Compressive goitre is a public health emergency due to the risk of asphyxia caused by compression of bronchial tree. We report the case of a 48-year-old female patient presenting to the emergency department with laryngeal dyspnea due to compressive goitre. We conducted a study and a literature review focusing on the clinical and radiological features of compressive goitre and different treatment options. The management of a patient with compressive goitre is difficult and must be rapid, due to the high risk of asphyxia. It requires a multidisciplinary approach of an experienced surgical team, including ENT and thoracic surgeons. [ABSTRACT FROM AUTHOR]

INTRODUCTION: The aim of this retrospective study is to review patients with malignant goiter, as regards their nature, demographic characterization, clinical presentation and preoperative histopathological data. METHODS: the study focused on a period of 4 years from December 2015 to January 2019. Patients´ demographic data, clinical presentation, Intra-operative findings, Pre and postoperative histopathological staging and grading were all recorded. Postoperative follow up whether early or late were also included. RESULTS: a total of 100% (n = 65) patients underwent surgery. The female to male ratio was found to be 5:1 (48 females and 17 males). Solitary nodule was the main clinical presentation in 80% (n = 52) of cases while 20% (n = 13) were multinodular swellings. Papillary carcinoma was recorded in 80% (n = 52) of patients while in 15.4 % (n =10) of patients were having follicular carcinoma. The remaining 4.6% (n = 3) suffered of lymphoma; no medullary or anaplastic tumors were reported. CONCLUSION: Thyroid cancer is the most commonly encountered endocrinal malignancy at our institute. Fine Needle Aspiration Cytology (FNAC) showed a high percentage of reliability in diagnosing thyroid cancer among our series. It is recommended to adapt this technique in initial screening of goiter patients in our local setting.

Introduction: Surgical campaigns for thyroid surgery in low-income environments are very efficient, but there is little literature reporting results. These campaigns are complex due to multiple particularities: highly evolved cases, the need for professionals to travel or an obvious socio-cultural barrier influence towards the surgical act. We describe a surgical campaign in Cameroon to treat patients with goiter and issue some medical and sociocultural recommendations in view of our experience for its implementation with guarantees., Material and Methods: An experienced group carried out an 11-day campaign at the Saint Martin de Porres Dominican Hospital, Yaounde, Cameroon. Demographic data, TSH values, surgery and complications after a 12-month follow-up were analyzed., Results: Thirty-eight patients with goiter were selected for the campaign and 32 patients (mean age, 40-years-old; 30 females) were operated. Bilateral goiter, as assessed with echography, was diagnosed in 13 patients (41%). Ten patients (31%) had a WHO grade II goiter (visible with the neck in a normal position). The surgical procedures were 18 unilateral thyroidectomy with isthmectomie, 13 total thyroidectomy, and 1 totalizing thyroidectomy, due to previous unilateral thyroidectomy (cancer recurrence). A pathological study in 13 patients (40%, extra cost 60 €) showed benign multinodular goiter/thyroid nodule (12 patients) and an extensive papillary carcinoma (one patient). Six months postoperatively, 3 patients had a slight dysphonia and one patient had persistent hypocalcemia. Follow-up was completed in all patients, either face to face (75%, 24 patients) or by phone (25%, 8 patients who failed to have a TSH test because of its cost, 23 €)., Conclusions: Surgical campaigns to treat thyroid pathology can be carried out with guarantees if a series of important steps are followed: active participation of the patient's environment, thyroid ultrasound by the surgical team to decide which technique, intense awareness about monitoring and hormone replacement therapy, and the participation of local personnel for long-term follow-up., Competing Interests: Les auteurs déclarent qu'ils n'ont aucun conflit d'intérêts., (Copyright © 2024 SFMTSI.)

