Aim: Both teachers and learners had clear reservations in the beginning about the usefulness and benefits of supervision and feedback, which were to be implemented as a teaching method in the competency-based, post-graduate curriculum in general ambulatory pediatrics, known as PaedCompenda (www.paedcompenda.de). This paper investigates the different perceptions of the physicians undergoing specialist training () and elucidates these differences.Method: The following data were collected as part of the research on the three-year-long implementation (2019-2023):1) Four focus group discussions (N=28) with physician trainees who had no experience in primary care pediatrics or with the post-graduate curriculum;2) Problem-oriented interviews, one at the beginning and again at the end, with physician trainees (N=28) undergoing specialist training at 19 participating medical practices belonging to two post-graduate education networks;3) Videos of patient consultations with the physician trainees (N=23);4) Videos of feedback conferences regarding the videotaped patient consultations (N=7).This data was evaluated using reconstructive grounded theory.Results: A distinctly more positive perception of the benefits of supervision and feedback as a teaching and learning method was seen in the physicians who received specialist training at the practices following the PaedCompenda curriculum. In regard to method, it is crucial that the educational setting can be experienced as a learning opportunity. Playing a central role in this is constructive and conducive feedback (a. dysfunctional routines, b. underlying lack of confidence, and c. overlooked problems).Conclusions: This paper shows the opportunities of an institutionalized form of supervision and feedback as part of a competency-based, post-graduate curriculum while also making it clear that implementation is challenging. Furthermore, the medical specialists who serve as trainers need to be specifically trained to know which approaches promote learning effectively.

Für die Erforschung von Verhaltensweisen, die in Verbindung mit dem Smartphone stehen, eignet sich insbesondere die Mobile Experience Sampling Methode (MESM). Sie wurde mit dem Ziel entwickelt, menschliches Verhalten unmittelbar und direkt in der natürlichen Umgebung der Befragten zu erheben. Damit hat sie besonders für den Einsatz in der Kinder- und Jugendmedienforschung Potenzial: Heranwachsende sind in Deutschland beinahe flächendeckend mit Smartphones ausgestattet und tragen sie fast immer bei sich. Die Methode ermöglicht durch wiederholte Messungen nicht nur die Erfassung situativer Effekte in der Mediennutzung, sondern auch die Analyse intraindividueller Unterschiede. Gerade in Bezug auf Minderjährige als besonders vulnerable Gruppe sind bei solchen stark in die Privatsphäre eingreifenden Forschungsdesigns unbedingt forschungsethische Aspekte zu berücksichtigen. Unser Beitrag befasst sich daher mit dem Mehrwert der MESM hinsichtlich ihres Einsatzes in der Kinder- und Jugendmedienforschung und beleuchtet die spezifischen Herausforderungen, die sich aus dieser Zielgruppe ergeben. Diesen Themen widmen wir uns sowohl auf einer projektübergreifenden Ebene als auch in Form eines Werkstattberichts, in dem wir konkrete Fallstricke aufgreifen, die uns in der Forschungspraxis begegnet sind.

Ausgehend vom Modell von Gotlieb und Sarge (2021), in welchem Attribute partizipativer Mediennutzung (sich ausdrücken, sich darstellen und mit anderen zusammenarbeiten) mit motivationalen Prozessen der Selbstbestimmung sowie der Ausbildung zivilgesellschaftlicher Fähigkeiten verbunden werden, modelliert dieser theoretisch-konzeptionelle Beitrag den Zusammenhang zwischen partizipativen Praktiken von Kindern und Jugendlichen in digitalen Medienumgebungen und ihrem zivilem Engagement. Dabei wird auf das heuristische Strukturmodell einer partizipativen Mediendidaktik nach Mayrberger (2019) sowie Überlegungen zum selbstbestimmten Lernen zurückgegriffen, um kommunikationswissenschaftliche Konzepte mit medienpädagogischen und bildungswissenschaftlichen Ansätzen zu kombinieren. Die Herausbildung und Förderung partizipativer Fähigkeiten von Heranwachsenden im Rahmen partizipativer Mediendidaktik in formalen und informellen Bildungskontexten begreifen wir demnach als potenziellen Zugang für ziviles, staatsbürgerliches Engagement der jungen Zielgruppe. Vor diesem Hintergrund argumentieren wir, dass der Gestaltung der persönlichen Lernumgebung (PLE) und der damit verbundenen Zusammenstellung medialer Arrangements in unterschiedlich gelagerten Lernkontexten durch die Heranwachsenden eine wichtige Bedeutung zukommt. Anhand quantitativer Befragungsdaten zur Nutzung des Study Webs im Rahmen selbstbestimmten Lernens exploriert der Beitrag beispielhaft das Potenzial digitaler persönlicher Lernumgebungen und damit partizipativen Handelns in Bildungszusammenhängen für das zivile Engagement Heranwachsender.

