The introduction of intracytoplasmic sperm injection (ICSI) into the spectrum of assisted reproductive technologies has offered men who suffer from severe disorders of spermatogenesis and azoospermia the possibility of fathering a child. Different surgical techniques can be used to extract spermatozoa from these men from either the epididymis and/or the testis. Surgical sperm retrieval offers a treatment for patients with testicular and/or obstructive azoospermia in cases where microsurgical refertilization is not an option or has already failed. Among surgical techniques that have been explored over the years, microsurgical epididymal sperm aspiration (MESA) and testicular sperm extraction (TESE) have become the most popular. Percutaneous techniques (such as TEFNA) are available but have disadvantages versus open surgical procedures. Together with cryopreservation of extracted spermatozoa, these techniques facilitate retrieval of spermatozoa for several ICSI attempts by a single surgical intervention.

Especially young women with cancer face rising survival rates due to remarkable progress in oncologic therapies. Preserving fertility is a major concern for both patients and their next of kin. Well established reproductive technologies such as cryopreservation of fertilized oocytes after in vitro fertilization already make fertility preservation possible for some patients. This review is dedicated to the emerging techniques that are becoming widely accepted for fertility preservation in young women and girls with cancer, such as auto transplantation of ovarian tissue cryopreservation and in vitro maturation (IVM) of either oocytes or follicles. First results are encouraging. But some challenges still have to be tackled in order for these novel technologies to be routinely employed with the aim of successful fertility preservation.

Deep wound infection or prosthetic vascular graft infection is one of the most challenging complications in vascular surgery with a substantial early and late morbidity and mortality. Surgical treatment usually consists of complete removal of infected vessels or prosthetic vascular grafts followed by extraanatomic bypass procedures. However, this method is associated with significant mortality and amputation rates. Herein, we report two patients with deep wound and prosthetic vascular graft infection who underwent successful in situ reconstruction with cryopreserved arterial homografts. Although the long-term results are missing, this approach may offer a possible treatment alternative for this potentially life-threatening complication.

The transmission of old methods and the transformation of new knowledge in to animal breeding practice has an cultural tradition lasting millenniums. Each epoch had its own strategy of solving problems because progress is independent of religions and ideologies, and therefore, it can not be hindered. In the past different biotechnologies have increased progress in animal breeding exponentially, and it can be supposed that the course of the exponential curve will not change in the foreseeable future. Anyone who intends to compete internationally is compelled to use the new technologies, wherever old methods can not longer compete. The transition to the new biotechnology especially in Germany, hits limitations an two levels: on the one hand, an exaggerated desire for safety that has created the German gene law and, on the other hand the animal breeding industry that can hardly afford the expensive biotechnology because of its economic condition. It seems advisable to revise both the gene law and the expense of the technology. Animal breeding research should foster simplification (make less expensive) of biotechnology, and politicians should ask weather it is necessary for the gene sector to migrate further abroad. In animal breeding, the gene transfer is practical (e.g., liposome mediated gene transfer via sperm cells) because of its low costs, and so it is absurd not to allow its use. In Germany, transgenic individuals have to bee discarded at the knackery, whereas in the Netherlands heifers derived from the bull HERMAN, that is transgenic for lactoferrin are allowed to calf (and produce milk). Currently, gene transfer is not a viable alternative because of the negative emotional impact of biotechnology. Only someone who uses this technique may be responsible for handling and creating it without having, therefore, to overstep ethical borders.
