Objective: The aim of this study was to record the current care and control structures in place for patients with ethyltoxic liver cirrhosis while being prepared for a liver transplant (LTX) at German transplant centers. In addition, it was also intended to analyze the associated barriers as well as the view of the practitioners on ways to improve care of this patient group., Methods: In an exploratory descriptive qualitative design, 11 interviews with practitioners from 10 of the 22 German LTX centers were conducted and analyzed using qualitative content analysis., Results: There were considerable differences in the care and control structures in place at the LTX centers. Addiction therapy counseling or treatment were not integrated into the treatment concept at all centers. Structural barriers arose from insufficient funding and staffing. Practitioners recommended expansion of treatment options as well as standardizing treatment concepts., Discussion: The results of our study point to a need for action both in the area of the structures of the individual LTX centers and overall at the system level. Fast-track treatment pathways reduce the frequency of postoperative complications in elective colorectal resections by approximately 40% and due to the rapid recovery reduce the postoperative duration of hospitalization by approximately 50%. Specialized nursing personnel (enhanced recovery after surgery, ERAS, nurses) have already been appointed internationally to accompany and monitor the execution of multimodal perioperative treatment. In November 2018 a fast-track assistant was appointed in the Clinic for General and Visceral Surgery of the Municipal Clinic in Solingen for coordination of the fast-track treatment pathway. The results confirmed that a high adherence to perioperative fast-track treatment concepts can also be achieved in the German healthcare system by the assignment of specialized nursing personnel, with the known advantages for patients, nursing personnel, physicians and hospital sponsors., (© 2021. Fast-track or ERAS programs can give clinically relevant benefits for patients undergoing surgery. They also have financial advantages for the hospital. Despite these facts, fast-track has been implemented in only very few hospitals in Germany. Neither scientific evidence nor reimbursement regulations explain this lack of fast-track acceptance. It seems however that a structured process of change from traditional perioperative medicine to evidence based fast-track therapy is missing in Germany. In other countries, structured programs for fast-track or ERAS implementation have been successful in many hospitals. This article describes structured fast-track implementation divided into three consecutive steps: organisational preparation, clinical implementation and continuous examination of fast-track adherence, functional recovery, morbidity and postoperative length of hospital stay. The 2015 European Guidelines on Diagnosis and Treatment of Pulmonary Hypertension are also valid for Germany. While the guidelines contain detailed recommendations regarding pulmonary arterial hypertension (PAH), they contain only a relatively short paragraph on other, much more common forms of PH such as PH due to left heart disease. Despite the lack of data, targeted PAH treatments are increasingly being used for PH associated with left heart disease. This development is of concern because of limited ressources and the need to base treatments on scientific evidence. On the other hand, PH is a frequent problem that is highly relevant for morbidity and mortality in patients with left heart disease, representing an unmet need of targeted PH therapies. It that sense, the practical implementation of the European Guidelines in Germany requires the consideration of several specific issues and already existing novel data. This requires a detailed commentary to the guidelines, and in some aspects an update already appears necessary. In June 2016, a Consensus Conference organized by the PH working groups of the German Society of Cardiology (DGK), the German Society of Respiratory Medicine (DGP) and the German Society of Pediatric Cardiology (DGPK) was held in Cologne, Germany. This conference aimed to solve practical and controversial issues surrounding the implementation of the European Guidelines in Germany. To this end, several working groups were initiated, one of which was specifically dedicated to PH associated with left heart disease. This position paper summarises current developments in chronic thromboembolic pulmonary hypertension (CTEPH) including diagnostic approaches and treatment options. Based on the guidelines of the task force of CTEPH experts at the 5th World Symposium on Pulmonary Hypertension in Nice 2013. Open questions arising during the treatment of patients with CTEPH are addressed. Patients with suspected CTEPH should undergo echocardiography and cardiopulmonary exercise testing. A ventilation/perfusion scan is the recommended imaging test for screening in the diagnostic algorithm for the evaluation of CTEPH. CTEPH-patients should be discussed in an expert center with an interdisciplinary team and an experienced PEA surgeon to decide the further treatment. Pulmonary endarterectomy (PEA) is the treatment of choice for patients with CTEPH. Medical therapy with PH-targeted medications for inoperable CTEPH and residual disease after PEA should only be initiated if evaluation reveals that the patient is no candidate for a PEA. In the 2009 European Guidelines on pulmonary hypertension one section covers aspects of pathophysiology, diagnosis and treatment of chronic thromboembolic pulmonary hypertension (CTEPH). The practical implementation of the guidelines for this disease is of crucial importance, because CTEPH is a form of pulmonary hypertension which can be surgically cured. It is, however, frequently diagnosed late in the course of disease and often treated not correctly. In the European Guidelines CTEPH is addressed relatively briefly, although it is a common form of PH which is often overlooked. Any patient with unexplained PH should be evaluated for the presence of CTEPH. A ventilation/perfusion lung scan is recommended as the first step to exclude CTEPH. If the ventilation/perfusion lung scan or multislice CT angiography reveals perfusions defects suggesting the diagnosis of CTEPH, the patient should be referred to a centre with expertise in the medical and surgical management of these patients. After diagnosis of CTEPH the case has to be reviewed by an experienced surgeon in a PEA centre for assessment of operability. Based on the evidence from large clinical and epidemiological studies indicating an independent prognostic role of heart rate in cardiovascular disease, the assessment and correction of elevated heart rate is of significant prognostic relevance. In the present study conducted with the support of 74 specialists of Internal Medicine in 2009 in Austria, heart rate in patients with coronary heart disease (CHD) and chronic stable angina pectoris was evaluated in relation to pre-existing and concomitant diseases, angina-severity (CCS), angina-symptoms and treatment. For all variables, descriptive statistical analyses were performed according to three predefined groups with heart rates <60 bpm (HR-1), 60-70 bpm (HR-2), and >70 bpm (HR-3). Of the 1280 patients 21.8% had a heart rate of <60 bpm, 39.6% of 60-70 bpm, and 38.5% of >70 bpm. A significant association was shown between elevated heart rate and concomitant disease, e.g., peripheral artery disease (p = 0.046), psoriasis (p = 0.029), previous acute coronary syndrome (p = 0.001), COPD (p < 0.001), diabetes mellitus (p = 0.004), and depression (p < 0.001). CCS-severity was correlated with heart rates (mean values; CCS-0: 66.8 bpm, CCS-IV: 77.5 bpm). Angina-pectoris (AP) symptoms were more common in patients with heart rates of >70 bpm (HR-3: 1,2 AP-events/week; HR-2: 1 AP-events/week; HR-1: 0,7 AP events/week; each time p < 0.001). The majority of patients were treated with betablockers (74%); yet, the average dose was approximately half the maximal recommended dose. Despite inadequate heart rate reduction in patients on betablockers, selective heart rate lowering agents such as ivabradine were used in only 1.6% of patients. Overall, these results illustrate that heart rate as an important therapeutic target in CHD-patients with chronic stable angina is still underestimated in contemporary clinical practice.

