The article focuses on the evolution of the Metal Inert Gas (MIG) welding process and its various process control variations, as outlined in the German DVS (Deutscher Verband für Schweißen und verwandte Verfahren e.V.,) 0973 guideline. It emphasizes that advancements in MIG welding aim to enhance effectiveness and efficiency, addressing factors such as automation, gas selection, electrode diameter, and seam characteristics.

Introduction: Infection of the knee joint after primary total knee arthroplasty is a serious complication. In this work, we would like to evaluate the mid-term results after two-stage revision of total knee replacement in periprosthetic infection using dynamic spacer, in particular with regard to the function of the knee joint after reimplantation.Patients and methods: In this retrospective study, we included patients who were treated in our clinic between 2005 and 2013 due to infection of the knee after total knee arthroplasty. All patients included have had a mobile antibiotic-coated cement spacer implanted after surgical debridement and removal of the components of total knee replacement. Subsequently, reimplantation of total knee replacement was performed when no clinical or paraclinical signs of infection were found. We analyzed all included cases for potential reinfection, examined the range of motion of the knee joint and evaluated the Merle d’Aubigné-Postel score. Statistical evaluation was performed with SPSS 24.0.Results: This study group contains 16 patients (9 women and 7 men) with an average age of 72.0 ± 8.3 years. All patients were followed up for at least 6 months with an average follow-up of 22.5 ± 16.6 months. In all patients a pathogen was isolated intraoperatively during the first-stage surgery (explantation of the knee). were detected in 94% of the cases, in only one patient. Reimplantation was carried out after 6.2 ± 5.2 months. The average knee flexion in the group of patients without relapse of infection was Only 3 patients showed extension deficit of max. 20°. The Merle d’Aubigné-Postel Score was 14.4 ± 1.9.Conclusion: Two-stage surgery of total knee replacement with the use of a mobile spacer has its high value in the treatment of periprosthetic infections. The mobile spacers contribute to an advantageous range of motion of the knee joint after reimplantation of a total knee endoprosthesis. However, further studies are required that compare the results after using mobile or static spacer, but with the inclusion of homogeneous patient collective.

Introduction: Deep sternal wound infections (DSWI) are a rare but devastating complication after median sternotomy. Minor perfusion in bone and soft tissue, especially after recruiting the internal mammary artery for bypass supports the development of wound infection and nonunion of the sternal bone. The aim of the study was the macroscopic and radiological presentation of the vascular system supplying the sternum, in particular the compensating blood supply routes in the event that the internal mammary artery is no longer available after use as a bypass vessel.Method: This anatomic study was carried out on the anterior chest wall of 7 specimens. The thorax plates of 7 specimens were analyzed macroscopically after microsurgical preparation. Different anatomic preparations were produced using different contrast or form-giving substances. Radiological analysis and three-dimensional reconstructions were performed to show alternative, collateral sternal vessel perfusion under estimation of the loss of the internal thoracic artery due to a bypass.Results: The length of the ITA (internal thoracic artery), measured from the beginning of the first rib to the division into the superior epigastric artery and musculophrenic artery, was an average of 16.3 cm. On average, 18.5 branches were delivered from each artery, 10 medially to the sternum supply, and the intercostal muscle. Conclusion: Our analysis gives an overview of the macroanatomic vessel system supplying the sternal bone, describing especially a common trunk deriving from the ITA and supplying multiple branches and playing an important role in building a collateral circulation of the sternum.For better evaluation, in vivo CT analysis with contrast media should be performed in patients prior to the operation and directly after the use of the double ITA to demonstrate the change in perfusion of the sternum.In the future, preconditioning of the sternum by coiling the deriving branches could become an option, although patient selection has to be improved and further analysis of the topic performed.

Introduction: Revision arthroplasty of the hip is becoming increasingly important in recent years. Early primary arthroplasty and longer life expectancy of the patients increases the number of revision surgery. Revision surgery of hip arthroplasty is major surgery for the patients, especially the elderly, with significant risks concerning the general condition of the patient. The aim of this work is to evaluate the outcome of bipolar hip arthroplasty as a salvage procedure for treatment of loosening of the acetabular cup with significant acetabular bone defects after total hip replacement (THR) in multi-morbid patients.Patients and methods: During the period from January 1 2007 to December 31 2011 19 revision hip surgeries were performed in , in which the loosened acetabular cup was replaced by a bipolar head. The examined patient group consisted exclusively of female patients with an average of 75 years. The predominant diagnosis was “aseptic loosening” (84.2%). All patients in our study were multi-morbid. We decided to resort to bipolar hip arthroplasty due to the compromised general condition of patients and the major acetabular bone defects, which were confirmed intraoperatively. The postoperative follow-up ranged from 0.5 to 67 months (average 19.1 months). Results: Evaluation of the modified Harris Hip Score showed an overall improvement of the function of the hip joint after surgery of approximately 45%.Surgery was less time consuming and thus adequate for patients with significantly poor general health condition. We noticed different complications in a significant amount of patients (68.4%). The most common complication encountered was the proximal migration of the bipolar head.The rate of revision following the use of bipolar hip arthroplasty in revision surgery of the hip in our patients was high (21%). Despite the high number of complications reported in our study, we have noticed significant improvement of hip joint function as well as subjective pain relief in the majority of patients. We clearly achieved clinically satisfactory results in 14 patients.Conclusion: Bipolar hip arthroplasty is by no means to be regarded as standard procedure in revision surgery of THR. It provides an option or salvage procedure for patients with poor general condition in whom the quickest possible surgical intervention preserving mobility is required. This is particularly true for multi-morbid patients in whom sufficient acetabular fixation is not possible.

