The coronavirus has challenged all medical systems worldwide. Herein both waves of COVID-19 (coronavirus disease 2019) in spring and autumn 2020 differ principally. Whereas Europe was hit by the first wave more or less unprepared, which was aggravated by the high virulence of COVID-19, the second wave is characterized by a much higher contagiosity of the virus with very high incidences. On the other hand the virus has attenuated, which is reflected by the significantly lower incidence-related mortality rate. However, the overall increasing number of infected patients represents again a great challenge for the medical management of the disease. France and Spain are doing better in comparison to Germany and Italy this time. The absolute number of deaths per week is higher than during the peak of the first wave. However, urologists in these countries have also experienced greater restrictions in their activities in the second shutdown than in Germany, where there is only a reduction of beds to between 75 and 90%. Mostly all levels are operated. Of importance for Germany, however, is the plateau on a high level for several weeks probably due to the reduced efficacy of a light lock-down. This finally resulted in a total lock-down in mid-December 2020. Subsequently in Germany some hospitals are also reaching their limits with similar consequences for the departments of urology facing a 50% reduction of beds and operating only level III and IV indications. Nevertheless, the management of urologic patients during the COVID-19 pandemic is carried out in Europa on a high standard. Therefor the risk of secondary harm to our patients is expected to be rather minimal in the long run.Das Coronavirus hat weltweit die Medizin vor große Herausforderungen gestellt. Dabei unterscheiden sich die beiden COVID-19-Wellen („coronavirus disease 2019“) im Frühjahr und Herbst 2020 grundsätzlich. Während die erste Welle Europa sehr unvorbereitet traf und sich das Virus durch eine hohe Virulenz auszeichnete, ist die zweite Welle im Herbst von einer hohen Kontagiosität des Virus mit sehr hohen Inzidenzen geprägt. Dabei findet sich aber eine deutlich abgeschwächte Virulenz, was sich in der niedrigeren inzidenzbezogenen Mortalitätsrate widerspiegelt. Andererseits stellt die insgesamt hohe Anzahl betroffener Patienten erneut hohe Anforderungen an die medizinische Versorgung. Im Vergleich schneiden Frankreich und Spanien bei der Bewältigung der zweiten Welle deutlich besser ab als Deutschland und Italien. So liegt die absolute Mortalität pro Woche in Deutschland trotz vergleichbar geringerer Inzidenz über den Werten der ersten Welle. Allerdings waren die Urologischen Kliniken in diesen Ländern auch von dem schärferen zweiten Lockdown mehr betroffen als dies in Deutschland aktuell der Fall ist, wo eine Bettreduktion zwischen 75 und 90 % besteht und immer noch alle Level operiert werden. Entscheidend für Deutschland ist aber das jetzt beobachtete mehrwöchige Plateau auf hohem Niveau, wohl bedingt durch den nur beschränkten Lockdown light. Die wieder steigenden Zahlen führten zum totalen Lockdown Mitte Dezember. Damit kommen auch in Deutschland Kliniken an ihre Grenzen, was auch entsprechende Auswirkungen auf die Situation in den Urologischen Kliniken hat mit ebenfalls 50%iger Reduktion der Operationskapazität und Reduktion auf nur Level-III- bis -IV-Eingriffe. Dennoch ist das Management urologischer Patienten inzwischen europaweit von hoher Qualität und Umsicht, so dass langfristige Sekundärschäden für unsere Kranken eher selten zu erwarten sind.

The article focuses on the fate of a personal library held by two librarians and bibliographers, dr. Franc Simonič (1847-1919) and his son in law dr. Janko Šlebinger (1876-1951) at the family estate in Gornja Radgona. Today we can witness only the remains of what was once an extensive library curtailed by the owners’ migrations and their financial hardships. Besides domestic and foreign literature, the library contained plenty of journals, for example the complete bind editions of Novice, Zgodnje danice, Čas, Kres, Veda, Ljubljanski zvon; it also included valuable manuscripts, old ephemera, 17th and 18th century books and plenty of rare editions such as books from the Prekmurje region and prints written in dajnčica. It was not only the professional selection that made the library exceptional but also its appearance – bookshelves full of prime bindings.