The Club of Female Professors of the Medical University Vienna (MUW) was founded in 2005 to strengthen and improve the impact of common concerns and ideas. The mission of this continuously growing club is to improve the situation of women, including colleagues, coworkers, students and patients, at the MUW. This important goal is pursued by continuous interaction with rectorate, academic senate and politicians.

The spectrum of venous thromboembolism encompasses deep vein thrombosis, acute thrombus passing through the right heart, acute pulmonary embolism and chronic thromboembolic pulmonary hypertension (CTEPH). Pulmonary embolism may recur and have severe sequelae, such as the post-thrombotic syndrome and CTEPH. It is thought at present, based on a long-term prospective study of about 300 patients, that CTEPH results within two years in 3.8% of patients who had survived the first symptomatic pulmonary embolism. Previous incidence of pulmonary embolism, young age, a large perfusion deficit and no known cause were associated with an increased probability of subsequent CTEPH. The diagnosis and treatment of CTEPH call for an interdisciplinary approach, the former still relying on finding a positive perfusion scan together with pulmonary hypertension. Treatment of choice is surgical endarterectomy of the pulmonary obstruction which will result in functional healing in 80% of patients.

The German Endoprostheses Register started its documentation of primary and revision surgery on major joints in 1997. About 7000 primary arthroplasties and 1100 revisions on hip joints as well as 3000 primary arthroplasties and 250 revisions on knee joints are registered annually by 41 hospitals, which amounts to 60% in primary or 80% in revision surgery of the Swedish arthroplasty register. 95% of the revisions are performed on implants which have not been primarily operated on by the hospitals enrolled to the register, normally recruiting the classical "lost-to-follow-up-cases". Especially the removed implants show a conspicuous diversity of models. More than 10 specimens were removed in only 24 of 173 different types of hip-stems and 21 of 133 different types of hip-sockets. Thus, in order to cover not only a small number of types of implants by statistical analysis, large numbers of surgery have to be registered to yield case-numbers which can not be achieved by registers of small countries like the Scandinavian. Germany with approximately 1900 relevant clinical departments owns a slumbering potential that could easily be recruited.

Whether asymptomatic patients with severe aortic stenosis benefit from surgery remains unclear. We report our data recently published in the New England Journal of Medicine on the natural history of this disease and predictors of outcome.

Chronic recurrent pulmonary embolism can lead to extensive pulmonary hypertension by obstruction of the pulmonary vessels. Pulmonary thrombendarteriectomy is a new approach to normalizing the elevated pulmonary vascular resistance by removal of the adsorbed thrombi. Between 1992 and 1994 we have operated on 8 patients aged between 34 and 62 years. The first patient died due to extensive reperfusion edema, all others showed significant improvement in hemodynamic parameters (mean pulmonary artery pressure preop. 63 +/- 5 mmHg; postop. 30 +/- 9 mmHg; Cardiac Index preop. 2.0 +/- 0.2 l/min; postop. 3.5 +/- 0.5 l/min; pulmonary vascular resistance preop. 1169 +/- 75 dyn; postop. 228 +/- 55 dyn) and exercise performance (NYHA classification preop. III-IV, postop. I-II). Pulmonary thrombendarteriectomy represents an efficient method to normalize elevated pulmonary pressure and exercise performance of patients with far-advanced chronic thromboembolic pulmonary hypertension.