Clinical manifestations and visceral involvements described in patients with thyroid disease are many and varied. However, cases of patients with hepatic manifestations have been rarely reported in the literature. We here report the case of a 50 year old patient diagnosed with Graves' disease without a goiter or ocular manifestations. Follow-up laboratory testing led to the detection of anicteric cholestasis. Investigations of the potential causes were negative and given the normalization of the liver parameters after appropriate treatment of hyperthyroidism, the diagnosis of hepatic involvement secondary to Graves' disease was retained. Одной из важнейших задач XXI века. остается ликвидация йододефицитных заболеваний, которыми болеют около 30% населения планеты (по данным ВООЗ). Деятельность исследователей Черновицкого государственного медицинского института в течение 1940-1950-х гг. в деле ликвидации эпидемии эндемического зоба в Черновицкой области в условиях послевоенной разрухи, голода и антисанитарии имеет далеко не местный характер. Теоретические разработки наших ученых, тщательное изучение местной ситуации, схема практического решения проблемы в районах эпидемии требуют изучения и применения в более широких масштабах. Participation of Chernivtsi state medical institute in the elimination of focal goitre endemic in the Chernivtsi region during the period 1945-1949 years. One of the biggest challenges of the 21st century is the elimination of iodine deficiency disorders which about 30% of the world's population suffers from (according to WHO). The scientists of Chernivtsi State Medical Institute provided activity in eradicating the epidemic of endemic goiter in Chernivtsi region during the 1940s-1950s in conditions of postwar devastation, hunger and poor sanitation. This experience could be globally used. Theoretical development made by our scientists, their careful study of the local situation, the scheme of a practical solution to the problem in the epidemic areas require further study and application on a wider scale. Given this situation article considers such issues that are particularly relevant to the research methods, ways and forms of elimination of endemic goiter in certain regions of our country. The problem of iodine deficiency was first considered at the state level in Ukraine in the 1940. At the time of the Soviet Union in Ukraine to regions of maximum risk of IDD traditionally according Lviv, Chernivtsi, Ivano-Frankivsk, Zakarpattia, Ternopil and Volyn regions. In order to prevent the disease in endemic areas was introduced a set of preventive and therapeutic measures. Despite the fact that in the 1960s -1980s endemic goiter problem was almost solved in the 1990s. Due to the weakening of attention to this important medical and social problems as well as man-made pollution by the antagonists of the iodine observed increase in the number of regions where fixed iodine deficiency endemic goiter and features. The increasing prevalence of thyroid pathology was also contributed to Chernobyl tragedy. A large amount of archival documents reveals to us the real view of goiter endemic liquidation in Chernivtsi region and the role of employees of Bukovinian state medical institute in that process. The active dissemination of goiter disease coincided with the establishment of the Chernivtsi State Medical Institute, which has been restoring facilities once in a while and worked to develop a clinical component and medical researches. Scientific research of BSMI scientists formed the basis of prevention methods and treatment of health edge, which has been contributed to the recognition of Chernivtsi Medical Institute around the world, this medical establishment became a research center in this field. Thyroid lymphomas are rare. They may be primitive, representing less than 5% of thyroid malignancies and developing in most cases on a pre-existing thyroid disease including Hashimoto's thyroiditis, or constituting a secondary localization of disseminated lymphoma. This is an aggressive disease, in the form of painful goiter, quick installation with signs of compression. The majority are non-Hodgkin lymphoma type B. We report a case of primitive thyroid lymphoma diagnosed in a patient admitted to emergency with respiratory distress on a massive goiter appeared for 4 months, without pathological antecedents. The histological diagnosis was made by a biopsy made during tracheotomy. The treatment consisted of chemotherapy alone. The outcome was favorable. We discuss, after review of the literature, diagnostic difficulties and therapeutic modalities, while specifying the place of surgery. [ABSTRACT FROM AUTHOR]