Purpose Adjuvant carboplatin is one of three management strategies that may follow inguinal orchiectomy in clinical stage I seminoma. However, little is known about the outcome of patients who experience a relapse after such treatment. Patients and Methods Data from 185 patients who relapsed after adjuvant carboplatin between January 1987 and August 2013 at 31 centers/groups from 20 countries were collected and retrospectively analyzed. Primary outcomes were disease-free survival and overall survival. Secondary outcomes were time to, stage at, and treatment of relapse as well as rate of subsequent relapses. Results With a median follow-up of 53 months (95% CI, 48 to 60 months) the 5-year disease-free survival was 82% (95% CI, 77% to 89%), and the 5-year overall survival was 98% (95% CI, 95% to 100%). The median time from orchiectomy to relapse was 19 months (95% CI, 17 to 23 months); 15% (95% CI, 10% to 21%) of relapses occurred > 3 years after treatment. The majority of relapses were detected by computed tomography scan during routine follow-up, 98% in the International Germ Cell Cancer Collaborative Group good prognosis group. Chemotherapy was administered to 92% of patients, mostly as standard first-line treatment corresponding to stage; 8% of patients had additional local treatments. Only 28 patients experienced a second relapse. At last follow-up, 174 (94%) of 185 patients were alive without disease, and four patients with disease. Seven patients died, three of whom due to progressive disease. Conclusion Within the limitations of a retrospective analysis, the results suggest that the majority of patients who experience a relapse after adjuvant carboplatin for clinical stage I seminoma can be successfully treated with a cisplatin-based chemotherapy regimen adequate for stage. Because 15% of the relapses occurred > 3 years after adjuvant treatment, a minimum of 5 years follow-up is recommended.

In diesem Band sind Beiträge versammelt, die ursprünglich als Vorträge für die internationale Tagung "Materialistische Dialektik - Marxlektüren im Dialog" (30. u. 31. 10.2015) konzipiert wurden. Sie befassen sich mit der Frage nach der epistemologischen und inhaltlichen Spezifik einer materialistischen Dialektik - und zwar sowohl aus dem Blickwinkel der Marxforschung als auch aus dem der marxistischen Theorie im weitesten Sinne.

