMA study adolescents received pharmaceutical care provided by community pharmacists in addition to usual care provided by diabetologists and diabetes educators. Patients in the intervention group received monthly scheduled visits with the community pharmacists and on-demand telephone calls. Fundamental contents of the pharmacist᾽s interventions were in particular self-monitoring of blood glucose, prevention of acute and long-term diabetes complications and conscientious consumption of alcohol. Furthermore difficulties in individual insulin therapy and current life conditions were addressed.

The antiretroviral therapy (ART) enables an almost normal life with HIV infection. Does this also hold true for sexual intercourse, i.e. safe sex without condoms? The European PARTNER-study provides evidence for a very low risk of transmission for condomless sex under optimized conditions such as early, adherent and lasting ART (200 RNA copies/ml) as well as stable partnerships. The risk of transmission is enhanced with non-compliance or shortlasting ( 6 months) ART, increased HIV-1 RNA load and frequent partnerships.

Addressing maladherence in schizophrenia Abstract. To prevent neurotoxic and demoralizing relapses in the course of schizophrenia maladherence was shown as one of the most important factors. Empirical evidence is presented and organized in a conceptual four level framework to organize an individually tailored approach to promote adherence more effectively and prevent the demoralizing function of classical insight by neglecting the underlying motivational processes in routine care. This four level framework to address maladherence encompasses (a) functional analysis of (mal-)adherence as an instrumental goal oriented behaviour, (b) addressing dysfunctional attitudes related to disorder and treatment,Zusammenfassung. Mangelnde Behandlungsbereitschaft ist der gewichtigste Einzelfaktor für Rückfälle bei schizophrenen Störungen. Rückfälle ihrerseits wirken als kumulativ fortschreitende Recovery vernichtende neurotoxische Prozesse. Patient- vs Physician-Reported Implementation of and Compliance to Anti-Osteoporotic Medication One Year after Sustained Fragility Fracture Abstract. We were interested why therapy recommendations made by specialists are often not followed by general practitioners (GPs) and patients. We evaluated systematic questionnaires comparing both, patient and GP statements (n = 151 each) with regard to the implementation of and compliance to specific therapy recommended by an osteologic specialist one year after an osteoporotic fracture. In 53 % GPs prescribed antiosteoporotic drugs, more often if the indication for treatment was less aggressive (p0.001). Once prescribed, in 94 % of cases the GPs' medication followed the specialists' recommendations. 74 % of patients followed their GP's prescription. Patients most often stated a missing prescription as the reason for not taking drugs (39 %), whereas GPs cited a missing interest of their patients (44 %). The observed discrepancies call for a melioration in the communication between all parties involved.Zusammenfassung. Im Rahmen eines Qualitätsprojekts zur Behandlung von Osteoporosefrakturen fiel auf, dass fachärztliche Therapie-Empfehlungen oftmals nicht umgesetzt wurden, was genauer untersucht werden sollte. Durchgeführt wurde eine systematisch vergleichende Fragebogen-Evaluation bei Patienten wie Hausärzten (n = 151) ein Jahr nach empfohlener Osteoporosetherapie. In 53 % der Fälle folgten die Hausärzte der vom Spezialisten empfohlenen Osteoporosetherapie-Indikation, häufiger, wenn diese medizinisch enger gestellt wurde (p0,001). Sofern der Patient eine hausärztliche Verordnung erhalten hatte, entsprach diese meist der Spezialisten-Empfehlung (94 %). 74 % der Patienten folgten der hausärztlichen Verschreibung. Patienten gaben am häufigsten eine fehlende Verordnung als Grund für die Nichteinnahme der vom Spezialisten empfohlenen Medikamente an (39 %), hingegen nannten Hausärzte am häufigsten ein Patienten-Desinteresse (44 %). Die beobachteten Diskrepanzen sprechen für einen Bedarf an verbesserter Information, insbesondere im Fall einer eher aggressiv gestellten Therapieindikation.

Allergies are steadily gaining in importance in the Western world. For over one hundred years, immunology has been the only causal treatment. Specific immunotherapy (SIT) aims at the cure of allergy or at least freedom from allergy symptoms. In association with this, adherence poses a complex problem. Both treatment applications commonly used in Germany-sublingual and subcutaneous immunotherapy-show poor persistence on the part of the patients. In most cases, SIT is not carried out to the end of the recommended duration and instead is discontinued prematurely. Corresponding figures from 3‑year studies in the literature range from 41- 93% for uncompleted SLIT and from 40-77% for uncompleted SCIT. Patient adherence is subject to influencing factors of various dimensions that are interdependent in complex relationships. The physician-patient relationship is just as decisive a factor for treatment success as the patient's understanding of allergy, treatment, and the importance of adherence.

