Background: Pensioners of Turkish origin residing in Germany often engage in circular migration between the two countries, thereby experiencing discontinuities in prescribed medication and medical care for chronic diseases, with the risk of deteriorating health status. So far, discontinuities in prescriptions and care have not been analyzed quantitatively. Methods: Routine data of elderly chronically ill Turkish-origin members of a German statutory health insurance scheme were analyzed for discontinuities in prescriptions and care compared to non-Turkish members. The study was based on data on bronchial asthma, diabetes mellitus, hypertension, and coronary heart disease from the AOK Rhineland/Hamburg from 2012-2018. Results: Discontinuities in prescribed medication and general care were significantly more frequent among Turkish than non-Turkish persons for several chronic diseases. Particularly for diabetes mellitus and bronchial asthma, Turkish persons had high odd ratios for discontinuities. Discontinuities tended to occur less frequently among Turkish persons aged 60-64 years compared to those of retirement age. Conclusion: A relevant proportion of Turkish persons show healthcare disparities. Comparing the pattern of discontinuities among groups, age, and disease, circular migration seems a likely cause. Family physicians in Germany need to be aware of circular migration patterns. They should address longer stays in Turkey as one possible reason for irregular medication and health care early, to prevent inadequate care. [ABSTRACT FROM AUTHOR]

Microvascular angina is a common manifestation of coronary microvascular dysfunction, particulary prevalent in post-menopausal women above the age of 50 and associated with impaired quality of life and poor clinical outcomes. However, microvascular angina remains largely undetected given the underuse of diagnostic tools for the assessment of coronary microvascular function. As a consequence, many of these patients suffering from coronary microvascular dysfunction fail to receive the appropriate medical treatment and remain in the long term symptomatic. Invasive coronary catheterization with measurement of coronary flow reserve and intracoronary acetylcholine provocation testing allows for the assessment of coronary microvascular dysfunction, and a therapy targeting specific physiological pathways can be implemented. A targeted therapy includes lifestyle modifications, secondary prevention measures, and anti-anginal medication. Ongoing clinical research in the field is expected to deliver novel diagnostic and therapeutic concepts for an improved management of patients with coronary microvascular disease. The study compares the accuracy and outcome of coronary CT and coronary angiography in patients suspected of having obstructive coronary heart disease. A total of 2052 women and 1615 men were included in the study. After a follow-up period of 3.5 years, no significant differences were found in the primary outcome (cardiovascular events) and secondary outcome (extended cardiovascular events) between the two methods and the two genders. 25. P2X7 Genpolymorphismen bei Patienten mit koronarer Herzerkrankung mit und ohne Parodontitis

Der durch extrazelluläres Adenosintriphosphat (ATP) aktivierbare, als nichtselektiver Kationenkanal fungierende P2X7-Rezeptor trägt zur proinflammatorischen Antwort auf Verletzungen, Entzündungen und bakterielle Invasionen bei und ist in der Lage, Apoptose zu vermitteln. Da laut Literatur koronare Herzerkrankungen (KHK) einen Einfluss auf das Ausbilden einer parodontalen Erkrankung (PA) haben und beide Erkrankungen eine entzündliche Komponente unter Beteiligung von extrazellulärem ATP aufweisen, wurden hier einzelne Polymorphismen (single nucleotide polymorphisms, SNPs) und P2X7-Rezeptor SNP Genotypen untersucht. Für die Fall-Kontroll-Studie wurde eine Kohorte von 263 Patienten mit koronarangiographisch gesicherter KHK und 526 Kontrollen aus Ulm und Umgebung untersucht. Zusätzlich wurde als zahnmedizinische