Résumé: Nous rapportons le cas d’un adolescent présentant une dyspnée liée à un volumineux goitre à extension intrathoracique, dans le cadre d’une hypothyroïdie due à un trouble de l’hormonosynthèse. Le patient s’était présenté à l’unité d’accueil des urgences en raison d’une sensation de dyspnée, accompagnée au domicile d’un stridor aux 2 temps, sans signe de détresse respiratoire. Le cou était déformé par un volumineux goitre. L’adolescent reconnaissait avoir interrompu le traitement par l-thyroxine. Les radiographies du thorax et du cou révélaient une compression trachéale ; le scanner cervicothoracique quantifiait une diminution de calibre de la trachée de 60 %. Il n’y a, à notre connaissance, qu’un seul cas publié de goitre compressif lié à une hypothyroïdie congénitale. Cependant, le goitre n’est pas une affection si rare chez l’enfant : sa prévalence est estimée entre 2 et 4 % en France. Les données de la littérature chez l’adulte montrent qu’en cas de goitre compressif, la dyspnée est beaucoup plus fréquente que la détresse respiratoire même en cas de compression importante de la trachée. L’absence de signe objectif de détresse respiratoire n’exclut donc pas qu’une dyspnée soit due à une compression anatomique et l’examen clinique ne doit pas omettre la palpation du cou à la recherche d’un goitre. Objective: To report on a case of rare giant anterior cervical lipoma., Patients and Methods: This was a 60-year-old male patient received in March 2020 for an anterior cervical swelling in progressive evolution since 20 years. The condition was negatively impacting the patient's quality of life with a feeling of cervical heaviness, discomfort, head movement limitations, with no sign of compression. Despite this significant discomfort, the patient first consulted several traditional practitioners with different traditional treatments without success, the mass having been taken for a goiter. It is in the face of the failure of traditherapeuts that the patient finally decided to consult in our care structure., Results: Physical examination found a large left paramedian formation which appeared soft, mobile vis-a-vis the two plans, and sensitive; it measured 13 cm on its longer axis, and surrounding skin showed scarifications, witness to previous traditional treatments. Cervical CT eliminated goiter and made it possible to diagnose a giant cervical lipoma. A cervicotomy was performed under general anesthesia with a total one-block excision of an encapsulated mass. The evolution was favorable with a good healing. The histological examination of the mass identified a well-differentiated lipoma. There was no relapse up until 24 months later., Conclusion: The giant anterior cervical lipoma is rare. The differential diagnosis is mainly posed with a large goiter or liposarcoma. CT or MRI make it possible to confirm the diagnosis. Its treatment is exclusively surgical and the examination of the operative piece by the pathologist confirms the diagnosis. The article examines the histological features and hormonal traits of goitrous and normal camels in south of Tunisia. It notes that the goitrous colloidals found in the animals present a structural change of the vesicles with thyroid follicles. It mentions the densification of the fibrous thyroid stroma of the camels as depicted in the estimated percentage of interstitium. It also emphasizes the significant decline of concentrations of thyroid stimulating hormone (TSH) and serum in goitrous animal when compared to normal ones.

Abstract: Struma ovarii, a rare form of ovarian tumor, represents less than 3% of ovarian teratomas. The peak frequency occurs during the fifth decade of life. Diagnosis is usually a surprise of pelvic ultrasound or histological analysis. Surgery is the main mode of treatment and prognosis is excellent. Struma ovarii can be associated with thyroid biology abnormalities, or exceptionally become malignant, and thus managed as a thyroid cancer. [Copyright &y& Elsevier]

