Résumé Le choix de l’annexectomie homolatérale en cas de tumeur limite unilatérale doit répondre à la fois à un objectif carcinologique (limiter le risque de récidive), de fertilité et s’intégrer dans la prise en charge globale d’un couple en cas de désir de grossesse. L’annexectomie unilatérale est une option légitime en cas de désir de grossesse par rapport au risque de récidive et de survie. L’identification d’un groupe à haut risque de récidive est nécessaire. L’annexectomie unilatérale est une option envisageable afin de préserver la fertilité en cas de tumeur unilatérale bien que les données sur la réserve ovarienne sont manquantes. L’annexectomie unilatérale doit être envisagé en situation d’infertilité. Une fécondation in vitro, si elle est indiquée, est possible en particulier en l’absence de facteur de mauvais pronostic après annexectomie. La place de la cryopréservation de tissu ovarien ou de la maturation in vitro reste à définir. Abstract: Ovarian low malignant potential tumor account for 10 to 20 percent of ovarian epithelial tumors. They differ from typical ovarian cancers in that they do not grow into the ovarian stroma. Likewise, if they spread outside the ovary, for example, into the abdominal cavity, they do not usually grow into the lining of the abdomen. These cancers tend to affect women at a younger age than the typical ovarian cancers and are less life-threatening than most ovarian cancers. Guidelines for surgical treatment of borderline ovarian tumors are similar to those for ovarian cancer and include hysterectomy with bilateral salpingo-oophorectomy. However, patients with borderline ovarian tumors tend to be younger than women with invasive ovarian cancer. For many of these patients, fertility is an important issue. Previous studies have suggested the safety of conservative surgery with unilateral salpingo-oophorectomy or cystectomy for patients with stage I borderline ovarian tumors. Despite infrequent data, this observation has been expanded to include women with advanced-stage disease. Recurrence is noted more often after this type of treatment, but does not seem to have a negative effect on survival. Management of conservative treatment (complete staging, cystectomy or oophorectomy, oophorectomy or adnexectomy) are still under debate since none avoids the malignant transformation risk. Thus, close follow-up is mandatory and the optimal moment for final oophorectomy remains unclear. When ovarian preservation is impossible, oocyte/ovarian cryopreservation or emergency ovarian induction before the surgical procedure to obtain embryos are promising but still under evaluated options. Objectives: The aim of this study was first to describe the adnexal masses diagnoses and their management and secondly to assess the oncological relevance of these strategies. Patients and methods: A prospective multicentric observational study of organic adnexal masses was conducted between June and November 2005. All patients presenting an organic adnexal tumor and for which a definitive histological diagnosis was subsequently available were eligible. Baseline characteristics, mode of discovery, preoperative assessment, peroperative findings, surgical treatment and pathological findings were collected. Results: Among the 278 patients treated for an adnexal mass during the study-period, 166 were included. Mean age was 42.8 years with a 25.3% menopause rate. The radiological assessment comprised an ultrasound examination in 98.8% of cases and an MRI in about one-fourth. The CA 125 marker was measured in half the patients and the carcinoembryonic antigen (CEA) marker in 19.9%. In all, 83.1% of tumors were found benign, 12.7% were malignant and 4.2% were borderlines on definitive histological examination. The surgical procedure comprised a cystectomy in 88 cases and an adnexectomy in 64 cases. Among those treated by cystectomy, one borderline tumor was found, whereas in those treated by adnexectomy, five borderline and 12 malignant masses were discovered. Peroperative conversion rates were 16% and 50% in borderline and malignant tumors. Accidental rupture of the cyst occurred in 29% of cases. A minimal preoperative assessment was defined, based upon the dimensions of the tumor and the dosage of at least one marker. These minimal criteria were met in only 28% of initial assessments. Discussion and conclusion: Ovarian borderline tumors and carcinomas are an important contingent of this study (16.9%). Twenty percent of them are diagnosed in an emergency situation. The management observed in this study is adapted to benign lesions. On the other hand, the rate of inappropriate procedures as well as incomplete staging in case of borderline or carcinomas is quite high. [Copyright &y& Elsevier]

Introduction: Rare ovarian tumors include complex borderline ovarian tumors, sex-cord tumors, germ cell tumors, and rare epithelial tumors. Indications and modalities of fertility preservation, infertility management and contraindications for hormonal contraception or menopause hormone therapy are frequent issues in clinical practice. A panel of experts from the French national network dedicated to rare gynaecological cancers, and of experts in reproductive medicine and gynaecology have worked on guidelines about fertility preservation, contraception and menopause hormone therapy in women treated for ovarian rare tumors., Methods: A panel of 39 experts from different specialties contributed to the preparation of the guidelines, following the DELPHI method (formal consensus method). Statements were drafted after a systematic literature review, and then rated through two successive rounds., Results: Thirty-five recommendations were selected, and concerned indications for fertility preservation, contraindications for ovarian stimulation (in the context of fertility preservation or for infertility management), contraceptive options (especially hormonal ones), and menopause hormone therapy for each tumor type. Overall, prudence has been recommended in the case of potentially hormone-sensitive tumors such as sex cord tumors, serous and endometrioid low-grade adenocarcinomas, as well as for high-risk serous borderline ovarian tumors., Discussion: In the context of a scarce literature, a formal consensus method allowed the elaboration of guidelines, which will help clinicians in the management of these patients., (Copyright © 2017 Société Française du Cancer.