Background: Infection after total knee replacement (TKR) is one of the serious complications which must be pursued with a very effective therapeutic concept. In most cases this means revision arthroplasty, in which one-setting and two-setting procedures are distinguished. Healing of infection is the conditio sine qua non for re-implantation. This retrospective work presents an assessment of the success rate after a two-setting revision arthroplasty of the knee following periprosthetic infection. It further considers drawing conclusions concerning the optimal timing of re-implantation.Patients and methods: A total of 34 patients have been enclosed in this study from September 2005 to December 2013. 35 re-implantations were carried out following explantation of total knee and implantation of cemented spacer. The patient’s group comprised of 53% (18) males and 47% (16) females. The average age at re-implantation time was 72.2 years (ranging from 54 to 85 years). We particularly evaluated the microbial spectrum, the interval between explantation and re-implantation, the number of surgeries that were necessary prior to re-implantation as well as the postoperative course. Results: We reported 31.4% (11) reinfections following re-implantation surgeries. The number of the reinfections declined with increasing time interval between explantation and re-implantation. Patients who developed reinfections were operated on (re-implantation) after an average of 4.47 months. Those patients with uncomplicated course were operated on (re-implantation) after an average of 6.79 months. Nevertheless, we noticed no essential differences in outcome with regard to the number of surgeries carried out prior to re-implantation. Mobile spacers proved better outcome than temporary arthrodesis with intramedullary fixation.Conclusion: No uniform strategy of treatment exists after peri-prosthetic infections. In particular, no optimal timing can be stated concerning re-implantation. Our data point out to the fact that a longer time interval between explantation and re-implantation reduces the rate of reinfection. From our point of view, the optimal timing for re-implantation depends on various specific factors and therefore it should be defined individually.

Background: Instead of waiting for challenges to confront hospital management, doctors and managers should act in advance to optimize and sustain value-based health. This work highlights the importance of scenario planning in hospitals, proposes an elaborated definition of the stakeholders of a hospital and defines the influence factors to which hospitals are exposed to. Methodology: Based on literature analysis as well as on personal interviews with stakeholders we propose an elaborated definition of stakeholders and designed a questionnaire that integrated the following influence factors, which have relevant impact on hospital management: political/legal, economic, social, technological and environmental forces. These influence factors are examined to develop the so-called critical uncertainties. Thorough identification of uncertainties was based on a “Stakeholder Feedback”. Results: Two key uncertainties were identified and considered in this study: According to the developed scenarios, complementary education of the medical staff as well as of non-medical top executives and managers of hospitals was the recommended core strategy. Complementary scenario-specific strategic options should be considered whenever needed to optimize dealing with a specific future development of the health care environment. Conclusion: Strategic planning in hospitals is essential to ensure sustainable success. It considers multiple situations and integrates internal and external insights and perspectives in addition to identifying weak signals and “blind spots”. This flows into a sound planning for multiple strategic options. It is a state of the art tool that allows dealing with the increasing challenges facing hospital management.

Peripheral nerve catheters are effective and well-established tools to provide postoperative analgesia to patients undergoing orthopedic surgery. The performance of these techniques is usually considered safe. However, placement of nerve catheters may be associated with a considerable number of side effects and major complications have repeatedly been published. In this work, we report on a patient who underwent total knee replacement with spinal anesthesia and preoperative insertion of femoral and sciatic nerve catheters for postoperative analgesia. During insertion of the femoral catheter, significant resistance was encountered upon retracting the catheter. This occurred due to knotting of the catheter. The catheter had to be removed by operative intervention which has to be considered a major complication. The postoperative course was uneventful. The principles for removal of entrapped peripheral catheters are not well established, may differ from those for neuroaxial catheters, and range from cautious manipulation up to surgical intervention.

[english] A simulation of complex clinical processes is a challenging task and suitable methods need to be found which can capture the influence of relevant factors and their relationships. The Manchester triage system (MTS) is widely used in German emergency departments (ED), however the impact on patient waiting times remain difficult to predict. The purpose of this work is the assessment of MTS particularly with regard to the waiting times of different degrees of severity. The methodology of agent based simulation was found suitable for the ED domain and the agent based simulation tool SeSAm was chosen due to its intuitive user interface and easy adaption of the simulation models. Altogether four agent classes could be implemented based on the information derived from a process model. The model permits a dynamic simulation of the ED processes and a reliable assessment of patient waiting times. In addition, the implementation of a triage nurse allowed the simulation of the triage process and a direct comparison to the current state without a standardized triage procedure. Essential influencing factors (e.g. number of patients, manning level) were implemented and their effects on the ED processes and patient waiting times assessed. The simulation runs delivered correct results based on the underlying process model and the collected statistical data. The process flow and the waiting times of an ED could be mapped exactly. In all simulation runs the waiting times of high triage levels (MTS-levels 1 and 2) could be reduced. Especially patients of MTS-level 2 in the waiting area of the ED benefit significantly from the implementation of a standardized triage procedure and the associated permanent monitoring.
