The goal of this phenomenological study was to document the practice of music therapy in adolescent mental health, more specifically through brief group therapy sessions involving this age group. Despite proven needs in this population and the promising effects of music therapy as confirmed by mental health research, the Canadian literature contains scant documentation about this specific context. This study, in the form of semi-structured interviews, sought to learn about the professional experience of three Canadian music therapists who work primarily with adolescents who have mental health issues. An analysis of their responses revealed three main themes: (a) the primary goals in this type of context, (b) the program structures generally established, and (c) the professional skills required. While specific approaches are required for groups of adolescents who present with psychiatric and psychosocial problems, each music therapist uses their own specific work methods, which can therefore vary greatly from one practitioner to the next. The study highlights, in particular, the importance of community music therapy, as well as the need to value individuality--each person's unique story--in the therapeutic process. For several years, there has been a disinvestment of spaces of subjectivation and symbolization within the Quebec health system, in favor of a movement of overmedicalization and biologization of psychic distress. Because of the political and administrative policies that guide institutions, including the implementation of clinical restraints based on efficiency and "performance," creativity seems to be abandoned by both caregivers and patients. Thus, our aim, in this writing, is to emphasize the relevance and define certain functions of a group of therapy by artistic mediation, through the study of the therapeutic process of psychiatric patients with traumatic suffering. The process and some artistic productions of two subjects who participated in this therapeutic modality, which is rare in Quebec, are presented and analyzed by the co-authors. Then the effects of such a group, mainly reflexivity, containment, and restarting symbolization (Roussillon), are raised and detailed. [ABSTRACT FROM AUTHOR]

In 1997, a music therapy program was first piloted in a community mental health clubhouse and six months later expanded to five community mental health group homes, all administrated by an accredited agency in the Lower Mainland of British Columbia, Canada. This agency serves persons with a diagnosis of chronic and persistent mental illness and adheres to a consumer-initiated model of practice. In an article published in the Canadian Journal of Music Therapy, Baines (2000) described the predominantly song-based approach of that program and documented a consumer evaluation of it, concluding that the client-centred empowerment model of consumer inclusiveness described appeared to contribute significantly to the rehabilitative effect of the program. In this article, the authors report on the current status of the program, including new data from surveys of consumers and staff as well as in-depth consumer interviews regarding their awareness of how and why the consumer-initiated, song-based music therapy program is effective for them. By resorting to an inter- and transdisciplinary approach, especially developed in the field of bioethics, the present article explores the possibilities for a critical "dialogue " between the contemporary clinical practice and the phenomenology of revolt and its pathological forms, formulated in the work of Albert Camus. The reading of the clinical case ofTheo through the lens of the 1957 Nobel Price for Literature's philosophical writings, in particular, L'Homme révolté (Paris, 1951), allows to challenge some of the current therapeutical approaches - from neuropsychological to systemic and psychodynamic psychotherapy - in the light of Camus's analysis of the contemporary psychopathology of revolt. Résumé À partir de la présentation d’une situation clinique d’adoption, ce travail décrit le fonctionnement d’une consultation groupale d’adoption internationale. Les questionnements qui y sont déposés par une famille sont analysés à plusieurs niveaux : à un niveau collectif, ils interrogent les représentations parentales des appartenances culturelles de l’enfant ; à un niveau intersubjectif, ils sont révélateurs de la dynamique familiale et plus particulièrement de la problématique de couple ; à un niveau individuel, ils s’inscrivent dans la problématique intrapsychique respective des parents. Ce travail relate ainsi, comment la question de l’altérité de l’enfant, vient se déployer sur plusieurs scènes à la fois. Elle n’est ainsi jamais à comprendre au premier degré, mais nécessite de multiples niveaux de lecture, du plus singulier au collectif. This article explores the dynamic of a therapeutic group setting addressed to adopting families. On the basis of a case study, the problematics presented by the adopting family are analyzed in reference to the collective, intersubjective and individual perspectives. From a collective perspective, the problematics