Background: Oligometastatic, hormone-sensitive prostate cancer (omHSPC) is increasingly diagnosed due to the implementation of molecular imaging. OmHSPC is mostly defined as a maximum of four bone metastases without visceral metastases on conventional imaging., Objectives: This study highlights the existing evidence regarding local treatment of omHSPC, taking into account molecular imaging and modern therapies., Materials and Methods: Narrative review article based on expert consensus and national/international guideline recommendations, supported by a nonsystematic literature search in PubMed (MEDLINE). The authors consider the cited studies as the most significant works in this regard and these were selected to illustrate developments and fundamental concepts, without claiming completeness., Results: Initially, the STAMPEDE study prospectively demonstrated an oncologic benefit of radiotherapy (RT) to the prostate in addition to androgen deprivation therapy for omHSPC. At 3 years, overall survival (OS) was 81% with RT versus 73% without RT (hazard ratio [HR] 0.68; 95% confidence interval [CI] 0.52-0.90; p = 0.007). However, this benefit was not observed in polymetastatic HSPC (HR 1.07; 95% CI 0.90-1.28; p = 0.4). In a study by Dai et al., local therapy for omHSPC was performed surgically in 85% of cases, also demonstrating an OS advantage (HR 0.44; 95% CI 0.24-0.81; p = 0.008)., Conclusion: OmHSPC should be treated using adjunctive RT. Preliminary prospective evidence shows comparable efficacy with prostatectomy. Modern systemic combination therapies challenge the role of local therapy., (© 2024. Anorexia nervosa is a perilous disease of unknown etiology that causes a variety of endocrine effects. Characteristic for anorexia nervosa are a reduced food intake and thus significant underweight, as well as the fear of gaining weight. Often sufferers also have a distorted self-perception, the urge to move and amenorrhea. AN is difficult to treat and often has a chronic course, and is associated with an increased mortality risk. The endocrinological changes occur in several endocrine axes, their extent is related to the degree of malnutrition. Low leptin levels, due to the underweight, signal a potentially dangerous lack of energy to the brain. There is a cascade of neuroendocrine adaptive responses to help the organism to survive. The effects of starvation are extensive, affecting the pituitary gland, thyroid gland, as well as the adrenal glands, gonads and bones. In positive cases, most dysfunctions are reversible; the compromised bone stability recovers only slowly. [ABSTRACT FROM AUTHOR]

Der Einfluss einer Schilddrüsendysfunktion auf die sportliche körperliche Leistungsfähigkeit ist ein eher noch unbekanntes und nicht zu unterschätzendes Thema in der Sportphysiotherapie, da es bisher nur vage wissenschaftlichen Erkenntnisse über die Wirkungsweise und den Einfluss des Schilddrüsenhormon-Stoffwechsels auf den Organismus bei körperlicher Belastung gibt. Durch die annähernd im gesamten Organismus vorhandenen Schilddrüsenhormon-Rezeptoren hat die Schilddrüse einen weitreichenden Einfluss auf viele Funktionen im Körper. Eine Fehlfunktion des Hormonstoffwechsels der Schilddrüse kann somit zu einer Fehlfunktion im betroffenen Organismus führen. Die Ursache einer Funktionsstörung des Schilddrüsenhormonhaushalts kann pathologisch oder durch körperliche Belastung ausgelöst werden, da die Schild-drüsenhormone Einfluss auf den Energiestoffwechsel, die Muskel und Gewebsphysiologie sowie das Plasma- und Sauerstoffvolumen haben. Dadurch werden Training, aber auch Regenerations- und Heilungsprozesse negativ beeinflusst, was erhebliche Auswirkungen auf die sportliche Leistungsfähigkeit zur Folge haben kann. Dieser negative Einfluss auf den Organismus wird meist durch eine unzureichende Versorgung von Schilddrüsenhormonen in den betroffenen Organen und Gewebsstrukturen verursacht. Die Diagnosestellung gestaltet sich dabei schwierig, da die Symptome leicht mit anderen Pathologien verwechselt werden können. Bei Feststellung einer Dysfunktion wird meist versucht, eine solche Schilddrüsendysfunktion durch ein entsprechendes Hormonsupplement zu kompensieren und die normale Stoffwechsellage wieder herzustellen. Trotz der Behandlung von Dysfunktionen der Schilddrüse scheint es nicht immer möglich zu sein, die Auswirkungen der Veränderungen im Hormonhaushalt der Schilddrüse vor allem bei physischer Belastung so zu steuern, dass keine Defizite in der körperlichen