The purpose of this study was to examine the influence of treatment on long-term prognosis of patients with aminorex-induced plexogenic pulmonary hypertension. The study included 104 patients (13 males, 91 females) with an aminorex (menocil) intake between 1966 and 1968. All patients were treated with digitalis and diuretics, 52% received an anticoagulant medication with warfarin after pulmonary hypertension was diagnosed. During follow-up, a second right-heart catheterization was performed in 37 patients with a mean interval of 5 years. The longest mean survival time, 8.3 years, was found in patients treated with anticoagulant medication, compared to the 6.1 years found in the non-anticoagulated aminorex patients. Also, in the 5- and 10-years survival rate, patients with an anticoagulant therapy have shown better results (62.9 vs. 38% and 39 vs. 20%, respectively). Patients who received anticoagulant therapy soon after the onset of symptoms showed a better mean survival (10.9 years) than those who commenced treatment more than 1 year thereafter (mean survival 5.9 years). In 57% of the patients who had a second right-heart catheterization a decrease of pulmonary pressures could be diagnosed. Two-thirds of these patients with pulmonary pressure decrease were under anticoagulant therapy. An improvement in the NYHA-classification was seen in 44.8% of the patients treated with warfarin, in comparison to 22.2% of those who did not receive anticoagulant therapy. Although this study is retrospective, it shows a positive influence of anticoagulant therapy on survival in patients with a history of anorectic drug intake.

Since 1967, 166 patients with primary pulmonary hypertension of unknown etiology have been followed-up at the Department of Cardiology at the University of Vienna. Although an exact etiopathogenetic differentiation was not possible, three groups of patients were identified clinically: patients with so-called primary pulmonary hypertension (PPH)--group 1; patients with pulmonary hypertension induced by anorexipens (aminorex fumarate)--group 2; and patients with pulmonary hypertension due to emboli in the large pulmonary vessels--group 3. In addition to non-invasive echocardiographic assessment of pulmonary pressures, regular cardiac catheterizations were carried out, which revealed right atrial mean pressure, cardiac output, and mixed venous saturation to be prognostic predictive factors. Patients with anorexigen-induced pulmonary hypertension have a clearly better life expectancy than those with PPH. Owing to the temporally limited effect of the pulmonary hypertension-inducing agent, aminorex fumarate (Menocil), patients with anorexigen-induced pulmonary hypertension may be considered a model group for drug-induced vascular pathology.

In 8 patients with chronic peritoneal dialysis before and after 24-hour peritoneal dialysis examinations on the cellular immunity were performed (lymphocyte blast transformation with PHA and Con A, ADCC). There was no difference to the normal findings in healthy control persons. Blast transformation decreased during peritoneal dialysis, no changes were visible with ADCC. Compared with reports in literature concerning regular hemodialysis therapy the state of cellular immunity in chronic peritoneal dialysis seemed to be superior.

Penicillins are the most commonly used antibiotics in dermatology owing to their bacteriocidal effect and the high responsiveness of most bacterial dermatoses to penicillin treatment. Although there is a wide range of adverse reactions to penicillins, severe complications are seen very rarely. We report on two cases of penicillin-induced acute interstitial nephritis and summarize the characteristic features of this serious adverse reaction to penicillin administration.

The case of a woman patient of 25 years of age with acute cardiac decompensation is presented. The chest x-ray as well as the lung scan showed the typical features of absence of the right pulmonary artery. The diagnosis was confirmed angiographically. The sudden and late onset of symptoms of pulmonary hypertension is a remarkable feature.

We report the case of a patient who acquired cutaneous leishmaniasis in Israel, with multiple lesions on the face and wrists. The diagnosis was made several months after the onset of disease. We observed large multiple ulcers, which at first responded to sodium stibogluconate (600 mg daily for 2 weeks). After initially decreasing in size, the lesions remained unchanged. Several weeks later clinical exacerbation of the cutaneous leishmaniasis was observed. Fearing late complications on the upper eyelid, we administered two 2-week courses of ketoconazole, 800 mg daily. The early and rapid improvement achieved indicates that ketoconazole was successful in this case of cutaneous leishmaniasis.

In reviewing the literature, we made an attempt to expand the term "transient acantholytic dermatosis" (TAD) beyond the criteria of classical Grover's disease to a broader spectrum of diseases and to systematically categorize the variety of reported clinical pictures. Moreover, we endeavoured to separate these conditions from Darier's and Hailey-Hailey's disease and from immunofluorescence-positive acantholytic dermatoses. Three patients of our own are presented with a heretofore undescribed bullous exanthematic variant.