Hintergrund: In operativen Kliniken gehen 20% der Patienten einem chronischen Alkoholmissbrauch nach, womit diverse postoperative Komplikationen verbunden sind. So liegt das postoperative Blutungsrisiko um 25% höher als bei Nichtalkoholkranken. Ursachen solcher Nachblutungen sind eine gestörte Synthese von Gerinnungsfaktoren aufgrund von Leberveränderungen und toxisch bedingten Thrombozytenpathologien. Bei der mangelhaften Produktion von Gerinnungsfaktoren spielt der Faktor XIII (F XIII) bei alkoholkranken Patienten eine entscheidende Rolle. F XIII ist ein fibrinstabilisierender Faktor und sowohl für die Blutgerinnung wie auch für die Wundheilung von Bedeutung. Ziel dieser prospektiven Studie war es, einen Zusammenhang zwischen der prophylaktischen Gabe von F XIII und der Blutgerinnung bei chronisch alkoholkranken Patienten nach einer Neck-Dissektion zu untersuchen. Methodik: 23 Patienten mit Neck-Dissektion wurden in diese randomisierte, doppelblinde und placebokontrollierte Pilotstudie eingeschlossen. Das Studienmedikament Fibrogammin® wurde am Operationstag präoperativ mit einer Dosis von 2500IE verabreicht und unmittelbar postoperativ sowie am ersten, zweiten und dritten postoperativen Tag mit je 1250IE. Die Patienten wurden am Operationstag dreimal und anschließend bis maximal zum zehnten postoperativen Tag einmal täglich aufgesucht. Blutentnahmen für die Analytik erfolgten präoperativ vor Studienmedikamentengabe, 30 Minuten nach der Gabe, unmittelbar nach OP sowie am ersten, vierten und zehnten postoperativen Tag. Hierbei wurde sich der klassischen Laboranalyse wie auch der Rotationsthrombelastometrie (ROTEM®), mit der eine spezifische Diagnostik zu Gerinnungsfaktoren, Inhibitoren und Zellkomponenten möglich ist, bedient. Ergebnisse: Im Rahmen dieser Studie konnte nicht nachgewiesen werden, dass chronisch alkoholkranke Patienten einen verminderten F XIII-Spiegel im Blut oder veränderte Werte in der ROTEM® aufweisen. In der Diagnostik mit dem ROTEM® konnten keine signifikanten Ergebnisse erzielt werden. Möglicherweise ist die Gerinnung der Verum-Gruppe qualitativ hochwertiger, aber da die Placebo-Gruppe vorrangig Normwerte in den ROTEM®-Messungen aufzeigte, bleibt die prophylaktische Gabe von F XIII fraglich. Schlussfolgerung: Anhand der Daten dieser Studie kann lediglich ein tendenzieller, aber kein signifikanter Vorteil der prophylaktischen Gabe von F XIII bei chronisch alkoholkranken Patienten mit Neck-Dissektion nachgewiesen werden. Zur weiteren Beurteilung dieses Aspektes sind in Zukunft zusätzliche Untersuchungen notwendig., Background: In surgical medical institutions 20% of the patients are addicted to alcohol what various postoperative complications are connected with. One of these postoperative complications is that the postoperative bleeding risk is increased by 25% compared to non-chronic alcoholics. These postoperative bleedings are caused by a disturbed synthesis of coagulation factors due to liver changes and toxically conditioned pathologies of platelet. With respect to the inadequate production of coagulation factors, Factor XIII (F XIII) is crucial for chronic alcoholic patients. F XIII is a fibrin-stabilising factor and therefore is important for coagulation and wound healing as well. The objective of this prospective study was to investigate the correlation between the prophylactic application of F XIII and the coagulation for chronic alcoholic patients after a neck dissection. Methods: 23 patients with neck dissection were enclosed in this randomised, double-blind and placebo- controlled pilot study. Within this study the drug Fibrogammin® was given preoperatively on the day of surgery and immediately after operation and on the first, second and third postoperative day. The initial dose was 2,500IE, the remaining ones were 1,250IE. The visit of the patients was three times on the day of surgery and afterwards once-a-day until the tenth postoperative day. The blood samples were collected preoperatively before application of the study drug, 30 minutes after the application, directly after surgery and on the first, fourth and tenth postoperative day. Within this study a combination of two different procedures was used to analyse the coagulation, the classical lab analysis and the Rotation Thrombelastometry (ROTEM®). The ROTEM® allowed a specific diagnostic with regard to coagulation factors, inhibitors and cell components. Results: Within the scope of this study there is no evidence that chronic alcoholic patients show a decreased F XIII level or changed values in the ROTEM® analysis. However, the diagnostic by the ROTEM® showed no significant results. Potentially, coagulation of the verum-group is qualitatively higher, but due to standard values of the ROTEM® in placebo- group, the prophylactic application of F XIII is still questionable. Conclusion: Based on this study, there is only the evidence for trends, not for significant advantages of the prophylactic application of F XIII for chronic alcoholic patients with neck dissection. Further research is needed to finally assess the investigated aspects.

The author presents the development of the labour exchange service in a public library.lt is the first information service of this kind in Slovenia designed for the unemployed,for those looking for jobs and for all who are interes ted in issues of employment.The labour exchange service resulted from a project af active employment policy carried out by the library and State Employment Office. After one year of operation it can be said that the new information office has successfully passed the project period and become an inseparable part of library services.