Leistungsfähigkeit vorhanden sind. Dies kann in der Leistungsdiagnostik sowie in der Trainings- und Wettkampfdurchführung durch nicht nachzuvollziehende Ergebnisse sichtbar werden. Auch der Versuch der Leistungssteigerung durch Schilddrüsenhormonsupplemente bei euthyreoten Sportler*inn*en ist nicht zu unterschätzen, was jedoch auf die internationale Dopingkontrolle bisher keine Auswirkungen hat. Da das Thema sehr komplex und umfangreich ist, liegt der Fokus dieser Arbeit auf der Funktion der Schilddrüse im Organismus sowie den Einfluss der Schilddrüsenhormone auf einzelne physiologische Vorgänge bei körperlicher Belastung. The influence of a thyroid dysfunction on the athletic physical performance is a rather unknown and not to be underestimated topic in sports physiotherapy, since there is only vague scientific knowledge about the mode of action and the influence of the thyroid hormone metabolism on the organism during physical stress. Due to the thyroid hormone receptors present in almost the entire organism, the thyroid gland has a far-reaching influence on many functions in the body. A malfunction of the thyroid hormone metabolism can thus lead to a malfunction in the affected organism. The cause of a dysfunction of the thyroid hormone balance can be pathological or triggered by physical stress, since thyroid hormones have an influence on energy metabolism, muscle and tissue physiology as well as plasma and oxygen volume. As a result, training, but also regeneration and healing processes are negatively influenced, which can have a significant impact on athletic performance. This negative influence on the organism is usually caused by an insufficient supply of thyroid hormones in the affected organs and tissue structures. The diagnosis is difficult, because the symptoms can easily be confused with other pathologies. If a dysfunction is detected, an attempt is usually made to compensate for such a thyroid dysfunction with an appropriate hormone supplement and to restore the normal metabolic state. Despite the treatment of thyroid dysfunction, it does not always seem possible to control the effects of changes in thyroid hormone balance, especially during physical exertion, so that there are no deficits in physical performance. This can become visible in performance diagnostics as well as in training and competition by results that are not comprehensible. Also the attempt of performance enhancement by thyroid hormone supplements in euthyroid athletes is not to be underestimated, but this has so far no effect on international doping control. Since the topic is very complex and extensive, the focus of this work is on the function of the thyroid gland in the organism and the influence of thyroid hormones on individual physiological processes during physical exercise. Endocrine disrupting chemicals (EDCs) are universally occurring substances with the potential to interrupt the hormonal system, although the definition of EDCs is not yet standardized. Similar to hormones, EDCs are potentially able to act at very low concentrations, which might be considered save from a toxicological viewpoint. Research over the past years showed that EDCs, even in low doses, might have a relevant impact on human health and the environment as well.The increase of disease burden and disability induced by EDCs causes substantial costs for society. Thus, besides the implications on personal and global health, EDCs affect the economy as well and pose a universal threat to society and environment. Many of the potentially hazardous substances are still inadequately examined and regulated.In this review, we give a brief summary on the previous efforts to classify EDCs and provide a short summary of the current knowledge.Eine einheitliche Definition endokriner Disruptoren gibt es noch nicht. Inzwischen wurden jedoch Schlüsselcharakteristika definiert, welche die Einordnung einer Substanz als endokrinen Disruptor vereinfachen sollen.Der Einfluss endokriner Disruptoren auf den menschlichen Organismus wurde bisher wenig untersucht. Die Schwerpunkte der bisherigen Forschung liegen auf Stoffwechsel-Erkrankungen und Reproduktionsmedizin. AKTUELLE FORSCHUNGSBEMüHUNGEN UND AUSBLICK: Die bisherigen Erkenntnisse zu endokrinen Disruptoren beruhen auf epidemiologischen Beobachtungen und Expositionsstudien in Zellkultur und Tierexperimenten. Auch Ökologie und Ökonomie werden durch endokrine Disruptoren beeinflusst. Daher sind globale Bemühungen notwendig, um irreversible Schäden aufzuhalten.

