3. [Utility of coronal oblique slices in cervical spine MRI: Improved detection of the neuroforamina]

Angulated projections are standard in conventional radiography of the cervical spine, but rarely used in magnetic resonance imaging (MRI). As neuroforaminal pathology plays an important role in the etiology of radicular syndromes and may influence an operative approach, the utility of coronal oblique slices in MRI is explored.In a retrospective setting, 25 consecutive patients with neurologically diagnosed cervical monoradiculopathy were identified. T2-weighted sagittal, coronal oblique, and transversal slice orientations were anonymized. Two radiologists and two neurosurgeons independently assessed the cases. Criteria were site, cause, and grading of the neuroforaminal stenosis and the level of confidence on a 100-point visual analog scale (VAS). We computed interrater agreement, sensitivity, and t tests.Using only one slice orientation, the sensitivity in detecting the relevant neuroforamen was 0.40 for transversal, 0.68 for sagittal, and 0.64 for coronal oblique scans. A combination of the different angulations increased sensitivity and in 4 cases only the coronal oblique scans proved diagnostic. The readers felt significantly more confident in attributing the cause of the pathology on coronal oblique planes (a mean of 72 VAS points, p = 0.0003 vs 58 (sagittal) vs 64 (transversal)). Interrater agreement was significantly better for experienced (kappa 0. 48) than for inexperienced readers (0.32, p = 0.02).Adding coronal oblique planes in cervical spine MRI increases sensitivity and confidence in attributing the cause of neuroforaminal pathology. They are regarded as useful by all the readers.

Magnetic resonance imaging (MRI) plays a very important role in the diagnosis of musculoskeletal conditions; its importance in orthopedic trauma continues to grow. To ensure optimal imaging and to be able to answer all clinically relevant questions, some prerequisites must be taken into account. Of uttermost importance is a functioning communication between surgeons and radiologists. To adapt the best sequences, the radiologist needs to know all suspected injuries and the mechanism of trauma. Second, the surgeon must have basic knowledge regarding this technology to optimally use all its possibilities. The aim of this article is to familiarize the reader with basic MRI in traumatology focusing on weightings and sequences.

Cranial magnetic resonance imaging (MRI) was performed in 250 patients who had been unconscious post-trauma for at least 24 hours. The frequency and the characteristics of injuries to the upper cervical myelon were determined.Between 1996 and 2009, MRI was carried out within 8 days of trauma.No lesions of the upper cervical medulla were found without accompanying damage to the medulla oblongata. Two groups were found to have a lesion in the upper cervical myelon. (i) In 3.2 % of the patients in a state of deep coma MRI revealed lesions in the entire brain stem. These died without waking from coma. (ii) 2 % of the patients were found to have additional damage to the distal medulla oblongata. These victims of high-speed traumas awoke from coma after 2-3 days. They revealed frontal contusions of the brain and traumatic subarachnoidal hemorrhages. Injuries to the bony upper cervical spine and/or the skull base were frequent. Four of them died, one patient survived with severe disabilities.Two types of lesions involving the upper cervical myelon could be differentiated, both of which occur only in association with lesions in the medulla oblongata.

Diagnostic and interventional procedures in children often need to be performed under sedation. This prevents pain and stress in children and provides optimal examination conditions. For complete immobilization and stress shielding the depth of sedation often corresponds with general anesthesia. Therefore, established safety standards need to be observed and a fundamental precondition is implementation by a skilled anesthesiologist who can handle the anesthesiology procedure and its possible complications. Organization, schedule, medication, equipment, monitoring and post-anesthesiology care should be institutionally defined. A professional anesthesiology management of pediatric patients is an important factor to increase the quality of care, patient safety and patient satisfaction.

10. [Life-threatening macroglossia following cleft palate palatoplasty]

The case of a 13-month-old child who developed a life-threatening macroglossia with airway obstruction following palatoplasty for a cleft palate is reported. As direct laryngoscopy was not feasible a laryngeal mask (LM) was inserted to secure the airway. Under fiber optic guidance an endotracheal tube was then introduced via the LM. In this article the incidence, pathophysiology, clinical dynamics, options for emergency anesthesia management and organizational implications of this rare but typical complication in the field of oral and craniomaxillofacial surgery are reported.

11. [First description of an intradural disc herniation at the C7-T1 level]

41 cases of cervical intradural disc herniation have been reported in the literature, none at the C7-T1 level. We describe a case of myelopathy after a fall. The female patient suffered an ataxia of the legs in combination with unilateral C8 cervicocephalgia and hypaesthesia. Magnetic resonance imaging revealed a space-occupying lesion at C7-T1, so we suspected a tumour. After a dorsolateral approach, an intradural disc herniation was found. Six months after the operation, the patient is free of complaints.