Parameter der Community Periodontal Index of Treatment Needs (CPITN) erhoben. Die SNPs 498, 946, 1405, 1513 und 1729 des P2X7-Rezeptors wurden mittels Pyrosequenzierung sequenziert. SNPs 946 und 1729 wurden in keiner homozygot mutierten (hm) Konstellation gefunden. Eine tendenziell erhöhte Chance an KHK zu erkranken, jedoch nicht signifikant, ließ sich für SNP946 mit einem p-Wert von 0,075 erkennen, was jedoch im Vergleich zu den Variablen Diabetes mellitus mit einem p- Wert von 0,004 und Blutdruck mit einem p-Wert von 0,02* 103 als Vergleichsparameter auf einen sehr schwachen Zusammenhang schließen ließ. Statistisch konnte kein signifikanter Zusammenhang festgestellt werden. Mit einem p-Wert von 0,069 kann man jedoch die Tendenz erkennen, dass eine Mutation an SNP1729 tendenziell vor einer Erkrankung an Parodontitis (PA) schützen könnte. Bei Betrachtung der Genotypen konnte festgestellt werden, dass die hier untersuchten Genotypen mit einer heterozygoten Mutation an Stelle 498 und 1405 (Genotyp: 21211, p=0,040, OR=1,58) und an Stelle 946 (Genotyp: 12111, p=0,067, OR=4) die Chance auf eine Erkrankung an KHK erhöhen. Protektive Wirkung konnte bei keinem Genotyp festgestellt werden. Die Chance an PA zu erkranken, war für die Genotypen 12111 (p=0,077, OR=0,240), 21212 (p=0,046, OR= 0,120), statistisch signifikant vermindert. Die Chance auf eine Erkrankung an PA erhöhen die Genotypen 11111 (p=0,031, OR= 1,66), 21111 (p=0,031, OR= 1,66). Insgesamt zeigen die Ergebnisse, dass die Kombinationen von SNPs des P2X7-Rezeptors (Genotypen), einer isolierten Betrachtung einzelner SNPs vorzuziehen ist. Die Unterschiede von Genotypen mit protektiven oder Krankheits-. prädisponierendem Effekt für KHK und PA lassen den Schluss zu, dass sich eine KHK nicht auf der Basis einer bakteriell bedingten Entzündung manifestiert. Letzteres ist aber bei der PA eindeutig der Fall. Risikogenotypen der KHK deuten auf eine Bedeutung eines funktional überdimensional starken P2X7-Rezeptors hin (21211), wohingegen bei PA schwach aktive P2X7-Rezeptor Genotypen das Erkrankungsrisiko erhöhen. Die Bedeutung der hier erhobenen Ergebnisse lassen sich mit mehr funktionalen Tests und der zusätzlichen Genortypisierung des funktionssteigernden SNPs rs1718119 weiter verbessern.

Cardiac computed tomography (CT) has made substantial progress in recent years. The main field of application is CT coronary angiography for visualization of the coronary arteries and for the detection and exclusion of coronary artery stenosis. This has been included in international guidelines for the management of stable coronary artery disease or chronic coronary syndrome as well as for the diagnostic work-up of patients with acute chest pain; however, it must be taken into account that the diagnostic validity is only sufficiently high when the image quality is good and therefore alternative diagnostic procedures should be included in patients where an unrestricted good image quality is not to be expected. The fact that CT angiography enables the detection not only of coronary stenosis but also of nonobstructive atherosclerotic plaque is interesting for the estimation of the risk of atherosclerotic events. It is practically certain that in the absence of detectable atherosclerotic plaque in CT angiography, statin treatment does not lower the risk of atherosclerotic events. To what extent CT is suitable to provide indications for statin treatment and the threshold for which the presence of nonobstructive plaque should prompt initiation of statin treatment are currently the subject of intensive research., (© 2023. This article summarizes the innovations and most important points of the recently published and updated ESC guidelines 2021 on the prevention of cardiovascular diseases. Developments in risk stratification and further developments with respect to targets and options for the treatment of risk factors necessitated an update of the guidelines from 2016. The innovations incorporate a staged and individualized approach to the prevention