question the parental representations of their child's cultural belonging. From an intersubjective perspective, they shed light on the family's dynamic, notably the couple's difficulties. From an individual perspective, the problematics shed light on intrapsychic dynamics of each parent. Finally, the case study highlights the multiple scenes of the child's otherness display that necessitate a multidimensional analysis. Systemic, psychoanalytical and transcultural perspectives are needed to better understand the complex intertwining of the collective, intersubjective and individual dynamics underpinning the suffering in an adopting family. Résumé But de l’étude Les dépressions postnatales (DPN) concernent 10 à 15 % des accouchées. Le groupe « dépression postnatale » est un outil de prise en charge mis en place dans l’unité de périnatalité du service de pédopsychiatrie du CHU de Reims. L’objectif de l’étude est d’évaluer le ressenti de l’expérience groupale chez les mères ayant présenté une DPN, afin d’en évaluer les résultats et d’en améliorer sa délivrance et sa qualité. Patients et méthode La population étudiée est de 9 mères ayant participé au groupe DPN ces dernières années. Il s’agit d’une étude qualitative basée sur un entretien semi-dirigé avec questionnaire. Résultats L’ensemble des mères interrogées a été très satisfait de la prise en charge groupale avec un ressenti global « très positif ». L’amélioration symptomatique est marquée notamment par une déculpabilisation, une resocialisation et une renarcissisation. Les résultats soulignent l’importance du dialogue au sein du couple, permettant une amélioration relationnelle avec le conjoint. La psychothérapie groupale permet d’élaborer autour d’un vécu émotionnellement intense avec la construction progressive d’un récit dans un espace contenant et étayant où la mise en mots est possible. Conclusion Tenant compte des caractéristiques et limites méthodologiques de l’étude (faible panel d’étude, entretien réalisé dans des délais différents), les résultats de cette étude préliminaire sont encourageants et mériteraient d’être reconduits à plus large échelle. La prise en compte de la souffrance paternelle reste à développer. Les bénéfices de l’abord groupal se confirment et une généralisation de cette prise en charge singulière nous semble pertinente. Introduction Postnatal depression (PND) affects 10 to 15 % of the women who gave birth. The “postnatal depression group” is a group therapy set up in the parents-infant unit of the child psychiatry department of the university hospital of Reims city in France. The purpose of this paper is to evaluate how the women who suffered from postnatal depression feel about this group therapy in order to improve its quality and broaden its use. Method It is a qualitative study based on a semi-directed interview with a survey. The study sample is a population of 9 mothers who participated in the PND group in the last six years. Results All the mothers we interviewed were very satisfied with the PND group and considered their experience as “very positive”. They felt that the group therapy helped them to feel free of their guilt, to resocialize and to improve their self-image (renarcissization). The results show that talking with their husbands improved the couple's relationship, which was an important part of the process. Group therapy enabled patients to speak about their feelings in a space where they felt safe and supported. Conclusion We are aware that this study is based on a limited number of women and that some interviews were conducted several years after their participation in the PND group, but the results are so encouraging that we think this method should be broadened. For example, it could also be used on fathers. The benefits of the group therapy are real and it could be applied to various psychological problems. [ABSTRACT FROM AUTHOR]

The natural disaster catches the psychologist by surprise and plunges her into the heart of distress, hers and that of thousands of others around her. Can she intervene when she herself is affected by trauma? This article recounts one experience of group intervention in Haiti's 2010 post-earthquake environment. The treatment was carried out by two psychologists who lived through the earthquake and the internal trauma. The responsibility of acting as a professional is a major challenge as well as a powerful driver of resilience. The strong support coming from the group makes group treatment a preferred intervention strategy. Working as co-therapists throughout the group treatment ensures support during this process, which can prove difficult for therapists who have experienced the trauma. The clinical experience cannot be disassociated from one's personal experience in such a context. It embodies a reflection on the role of the local psychologist in a disaster setting and highlights the factors which can strengthen the effectiveness of this type of intervention. Abstract: The questions bound to the care of the authors of sexual offenses are complex and question the intrication between the logics of care and control. The aspects of disease and