Background: In Germany, the (model) regulation for postgraduate medical education 2018, the professional codes of conduct of the regional medical councils and the health professions chamber laws of the federal states are the formal basis of postgraduate medical education, but say little about its structure, processes and results. The World Federation for Medical Education (WFME) has developed global standards for improving the quality of postgraduate medical education and published them in a revised edition in 2015. A German version which takes the specifics of medical training in Germany into account has not been published to date.Objective: The Committee for Postgraduate Medical Education (PGME) of the Society for Medical Education (GMA) has set itself the goal of firstly translating the WFME standards into German and secondly making recommendations for physicians with a license for post-graduate training (PLT) and training agents (TA) in clinics and practices which have been adapted to the German context.Methods: The WFME standards were translated into German by a working group of the GMA Committee for PGME, the terminology adapted to PGME in Germany and checked by an interdisciplinary panel of experts made up of 9 members of the committee. In a second step, the WFME basic standards and quality standards for PGME relevant to PLTs and TAs in Germany were iteratively determined by this panel of experts using the Nominal Group Technique (NGT) and compiled in the form of recommendations. Results: The translation of the WFME guidelines was approved by the expert group without any changes to the content, taking into account the terminological system of PGME in Germany. In a second step, 90 standards were identified which were considered helpful for PGME in Germany, especially for PLTs and TAs (such as development of a professional identity, a more patient-centered approach or support of self-directed learning). Care was taken to only give recommendations which can be influenced by PLTs and TAs. These standards have been summarized as recommendations to PLTs and TAs and take into account all chapters of the WFME standards. Conclusion: The WFME standards selected here are recommended to PLTs and TAs in clinics and practices to achieve high-quality PGME. Empirical longitudinal studies will be required to examine both the implementation and the results of applying the modified WFME criteria in Germany.

Migrants who live in Switzerland without valid residence papers find themselves in a complex and contradictory legal position. Even though they are entitled to a great number of rights, they are rarely able to claim them in practice. This paper analyzes the challenges that migrants with irregular status face when enforcing their rights. Furthermore, a strategy that could potentially serve as a remedy to their lack- ing access to justice is explored; the approach of ur- ban citizenship and the introduction of city ID cards.

Aim: The seminar program of the KWBW Verbundweiterbildungplus® is offered by the Competence Center for Postgraduate Medical Education in Baden-Württemberg (KWBW) for physicians specializing in general practice (GP trainees). Attendance is a voluntary one comprised of 48 curricular units of 45 minutes each per GP trainee. This seminar program is meant to be attended in parallel to the postgraduate medical education in clinic or practice. The intention behind this project was to develop objectives, topics and a feasible structure for a seminar curriculum while taking time and financial constraints into account.Method: The Kern cycle was applied in an open, modified nominal group consensus technique in the form of an iterative process. Participating were 17 experts from the departments of general practice at the universities in Freiburg, Heidelberg, Tuebingen and Ulm, plus a pediatrician.Results: The main objective was defined as empowering GP trainees to independently provide high-quality primary care, including in rural areas. A basic curriculum was defined based on relevant frameworks, such as the 2018 Model Regulation for Postgraduate Medical Training (Musterweiterbildungsordnung/MWBO) and the Competency-based Curriculum General Practice (KCA). Overall, the seminar curriculum has 62 basic modules with 2 curricular units each (e.g. Basic Principles of General Practice, Chest Pain, Billing) and another 58 two-unit modules on variable topics (e.g. digitalization, travel medicine) adding up to 240 (124+116) curricular units. A blueprint with a rotation schedule for all of the teaching sites in Baden-Württemberg allows regular attendance by n=400 GP trainees over a period of five years, with individual variability in terms of program length.Conclusion: The model entails a five-year, flexible program to accompany the postgraduate medical education in general practice which can also be implemented in multicenter programs and those with high enrollments. The model’s focus is on acquisition of core competencies for general practice. Despite the current shift to eLearning seminars due to SARS-CoV, the program’s implementation is being continued, constantly evaluated, and used to further develop the KWBW Verbundweiterbildung® program.