Im vorliegenden Artikel werden Besonderheiten im Krankheitsverlauf und in der Pharmakotherapie bipolar erkrankter Frauen diskutiert. Geschlechtstypische Unterschiede betreffen sowohl Symptomatik und Krankheitsverlauf als auch Aspekte, die mit der speziellen biologischen Situation der Frau zusammenhangen. Typisch fur bipolare Frauen ist die Dominanz depressiver Krankheitssymptome und das im Vergleich zu bipolaren Mannern etwa 8fach hohere Risiko fur einen ungunstigen Krankheitsverlauf im Sinne eines „rapid cycling“. Abhangig von der gegenwartigen Lebensphase sind bezuglich der pharmakologischen Behandlung weiblicher Patientinnen die Aspekte Fertilitat, Kontrazeption und Schwangerschaft in besonderem Mase mit einzubeziehen. Trotz hoher Relevanz fur den Krankheitsverlauf ist das sexuelle Erleben und Verhalten bipolarer Frauen bisher ebenso wenig untersucht worden wie spezifische sexuelle Veranderungen in manischen und depressiven Erkrankungsphasen.

13. [Fears, knowledge, and need of counseling for women with epilepsy. Results of an outpatient study]

Women with epilepsy are particularly affected by their condition and need therefore specific counseling and comprehensive information about issues related to contraception, pregnancy, hormone effects on seizure control, bone mineral density, etc. The primary aim of this study was to investigate the knowledge of women with epilepsy about their condition and their need for information and counseling.A total of 365 women with epilepsy aged from 16 to 75 years of age took part in this prospective, cross-sectional study. All were treated by neurologists in private practice or outpatient clinics. The physicians distributed anonymous questionnaires to the women, who mailed them unsigned to the Society for Epilepsy Research. Sociodemographic and epilepsy-specific data of the women, their experiences and fears regarding partnership, family planning, pregnancy, care of children, and their self-rated and actual knowledge were assessed.Most women (80.9%) lived together with partners, and about half of the women (44.9%) had children. The majority of those with children were greatly worried during pregnancy about risks to their newborn child. They were afraid of potential handicaps (57.9%) and potential epileptic seizures of the child (52.5%) because of their own epilepsy or antiepileptic drugs. Another 39.0% had had great reservations about becoming pregnant. About every fifth woman (18.2%) stopped or reduced antiepileptic medication during pregnancy without consulting her doctor. Women who consciously abstained from having own children (n=87, 23.8%) often reported epilepsy-related reasons (e.g. risk of malformation caused by antiepileptic drugs, 40.8%). Epilepsy also affects the mother-child relationship. For example 40.8% of the women were worried that the child could be harmed during a mother's seizure and 36% reported that their seizures would frighten their children. About 56-66% of the epileptic women stated that they were well to very well informed about topics on contraception, pregnancy, prophylaxis of malformations, and heredity of epilepsies. The knowledge questionnaires revealed however considerable knowledge deficits. These were especially widespread concerning older women and epilepsy, e.g. antiepileptic drugs and osteoporosis, and more pronounced in older women (50 years) and those with low school education. In all, self-rated and actual knowledge correlated only slightly (r=0.25). Of the women, 60% preferred personal counseling by their physicians. However, especially younger women wanted further information, primarily easy-to-understand brochures (32%) and educational courses (31%).Our results are in accordance with other studies from Great Britain and the U.S. They confirm that women with epilepsy have considerable deficits in epilepsy-specific knowledge and a great need of counselling and information.

Fetal magnetic resonance imaging (MRI) is a reliable method to further evaluate brain anomalies detected on ultrasonography. MRI can reveal additional brain abnormalities which are consequential for counselling parents about the fetal prognosis and subsequently influence the decision about continuing the pregnancy. In case of fatal malformations, MRI can confirm a diagnosis established on ultrasonography, supplying more reliability.

Acute cerebral vein and dural sinus thrombosis are usually treated with unfractionated intravenous heparin. Progression of symptoms after appropriate heparin dosage appears to be a rare event. In the literature only single case reports or small case series dealing with local intravenous fibrinolysis are available. Here we describe a 44-year-old woman with progressive thrombosis of the sinus rectus and right sinus transversus upon an appropriate dosage of heparin who was successfully treated with urokinase 12 h after symptom onset. This case demonstrates that local thrombolysis should be discussed in patients with progression of the clinical or radiological picture under sufficient heparin therapy.

