Improvement in the quality of life (QoL) is a major goal of therapy for heart failure (HF) patients. Physical well-being as an important component of QoL has not yet been sufficiently covered by disease-specific assessment instruments. The aim of the study was to validate the questionnaire for assessing subjective physical well-being (FEW16) in HF patients with preserved ejection fraction (HFpEF) from the exercise training in diastolic heart failure (Ex-DHFaEuroP) trial. A total of 64 HFpEF patients (65 years, 56 % female) were randomized to usual routine treatment with (n = 44) or without training (n = 20). At baseline and 3 months, patients were clinically evaluated and assessed using appropriate questionnaires on the QoL (SF36), physical well-being (FEW16) and depression (PHQ-D). The FEW16 showed good values for Cronbachs' alpha coefficients (0.85-0.93). The cross-validity with SF36 and PHQ-D was highly significant but more so for psychological aspects. At baseline, the FEW16 score correlated with age, the subscale resilience with age and the 6 min walking distance test. At follow-up, the total and resilience scores had improved in the training group. In contrast to the SF36, the FEW16 did not detect differences between the groups in Ex-DHFaEuroP. The FEW16 questionnaire showed good internal consistency and correlation with SF36, its total score and resilience had improved after training; however, it did not reflect different changes between the study groups. The FEW16 is therefore more suited to assess general/mental well-being than the subjective physical well-being.

Ziel der Studie war die Untersuchung, in welchem Maß eine regelmäßige sängerische Aktivität bei Kindern und Jugendlichen deren Stimmleistungsparameter verbessert und ihre Eigenwahrnehmung und den Umgang mit der ihrer Stimme positiv beeinflussen kann.Wir untersuchten 186 Kinder und Jugendliche zwischen 6 und 19 Jahren (mittleres Alter: 13,5 Jahre) und bestimmten Tonhöhenumfang, Schwelltonvermögen, mittlere Sprechstimmlagen, Tonhaltedauer und Stimmstärke. Wir ließen jeden Probanden in einem Fragebogen mit fünf Fragen und 21 Items die Wahrnehmung und die bewusste Steuerung seiner Stimme einschätzen.Mit zunehmender stimmlicher Belastung (Tau=0,195, p=0,017) und zunehmender gesangspädagogischer Betreuung (Tau=0,238, p=0,001) wird auch der Tonhöhenumfang größer. Bei der Diagnostik und Therapie von Stimmstörungen bei Kindern und Jugendlichen hat die realistische Einschätzung ihrer stimmlichen Aktivität eine entscheidende Bedeutung. Wir entwickelten eine Klassifikation unter Berücksichtigung der stimmlichen Belastung, der gesangspädagogischen Betreuung und einer möglichen Zusatzbelastung durch Hochdruck-Blasinstrumente.Für die Evaluation der Reproduzierbarkeit wurden von 186 Kindern und Jugendlichen zwischen 6 und 19 Jahren (mittleres Alter: 13,5) Interviews aufgezeichnet, die der Einteilung in die Klassifikationsgrade dienten. Anschließend klassifizierten 124 Ärzte/Logopäden, Chorleiter, Musiklehrer und Laien die sängerische Aktivität anhand von jeweils 6 randomisierten Interviews in insgesamt 744 Beurteilungen. Alle Beurteilergruppen zeigten unabhängig von der Berufsgruppe eine gute Übereinstimmung mit den Interviewern (ĸ=0,65 bis 0,83). Bei einer Klassifikation ohne Befragung der Probanden resultierte eine signifikant schlechtere Übereinstimmung (ĸ=0,29 bis 0,47). Die Einschätzung der Zusatzbelastung durch das Instrument erfolgte dagegen auch mit einer sehr hohen Übereinstimmung bereits in der Vorauswahl (ĸ=0,88 bis 0,93).Die Klassifikation ist für die praktische Anwendung auch durch Laien geeignet und kann die sängerische Aktivität sicher und gut reproduzierbar einschätzen. Damit ist ein Instrument entwickelt worden, das die Kommunikation zwischen behandelnden Arzt, Logopäden und Gesangspädagogen vereinfacht und vereinheitlicht.