Einleitung: Das Nichteinhalten des verordneten Behandlungsplans führt bei rheumatoider Arthritis zu einer negativen Auswirkung auf die Funktion des Bewegungsapparats und den radiologischen Status. Um effektiv gegen Nonadhärenz vorgehen zu können sind Assessments notwendig die dieses Verhalten messen. In dieser Studie werden aktuelle Assessments zur Adhärenzmessung recherchiert und mit der ICF verlinkt. Methodik: Um dieses Ziel zu erreichen wurde in den Datenbanken Cinahl, Pubmed und Cochrane im Zeitraum von 12/2016 bis 02/2017 eine Recherche mit den Mesh-Terms arthritis, rheumatoid und patient compliance durchgeführt. Insgesamt konnten 17 Assessments identifiziert werden. Davon konnten neun mit den Regeln von Cieza et al. (2016) mit der ICF gelinkt werden, um im Anschluss mit Daten aus Interviews von PatientInnen mit RA verglichen zu werden. Resultate: Die Aufteilung und die Anzahl der Assessments konnte aufgrund der aktuell gewonnenen Ergebnisse zur Vergleichsstudie um neun Messmethoden erweitert werden. Der durchgeführte Linkingprozess der neun linkbaren Assessments ergab 110 Items die zu 32 verschiedenen ICF Kategorien verbunden werden konnten. Der Vergleich mit den Linkingdaten, die von Ritschl et al. (2017) aus den Interviews mit RA PatientInnen gewonnen Items, lässt lediglich bei den zwei Assessments CQ und MMAS-4 eine annähernde Übereinstimmungen mit den Perspektiven, Werten und Bedürfnissen der PatientInnen erkennen. Conclusio: Die Fragebögen CQ und MMAS-4, die annähernd ähnliche Linkingergebnisse mit den gelinkten Interviewdaten abbilden, sind kurz, prägnant und einfach in der Handhabung und können dadurch sehr gut in den klinischen Alltag impliziert werden. Ein wichtiger Forschungsansatz wäre, anhand der Linkingergebnisse der Interviewdaten, einen kompatiblen Fragebogen zu erstellen und die derzeit nicht im Detail übereinstimmenden Assessments abzulösen. Introduction: Non-compliance of prescribed treatment plan in case of rheumatoid arthritis leads to negative impacts on the functionality of the musculoskeletal system and as well radiological status. To work in an effective way against this non-adherence it is essential to implement assessments which provide measurements of this behavior. Within this study current assessments concerning measurement of adherence should be researeched and linked with ICF. Methods: To finally reach that goal researches with Mesh-Terms arthritis, rheumatoid and patient compliance were executed from 12/2016 until 02/2017 in following databases Cinahl, Pubmed and Cochrane. Within that research in total 17 assessments could have been identified. Form these assessments nine could have been linked with the rules from Cieza et al. (2016), which were afterwards compared with data gathered from interviews with RA patients. Results: Because of obtained findings within the current research it was possible to expand distribution and amount of assessments compared to the comparative study by nine measurement methods. The result of the executed linking process of these nine linkable assessments were 110 items which could have been linked to 32 different ICF-categories. A comparison with linking data from Ritschl et al. (2017) out of interviews with RA-patients only provides an approximate match among the assessments CQ and MMAS-4 with perspectives, values and needs of the patients. Conclusion: CQ and MMAS-4 questionnaires, which show an approximate match with the linked data from the interviews are short, precise and simple to handle. Because of these characteristics they can be easily implemented into the clinical daily routine. An important research approach would be to create a compatible questionnaire which is based on linking results coming from interview data. Background Multiple sclerosis is a chronic disease with unknown cure. Though several therapies exist, only consequent medication receipts lead to satisfying results. To reach this, qualified patient education seems to be the relevant key to constructive adherence. Therefore, a patient academy was introduced and the benefits of this academy were examined. In addition, the comparatively new oral drugs have been examined as they seem to be more comfortable in receipts, but the concrete reasons for changing the treatment have not been well examined prior to this study. Material and Methods A prospective online survey as a questionnaire was carried out in Germany to gain data on treatment adherence and reasons for changes in medical treatment of MS patients. Patients were asked about their own perception of knowledge and adherence to the treatment of their disease. Furthermore, the reasons and recommendations for treatment change were examined. Results 214 patient questionnaires were used as a base for this analysis. Approximately 87% recorded a RRMS. 113 patients changed from parenteral treatment to oral drugs. The self-assessment of treatment adherence was 1.46 on average, the self-assessment of the level of knowledge was 2.1 (1=very good, 5=very bad). Most patients visited 5 or fewer meetings at patients’ academy. There was a significant correlation in linear regression between knowledge and adherence (p=0.0006, r²=0.05). The most important reason for treatment change was the general hope for improvement. The most influential factor on the decision to change treatment was the advice given by the medical professionals, namely treating physicians and MS nurses. Conclusions The most important factor for adherence is professional mediated knowledge. The recommendations for treatment change by commercial advertising has not been identified to be a relevant factor.