Abstract: Fetal goiter is a rare occurrence of which neonatal consequences are not always predictable. Concerning three cases of goiters associated with hypothyroidism discovered in utero, the authors describe the way to take care of in this bad codified situation. They insist upon the major role of ultrasound for goiter diagnosis and its impacts and for control of treatment efficiency. They also discuss intra amniotic L-Thyroxine® injection and insist upon the necessity to obtain quick and definite thyroid evaluation after birth before decision to abstain from neonatal therapy. Abstract: Aim of the study. – To demonstrate the feasibility of endoscopic thyroidectomy for unilateral lobectomy and isthmectomy. Materiel and methods. – This prospective study included 40 patients operated on between January 1999 and March 2005 by a total endoscopic approach. Only lobectomies and isthmectomies were performed for nodules inferior to 3 cm in diameter. Results. – Forty patients (36 females, 4 males) underwent 35 lobectomies and five isthmectomies. Twenty-four patients were operated on without ultrasonic shears (US) the rate of conversion in this group was 33%. Sixtine patients were operated on with US: the rate of conversion was 0%. In the second group, the operative time was decreasing to the half, range 45 to 90 minutes. In both the two groups, there were no morbidity: no extensive emphysema, no hematoma, no wound abscess, no cord vocal palsy. The median hospital stay was 1,75 days. After three months of follow up, all the patients were satisfied, especially concerning the cosmetic results and the short recovery time. Conclusions. – Endoscopic thyroidectomy is feasible and safe for performing lobectomies and isthmectomies. In the near future, it could be extended to bilateral goitres in selected patients. Aim of the study: Total thyroidectomy has been advocated for the treatment of multinodular nontoxic and benign goiter. The aim of this study, based on our experience, was to define the surgical factors which permit to decrease morbidity related to total thyroidectomy for multinodular euthyroid benign goiter.Methods and materials: In a retrospective study performed between January 1996 and September 2000, all records of total thyroidectomy for initial treatment of multinodular euthyroid benign goiter were reviewed. This study allowed to specify recurrent and parathyroid morbidity after surgery.Results: There were 51 women and 13 men with a mean age of 47 years. Recurrent laryngeal nerve injury occured in 2 patients. It resolved in 1 patient but was permanent in another (1.6%). Transient hypocalcemia was found in 8 patients (12.5%). One patient had permanent hypocalcemia (1.6%).Conclusion: The results of our serie are comparable to previous reports. Systematic identification of the recurrent laryngeal nerve, and preservation of the parathyroid blood supply permit to decrease the surgical morbidity. [Copyright &y& Elsevier]

Cowden’s disease is an autosomal dominantly inherited syndrome characterized by mucocutaneous lesions and multiple hamartomas. We report here a 12 years-old boy case with craniomegaly, intestinal polyps, epilepsy and multiadenomatous goiter. All the lesions were begnins. The predisposing genetic defect has been assignated to chromosomal 10 (PTEN-gene mutation). A long term follow-up is necessary because of the risk of malignancies. [Copyright &y& Elsevier]

We report a rare case of thyrolipoma-thyrolipomatosis' association. A 48-year-old patient, with a past history of high blood pressure and chronic renal failure secondary to AA amyloidosis, presented with a midline neck swelling increasing in size since one year. Ultrasound showed a multinodular goiter, the worst nodule was left-lobar and categorized as EU-TIRADS 4. Grossly, cut section of the thyroïd specimen showed a yellow-tan left-lobar nodule measuring 2.5cm, the surrounding tissue had a diffuse yellow-brown appearance with scattered whitish areas. Histopathologic examination revealed that the nodule corresponded to a follicular adenoma.Thyroid fat infiltration was noticed both within the nodule and in the neighboring parenchyma. Thus, the diagnosis of an association thyrolipoma-thyrolipomatosis was rendered. The association of thyrolipoma-thyrolipomatosis is a very rare occurrence of unclear pathogenesis. To the best of our knowledge, this report describes the second case in the literature. Abstract: We describe a 62-year-old woman with slowly growing usual nodular goitre in whom diffuse giant cell arteritis (GCA) of the thyroid arteries was found upon thyroidectomy, revealing otherwise unsuspected biopsy-proven temporal arteritis. To our knowledge, this association had been previously reported in only three instances. In each case, GCA of the thyroid arteries appeared clinically silent, did not produce significant glandular dysfunction, and was uncovered thanks to a planned thyroidectomy for nodular goitre. These observations highlight that thyroid artery involvement by GCA, even widespread, as in our patient, may be overlooked clinically and may produce little or no thyroid dysfunction.