targets, an implementation of the systematic coronary risk evaluation 2 (SCORE 2 update) and the SCORE 2‑older persons (OP) systems on risk stratification, including age-dependent estimations for young (50 years old) and older people (≥70 years old) as well as for patients with diabetes mellitus and atherosclerotic cardiovascular diseases. Furthermore, specific recommendations are given for coronary heart disease, heart failure and chronic kidney insufficiency. In addition to recommendations on individualized treatment, the relevance of cardiovascular prevention at the population level, including environmental protection is underlined.Dieser Artikel fasst die Neuerungen und wichtigsten Punkte der kürzlich veröffentlichten und aktualisierten ESC-Leitlinie 2021 zur Prävention von kardiovaskulären Erkrankungen zusammen. Entwicklungen in der Risikostratifizierung sowie Weiterentwicklungen bezüglich Zielen und Optionen der Behandlung von Risikofaktoren bedurften einer Aktualisierung der Leitlinien von 2016. Die Neuerungen beinhalten einen stufenweisen und individualisierten Ansatz der Präventionsziele, eine Implementierung des SCORE2(Systematic Coronary Risk Evaluation [Update])- und SCORE2-OP(Older Persons)-Systems zur Risikostratifizierung inklusive altersabhängiger Abschätzungen für junge ( 50 Jahre) und ältere Menschen (≥ 70 Jahre) sowie bei Patienten mit Diabetes mellitus und atherosklerotischer kardiovaskulärer Erkrankung. Des Weiteren werden spezifische Empfehlungen für koronare Herzkrankheit, Herzinsuffizienz und chronische Niereninsuffizienz gegeben. Neben Empfehlungen zur individuellen Behandlung wird auch die Relevanz der kardiovaskulären Prävention auf Bevölkerungsebene inklusive Umweltschutz unterstrichen.

Background: Structured reporting allows a high grade of standardization and thus a safe and unequivocal report communication. In the past years, the radiological societies have started several initiatives to base radiological reports on structured reporting rather than free text reporting., Methods: Upon invitation of the working group for Cardiovascular Imaging of the German Society of Radiology, in 2018 an interdisciplinary group of Radiologists, Cardiologists, Pediatric Cardiologists and Cardiothoracic surgeons -all experts on the field of cardiovascular MR and CT imaging- met for interdisciplinary consensus meetings at the University Hospital Cologne. The aim of these meetings was to develop and consent templates for structured reporting in cardiac MR and CT of various cardiovascular diseases., Results: Two templates for structured reporting of CMR in ischemia imaging and vitality imaging and two templates for structured reporting of CT imaging for planning Transcatheter Aortic Valve Implantation (TAVI; pre-TAVI-CT) and coronary CT were discussed, consented and transferred to a HTML 5/IHR MRRT compatible format. The templates were made available for free use on the website www.befundung.drg.de., Conclusion: This paper suggests consented templates in German language for the structured reporting of cross-sectional CMR imaging of ischemia and vitality as well as reporting of CT imaging pre-TAVI and coronary CT. Background: Despite substantial success in the anesthetic and surgical management of cardiac surgery, patients frequently show postoperative complications and organ dysfunctions. This is highly relevant for mid- to long-term outcomes.Objectives: To evaluate cardioprotective strategies that may offer effective protection in vulnerable cardiac surgery patients.Methods: To demonstrate recent cardioprotective approaches for cardiac surgery patients, aiming to modulate the body's own protective mechanisms in cardiac surgery patients.Results: Both cardioplegia and hypothermia belong to the well-established protective strategies during myocardial ischemia. Volatile anesthetics have been repeatedly shown to improve the left ventricular function and reduce the extent of myocardial injury compared to a control group with intravenous anesthesia. Furthermore, patients