In the present study patients with an osteoporotic fracture were examined in order to determine how many patients decided to continue their therapy as outpatients having taken part in osteoporosis training at the Dr. Becker Rhein-Sieg Clinic.Patients who had previously suffered an osteoporotic fracture and thereafter received inpatient rehabilitation were selected for the study. Patients (n = 95) who had taken part in inpatient rehabilitation after an osteoporotic fracture were approached either via telephone or mail and were then followed up after 6 and 12 months with regard to their adherence to the treatment.From a total of 95 patients, after 6 months 82 % (n = 78) were contacted successfully and included in the study. 68 out of 78 patients (72 %) had continued their medication. After 1 year 76 % of patients (n = 72) were successfully contacted once. Here, 62 % of patients (n = 59) had continued their medication under medical supervision. In the 1‑year follow-up period there were eight cases of falls, and one of these resulted in a peripheral fracture, which was treated conservatively.Osteoporosis-specific information and osteoporosis training of patients after osteoporotic fracture are important tools to improve the motivation and compliance of patients. One possibility to realize this is osteoporosis-specific training with video presentations and individual care and education of the patients within outpatient or inpatient rehabilitation.

Patients' lack of knowledge about their discharge medication can endanger patients' safety after their hospital stay. This is especially the case with regard to medications that were newly prescribed during the hospital stay and are intended to be used after discharge or medications with an increased risk for adverse drug reactions (high-risk drugs). The aim of this study was to analyse the patients' level of knowledge about their discharge medication and to identify influence factors.In a bicentric survey patients were interviewed prior to their discharge from an acute and a geriatric rehabilitation hospital. They were asked about their discharge medication in a structured interview. Influence factors were statistically analysed by Tobit regression.In total, 179 patients were interviewed. On average, patients named 48% of their discharge medication correctly (95% CI: 46-50%). Influence factors for knowledge deficits were the lack of a medication plan, an older age, the hospitalization in a rehabilitation hospital and a long hospitalization. 81% of the patients had at least one drug in their discharge medication, which was newly prescribed during the hospital stay. 11% of those drugs were named correctly, the potency was named correctly in 6%, the indication in 8%. For almost two-thirds of the patients at least one high-risk drug was recommended in the discharge letter, among them most frequently oral anticoagulants and opioid analgesics. 38% of these high-risk drugs were named correctly.Our results demonstrate an urgent need to train patients about their discharge medication, especially if medications are included that were newly prescribed during the hospital stay and recommended for further use after discharge or medications with an increased risk of adverse drug reactions. Particularly older patients and patients of a rehabilitation hospital after long hospitalization should be intensively counselled and obtain a medication plan upon discharge.

Activity-based treatment regimens with anti-vascular endothelial growth factor (anti-VEGF) are currently the gold standard for treatment of exudative age-related macular degeneration (nAMD). Whereas injection frequencies of approximately seven injections in the first year and six in the second year are expected with a pro re nata (PRN) regimen, retrospective real life observations have recorded significantly reduced numbers of injections. This study was carried out to investigate the reasons for the reduction in follow-up control appointments and to find out whether a telemedicine network could influence the motivation and compliance for regular control examinations and treatment.The patient collective included 210 eyes from 191 patients with nAMD treated by anti-VEGF therapy in 2010 and 2011. The activity-based anti-VEGF treatment, control examinations and treatment intervals were performed according to the guidelines over a mean follow-up of 2 years. In another collective of 100 eyes from 100 patients with treatment of nAMD 2 groups were observed: 1 group with patients for whom control examinations were carried out close to home including an online transmission of the results to the treating retinal center and another group in which the patients had to be examined in the treatment center.After 140 weeks 50 % of the patients in the first collective regularly attended control examinations and after 1 year the number was 79 %. After 2 years the probability of continuous supervision is given for only 62 % of the patients, whereas in 38 % the treatment was terminated. Of these patients treatment was terminated in 8 % due to valid criteria, whereas in 30 % of the patients the termination was unintentional. The main reason (38 %) for an unintentional termination of examination and treatment was the frequent and long journey. Patients in the second collective had a significantly higher compliance with respect to the control examinations (p 0.001) and number of injections (p = 0.02) over the period of nearly 2 years due to the introduction of electronic transmission of images.A long-term therapy of nAMD in the clinical routine can be achieved by a close relationship with the ophthalmologist, continuous follow-up controls and therapy cycles. A close telemedical networking between the ophthalmologist and the treatment center can lead to better patient compliance. Furthermore, the construction of such platforms represents a challenge not only for the treatment of nAMD but also for other diseases.