The purpose of this study was to develop and evaluate a skill training aimed at increasing the social competence of caregivers of nursing home residents suffering from dementia. Herewith, the professional burden and occupational stress of the caregivers should be reduced and the quality of life of dementia patients should be increased. The contents of the training focused on problems and strategies in the communication with dementia patients and the communication with colleagues. The effectiveness of the intervention was tested in a controlled training study using a multiple control group design and process measurement. The participants of the trainings were 53 nursing home professionals, who were in daily contact with residents suffering from dementia. The results of the study verify effects for all relevant variables. The "social competence" of the caregivers increased and their "work stress" decreased while the "quality of life of dementia patients" increased. Therefore it can be concluded that training the social competence of nursing home professionals is a method to indirectly reduce their work stress and support dementia patients. The results of research in this program underline very clearly that the developed training is an effective option to improve the situation of dementia care in nursing homes. To make the intervention widely applicable we are currently developing a "multiplier program" in a follow-up project.

A number of recent studies suggest a link between in utero exposure to valproate (VPA) and low IQ and behavioural disorders in children of mothers with epilepsy. In this review, a commission of the German Section of the International League Against Epilepsy discusses the evidence in the literature and practical recommendations for the use of VPA in women of childbearing potential. It is concluded that despite methodological shortcomings--largely due to the complexity of the problem and small case numbers in prospective studies--the existing data are sufficiently alarming to require great caution in the use of VPA in women who could become pregnant. The underlying mechanisms of how antiepileptic drugs may lead to neurodevelopmental problems are unclear. Further prospective studies are urgently needed to clarify this clinically important issue, and a collaborative study is suggested based on the international network established by the European Registry of Antiepileptic Drugs and Pregnancy.

All old-generation antiepileptic drugs (AEDs) are considered to be teratogenic. In Germany, one out of 200 pregnant women (0.5%) is treated with AEDs for epilepsy. The risk of major malformations following exposure to AEDs during the first trimester of pregnancy is two to three times the rate reported in the general population, which is estimated at 2-3%. The risks associated with the treatment of epilepsy during pregnancy are therefore of major concern to all women of childbearing potential with epilepsy. Data on the comparative teratogenicity of these AEDs in humans are, however, conflicting, mainly due to inadequate sample sizes and other methodological shortcomings of previous studies. The teratogenic potential of newer AEDs is even less well known, which prevents a rational approach to AED treatment in women of childbearing potential. The European Registry of Antiepileptic Drugs and Pregnancy is a prospective international multicentre study of pregnancies with AEDs. In Germany the project was started in 2001 and so far more than 500 pregnancies have been enrolled. The enrollment rate is 4% of 4,000 pregnancies with AEDs reported annually.

With improvements in cancer survival rates, more patients with cancer are living longer and the influence of nutrition, lifestyle, physical activity as well as supportive care during and after chemotherapy is of increasing interest. In several malignancies smoking cessation increases cancer survival. Similar effects are expected by healthy nutrition. Regular physical activity of cancer patients reduces drug interactions of chemotherapy, decreases the number of comorbid conditions, and helps patients maintain independence as long as possible. For supportive care during chemotherapy the 5-HT3 receptor antagonists are more effective for the prevention of chemotherapy-induced nausea and vomiting. There are several colony-stimulating factors (e.g. GCSF, erythropoietin) for hematopoietic recovery post-chemotherapy. Altogether supportive care of chemotherapy reduces toxicity and increases efficacy.

Gender aspects are important when assessing the tolerability of antiepileptic drugs (AED). Due to its broad spectrum of action and to the lack of evidence for teratogenicity in monotherapy, lamotrigine (LTG) is an AED of first choice for women with epilepsy with child-bearing potential.In an observational study over 6 months including 832 women with epilepsy, we evaluated the efficacy and tolerability of LTG in monotherapy and add-on therapy, with a special focus on parameters of particular importance for women including changes in weight, skin, hair, and patterns of menstruation.Of the treated patients, 94% had seizure reduction of at least 50%. Adverse events occurred in 7.3%; 1% experienced serious adverse events, all of which were reversible. Ninety-six percent continued treatment throughout the observation period, and 83% reported improvement in quality of life by the end of the study. Cognition, functioning at work, and mood improved in 43-54% of patients. In most of them, weight remained stable. Those who were switched from valporate (VPA) or carbamazepine (CPZ) to LTG lost 3.2 kg and 3.1 kg (means), respectively. Many of the women described problems related to menstruation at the beginning of the study: variability in cycle length (31.2%), hypermenorrhea (17.3%), and pains (10%). Skin and hair problems were reported by 18.4% and 17.3%, respectively. There was improvement in all of these aspects at the end of the study, particularly for those women switched from VPA or CBZ to LTG.In summary, this observational study confirms the good tolerability of LTG with respect to issues particularly relevant to women. More complete elucidation of the correct LTG dosage, which varies widely according to the accompanying medication, will further improve treatment safety.