Das Erkenntnisinteresse dieser Diplomarbeit war es, herauszufinden, welches Verständ-nis von dem Begriff „Biografiearbeit“ in der deutschsprachigen Literatur vorherrscht und wie Biografiearbeit, in Theorie und Praxis der AutorInnen zufolge, zur Anwendung kom-men soll. Mittels einer Literaturrecherche und anschließender Literaturanalyse wurde zu-nächst der Begriff „Biografiearbeit“ herausgearbeitet und durch vorher festgelegte Katego-rien strukturiert. Die strukturierte Darstellung davon, welches Verständnis von „Biografie-arbeit“ den Texten zugrunde liegt, ermöglicht einen Erkenntnisgewinn innerhalb der jewei-ligen Literatur und verweist zudem auf Gemeinsamkeiten und Unterschiede zwischen den einzelnen Publikationen. Die Ergebnisse aus der Analyse zeigen, dass unter dem Begriff „Biografiearbeit“ ähnli-ches verstanden wird, jedoch keine einheitliche Begriffsdefinition vorherrscht. Da es kein einheitliches Bild über die pädagogische Biografiearbeit gibt, variiert die Ausübung der Biografiearbeit im Praxisfeld der Altenbildung und Altenpflege. Daraus ergibt sich ein Problem der Professionalisierung in diesem Bereich. Durch wissenschaftlich fundierte Konzepte könnte eine professionelle Biografiearbeit entwickelt werden, die dazu dienlich sein kann in der Praxis biografisch angeleiteten Methoden und einer professionellen Bio-grafiearbeit von Fachkräften für Anwendungen in institutionellen Rahmenbedingungen zu unterscheiden. Ausgehend von diesen Erkenntnissen wurden am Ende dieser Arbeit die Professionalisie-rung der Pädagogik im Bereich der Biografiearbeit diskutiert und zudem ein Ausblick dar-über gegeben, wie die Biografiearbeit eine professionelle Anwendung finden könnte., The main interest of this thesis was to find out the meaning of the term "biographical work" in German literature and how authors prevail it in theory and practice. In a literature review and subsequent analysis of the literature the term "biograhy work" was initially identified and structured by pre-defined categories. The structured represen-tation of the understanding of "biographical work" from the publications, enables a know-ledge gain within the relevant literature and also draws attention to the similarities and differences between the various publications. The results of the analysis show that the term "biographical work" is understood similarly, but it prevails no uniform definition of terms. Since there is no uniform conception of the biographical work, the application in the field of the geriatric education and care is very different. These results show the problem of professionalization of skilled employees in this area. Through science-based approaches a professional „biography work“ could be developed that would distinguish biographical guided methods from a professional biography work of professionals for use in institutional settings. Based on these findings the author of this paper discusses the professionalization of pe-dagogy in the field of biographical work and also gives an outlook on how it could find a professional application.

Histological examination of surgical specimens from 98 patients with carpal tunnel syndrome revealed amyloid deposits in 12% of the cases. Amyloid was located in the flexor retinaculum and to the same extent in the tenosynovium. Serial sections of the material containing amyloid deposits demonstrated that examination of only one level would detect solely 90% of the amyloid-positive cases. In one single case amyloid deposits were considered extensive enough to cause compression of the median nerve and thus could have played a role in the pathogenesis of the carpal tunnel syndrome. In 8 of the 12 cases the patients' histories proved no correlation to preexisting diseases. In our opinion, an individual disposition may promote amyloid deposits as a response to local and chronic irritation. It is noteworthy that the mean age in the amyloid-positive group (70 years) is significantly higher than in the amyloid-negative cases (55 years).

We have examined 16 patients with laryngeal carcinomas who had been submitted to laryngectomy and subsequent 60Co therapy (dose 60-70 Gy, thyroid exposure 50-60 Gy). After an interval of several years, some patients showed alterations of the thyroid function, i.e. a problematic hypothyreosis in two patients and a latent or compensated hypothyreosis in six patients. The latter could be demonstrated by several functional in-vitro tests (T3 saturation, T4, T3-RIA, TSHV, TRH). Especially the TRH test showed significant alterations in four patients. The results obtained from this relatively small number of cases correspond to the descriptions in literature of irradiation damages to the thyroid caused by the treatment of the laryngeal carcinoma. Despite the necessity of further control examinations of a greater number of patients, the results achieved hitherto suggest a prophylactic or substituting hormone treatment during the radiotherapy of the laryngeal carcinoma.