In metastatic prostate cancer, a trend towards longer survival has been observed over the last 15 years. Beyond progress due to new drugs, retrospective data also suggest a positive influence of a prior treatment of the primary tumor.Can treatment of the primary tumor improve the prognosis of patients later developing metastases, and if yes, what are the underlying mechanisms.In addition to a critical review and discussion of the literature, we analyzed the long-term outcomes of 115 patients with T4 prostate cancer, who had undergone radical prostatectomy after inductive hormonal therapy at our institution.Of the 115 patients, 84 developed prostate-specific antigen (PSA) recurrence during the further course of disease and must therefore be regarded as uncured. Tumor-specific and overall survival of these 84 patients after 150 months were 61 and 44%, respectively. A total of 47 patients were alive after a median follow-up time of 95 months, of whom 31 were still receiving standard hormonal therapy. Only 13 had developed resistance towards their primary hormonal therapy and, hence, received tertiary hormonal therapy. Again, long-term responses were found in some of these patients.Primary tumor resection, at least under the circumstances described here, seems to delay the development of castration resistance in metastatic prostate cancer or to completely prevent it in individual cases.HINTERGRUND: In den letzten 15 Jahren zeigt sich ein Trend hin zu einem längeren Überleben beim metastasierten Prostatakarzinom. Neben dem durch neue Medikamente bedingten Fortschritt deuten retrospektive Daten auch auf einen möglichen positiven Effekt einer früheren Primärtumorbehandlung hin.Kann eine Primärtumorbehandlung im Falle einer späteren Metastasierung die Prognose der betroffenen Patienten verbessern und wenn ja, über welche Mechanismen?Wir werteten die klinischen Langzeitergebnisse von 115 Patienten aus, die bei T4-Prostatakarzinomen nach induktiver Hormontherapie an unserer Klinik prostatektomiert worden waren. Weiterhin erfolgte eine kritische Durchsicht und Diskussion der zur oben genannten Fragestellung vorhandenen Literatur.Von den 115 Patienten hatten 84 im weiteren Verlauf ein biochemisches Rezidiv erlitten, waren also definitiv durch die radikale Prostatektomie nicht geheilt. Das tumorspezifische und das Gesamtüberleben dieser 84 Patienten lag nach 150 Monaten bei 61 % bzw. 44 %. Bemerkenswert war die Beobachtung, dass diese Patienten ein überraschend gutes und langes Ansprechen auf eine Hormontherapie zeigten. Von den 84 Patienten waren nach durchschnittlich 95 Monaten Nachbeobachtungszeit noch 47 am Leben. 31 von ihnen, also ungefähr zwei Drittel, standen immer noch unter einer Standardhormontherapie. Nur 13 hatten eine Resistenz gegen die primäre Hormontherapie entwickelt und entsprechend eine tertiäre Hormontherapie erhalten, auf die sie teilweise aber auch wieder langfristig sensibel blieben.Die Primärtumorentfernung, zumindest unter den beschriebenen Begleitumständen, scheint die Entwicklung einer Hormonresistenz beim metastasierten Prostatakarzinom hinauszögern und in Einzelfällen sogar ganz verhindern zu können.