recurrence are in the center of the therapeutic accompaniment of these patients outside the common fields of the mental health. From these questionings, the article presents an original model of care associating social meeting, individual therapy and group therapy. The analytical and systematical references want to be complementary, meeting the subject on the one hand in its conflicts intrapsychic and on the other hand in its relational sufferings with its pairs and its family. The group therapy is the particular development by showing the interest of systemic epistemology in this specific private clinic. We stress also the history of the first bonds and the process of setting in account distinguishing the act from the circumstances from the act. Abstract: Objectives: This article develops the psychoanalytical theories of trauma in Freud''s work and that of other psychoanalysts. The author will show the place of traumatism prior to language acquisition and its specificities. We will propose a method of treatment, group therapy. Freud''s theory: Usually traumatism designates a psychological impact of events, which are painful for a person and his life. This term is over-used in common language. Actually in France “trauma” usually results in immediate intervention of psychological treatments. In psychoanalytical theory, the notion of trauma is at the heart of Freud''s work. This notion has varied greatly from 1905 to 1931. Many psychoanalysts work on the place of trauma in self-development. But in this article, we will examine the work of only two other authors: Ferenczi and Winnicott. These psychoanalysts work on “narcissic damage”. This problem corresponds to the absence of an adequate response from the object to a stressful situation and constitutes a mutilation of the self. The conception of trauma has many meanings: the first theory is that of the overwhelming of the defense mechanism of the subject and the other is repressed memory in relationship to theories of sexuality. The second theory is that trauma interrupts the self-continuity. In this situation, the subject clings to perceptive sensations. There is an interruption in the cycle of representation. It is not possible to use the representation mechanism. Actually many children who turn to child psychiatrists have pasts filled with traumas. There have not constructed transitional areas, which would permit them to create psychological spaces differentiating the imaginary from the real. For other children, however, the notion of trauma is difficult to recognize in their own lives. Clinical case: Here, the author will present an observation of a child who had a trauma at the age of 8 months. His trauma was very precocious, occurring at a time prior to the child''s language acquisitions and he didn’t have the skills to express his emotion. In therapy groups, he is able to remember the sound of the American ambulance siren, and with this sound, he expresses this psychological disorganization with great anxiety. It is probably the only memory he has been able to keep of this extremely traumatic experience. This analysis will show that the only memory of the child is an auditory one. Conclusion: The author concludes with the interest of group''s therapies permitting the emotional expression through play rather than through language. When there are group therapies, the children are going to have a certain excitation, and they feed off of each other; this group dynamic helps to transform the excitation into representation. The group therapies enable the children to reconstruct their pasts and repress the trauma. This therapy results in an associative representation: the children live emotional experiences and they translate these emotions into images (figurability). Then they arrive at a representation with a scenario and exceptionally at verbalization. [Copyright &y& Elsevier]

Abstract: Obsessive-compulsive disorder (OCD) is a chronic, recurrent, and often disabling disorder. Comorbid emotional and personality disorder are common and can serve to complicate both course of illness and treatment outcome. Controlled trials with adults have shown cognitive-behavior therapy (CBT) that includes exposure and response prevention (ERP) to be highly effective for treating the disorder. It means exposing the patient to the very thing that causes fear and anxiety, and using all his strength to simply accept the experience instead of relying on compulsions to counteract it. The basic concept of CBT is: by changing the behavior, the patient eventually changes how he thinks and feels. CBT usually works fairly quickly, and it relies mostly on action, not insight. In this article we describe a cognitive-behavioral group treatment for both OCD symptoms and comorbid personality disorder. By an evaluable patient analysis; statistically and clinically significant pretreatment to posttreatment change occurred in OCD diagnostic status and severity. Some personality traits also changed. The results of this study, the first to evaluate this dimension of the personality disorders comorbidities in the OCD and their evolution with a behavioural and cognitive psychotherapy, are very encouraging. Indeed, evolution of the fields and facets of the personality changed positively. [Copyright &y& Elsevier]