The program Kommunikations-TAnDem was developed with the primary purpose of increasing the communication competence of family caregivers of dementia patients. Further aims were to reduce the burden of family caregivers and to increase the quality of life of dementia patients. The effectiveness of the Kommunikations-TAnDem was tested in a controlled training study with 22 participants using a waiting-group control design and process measurement. The results of the study verify effects for the variables "Communication competence", "Caregivers knowledge about communication in dementia", "Quality of life of dementia patients" and "Directly perceived burden of family caregivers. Therefore it can be concluded that the Kommunikations-TAnDem is a method of indirectly supporting dementia patients by training the family caregivers. The results of research in this program underline very clearly the relevance of the inclusion of the family caregivers in intervention programs and the importance of communication competence for the well-being of dementia patients and their caregivers.

The available data have practical implications for the safe use of Lamotrigine in women.

The aim of this study was to compare total intravenous anaesthesia (TIVA) using propofol and remifentanil (P/R-group) and balanced anaesthesia (BA) using sevoflurane and remifentanil (S/R-group) for paediatric surgery.A total of 120 patients aged 6 months to 16 years scheduled for elective minor lower abdominal surgery were randomly assigned to receive either propofol (5-10 mg/kg/h) and remifentanil (0.125-1.0 microgram/kg/min) or sevoflurane (1.0-1.5 MAC) and remifentanil (0.125-1.0 microgram/kg/min). Perioperative haemodynamics as well as recovery and discharge times, PONV and side-effects were studied. The patients vigilance, comfort and pain intensity were assessed postoperatively using the objective pain discomfort scale, the Steward post-anaesthetic recovery score and a visual analogue scale.Postoperative recovery (9.0 vs 11.6 min) and extubation times (11.8 vs. 15.0 min) as well as the time taken until a Steward post-anaesthetic recovery score3/4 (15.2 vs. 21.4 min) was reached were significantly shorter in the P/R-group. However, the length of time until discharge to the ward, postoperative comfort, pain intensity and analgesic requirements as well as PONV were comparable in both groups.With regards to the investigated parameters, TIVA with propofol and remifentanil is equally effective as BA with sevoflurane and remifentanil in paediatric patients. However, considering the selected dosing regimen, recovery times were significantly shorter for children after TIVA.

In locally advanced or recurrent tumors of the female genital tract anterior or total exenteration may be mandatory in case of tumor invasion into the lower urinary tract or if a second course of radiation therapy is not feasible. The management of resection and reconstruction of the affected lower urinary tract has to be well integrated into the gynecological therapeutic concept. In 11/32 patients the reconstruction of the partially resected lower urinary tract was feasible with preservation of a functionally intact urinary bladder. Urinary diversion following pelvic exenteration was achieved in 13/17 patients with a continent urinary reservoir and in 4/17 patients with an ileal conduit. Operative reinterventions were needed only in patients with continent urinary diversion in 5 cases. All these patients had a past history of primary radiation therapy of their gynecological tumor. In the remaining other 11 patients with a history of primary radiation therapy no complications occurred. 9 of 32 patients survived the operative procedure 40.8 (25-57) month with no evidence of recurrent tumor. Continent urinary diversion represents an excellent therapeutic option for replacement of function lost due to exenterative pelvic surgery. Stringent selection of patients is mandatory to consider the presented therapeutic concept a reasonable tool in the management of the described clinical situations.

The loss of pipeline pressure in a central gas supply system is a rare but potentially hazardous complication in anaesthesia and critical care. In an anaesthesia simulator study, reactions of 20 anaesthetists to this simulated critical incident were monitored and evaluated. A comparison between novice (n = 10) and experienced anaesthesia residents/consultants (n = 10) determined a significantly quicker and more on-target reaction by the experienced anaesthetists. Unlike older cycle system anaesthesia machines, update anaesthesia ventilators (CICERO EM, Dräger, Lübeck) do not permit manual ventilation of a patient in a "closed-system" once pipeline pressure drops to zero. In this highly hazardous event, the patient has to be ventilated by reservoir bag until a sufficient back-up system delivering high inspiratory oxygen concentrations can be installed, because he is otherwise prone to diffusion hypoxia. Installation of mandatory (anaesthesia-machine integrated) back-up systems for respirators without cycle systems would therefore increase patient safety. A general algorithm for loss of pipeline pressure can be described only after a back-up system has been installed.