Background: Risk stratification of patients with metastatic hormone-sensitive prostate cancer (mHSPC) has undergone significant changes in recent years in light of new therapies and innovative imaging., Objectives: Established and innovative methods for detection of metastasis, risk group stratification, and treatment of mHSPC are outlined and compared., Materials and Methods: Background knowledge and treatment-relevant guideline recommendations on mHSPC are presented and complemented by recent study results., Results: Integration of modern imaging techniques, especially prostate-specific membrane antigen (PSMA) PET/CT, into the diagnostic algorithm has the potential to significantly improve risk stratification and treatment of mHSPC. By using PSMA PET/CT, metastases are detected early and sensitively. This leads to the definition of new subgroups amenable to modern therapeutic strategies. The prognostic value of using PSMA PET/CT with regard to established risk categories in mHSPC is currently being evaluated., Conclusions: Modern imaging, especially PSMA PET/CT, has significant added value for the diagnosis and treatment of mHSPC in almost all subgroups. In particular, it helps to select patients who will benefit from intensification or de-escalation of systemic therapy., (© 2023. Pituitary insufficiency is a partial or complete failure of secretion of one or more hormones from the pituitary gland. The pituitary gland is located in the hypophysial fossa of the sella turcica of the os sphenoidale and produces ACTH, LH, FSH, GH, TSH, and prolactin. Pituitary insufficiency can be caused by acute damage, such as secondary to traumatic brain injury. It can also be a result of chronic alterations, such as increasing tumor expansion.Pituitary insufficiency often presents with nonspecific symptoms (e.g. fatigue, listlessness, decreased performance, sleep disturbances, weight change) that leads to a challenging and sometimes delayed diagnosis. The symptoms correspond to the failure of the corresponding endorgans. Occasionally, symptoms such as a loss of libido, secondary amenorrhea or nausea in stressful situations are diagnostically indicative.Further clarification includes a clinical examination with endocrinological testing of the pituitary function. Alteration of pituitary hormone secretion can also occur physiologically as in pregnancy, depression or obesity. Substitution therapy of the failed corticotropic, thyrotropic and gonadotropic axis is corresponding to the therapy of a primary endorgan insufficiency. Space-occupying lesions of the adrenal glands are one of the most frequent tumors; however, only a fraction of approximately 20% need further diagnostics and treatment. The diagnostic standard is native computed tomography (CT). For larger tumors and those that cannot be clearly classified as benign, the supplementary radiological modalities magnetic resonance imaging (MRI), contrast CT and
18 F-fluorodeoxyglucose positron emission tomography (18 F-FDG-PET) should be used. Adrenal gland neoplasms can be hormone-active or hormone-inactive. The most important hormone-active adrenal gland neoplasms are those with autonomous cortisol secretion and autonomous aldosterone secretion as well as pheochromocytomas. Several laboratory tests are available each for the respective hormone activity. The indications for surgery are dependent on the dignity, size and hormone activity. Minimally invasive surgical techniques have become more important due to advantages such as lower blood loss; nevertheless, open surgery is still indispensable for large suspected tumors. Care must be taken preoperatively and postoperatively, particularly in cases of hormone-active tumors., (© 2023. Despite major developments in the therapy of advanced prostate cancer over the past decade, there was still an unmet medical need for efficacious and safe therapies suitable for a wide patient population of patients with metastatic hormone-sensitive disease.Since its label extension for the treatment of metastatic hormone-sensitive prostate cancer (mHSPC) at the beginning of 2020, apalutamide in combination with androgen deprivation therapy (ADT) has filled this gap 1. The novel androgen receptor inhibitor showed good efficacy and safety in comparison with placebo in the pivotal study TITAN while maintaining quality of life - the study included patients irrespective of risk, disease volume, or time of diagnosis 2.Trotz umfassender Entwicklungen in der Therapie des fortgeschrittenen Prostatakarzinoms im letzten Jahrzehnt, bestand bei Patienten mit metastasierter hormonsensitiver Erkrankung weiterhin ein Bedarf an gut wirksamen und zugleich gut verträglichen Therapien, die bei einer breiten Patientenpopulation zum Einsatz kommen können.Seit Anfang 2020 wird nach entsprechender Zulassungserweiterung die Lücke beim metastasierten hormonsensitiven Prostatakarzinom (mHSPC) durch Apalutamid in Kombination mit einer Androgendeprivationstherapie (ADT) geschlossen 1. Der moderne Androgenrezeptor-Inhibitor zeigte in der Zulassungsstudie TITAN, in der mHSPC-Patienten unabhängig von Risiko, Metastasenlast oder Diagnosezeitpunkt einschlossen wurden, gegenüber Placebo plus ADT eine gute Wirksamkeit und Verträglichkeit, bei gleichzeitig guter Lebensqualität 2.

