Surgical treatment of acral lentiginous melanomas with wide excisional margins as recommended often requires amputation of distal extremities. The current study analyzes, if smaller excisions in combination with a complete histological evaluation of the excisional margins (3D histology) have a negative impact on the prognosis.304 patients were retrospectively evaluated. 192 patients with reduced excisional margins followed by 3D histology (group A) were compared with 112 patients treated with conventional wide margins (group B). The outcome of both groups was compared.The median tumor thickness was higher in group A (p = 0.022) and ulcerations occured more frequently (p = 0.017). The median excisional margin was 8 mm in group A and 20 mm in group B (p0.0001). 10.9 % (4.2 % invasive) of group A und 8.9 % (4.5 %invasive) of group B (p = 0.577) developed a local recurrence in a median of 40 months.The melanoma specific 10-year survival was 66.8 % in group A und 63.4 % in group B(p = 0.531). Survival of patients with or without local recurrence showed no difference(p = 0.643).Excision with small margins followed by 3D histology is not inferior to conventional surgery with wide margins concerning the rate of local recurrences and has no negative impact on prognosis or the further course of the disease. Local recurrences are rare and do not influence the survival.

A 75-year-old man presented after recurrence of a pigmented macule on his left cheek. Approximately 8 month before a seborrheic keratosis had been diagnosed clinically and treated with cryosurgery and curettage. Dermatoscopy of the recurrent lesion revealed a number of criteria associated with lentigo maligna including asymmetric pigmented follicular openings, streaks, rhomboidal structures, and homogeneous slate-gray areas. Histopathology confirmed a lentigo maligna melanoma with a Breslow tumor thickness of 0.3 mm.

Since early February 2010 we have been implementing the latest version of the 2009 AJCC Melanoma Staging and Classification in our institution. Since, according to the guidelines for stage pT1 melanomas, the number of mitoses/mm(2) is of particular significance, we have been able to observe a notable shift from pT1a to pT1b. Highlighting the mitotic count as one of the key features of the diagnosis of malignant melanoma, we observed that the major part of stage-switched melanomas belonged to a minimally invasive subset of melanomas previously categorized as pT1a UICC (7(th) edition). A level of reasonable doubt remains regarding the distinct histogenetic classification of mitosis as early stage melanoma with regard to their epithelial or melanocytic origin.

The dynamics of tumor growth of malignant melanoma may be reconstructed by evaluation of suitable private photographs of the patient. Photohistorical investigations can greatly aid in following the course of development of malignant melanomas and show impressively the slow and protracted growth of initial melanomas. We report on a 90-year-old patient with an in situ melanoma (lentigo maligna) and an invasive lentigo maligna melanoma in the facial region. We were able to obtain complete series of photographs from this patient, which show the different development of the two melanomas over a period of more than 30 years. The first tumor to appear developed very slowly while the later one showed invasive growth after a short time period. Development of multiple primary melanomas is a well recognized phenomenon. The presence of multiple primary melanomas does not appear to be a negative prognostic factor. However, patients with primary melanoma should be made aware of increased risk of development another primary and physicians should do careful total body skin examinations for new primary melanomas as well as for recurrences of the original melanoma.

Excisional biopsy is recommended as the procedure of choice whenever there is suspicion of malignant melanoma. Incisional biopsies are only rarely indicated. For nearly seventy years the debate about the optimum resection safety margin around the primary tumor was influenced by historical case reports and paradigms. Recently, controlled clinical studies have provided new insights. Accumulating evidence over the last two decades shows that narrower surgical margins influence neither the rate of satellites or in-transit-metastases nor the occurrence of advanced metastatic disease. Local recurrence is rare (approx. 0.1%) when primary tumors are thin and is seen more often (approx. 10%) in primary tumors of greater thickness (4 mm). Analysis of the overall survival in randomized trials shows equal prognosis for malignant melanoma for narrow and wide resection margins. Due to these findings in-toto excisional biopsy for in-situ melanoma, a resection margin of 1 cm for thin primary tumors (1 cm tumor thickness) and a resection margin of 1 to 2 cm for primary tumors greater than 1 mm appears sufficient. With these recommendations, primary closure of wounds will be possible in nearly all cases, reducing surgical costs and morbidity. This article should serve as a basis of discussion for the proposed revision of the current guidelines of the German Dermatologic Society (DDG) on the primary surgical care of melanoma patients.