Ziel der Studie war es, an einem großen Patientenkollektiv den Einfluss möglicher Faktoren auf persistierende Beschwerden der Stimme und des Schluckens nach Anästhesie zu untersuchen, wobei als primärer Endpunkt das subjektive Befinden des Patienten definiert wurde.An 10.013 erwachsenen Patienten, bei denen zwischen 07/02-03/04 eine Intubation, Larynxmasken-, Maskennarkose oder Regionalanästhesie erfolgte, führten wir am 1. postoperativen (p.o.) Tag Befragungen hinsichtlich des Auftretens von Heiserkeit und Schluckbeschwerden mit einer visuellen Analogskala durch. Am 3. bzw. 7. p.o. Tag erfolgten gegebenenfalls weitere Befragungen. Im Falle des Fortbestehens wurde den Patienten eine phoniatrische Untersuchung angeraten.Nach einer Intubationsnarkose haben folgende Faktoren einen signifikanten Einfluss auf die Entstehung der Beschwerden: Alter, Geschlecht, Intubationsdauer, Verwendung von Lokalanästhesie-Gel, Rachentamponade oder Magensonde sowie postoperatives Erbrechen. Keinen signifikanten Einfluss hatten der Ausbildungsstand des Anästhesisten sowie der Schwierigkeitsgrad der Intubation. Der phoniatrischen Diagnostik unterzogen sich 36 von 55 Patienten (65,5%), die noch am 7. p.o. Tag über Beschwerden klagten. Dabei waren in nur drei Fällen (0,04%) eine partielle Recurrensparese bzw. in einem Fall (0,01%) ein Stimmlippenhämatom nachweisbar. 21. Sängerformanten im Kindes- und Jugendalter?