receiving volatile anesthetics showed a significantly shortened stay in the ICU and in hospital after cardiac surgery. In contrast, numerous other protective strategies failed translation into the clinical practice. Despite the published reduction of troponin release after remote ischemic preconditioning, two recent large-scale randomized multicenter trials were unable to demonstrate a clinical benefit.Conclusions: Beside the use of cardioplegia and hypothermia, the use of volatile anesthetics is well-established during cardiac surgery because of its conditioning and protective properties. Regardless of the promising results derived from experimental studies and small clinical trials, the majority of other approaches failed to translate their findings into the clinic. Coronary heart disease (CHD) is a dynamic process with acute instable events and chronic periods leading to an increased mortality. Patients with CHD benefit from a differentiated antithrombotic therapy consisting of dual antiplatelet therapy in the acute phase and antiplatelet monotherapy or in combination with low dose anticoagulation (Xa-Inhibition) in the chronic phase. Bei älteren Patienten besteht häufig eine Koprävalenz einer hochgradigen, symptomatischen Aortenklappenstenose und einer koronaren Herzerkrankung (KHK). Bei älteren komorbiden Patienten mit einem hohem Operationsrisiko hat sich die transfemorale Aortenklappenimplantation (TAVI) als Therapie der Wahl etabliert. Die KHK stellt hierbei einen negativen prognostischen Faktor hinsichtlich des Überlebens dar. Dies verdeutlicht die Bedeutung der Frage nach einer optimalen Revaskularisierungsstrategie für KHK-Patienten, die sich einer TAVI unterziehen. Ziel dieser Studie war es, die Prävalenz eine KHK in einer großen TAVI-Kohorte, sowie den Einfluss des Schweregrades der KHK gemessen am SYNTAX (Synergy Between Percutaneous Coronary Intervention with Taxus and Cardiac Surgery) Score auf das Outcome nach TAVI bei KHK-Patienten zu evaluieren. Darüber hinaus sollte eine mögliche langfristige Auswirkung einer PCI (perkutane Koronarintervention) auf das klinische Gesamtergebnis nach TAVI untersucht werden. Etwaigen Verzerrungen zwischen den Kontrollgruppen durch das nicht-randomisierte Studiendesign sollte durch verschiedene geeignete Analyseverfahren begegnet werden. 60,2% der Patienten, die in unserem Zentrum mit einer TAVI versorgt wurden, waren an einer KHK erkrankt. Der anatomische Schweregrad der KHK wurde anhand des SYNTAX Scores bestimmt. 36,0% dieser Patienten erhielten im Rahmen der TAVI eine PCI. Der mediane baseline SYNTAX Score konnten durch eine PCI signifikant gesenkt werden. Die Rate an Myokardinfarkten war in der nicht behandelten KHK-Kohorte signifikant höher im Vergleich zu den Patienten ohne begleitende KHK, dagegen war die Rate an Myokardinfarkten vergleichbar mit der PCI-Gruppe. Die Häufigkeit eines Major Adverse Cardiac Events (MACE) sowie die Mortalität der TAVI-Patienten war in der PCI-Kohorte niedriger. Dementsprechend war in der KHK-Kohorte der residuale SYNTAX Score zusammen mit dem neu aufgetretenen Linksschenkelblock (LSB) ein unabhängiger Prädiktor für die Mortalität nach TAVI. In der Gesamtkohorte dagegen war der EuroScore (European System for Cardiac Operative Risk Evaluation) II neben einem LSB unabhängiger Prädiktor für die Mortalität nach TAVI. In Zusammenschau liefert diese Studie weitere Daten, die darauf hinweisen, dass TAVI-Patienten mit einer begleitenden schweren KHK eine signifikant höhere Sterblichkeit aufzeigen, insbesondere wenn eine ausreichende Revaskularisierung nicht erreicht wurde. Dies konnte anhand des anatomischen SYNTAX Scores quantitativ validiert werden: ein nach der PCI verbliebener Syntax Score > 7 war ein signifikanter negativer Prädiktor für die 1-Jahres-Mortalität nach TAVI. Diese Daten könnten im Entscheidungspfad helfen, das individuelle Risiko von TAVI-Patienten mit begleitender schwerer KHK mit hohem SYNTAX Score besser zu erkennen und entsprechend einer periinterventionellen PCI zuzuführen.