Dural sinus thrombosis is not uncommon. Due to the nonspecific symptomatology, as well as the manifold etiology, clinical diagnosis may be difficult. In these cases imaging procedures are frequently crucial in deciding how to proceed and how to treat. The aim of our study was to evaluate the diagnostic utility of helical CT in the detection of dural sinus thrombosis. In 20 patients with clinically suspected thrombosis CT angiography was performed. In 6 patients dural sinus thrombosis was diagnosed. In order to acquire also arterial vessels, a short delay of about 22 s after the onset of the application of contrast medium was selected. By this method we found an occlusion of the MCA in two patients with clinically suspected sinus thrombosis. In all patients the transverse slices and the multiplanar reconstructions showed filling defects or an "empty delta" sign. With irregular outlines the thrombus could be depicted over the complete course of the sinus. The MIP reconstructions were particularly helpful in the evaluation of the vessel anatomy and the pathological collateral venous drainage. In three patients MR angiograms were available for comparison. The smaller veins, such as the v. vermis inferior, were less clearly depicted than in CT angiography. CT angiography is a fast and reliable method to exclude or verify a sinus thrombosis. It can be performed immediately after non-enhanced CT. According to our present experience CT angiography is sufficient for the diagnosis of a sinus thrombosis.

Collagen vascular diseases, representing systemic soft tissue disorders, may cause a broad spectrum of pathologic changes of the respiratory tract. The type and extent of manifestations can vary considerably among individuals and entities. This survey describes the chest radiographic and, in particular, high-resolution computed tomographic (HRCT) findings of individual lesions of the respiratory tract. It includes fibrosing alveolitis (alveolitis, interstitial pneumonia, pulmonary fibrosis) and bronchial (bronchitis/bronchiolitis, bronchiectasis), pleural and vascular manifestations, as well as lymphadenopathy and abnormalities related to therapy. We present typical patterns of changes in progressive systemic sclerosis (PSS, scleroderma), systemic lupus erythematosus (SLE), mixed connective tissue disease (MCTD, Sharp syndrome), Sjögren syndrome, overlap syndrome and rheumatoid arthritis (RA). Furthermore, we describe findings which are specific for individual entities such as esophageal involvement in PSS, acute pneumonitis and pulmonary hemorrhage in SLE, lymphoproliferative disease in Sjögren syndrome and necrobiotic nodules in RA.

The DEXA-method allows for bone mineral density measurements of different regions of the skeletal system. It is known that results obtained from the spine with this two-dimensional method may be biased in cases of spondylosis, spondylarthritis, and compression fractures; similarly biased results can be expected at the proximal femur due to morphometric peculiarities. The influence of anteversion and rotation of the femoral neck has been investigated in the present study. The results indicate that errors due to rotation as occurring in incorrect positioning or in arthrogenic variabilities mainly have effects on measurements of Ward's triangle: The average measurement error at 20 degrees external rotation amounts to 33.8% with a maximum of 66.5%. Measurements of the trochanteric region and of femoral neck region show a variation of 6.2% and 9.7%, respectively.

To investigate the diagnostic and clinical usefulness of a new non-ionic, hydrophilic gadolinium (III) chelate [Gd(HP-DO3A), gadoteridol, ProHance] and to compare it with Gd-DTPA (gadopentetate dimeglumine, Magnevist).In a Phase III clinical trial, 20 patients with rheumatic joint disease were examined before and after intravenous administration of gadoteridol in two different doses (0.1 and 0.3 mmol/kg bodyweight). Magnetic resonance imaging (MRI) was performed at 1.5 T with T1-weighted FLASH and T2-weighted spin echo sequences. Fourteen patients were examined with gadopentetate dimeglumine for comparison. Dynamic changes of signal intensity in the joints and muscle tissue were determined quantitatively.No significant changes in cardiovascular data and no adverse effects occurred after injection of gadoteridol. The 0.3 mmol/kg dose showed no advantage in diagnostic contrast over the 0.1 mmol/kg dose. No significant differences (p0.01) were noted between gadoteridol and gadopentetate dimeglumine in patients with early rheumatoid arthritis.Gadoteridol proved useful in the detection of early rheumatoid arthritis. No significant differences were observed between the two gadoteridol doses. There were no diagnostically relevant differences between gadoteridol and gadopentetate dimeglumine.