In der Literatur wird die Existenz von Sängerformanten bei Kindern und Jugendlichen kontrovers diskutiert. Dabei ist zu vermuten, dass diese - ähnlich wie bei Erwachsenen - insbesondere bei sängerisch aktiven, ausgebildeten Singstimmen und hauptsächlich bei männlichen Stimmen nachweisbar sind. Wir untersuchten 148 gesunde Probanden (Alter: 10,5-16 Jahre, mittl. Alter 12,8 +/- 1,6), von denen 82 (50 Knaben, 32 Mädchen) sängerisch aktiv, 66 (39 Knaben, 27 Mädchen) sängerisch nicht aktiv waren. Die Analyse der Formantfrequenzen und -schalldruckpegel erfolgte anhand eines ausgehaltenen Vokals /a/ in der mittleren ungespannten Sprechstimmlage mittels einer Linear-Predictive-Coding-Analyse (MDVP). Es zeigten sich ein 3. Formantbereich bei 2,5-3,5 kHz sowie ein 4. Formantbereich bei 3,4-4,4 kHz, die wir als Sängerformantfrequenzen interpretierten, wobei beim 4. Formant bei Mädchen signifikant höhere Frequenzen auftraten als bei Jungen (p=0,016). Vor der Mutation bestehen in beiden Gruppen geringe Schalldruckpegel in diesen Frequenzbereichen, wobei Jungen tendenziell höhere Schalldruckpegel aufweisen. Background: Research of SARS-CoV-2 has so far largely focused on symptomatic cases. The STAAB-COVID study therefore examined the seroprevalence of COVID-19 in the general population and the psychosocial effects of the pandemic., Methods: From June-October 2020, a sub-study was conducted within the "Characteristics and Course of Heart Failure Stages A-B and Determinants of Progression (STAAB)" cohort study. 4,860 study participants identified from a representative age-stratified sample of Würzburg residents were asked to provide a blood sample and to fill in a questionnaire. All participants also received an offer to take part in a point prevalence assessment (nasal swab taken from the participant at the beginning of November 2020)., Results: A total of 3,034 subjects took part in the STAAB-COVID program (response rate 62%). Antibodies against SARS-CoV-2 were detected in 33 participants (1.1%; 95% confidence interval 0.7-1.5%). Higher values on the GAD-7 anxiety scale were associated with lower rates of SARS-CoV-2 antibodies (Odds Ratio=0.78 for each+1 point in GAD-7; 95% confidence interval 0.65-0.95). Within this rather anxious group of subjects, however, the rate of cancellation of medical appointments was also increased (Odds Ratio=1.13 for each+1 point in GAD-7; 95% confidence interval 1.10-1.16). An acute infection was detected in six of a total of 2,451 participants in the point prevalence assessment (0.24%; 95% confidence interval 0.09-0.53%)., Conclusion: Between the first and second COVID-19 waves in Germany, we found a low level of SARS-CoV-2 contamination in the city of Würzburg. A more anxious personality was associated with a lower seroprevalence. - Abstract
Introduction: Improvement in the quality of life (QoL) is a major goal of therapy for heart failure (HF) patients. Physical well-being as an important component of QoL has not yet been sufficiently covered by disease-specific assessment instruments. The aim of the study was to validate the questionnaire for assessing subjective physical well-being (FEW16) in HF patients with preserved ejection fraction (HFpEF) from the exercise training in diastolic heart failure (Ex-DHF‑P) trial., Method: A total of 64 HFpEF patients (65 years, 56 % female) were randomized to usual routine treatment with (n = 44) or without training (n = 20). At baseline and 3 months, patients were clinically evaluated and assessed using appropriate questionnaires on the QoL (SF36), physical well-being (FEW16) and depression (PHQ-D)., Results: The FEW16 showed good values for Cronbachs' alpha coefficients (0.85-0.93). The cross-validity with SF36 and PHQ-D was highly significant but more so for psychological aspects. At baseline, the FEW16 score correlated with age, the subscale resilience with age and the 6 min walking distance test. At follow-up, the total and resilience scores had improved in the training group. In contrast to the SF36, the FEW16 did not detect differences between the groups in Ex-DHF‑P., Discussion: The FEW16 questionnaire showed good internal consistency and correlation with SF36, its total score and resilience had improved after training; however, it did not reflect different changes between the study groups. The FEW16 is therefore more suited to assess general/mental well-being than the subjective physical well-being.

- Abstract
Background: After the introduction of antiretroviral therapy, the life expectancy of HIV patients has increased to more than 30 years after initial diagnosis. Cardiovascular disease now is an important cause of death in HIV-infected patients., Patients and Methods: In the multicenter, prospective HIV-HEART study, 222 (38 %) patients suffered from lipodystrophy. Women were more often affected than men (41.5 % vs. 25.3 %). Patients with lipodystrophy were on average 5 years older and had been infected longer (10.4 vs. 6.6 years) then patients without lipodystrophy., Results: Lipodystrophy in HIV patients was a clinical sign of cardiovascular risk factors like hyperlipidemia (total cholesterol 19 mg/dl higher, HDL 2.8 mg/dl lower, triglycerides 53 mg/dl higher) and type 2 diabetes (11.3 % vs. 2.8 %). Patients with lipodystrophy were more likely to be co-infected with hepatitis B (34.7 % vs. 28.8 %, p = 0.122) or C (13.1 % vs. 9.3 %, p = 0.16) than patients without lipodystrophy. The quality of life was reduced in patients with lipodystrophy. In 6 of 8 scales of the SF-36 questionnaire, patients with lipodystrophy had lower scores., Conclusions: Lipodystrophy syndrome is an early warning system for a number of illnesses which reduce life expectancy. Dermatologists must help insure that HIV-infected patients receive treatment for these disorders.

