Despite the continuous technical progress regarding the transrectal ultrasonography of the prostate (TRUS) and its successful use in combination with magnetic resonance imaging (MRI) in MRI-targeted biopsy, there is no radiologic modality being able to rule out clinically significant prostate cancer without the need of systematic biopsy. In the past few years, TRUS regained more attention due to the development of high frequency ultrasound as well as the combination of different ultrasonic modalities like shear wave elastography and contrast-enhanced sonography (CEUS). Currently, multiparametric MRI (mpMRI)-targeted biopsy shows the best results concerning detection rates, sensitivity and specificity of clinically significant prostate cancer compared to systematic biopsy. In the future, transperineal biopsy is probably going to increasingly replace the transrectal biopsy approach. For both approaches, transrectal ultrasonography is necessary to display the prostate and to detect suspicious lesions. Therefore future improvements in transrectal ultrasonography can be expected.Trotz der kontinuierlichen Weiterentwicklung des transrektalen Ultraschalls (TRUS) und der Kombination verschiedener Bildgebungsmodalitäten gibt es aktuell noch kein Verfahren, das die systematische Biopsie der Prostata obsolet macht und eine alleinige läsionsgesteuerte Biopsie zulässt. Durch die Weiterentwicklung des konventionellen TRUS durch höhere Schallfrequenzen sowie die Kombination verschiedener Ultraschallmodalitäten (z. B. der Scherwellenelastographie und dem kontrastmittelgestütztem Ultraschall) rückt der transrektale Ultraschall zur Diagnostik des Prostatakarzinoms wieder zunehmend in den Fokus. Aktuell hat jedoch die multiparametrisch magnetresonanztomographisch (mpMRT) gestützte Target-Biopsie zusammen mit der systematischen Biopsie die beste Datenlage bezüglich einer Verbesserung der Detektionsrate von klinisch signifikantem Prostatakrebs im Vergleich zur alleinigen randomisierten Biopsie. Bezüglich des Biopsiezugangs wird voraussichtlich die transrektale Biopsie durch den transperinealen Zugang immer mehr in den Hintergrund geraten. Der TRUS wird jedoch in beiden Zugangswegen verwendet, weswegen von einer fortschreitenden Weiterentwicklung auszugehen ist.

In the future, precision medicine with agents targeting specific genetic alterations will play an important role in bladder cancer. This includes both single genetic alterations (e.g. FGFR3) and gene panel analyses in patients with no further therapeutic options, rare cancer subtypes or unusual clinical courses. These molecular analyses can be carried out on formalin-fixed paraffin-embedded tumor samples and the results should be discussed in interdisciplinary molecular tumor boards in order to either recommend approved targeted therapies or suggest patients for molecular-based clinical trials, compassionate use programs or off-label use of drugs. The remuneration of molecular diagnostics is largely well-represented for the outpatient sector in Germany; however, the covering of treatment costs must currently be approved by the health insurances.Beim Harnblasenkarzinom wird zukünftig die Präzisionsmedizin mit spezifischen molekularen Therapien eine wichtige Rolle spielen. Dies umfasst sowohl Einzelgenveränderungen (z. B. FGFR3) als auch Multigenanalysen bei austherapierten Patienten, seltenen Karzinomsubtypen und ungewöhnlichen Verläufen. Die Analysen können am formalinfixierten Patientenmaterial durchgeführt werden und sollten im Rahmen eines molekularen Tumorboards interdisziplinär diskutiert werden, um Patienten entweder spezifischen Therapien zuzuführen, diese in Studien einzuschleusen oder Empfehlungen für Härtefallprogramme und individuelle Heilversuche aussprechen zu können. Die Vergütung der molekularen Diagnostik ist dabei überwiegend für den ambulanten Bereich in Deutschland gut abgebildet. Die Übernahme der Therapiekosten muss dabei jedoch aktuell bei den Kostenträgern begründet beantragt werden.

