In August 2002 the Japanese Society of Psychiatry and Neurology decided to rename the Japanese expression for schizophrenia from Sêshin Bunretsu Byô to Tôgô Shicchô Shô. Currently the psychiatric classification systems ICD-10 and DSM-IV are under revision. Against this background the Japanese process of renaming a psychiatric disorder is of high interest as far as the clinical, social and cultural implications of the new name are concerned.The authors give an overview of the Japanese process of renaming schizophrenia. Its background and realization are explained and the expectations of Japanese physicians, patients and their families related to the new name are analysed. Furthermore, its effects are evaluated. The aim of the paper is to clarify in how far the Japanese example may serve as a model for evaluating the possible implications that a renaming or nosological redefinition of schizophrenia might have in the course of the revision process of ICD 10 and DSM IV.

Slipped capital femoral epiphysis (SCFE) is a common hip disorder in adolescence and should be diagnosed and treated surgically as soon as possible. The etiology, biomechanical, biochemical and hereditary factors are still under investigation. The classification of SCFE is based on the acuteness, clinical and radiomorphological findings. Avascular necrosis of the epiphysis (AVN) and chondrolysis occur more often in operated than in non-operated patients. Medium and long-term sequelae of SCFE are loss of function and degenerative joint disease due to femoroacetabular impingement (FAI) or consequences from complications such as AVN and chondrolysis. For mild slips the long-term prognosis is better than for moderate or severe slips. Higher grade unstable SCFE may benefit from reduction while in chronic slips corrective osteotomy may be indicated. Traditional osteotomy procedures, such as Imhäuser or Southwick intertrochanteric osteotomy are safe procedures but correct the deformity distant from the site of the deformity. The surgical dislocation with modified Dunn osteotomy according to Ganz allows the preparation of an extended retinacular soft tissue flap and offers an extensive subperiosteal exposure of the circumference of the femoral neck before reducing the slipped epiphysis anatomically. In cases of FAI due to mild deformities restoration of the head-neck offset via hip arthroscopy or surgical dislocation should be considered before higher grade cartilage damage occurs.

49 kidneys of male LBNF1-rats were transplanted into male Lewis-rats. BN-rats were used as a source of antigen. Control animals survived 16.1 +/- 1.8 days. 5 recipients were preteated with donorspecific living cells, 7 with semisoluble and 5 with low dosage of soluble antigen. The living cell-pretreatment showed the best effect for the active enhancement, the soluble antigen the least. The pretreatment with 6 mg protein/kg bw. caused the sensibilization of recipients. At the time of transplantation lymphcytotoxic antibody titers were detected in all recipients with pretreatment of living cells, but no titer in other groups. Renal allografts with elevated postopertive titer were rejected rapidly.

We grafted orthotopically 11 DA and 20 WiS-livers into LEW. The recipients of DA-livers survived 10.5 +/- 4.3 days; of the 20 recipients of WiS-liver, however, nine survived 18-37 days, and the other 11 survived indefinitely. The cells of recipients who survived more than 4 months showed GvHR of grade III, and their transfer showed no significant immunosuppressive effect. The indefinitely surviving liver recipients could accept specific skin grafts, but normally rejected third party skin. The serum of these recipients was able to prolong the survival of kidney grafts. This transfer factor is in our estimation responsible for the prolonged survival of rats with liver grafts.