- Abstract
Background and Purpose: HIV-infected patients exhibit an increased rate of cardiac diseases, due to an elevated rate of cardiac risk factors and side effects of antiretroviral therapy. The aim of the present study was to analyze the impact of B-type natriuretic peptide (BNP) in this patient population., Patients and Methods: The HIV-HEART (HIV infection and HEART disease) study is a prospective and multicenter cohort study. Outpatients with a known HIV infection were included over a 20-month recruiting period. BNP values were determined by a standardized laboratory test at study inclusion., Results: 802 HIV-infected patients (male: 83.4%, mean age: 44.2 +/- 10.3 years) were included. The BNP concentrations (median 11.3 pg/ml; BNP < or = 100 pg/ml: 96.2%) were associated with echocardiographic parameters and with the right ventricular diameter (p = 0.02) and the systolic pulmonary arterial pressure (p = 0.01). Patients with a BNP concentration of > 50 pg/ml had a significantly higher rate of heart failure (p < 0.001), cardiomyopathy (p < 0.001), and coronary artery disease (p < 0.001)., Conclusion: The data demonstrate that BNP is suitable for the detection of cardiac disorders in HIV-infected subjects. Therefore, BNP could be an appropriate tool for a screening program for HIV-associated disorders in this patient population.

Background: The incidence of squamous cell carcinoma is rapidly increasing and requires process-optimized treatment dependent on the course of the patient's individual disease. Patient-based data on squamous cell carcinoma were used to analyze the treatment modalities before DRGs were introduced, after their introduction and after discussion about process-optimization in context of clinical pathways., Patients and Methods: From the recorded data in the department of dermatology, all squamous cell carcinoma cases were identified and evaluated. In addition to patient characteristics, parameters describing the process, for example, length of stay in hospital, pre- and post-interventional days, were collected., Results: Between 1998-2002, 2004-2006 and January to June 2007, 658 patients were treated for squamous cell carcinoma. In contrast to the times before DRGs, the treatment process has been significantly optimized, reducing the pre- and post-operative days and thus the duration of stay. Analysis of clinical pathways as part of the continuous improvement process was hardly able to detect further improvement. However, the weekly distribution of the admitted patients has been improved., Conclusions: Patients with squamous cell carcinoma requiring surgical excision benefit from an optimized process, just as do impatient facilities. However, optimized treatment modalities carry hardly any potential of improvement by the invention of clinical pathways.

- Abstract
There is accumulating evidence from animal and epidemiologic studies that physical exercise is neuroprotective in healthy animals and humans and can prevent cognitive decline in chronic neurodegenerative processes like Alzheimer's dementia. However, data from well-designed interventional, randomized non-pharmacologic trials is lacking in contrast to other areas of medicine like prevention of hypertension, diabetes or the antipsychotic-associated metabolic syndrome. The demonstration of a potential positive effect of physical exercise on preventing dementia using a controlled study design would represent a significant progress in the prevention of dementia and public health, especially as long as other treatments for dementia prevention are lacking.

- Abstract
The influence of physical development and singing activity on vocal efficiency in children and adolescents is well known from clinical experience. There is, however, no comparative study between singing and non-singing children which also considers the influence of age and gender. Therefore, standard values for evaluation in clinical practice are missing. We examined the following parameters in 164 healthy children and adolescents (90 boys, 74 girls; 11-16 years), 86 without singing activity (group A) and 78 members of children's and youth choirs (group B): frequency and dynamic range of voice and its borders with a voice range profile, mean fundamental frequency (normal and loud phonation), maximum voice intensity, and maximum duration of intonation. The statistical analysis was performed using three-way ANOVA. We found significantly higher ranges of frequency and intensity in singing children and adolescents (p<0.0001). The borders of the dynamic range and the upper border of the frequency range were significantly higher in group B (p<0.0001). The boys in group B used a higher mean fundamental frequency during loud phonation. There were no significant differences between groups in maximum voice intensity (p=0.051) but a tendency towards higher values in singing children. As an unexpected result, we found significantly higher values in maximum duration of intonation in group A (p<0.0001) independent of age and gender, which seems to be related to the methods used. Regular training of the singing voice results in positive effects on several voice parameters in children as well as adolescents. Our results can be used for estimating standard values in professional clinical and educational care of young singing voices and non-singing children. For this purpose, voice range profile is particularly suitable.