The heart is closely connected with the endocrine system. On the one hand, the heart is an endocrine organ itself and produces several hormones like atrial and brain natriuretic peptides. On the other hand, cardiac structures are targets of many hormones like catecholamines, thyroid hormones, and corticosteroids. Therefore, many endocrine diseases come with cardiac symptoms. In this article, we describe three instructive clinical cases of patients with hormonal disorders that mimicked cardiovascular disease. Furthermore, we shortly discuss useful diagnostic and treatment algorithms. All cases emphasize the value of interdisciplinary management of patients with cardiac symptoms.

Hearing loss leads to impairments in communication, social interactions, and cognitive functions. This renders early treatment particularly important. A causal therapy is not yet available. Human and animal studies have shown that certain hormones can have a positive effect on hearing.This review provides an overview of the effects of various hormones on hearing and describes the potential benefit for future therapeutic approaches.A systematic literature review of reviews dealing with the effects of various hormones on hearing in humans and animals published in PubMed between 2015 and 2020 was conducted.Hormones may mediate antiapoptotic effects on structure-relevant cells of the cochlea and auditory pathway, and may influence hair cell functionality or the electrolyte balance of the endo- and perilymph. Current research focuses on glucocorticoids; the mineral corticoid aldosterone; the sex hormones estrogen, progesterone, and testosterone; the growth hormones GH (growth hormone) and IGF‑1 (insulin-like growth factor 1); thyroid hormones; and insulin. Study results are still inconsistent at this time, but various hormones appear to represent a possible future treatment option for acute hearing loss. Long-term hormone treatment, which would be necessary particularly in the case of age-related hearing loss, does not currently represent a sensible course of action due to the side effect profile of the systemic treatment/lack of practicable topical application options.The mode of action of hormones is complex. Whether they can be used in the future for individualized treatment of patients with acute hearing impairment requires further investigation.HINTERGRUND: Schwerhörigkeit führt zu Einschränkungen im Bereich der Kommunikation, im sozialen Umfeld und bei kognitiven Funktionen. Dies macht eine frühzeitige Behandlung besonders wichtig. Eine ursächliche Therapie ist bislang nicht verfügbar. In Human- und Tierstudien zeigt sich, dass bestimmte Hormone das Hörvermögen positiv beeinflussen können. ZIEL: Dieser Review verschafft einen Überblick über Auswirkungen verschiedener Hormone auf das Hörvermögen und beschreibt den möglichen Nutzen für zukünftige Therapieansätze.Es wurde eine systematische Literaturrecherche zu Reviews, die sich mit dem Einfluss verschiedener Hormone auf das Gehör von Mensch und Tier befassen und zwischen 2015 und 2020 in PubMed publiziert wurden, durchgeführt.Hormone können antiapoptotische Effekte auf strukturrelevante Zellen der Cochlea und Hörbahn vermitteln und Einfluss auf die Funktionalität der Haarzellen oder das Elektrolytgleichgewicht der Endo- und Perilymphe ausüben. Gegenstand aktueller Forschung sind Glukokortikoide, das Mineralokortikoid Aldosteron, die Sexualhormone Östrogen, Progesteron und Testosteron, die Wachstumshormone GH („growth hormone“) und IGF‑1 („insulin-like growth factor 1“), Schilddrüsenhormone und Insulin. Die Studienergebnisse sind zum aktuellen Zeitpunkt noch inkonsistent, allerdings scheinen diverse Hormone möglicherweise eine künftige Therapieoption bei akuten Hörstörungen darzustellen. Eine längerfristige Hormonbehandlung, wie sie insbesondere bei altersbedingter Schwerhörigkeit notwendig wäre, stellt aufgrund des Nebenwirkungsprofils der systemischen bzw. dem Fehlen einer praktikablen topischen Anwendungsmöglichkeit derzeit kein sinnvolles Vorgehen dar.Die Wirkungsweise von Hormonen ist komplex. Ob diese in Zukunft für eine individualisierte Therapie von Patienten mit akuten Hörstörungen genutzt werden können, muss weiter untersucht werden.