Cancer diagnostics can be supplemented by disease-related biomarkers. In the course of modern patient-tailored cancer treatment, the importance of correct risk stratification, prognosis and monitoring has significantly increased. In recent years, a multitude of biomarkers and related test procedures have emerged to fulfil this purpose. The following review article summarizes the most recent developments with respect to the use of biomarkers in the diagnostics of urological cancers.Biomarker können die Diagnostik maligner Erkrankungen ergänzen. Im Zuge der modernen individualisierten Krebstherapie haben Risikostratifizierung, individuelle Prognose und nicht-invasives Monitoring an Bedeutung gewonnen. In den letzten Jahren ist eine Vielzahl an Biomarkern und zugehöriger Testverfahren hierfür entwickelt worden. Der folgende Übersichtsartikel fasst die jüngsten Entwicklungen bezüglich der Verwendung von Biomarkern in der Diagnostik urologischer Krebserkrankungen zusammen.

With around 30,000 new cases annually bladder cancer (BC) is one of the most frequent cancers in Germany and the incidence is associated with advanced age and nicotine use. Urothelial carcinoma is the most frequent histological variant of BC in Central Europe. Nonmuscle-invasive BC can be resected endourologically and treated with intravesical instillation therapy. In the case of progression to nonmetastatic muscle-invasive disease radical cystectomy with accompanying neoadjuvant or adjuvant chemotherapy can be curative. Systemic treatment is the standard of care in metastatic disease. Although immunotherapy has made great progress in recent years, palliative chemotherapy remains the gold standard in first-line treatment. The armamentarium is continuously evolving: systemic immunotherapy is currently being investigated in nonmuscle-invasive BC as well as in perioperative and maintenance treatment after first-line chemotherapy and several studies are testing new targeted agents in palliative systemic therapy. This article gives an overview of current innovations and the expected paradigm shift in systemic treatment of BC.Mit jährlich etwa 30.000 Neuerkrankungen ist das Harnblasenkarzinom (BC) eines der häufigsten Karzinome in Deutschland, und die Inzidenz steigt mit dem Alter. Das Urothelkarzinom stellt die häufigste histologische Variante des BC in Mitteleuropa dar. Das nichtmuskelinvasive Karzinom der Harnblase kann endourologisch reseziert und über eine intravesikale Instillationstherapie behandelt werden. Bei Voranschreiten zur muskelinvasiven Erkrankung kann eine Zystektomie mit begleitender neoadjuvanter oder adjuvanter Chemotherapie kurativ sein. Die Behandlung des metastasierten BC erfolgt systemisch. Obwohl der Einsatz der Immuntherapie in den letzten Jahren große Fortschritte gemacht hat, bleibt die palliative Chemotherapie nach wie vor der Goldstandard. Dabei entwickelt sich das Armamentarium weiter: Die Immuntherapie wird sowohl beim nichtmuskelinvasiven BC als auch in der perioperativen Behandlung sowie in der Erhaltungstherapie nach Chemotherapie untersucht, und es laufen Studien zu neuen zielgerichteten Wirkstoffen für die palliative Systemtherapie. Diese Neuerungen und der zu erwartende Paradigmenwechsel der Systemtherapie des BC werden in diesem Beitrag beleuchtet.

Autopsy studies have confirmed the high prevalence of latent prostate cancer; however, only a certain portion of patients require definite treatment. Active surveillance is one of the treatment options which, according to national and international guidelines, should be offered to patients with newly diagnosed low-risk prostate cancer. Prostate cancer-specific survival is high in these patients; therefore, curative treatment, such as radical prostatectomy, external beam radiotherapy and brachytherapy may be initially deferred in order to avoid therapy-related side effects. In order to qualify for active surveillance, strict inclusion criteria have to be met; nevertheless, the reliable identification of low-risk prostate cancer patients is not always possible. Patients under active surveillance are followed up regularly with prostate-specific antigen (PSA) testing, digital rectal examination (DRE) and repeat prostate biopsies. Due to the heterogeneity of primary prostate tumors precise molecular diagnostic techniques could allow individualized treatment strategies in the future.

We report about a 25-year-old patient with transnasal ketamine abuse over years presenting with severe irritative urinary dysfunction (imperative urinary urgency, pollakisuria, dysuria) and severe alguria. Cystoscopia showed ketamine-induced vesicopathy with errosive cystitis; other etiologies could be excluded. Despite serious effort the patient was not motivated for abstinence from ketamine. After two ineffecient therapies with botulinum toxin A (200 and 400 I. E.) injected into the bladder, a prostate preserving cystectomia and ileum neobladder were mandatory.