For cardio-pulmonary resuscitation there are standardized treatment concepts, but there have been few prospective investigations examining the efficacy of prehospital advanced trauma life support and its effect on the outcome in patients with severe head injury and multiple trauma treated within the German emergency system. The results of this study underline the importance of intensive prehospital treatment and highlight some problems that should be taken into account in future in the training of emergency physicians. METHODS. A total of 179 patients with cerebral trauma were investigated. Data obtained included demographic and logistic data of the patients and the emergency physicians, diagnoses and treatment at the scene of the accident and state of the patient on admission in each case. Having divided the patients into three groups by severity of the trauma, we distinguished between sufficient and insufficient treatment and assessed infusion therapy, ventilatory support, positioning and immobilization, and analgesic and sedative therapy. For statistical analysis of the data we used chi 2-test and Fisher's exact test. P0.05 was considered significant. RESULTS. There were 102 patients who had sustained a cerebral trauma without other life-threatening lesions (score 1), 40 with multiple trauma (score 2) and 37 with multiple trauma (score 3). On average 2.4 IV lines were established and the patients received 1186 +/- 765 cc of crystalloid in addition to 801 +/- 411 cc of colloid fluids. In all groups, patients who received adequate infusion therapy had a better outcome; even in the group with score 1 significantly fewer had a fatal outcome. In all, 167 (93%) patients had endotracheal tubes placed, and in 150 cases (84%) ventilatory therapy was considered sufficient. The proportion of score 1 patients with sufficient ventilatory support who had a fatal outcome was significantly lower than that in the group with insufficient treatment. In patients with multiple trauma we could not separate the benefits of sufficient respiratory therapy and infusion therapy. In only 54% of the cases a vacuum mattress was used and in only 41% the patients were positioned with the upper part of the body elevated by 30 degrees. These were 28 patients (16%) who received neither analgesics nor sedatives. Regardless of the quality of prehospital treatment of isolated head injury, a Glasgow Coma Scale (GCS) score lower than 5 involved a very high mortality and all patients with a GCS score of 9 or more survived. In the group with GCS scores between 5 and 8, however, significantly more of the patients who received adequate treatment survived (82.5% vs 40%). CONCLUSIONS. The present study confirms that sufficient advanced trauma life support can improve the outcome of trauma victims with cerebral trauma. Adequate infusion and respiratory therapy reduce the mortality among such patients significantly. In patients with multiple trauma a clear positive effect of generous infusion therapy also is evident. The clearest effect of sufficient prehospital treatment is seen in patients with isolated cerebral trauma and a GCS score between 5 and 8. These results demonstrate the importance of advanced trauma life support and show emphatically that the so-called scoop-and-run strategy should be abandoned when resources are available for extended preclinical emergency treatment. On the other hand, we detected some problem areas in the prehospital treatment of trauma victims, such as positioning, immobilization and drug therapy with analgesics and sedatives. These findings allow us to pinpoint specific points that should be stressed in the training of emergency physicians and paramedics.

Transurethral resection of the prostate (TURP) and open adenectomy are regarded the "golden standard" in the treatment of benign prostate hyperplasia (BPH). Various alternative treatment forms (microwaves, laser, radio frequency, focused ultrasound) have been introduced recently. They are all aimed at reducing the morbidity related to TURP while achieving comparable efficacy at the same time. Since December 1992, 35 patients with BPH have been treated with high-intensity focused ultrasound (HIFU-P) in our department. By 6 weeks after HIFU-P, mean Qmax had improved from 6.5 ml/s to 12.1 ml/s. Residual volume dropped from 207 ml to 95 ml, IPSS from 20.1 to 9.7. Both subjective as well as objective improvements remained stable after 3 and 6 months. Urinary tract infections were observed in 3 patients, macrohemato-spermia in all, and macrohematuria (caused by the suprapubic catheter) requiring blood transfusion in 1 patient.

SPECT with the benzodiazepine receptor antagonist 123I-Iomazenil was performed in 33 patients with intractable partial seizures for pre-operative evaluation. The results combined with MRI and 99mTc-HMPAO-SPECT findings were compared with the video-assisted EEG monitoring ("gold standard") which localised the focus in 25 patients. 11 patients underwent surgical resection of the epileptogenic area and became seizure-free for a period up to 13 months. The Iomazenil SPECT had a significantly higher sensitivity compared to 99mTc-HMPAO in the visualization of an epileptogenic area and should be regularly used in the diagnostic of medically intractable partial seizures.

103 patients with 106 Müller straight stem prostheses could be evaluated clinically and radiologically after an average follow-up period of 6.8 years (range 5 to 10 ys.). Although until now no revision for aseptic loosening has become necessary 3 p.c. of the stems and 11 p.c. of the acetabular components showed radiological loosening. The clinical results were rated as satisfactory. After surgery 67 p.c. of the patients were completely painfree, 28 p.c. still had slight pain, 4 p.c. moderate and 1 p.c. severe pain. The investigation concentrated particularly on radiographic changes of the interface of the femur. Whereas radiolucency typically was non-progressive and located in the proximal part of the femur we found an increasing atrophy of the cortical bone on the calcar. Moreover, we found a continuous and increasing subsidence of the stem during the whole period of observation. At the time of follow up 56 p.c. of the straight stems had subsided about 2 mm and more. Calcar atrophy and radiolucency were significantly less present in femurs with a tapered medullary canal than in those with a cylindrical shape.