Abstract: Reporting on the clinical characteristics of adolescent sex offenders treated in a specialised outpatient unit of the Parisian region, the authors describe the clinical and psychodynamic profiles of this population. They develop a theoretical model based on the notion that sexual offences in minor adolescents are produced by a conjunction of three processes: 1) difficulties in integrating sensations due to puberty; 2) problems in modulating emotions and 3) dysfunctions in the normal ability for action. The authors develop a treatment model associating group and individual therapy, together with a thourough netting of the family and social context. The group focuses on a common creative activity (the making of a movie for instance) leading the adolescents to interact and team work, and individual sessions are focused on the expression of their thoughts and feelings. The main objectives are to help the adolescents in overcoming their inhibitions of action, better monitoring their emotions and developing a better sense of self. Abstract: Objective. – In the long term, people with traumatic brain injury (TBI) present with communication disorders impairing their professional and social integration. The aim of this study was to evaluate the feasibility of group therapy on communication disorders of people with TBI in the post-hospital period. Materials and method. – Four patients were followed in the medico-social department "Auprès-TC" and took part in various day programs. Two speech therapists participated for a 7-month period using group therapy to take into account communication disorders. The patients and families were evaluated at the beginning and end of care, by use of the Lille Communication Test, the Bordeaux Scale of Verbal Communication, the Revised Neurobehavioural Rating Scale, and part of the European document EBIS. Results. – Despite the delay since TBI, patients showed quantitative and qualitative evolution of communicative abilities. Discussion-conclusion. – The influence of group therapy on the improvement of communication in patients with TBI should be reconsidered, because in this study, familial and institutional environment influenced the communication. Group therapy allows for communication care from an ecological point of view, by enhancing interactions. [Copyright &y& Elsevier]

International audience; Résumé L’article concerne l’apport des groupes thérapeutiques dans la prise en charge des patients souffrant de douleurs chroniques. Il s’appuie sur une étude qualitative fondée sur des entretiens semi-directifs auprès de patients et de soignants ainsi que sur des observations. L’étude démontre que la participation des patients douloureux à des groupes thérapeutiques améliore leur qualité de vie et cela même quand les douleurs sont récalcitrantes. Les sentiments de dévalorisation et de culpabilité souvent associés à la douleur chronique ont tendance à s’apaiser et à laisser la place à des appréciations plus positives, permettant notamment une amélioration de la qualité des relations sociales. Summary The article concerns the benefit of group therapy as a treatment for patients suffering from chronic pain. It is based on a qualitative study in which semi-directive interviews are made with patients and nursing staff. The study is also founded on observations that were made. It shows that suffering patients, who participate in group therapy, see an improvement in the quality of their lives, even in cases of resistant pain. Feelings of guilt and self-depreciation, often associated with chronic pain, tend to diminish. A more positive outlook replaces these negative feelings and this allows for the marked improvement of the quality of social relations.

A support group brings together several people around a common theme. It aims to favour the expression of experiences, suffering, conflicts, problems as well as discussions about potential improvements or solutions. - Abstract
Cognitive behavior therapy (CBT) is recommended for the treatment of first-episode psychosis (FEP) patients. It can be offered in acute state or during the remission of the episode. Up to date, effects of CBT have been examined in several controlled and randomized trials in FEP. Combined with antipsychotic medications, results have demonstrated that CBT decreases positive psychotic symptoms, enhances quality of live, self-esteem and insight, and diminishes the intensity of comorbide symptoms, such as trauma or suicide behavior. CBT might be particularly efficient in individuals wishing to reintegrate premorbide functioning and those with low duration of untreated psychosis. Despite these encouraging results, psychosis orientated CBT are underused in France. The validation of group CBT specifically designed for FEP should enhance the range of patients receiving this treatment. Moreover, early CBT interventions for people at ultra high-risk for psychosis and assertive community treatments should be developed., (Copyright © 2013 L’Encéphale.
