Die akute arterielle Embolie ist eine seltene, aber schwerwiegende Komplikation nach einer Knie-Totalendoprothese (Knie-TEP). Es besteht ein allgemeiner Konsens darüber, dass in dieser Situation sofort eine Revaskularisation durchgeführt werden muss, aber die spezifische Behandlung ist immer noch umstritten. Wir berichten über zwei Fälle von Embolien der Kniekehlenarterie, die durch eine akute arterielle Thrombose nach Knie-TEP verursacht wurden. Bei beiden Patienten kam es nach der Operation zu einem Gefühls- und Bewegungsverlust der rechten unteren Extremität und einer Pulsationsschwächung der Arteria dorsalis pedis; eine Angiografie zeigte eine Embolie der Arteria poplitea. Einer der Patienten erhielt eine Thrombolysetherapie, entwickelte jedoch eine großflächige Infektion und Nekrose des rechten Wadenmuskels und benötigte nach erfolgreicher Thrombolyse ein mehrfaches Debridement und Hauttransplantationen. Bei dem anderen Patienten wurde eine Thrombektomie, eine Gefäßrekonstruktion und eine prophylaktische Fasziotomie durchgeführt; nach der Operation blieben ein Taubheitsgefühl im Fuß und eine leichte Streckschwäche zurück. Die Autoren empfehlen Chirurgen, Hochrisikopatienten mit Knie-TEP eine angemessene Aufmerksamkeit zu widmen. Vor der Operation sind eine sorgfältige Anamnese und körperliche Untersuchung erforderlich. Der chirurgische Eingriff sollte präzise und schonend durchgeführt werden, nach der Operation sind das Gefühl der Gliedmaßen und die Blutzirkulation aufmerksam zu beobachten. Bei abnormalem Fußgefühl und schwacher arterieller Pulsation sollten umgehend erforderliche Untersuchungen (Doppler-Ultraschall und Arteriografie) durchgeführt werden. Wenn eine arterielle Thrombose diagnostiziert wurde, muss die Blutversorgung sofort wiederhergestellt werden. The quality of reporting practice guidelines is often poor, and there is no widely accepted guidance or standards for such reporting in health care. The international RIGHT (Reporting Items for practice Guidelines in HealThcare) Working Group was established to address this gap. The group followed an existing framework for developing guidelines for health research reporting and the EQUATOR (Enhancing the QUAlity and Transparency Of health Research) Network approach. A checklist and an explanation and elaboration statement were developed. The RIGHT checklist includes 22 items that are considered essential for good reporting of practice guidelines: basic information (items 1 to 4), background (items 5 to 9), evidence (items 10 to 12), recommendations (items 13 to 15), review and quality assurance (items 16 and 17), funding and declaration and management of interests (items 18 and 19), and other information (items 20 to 22). The RIGHT checklist can assist developers in reporting guidelines, support journal editors and peer reviewers when considering guideline reports, and help health care practitioners understand and implement a guideline., (Copyright © 2017. History and Admission Findings: a 80-year-old women was admitted with a hypertensive crisis. Laboratory tests showed elevated cardiac enzymes (CK, CK-MB and troponin T). She was treated for suspected non-ST segment elevation myocardial infarction., Investigations: a cardiovascular examination, which included echocardiography, coronary angiography and magnetic resonance imaging, excluded a cardiac cause of the laboratory reults. After complete assessment the patient was found to have long-standing polymyosits positive for Mi-2 antibodies. This had caused troponin T elevation from the release of regenerating muscles after chronic inflammatory damage. Troponin I, however, is truly cardiomyocyte specific and distinguishes between cardiac and non cardiac origin of CK, CK-MB and troponin T., Treatment and Course: prednisone medication was started with a single dose of 50mg, then gradually reduced. Follow-up examination merely revealed minimally active polymyositis., Conclusion: troponin I should be measured in patients with inflammatory myositis/myopathies in order to diagnose and assess cardiac involvement.

Aim: This study investigated the fusion properties of a new intervertebral spacer that is directly fixed to the adjacent vertebrae by screws. The new spacer was compared with an identical spacer without direct fixation but with an additional plate and with an autologous bone graft., Methods: A single-level, cervical fusion was performed on 18 mature Merino sheep. Three treatment groups of six sheep each were used. Group A used an autogenous bone graft and an additional anterior plate. Group B used a titanium solid spacer with a titanium plasma-sprayed (TPS) surface and an additional anterior plate. Group C used a similar spacer as group B that was directly fixed with the adjacent vertebrae with 4 screws. The standing period was 12 weeks. The substrates were evaluated radiographically and histologically., Results: Dislocation of the spacer or the bone graft occurred in one of the 6 cases in each group A and B, while 5 of the 6 spacers were dislocated in group C. Successful fusion was found in 4 of the 6 cases in groups A and C and in 5 of the 6 cases in group B. The histomorphometry revealed a direct bone apposition at the interface of the bone graft (group A) at 11.6% of the surface. The surfaces of the spacers were osseointegrated in 1.1% in group B and 1.0% in group C., Discussion: As the dislocation rate shows, the fixation of the new spacer (group C) seems to be less stable than the fixation of the bone graft or the spacer with the additional plate. Although the intervertebral fusion is comparable for all groups, the extent of the osseointegration is much higher for the bone graft than for the spacers. In conclusion, the bone graft may offer a much better long-term stability than do the spacers.