The purpose of this study was to compare HMPAO- and IMZ-SPECT findings in treated and untreated patients with focal epilepsies. We examined 20 patients (untreated 13; treated 7). All patients had a normal CT and/or MRI scan. In all patients both whole-brain activity and regional count density (rcd) were evaluated by SPECT. Furthermore, the specific and non-specific binding of IMZ were calculated for each ROI and the results of both, treated and untreated patients were compared. Our data indicate that the normalized cerebral total binding of IMZ was significantly increased in all regions in the group of treated patients (p0.001). In contrast, no difference in the tracer binding was found with HMPAO-SPECT. We conclude that the difference in the IMZ-binding might be due either to exogenous or endogenous factors. In our study IMZ-SPECT had a higher sensitivity and a lower specificity in the detection of decreased rcd in areas with EEG findings that indicate an epileptogenic focus compared with HMPAO-SPECT.

Following irrigation-suction drainage and synovectomy in 68 knee joints for empyema between 1971 and 1987, all were available for clinical and radiological follow-up. Since in 24 cases the empyema occurred after total knee arthroplasty, this group of patients was evaluated separately. The mean follow-up time in the no-prosthesis group was 8.3 and that in the prosthesis group, 5.3 years. In particular the aetiological factors, the bacterial pathogens and the effect of infection on radiological signs of degeneration were studied. The clinical results, including subjective findings, were assessed with the aid of HSS score. Our late results show that the prognosis for knee joint empyema can be rated as good if it is diagnosed early and treated aggressively. The poor outcome in a few cases was rather the result of underlying local disease and concomitant factors, and seemed not to be an immediate consequence of the infection itself. The relatively poor results in prosthesis group have to be referred predominantly to cases dating from the early years of total arthroplasty, with implants of the metal-on-metal type.

Social behavior was analyzed as a function of these factors. Recent reviews restrict themselves to positive feeling states. Summarizing, Fiedler (1988) describes the information processing style in positive feeling states as "loosening" to capture its qualitative aspects. This study investigates the opposite direction of influence, i.e. the effect of cognitive style on positive feeling states. This study restricts itself to positive feeling states. The compatibility thesis which postulates a necessary interaction of feeling state and information processing style is tested. Equivalent states and productions have to go together to generate the mood effects. In a 2 x 2 x 5 mixed design 70 female students (non-psychologists) served as subjects. Using a 20-minute mood induction procedure (autobiographical recollection methodology) a positive or neutral feeling state was elicited in half of the participants. During the next 10 minutes half of each group worked either on a verbal creativity test (Schoppe, 1975) or on an intelligence test (Amthauer, 1973) to establish an creative or analytic style of information processing. Repeated sampling of a measurement repetition factor served as baseline assessments, manipulation checks, and measurements of the feeling states during the task completion of the creativity or intelligence test. The feeling states were assessed by means of a short version (BSK-1982) of the "Eigenschaftswörterliste" (JankeDebus, 1978). The results confirm the compatibility thesis. Only the group in which a positive feeling state and a creative processing style interact reported a positive mood throughout the task completion. Unexpectedly, a slight deterioration of mood was found in the group with a neutral feeling state and analytic reasoning, suggesting that the neutral mood induction might in fact, elicit a mild negative feeling state and that the compatibility thesis might, also be applicable to the field of negative emotions.

Femoral anteversion, anteversion of the acetabulum and tibial torsion of 47 patients with idiopathic osteoarthritis of the hip were determined by computerized tomography. From all these parameters only the anteversion of the acetabulum showed differing values in hips with and without osteoarthritis. There was a considerable range at the individual measured values in all parameters and differences between the male and female groups could be found in femoral anteversion and anteversion of the acetabulum.

A series of 90 patients, 85 males and 5 females, all with ACL-ruptures had a follow up between 5 and 12 years. All of them had a primary arthrotomy with or without meniscectomy but in no case an operative repair of the ACL-rupture was done. The most concomitant injury was a torn medial meniscus in 76.3%, a damaged lateral meniscus was found in 17.1%, chondromalacia was seen in 53%, either of tibia, femur or patella. Nearly 60% of the patients could resume their former sports activities in some cases on a lower level, whereas 33% had to change their sport or even had to give up any sport. 75%-80% of the patients with partial or total meniscectomy and 90% of those without meniscectomy estimated the outcome as good or fair. In contrast to this 61% rsp. 86% of the patients with partial or total meniscectomy had an osteoarthritis of grade III and IV, patients without meniscectomy in only 23%.

The first section of the paper presents the morphology, structure and function of the eosinophilic granulocytes, while the second part deals with major neurologic diseases in which eosinophils are present or represent significant findings. These data are compared with the results of the author's own examinations of 1,000 unselected cerebrospinal fluid (CSF) samples. Eosinophils appeared usually in conjunction with infection, followed by foreign body inflammation. If the proportion of eosinophils exceeds 4%, the condition is termed CSF-eosinophilia, which usually develops in the presence of foreign bodies. However, infection may produce similar findings, especially in parasitic disease, and tumours of the central nervous system occasionally present with CSF eosinophilia.