Having teaching staff with didactic qualifications in university teaching leads to a measurable improvement in academic skills among students. Previous recommendations on the type and scope of medical didactic qualification measures primarily apply to teaching staff at university and in-patient settings. The situation of primary care medicine, which often employs external lecturers and whose teaching takes place to a considerable extent in decentralized training facilities (teaching practices) is not adequately addressed.Taking into account a survey on the status quo at higher education institutions for General Practice in Germany, recommendations for minimum standards are made, based on national and international recommendations on the content and scope of medical didactic qualification measures.These recommendations include preliminary work by the Personnel and Organizational Development in Teaching (POiL) Committee of the Society for Medical Education (GMA), the MedicalTeachingNetwork (MDN), the Society of University Teaching Staff in General Medicine (GHA) as well as the experiences of the committee members, who hail from the field of general medicine, internal medicine and pediatrics amongst others.

Limitations to usual activities due to health problems impact quality of life and well-being. The Global Activity Limitation Indicator (GALI) was developed to assess the trends in limitations to usual activities. The GALI question is applied in a single or routed, multi-question version. GEDA 2014/2015-EHIS for the first time applied a three-question version. Three quarters of respondents reported that they did not experience any limitations to their usual activities. 18.7% of women and 17.0% of men said they had experienced moderate limitations. 6.4% of women and 6.8% of men reported severe limitations. The share of respondents experiencing limitations increases with age and shows a clear education gradient. Changes in methodology, however, mean that current results based on the GALI question cannot be compared to the results from earlier survey waves of the same study.

Demographic change, new health threats and inequalities in health and healthcare provision in and between European Union (EU) member states pose a great challenge to European health care systems. Not only for these reasons does it make sense to collect comparable European health data. Such information provides insights on the distribution of risk and protection factors, the prevalence of chronic diseases and the levels of care provided in the member states and supports the planning and implementation of (health) policy measures. Since 2013, in the context of the European Health Interview Survey (EHIS), all EU member states are obliged to collect data on the health status, the provision of healthcare, health determinants and socio-economic conditions of their populations. In Germany, the EHIS is integrated into health monitoring at the Robert Koch Institute (RKI). The RKI is thus Germany’s interface to the European health monitoring presented here. European health monitoring relies on different indicator systems such as the European Core Health Indicators (ECHI), EU social indicators and the health indicators of the European Sustainable Development Strategy. These are based on administrative and survey data, which stem for example from the EHIS or the European Union Statistics on Income and Living Conditions (EU-SILC) survey. Comparative data analyses must take into account the differences between health care systems, socioeconomic conditions and the age structures of societies. Variances in the prevalence of allergies for example are also due to differences in the available diagnostic tools. Significant differences in the prevalence of hypertension in Europe (with a range of 20% for women and 17% for men) are also related to different levels of awareness of hypertension. Comparative analyses can support the planning of and provide information for policy measures, and enhance the sharing of experiences between EU member states. A forthcoming EU regulation aims to harmonise the content of and intervals between health and social statistical data collection. Moreover, plans exist to establish a European Research Infrastructure Consortium (ERIC), which is set to develop and institutionalise European health monitoring.