Most intracranial bleedings in hemophiliacs occur in patients more than 6 months of age. In the neonatal period, this complication is rare and almost always observed in the first week of life. Based on a review of the literature, intracranial hemorrhage is an exceedingly rare occurrence in infants with hemophilia aged 2 weeks-6 months. We report on a male infant with hemophilia A who was referred to our hospital on day 18 because of pallor and jaundice. The neurological examination was normal. A cerebral ultrasound showed a left sided subdural hematoma with a shift of the midline structures to the right. Packed red blood cells (10 ml/kg) and factor VIII replacement (250 IU) were rapidly instituted and a craniotomy with evacuation of the hematoma was performed. The postoperative course was uneventful. On discharge, the neurological examination was considered normal.

Two years after the removal of an embryonic testicular tumour from a now 26-year-old man, his general health deteriorated and a holosystolic murmur was heard over the tricuspid valve area. A chest X-ray film demonstrated lung metastases. Biochemical tests were within normal limits, but the human chorionic gonadotrophin level was elevated to 81,030 mU/ml. Echocardiography revealed mild tricuspid regurgitation and a floating mass between the right atrium and ventricle, in contact with the tricuspid valve. The described findings did not support the diagnosis of embolus or inflammatory vegetation. Because of the danger of embolization and obstruction of the pulmonary outflow tract the mass was removed surgically. Enzyme-histological examination identified the tissue as a metastasis of the testicular tumour. The patient's condition at first improved, but he soon died of bleeding from the lung which was infiltrated by trophoblast-containing metastases of the testicular tumour.

The potentially ischemia-protective effect of ECG-synchronized coronary venous retroperfusion (SRP) with arterial blood via the coronary sinus (CS) was assessed in 26 patients (56 +/- 10 years, 22 male, 4 female) in the clinical scenario of PTCA of a proximal LAD stenosis. In six additional patients the SRP procedure failed due to anatomical or technical reasons. In an intraindividual comparison at least two standardized balloon inflations for 60 seconds at 6-8 atm were performed in randomized order with and without continuous SRP at a flow rate of 200 +/- 46 ml/min. Under both conditions echocardiographic regional wall motion, ST depression in leads V1-6, hemodynamic parameters and symptoms expressed in a pain score were continuously monitored during angioplasty. This study revealed that the echocardiographic regional wall motion score in the perfusion territory of the dilated artery increased from 1.65 +/- 1.81 at baseline to 5.65 +/- 2.88 (p < 0.001) during a one minute dilatation without SRP. With SRP-support the regional wall motion at 1 minute angioplasty was significantly improved to 3.55 +/- 2.80 (p < 0.025). Moreover, the ischemic ECG-changes were markedly less pronounced, whereas the subjective perception of anginal pain was not different as a function of SRP-support. Thus, the simultaneous coronary venous retroperfusion with arterial blood has ischemia-protective potential in elective PTCA of a proximal LAD stenosis and may reduce ischemic dysfunction with prolonged balloon inflations.

We analyzed performance and efficiency of the left ventricular myocardium on the basis of two new energetic parameters. The myocardial energy consumed during one cardiac cycle is related to performed work on the one hand (E1) and to the stress-time-integral on the other (E2). E1 was obtained by analysis of the pressure-volume integral divided by left ventricular muscle mass. E2 was obtained as follows: the stress-time integral was analyzed from pressure-volume data and wall thickness using an ellipsoidal calculation model. In order to transfer the stress-time integral into energy units, the value was multiplied by a constant factor which was obtained in experimental myothermal studies. In ten patients with coronary heart disease undergoing diagnostic heart catheterization, angiocardiography was performed before and after oral administration of nitroglycerin (1.6 mg). Total energy consumption (2E1 + E2) per gram myocardium per beat decreased from 6.1 +/- 1.3 mcal/g to 4.7 +/- 1.4 mcal/g (P less than 0.01), and myocardial efficiency (E1/[2E1 + E2]) increased from 27.0 +/- 3.1% to 28.4 +/- 4.3% (N.S.) on the average. This analysis explains quantitatively the beneficial effect of nitro-preparations on myocardial function and energetics.