The 2019 guidelines of the European Society of Cardiology (ESC) rename stable coronary heart disease to chronic coronary syndrome (CCS). Under CCS, six different scenarios have been defined, which take its heterogeneity into account. An important part of the current guideline plays the diagnostic assessment of the pre-test probability in the event of the possible presence of stenosing coronary heart disease. The addition of dyspnea and clinical probability as additional variables for better estimation of the pre-test probability are two important new features. Imaging techniques such as CT angiography have been significantly upgraded as diagnostic procedures for the detection of CHD, while exercise ECG is no longer routinely recommended in this regard. Invasive coronary angiography with revascularization option remains a central diagnostic and therapeutic procedure. In addition to antianginal, lipid-lowering and antithrombotic therapy, the current CCS guideline places special emphasis on prevention by means of lifestyle modification.In den aktualisierten Leitlinien der Europäischen Gesellschaft für Kardiologie (European Society of Cardiology, ESC) wird die stabile koronare Herzkrankheit zum chronischen Koronarsyndrom (CCS) umbenannt. Unter CCS werden 6 verschiedene Szenarien definiert, die der Heterogenität Rechnung tragen. Die diagnostische Beurteilung der Vortestwahrscheinlichkeit spielt auch in der aktuellen Leitlinie bei möglichem Vorliegen einer stenosierenden KHK eine wichtige Rolle. Die Hinzunahme von Dyspnoe und der klinischen Wahrscheinlichkeit als zusätzliche Variablen zur besseren Abschätzung der Vortestwahrscheinlichkeit sind 2 wichtige Neuerungen.Bildgebende Verfahren wie die CT-Angiografie wurden als diagnostische Verfahren zur Detektion von KHK deutlich aufgewertet, während das Belastungs-EKG diesbezüglich nicht mehr routinemäßig empfohlen wird. Die invasive Koronarangiografie mit Revaskularisationsoption bleibt ein zentrales diagnostisches und therapeutisches Verfahren.Neben der antianginösen, lipidsenkenden und antithrombotischen Therapie wird in der aktuellen CCS-Leitlinie ein besonderes Augenmerk auf Prävention mittels Lebensstilmodifikation gelegt.

The prevalence of a major depressive disorder in patients after myocardial infarction is 20%. Depression is a risk factor for incident coronary heart disease and poor prognosis after myocardial infarction. Poor lifestyle habits and adherence to cardiac therapy as well as metabolic and pathophysiologic changes may partially explain this link. The threatening experience of an acute coronary event and immune and inflammatory changes may be unique features contributing to incident depression after myocardial infarction. While psychotherapy, antidepressants, and physical exercise may alleviate depressive symptoms in patients with coronary heart disease, cardiac rehabilitation additionally reduces mortality risk. Attempts are being undertaken to identify the cardiotoxic characteristics of depression to develop even more effective therapies in the future. [ABSTRACT FROM AUTHOR]

49. [Social inequalities in healthcare provision for patients with coronary heart disease: Results from the GEDA (German Health Update) study 2014/2015]

Little is known about social inequalities in outpatient long-term care of coronary heart disease (CHD) in Germany.Regression analyses are based on the responses of women and men who participated in the national cross-sectional study "German Health update" (GEDA) 2014/2015 and had self-reported CHD (N=920). Outpatient healthcare of CHD was analysed on the basis of the self-reported administration of antihypertensive and cholesterol-lowering drugs, and the frequency of general practitioner (GP) contacts.On average, respondents visited their GP 7.5 times a year (mean). 46 % did not receive guideline-consistent treatment, i. e. both antihypertensive and cholesterol-lowering drugs. Respondents of lower social status consulted their GP more frequently (approx. two visits per year) than those of higher social status (AME: 1.94; 95% CI 0.56 to 3.31). Regarding treatment with antihypertensive and cholesterol-lowering drugs, there were no significant differences for either gender or social status. Nevertheless, the probability that respondents with increased levels of blood lipids or cholesterol took only one or none of the two medications recommended for long-term treatment of CHD was reduced by 54 percentage points (AME: -0,54; 95% CI -0,61 to -0,48).There are no social inequalities in the treatment of CHD patients with antihypertensive and cholesterol-lowering drugs, but inequalities exist in the frequency of visits to the GP who is more often consulted by the more socially disadvantaged patients.With about 7.5 consultations per year, CHD patients visit their general practitioner more often than average, but in about half of these patients the medication supply is less than optimal. This may indicate a deficit in the medical treatment of CHD that cannot be explained by social inequalities. A possible starting point for improving healthcare, especially for patients without other risk factors, is to focus more strongly on a guideline-based approach to prescribing medication for CHD patients.