- Abstract
Background: Osteopathy is a common clinical feature of chronic inflammatory bowel disease (IBD) in children and young adults at the time of primary diagnosis. The aim of the following study was to address the question of prevalance of a decreased bone density or increased bone metabolism in children with IBD., Patients: We examined 63 patients (mean age 13 years; 5 - 18 years): 36 Crohn's disease (MC) patients, 16 colitis ulcerosa (CU) patient and one patient with colitis indeterminata (CID). 10 children who had been referred to the gastroenterological outpatient department due to suspected IBD symptoms were later found not to suffer from IBD. These 10 patients therefore were included in the study as controls., Results: 8 of 10 CU patients and 18 of 28 MC patients showed a pathological bone density and abnormalities in bone metabolism. Repetitive bone density measurement was performed in 18 patients. In MC patients a - 0.39 SDS decrease of bone mineral density was found, without a simultaneous deterioration of clinical stage and inspite of a decreased cumulative prednisolon dosage. However in CU patients a + 0.06 SDS increase of bone mineral density was detected. These patients had a lower cumulative prednisolon dosage and a stable clinical course., Conclusions: In conclusion, pediatric IBD patients often show abnormalities in bone metabolism and decreased bone density. There is a need for multicentre, prospective randomised control trials to further identify therapeutic tools on the basis of the multifactorial etiology of bone disease in pediatric IBD patients.

- Abstract
Objective: Because of its complex profile of action (binding to dopamine, serotonin and histamine receptors), low rate of adverse effects and low cost as a medicinal preparation, metoclopramide is an interesting substance for the prophylaxis of post-operative nausea and vomiting (PONV). As a single substance its antiemetic effects are slight at the usual dose, so the aim was to test the efficacy of a combination of metoclopramide and dexamethasone for the prevention of PONV on a group of patients with the same operative trauma., Method: All patients (n = 204) were recruited prospectively (January-October 2002) and were to undergo a lumbar disc operation. The anaesthetic was administered according to a standard procedure as a balanced anaesthetic with fentanyl and isoflurane in oxygen/air. 60 min before the end of the operation, all patients were given 10 mg of metoclopramide and 8 mg of dexamethasone intravenously. The Würzburg-Oulu-Score served as an instrument for comparison, because no placebo group has been included. 24 hours after the operation, all patients were asked to report on nausea and vomiting, stating the time and the degree of discomfort (quantification by means of an analogue numerical scale from 0-10). The influence of age, height, weight, duration of the anaesthetic, operating position and increased dexamethasone dose was analyzed in addition to the risk factors according to the score. The cost analysis was based on the purchase prices of the hospital dispensary., Results: The expected PONV incidence was 35.8%; 10% nausea (average intensity 4.3) and 3% emesis (4.8) was reported for the 24-hour period. The rescue medication (dimenhydrinate) was requested 8 times. Nausea was mainly during the early part of the period (0-6 hours). Of 42 patients with a history of PONV, 71% had no symptoms. The Odds Ratios for female sex (2.9), non-smoker status (2.0) and post-operative opioid administration (1.9) correspond to the data given in the literature; it was not possible to determine the significance of a history of PONV as an independent risk factor. None of the other factors investigated had a significant influence on PONV. For the chosen combination of antiemetic drugs the number-needed-to-treat is 3.9 (95% CI: 3.3-4.7). The direct costs of the PONV prophylaxis are 0.65 euro per patient., Conclusions: The metoclopramide/dexamethasone combination proved to be effective and inexpensive, on the basis of these findings it is used prophylactically at our hospital if only one PONV risk factor exists.