- Abstract
Zusammenfassung Hintergrund  Die Vertr�glichkeit eines Antiepileptikums wird heute auch unter geschlechtsspezifischen Gesichtspunkten beurteilt. Lamotrigin (LTG) ist aufgrund eines breiten Wirkspektrums und bislang fehlenden Hinweisen f�r teratogene Effekte ein Antiepileptikum der ersten Wahl f�r Frauen im geb�rf�higen Alter. Methode  In einer gro�en Anwendungsbeobachtung (AWB) wurden Wirksamkeit, Vertr�glichkeit und frauenrelevante Vertr�glichkeitsparameter wie Menstruationsbeschwerden, Gewichts�nderungen, Hautbeschaffenheit und Haarwuchs bei 832 Frauen, die auf LTG in Monotherapie oder als Add-on-Therapie eingestellt wurden, �ber 6 Monate untersucht. Ergebnisse  Die Wirksamkeit und die allgemeine Vertr�glichkeit von LTG in Mono- wie Add-on-Therapie war sehr gut (94% hatten eine mindestens 50%ige Anfallsreduktion). Unerw�nschte Ereignisse traten bei 7,3% der Frauen auf; 1% der Patientinnen hatten schwerwiegende unerw�nschte Ereignisse, die ausnahmslos reversibel waren. 96% der Patientinnen nahmen LTG bis zum Ende der Beobachtungsperiode und dar�ber hinaus ein. Bei 83% verbesserte sich im Verlauf der Behandlung die Lebensqualit�t. Kognitive Funktionen, Arbeitsleistung und Stimmung besserten sich bei 43â54% der Patientinnen. Das Gewicht blieb unter LTG insgesamt stabil, reduzierte sich bei den Frauen, die auf LTG von Valproat (VPA) oder Carbamazepin (CBZ) umgestellt wurden, durchschnittlich um 3,2 kg bzw. 3,1 kg. Ein relativ gro�er Anteil der Frauen gab zu Beginn der Studie Menstruationsprobleme an. Probleme mit der Haut und mit dem Haarwuchs wurden von 18,4% bzw.17,3% angegeben. In jedem dieser Bereiche nahm der Anteil der Frauen mit Problemen w�hrend der AWB etwas ab. Dabei profitierten insbesondere Frauen, die von einer Vorbehandlung mit VPA oder CBZ auf LTG umgestellt wurden. Schlussfolgerungen  Insgesamt best�tigt diese AWB die gute Vertr�glichkeit von LTG auch unter frauenrelevanten Gesichtspunkten. Durch weitere Aufkl�rung �ber die LTG-Dosierungsempfehlungen, die sich nach der Begleitmedikation deutlich unterscheiden, kann die Anwendungssicherheit noch verbessert werden. [ABSTRACT FROM AUTHOR]

Lateral displacement of the femoral head was measured by the radiographs of 71 patients suffering from Perthes' disease. It is reported to be an early sign of the disease. Also it represents subluxation of the head of the femur, which is well accepted for a not favourable outcome. At the time of diagnosis average widening of the medial joint space was 1.9 mm. During the course of the disease the joint space was at its maximum width during fragmentation stage. A relation between lateral displacement and the extent of the disease could be shown, what suggests an importance in pathogenesis. Swelling of the soft tissues within the fossa acetabuli has been related to lateral displacement. On abduction films the femoral head could be demonstrated to migrate deeper into the acetabulum in those cases, when flattening of the femoral head was present. The consequences for the course and therapy of the disease is discussed.

A congenital bulging of the myocardial wall is a rare finding; little is known about its etiology and clinical course. The case of a 3-week-old girl in whom rapidly progressive cardiac insufficiency occurred after a completely normal postnatal development is described. Selective ventricular angiography revealed ventricular malfunction and an akinetic area occupying 57% of the basal free left ventricular wall. The child died in acute cardiac failure. Autopsy disclosed a basal fibrous aneurysm which extended to 34% of the free ventricular wall. There was no indication of a coronary or inflammatory etiology, so a focal disorder of myocardial architecture was presumed. Congenital bulging of the ventricular wall can be divided into broad-based aneurysms and diverticula, the latter being connected with the ventricle by a small orificium. A further distinction can be made on the basis of localization and histology. A reclassification of the congenital bulging of the left ventricular wall presented in the literature showed that apical muscular diverticula are the most frequent, whereas only four cases of fibrous aneurysms were reported. Differences were found, with regard to the distribution of age and race, the clinical course, and the possible etiology.
