Introduction/Objectives: If considering that experiential knowledge of people living with chronic conditions is one of the foundations of therapeutic education, the consolidation of partnerships requires a learning process and an evolution in the practice of the various players. This article presents the intervention approach of the regional patient education resource center – IdF in supporting the development of cooperative practices between health care professionals and patients. Contributions/Description: From a description of the co-construction of two multi-partner projects, we will explore the elements facilitating the commitment of partners and building solutions corresponding to patient needs. We emphasize the importance of co-designing projects, determining common values and principles collective governance, facilitating dialogue for practice analysis, tools for promoting cooperation, and participatory and formative evaluation. Discussion/Conclusion: The function of third-party "terceisation" of the Pôle ETP and the central role of the "patient perspective" are two elements to be leveraged in the co-construction process. Beyond the resulting material solutions, the immaterial effects include the empowerment of participants, on an individual level, but also potentially collectively and at an organizational level.
