Durant la décennie qui a suivi la publication en 2006 du rapport phare intitulé From cancer patient to cancer survivor: Lost in transition par l’Institute of Medicine (Hewitt, Greenfield et Stovall, 2006), la survie au cancer est peu à peu devenue une phase distincte de l’expérience globale du cancer. Si beaucoup de progrès ont été réalisés pour créer un système de soins qui réponde le mieux possible aux besoins des survivants, on dénote quand même encore la présence d’importantes lacunes. En octobre 2016, un symposium international a été organisé en marge de la conférence de l’Association canadienne des infirmières en oncologie (ACIO/CANO) tenue à Calgary, en Alberta, afin d’examiner et d’explorer les défis mondiaux en matière de soins aux survivants du cancer. Le présent article résume les présentations de ce symposium, qui ont survolé les soins offerts aux survivants du cancer d’un point de vue canadien, américain et international en décrivant leurs défis, leurs enjeux et leurs écueils. Pour débuter, Margaret Fitch, Ph.D., établit les bases pour examiner les soins aux survivants du cancer en présentant un aperçu des difficultés à définir le statut de « survivant », des besoins non comblés, des modèles de soins, et des occasions pour les infirmières en oncologie d’améliorer les soins offerts aux survivants. Les perspectives canadiennes, américaines et internationales sont ensuite abordées, chacune évoquant certaines lacunes de même que les ressources disponibles pour faire avancer la question de la survie. On discute finalement de stratégies individuelles et collectives pour aider les infirmières en oncologie à influencer le futur des soins aux survivants de manière à ce qu’ils soient mieux centrés sur la personne et offerts dans un souci d’équité.

Chaque infirmière est un leader. Plutôt que d’être seulement confiné dans un rôle ou un titre de leadership traditionnel, le leadership infirmier se réalise dans les domaines de la réflexion critique, de l’intervention et de la représentation et se manifeste dans tous les rôles, les milieux et les domaines de la pratique des soins infirmiers. Lorsqu’un leadership infirmier de haute qualité est adopté, des résultats positifs sont démontrés tant pour les patients que les prestataires de soins et le système (Cummings et al., 2010; National Expert Commission, 2012; Wong, Cummings et Ducharme, 2013). Le leadership commence dès les études universitaires et se poursuit pendant toute la carrière d’infirmière. Pour y arriver, l’infirmière doit élargir son champ de vision, et plutôt que de se concentrer uniquement sur l’individu soigné ou le contexte local, avoir une vue d’ensemble de la population et du contexte global (Association des infirmières et infirmiers du Canada, 2009; Institute of Medicine, 2010; International Council of Nurses, 2016; Commission nationale d’experts, 2012).

Every nurse is a leader. Rather than being situated only within a traditional leadership role or title, nursing leadership is about critical thinking, action and advocacy across all roles, practice settings and domains of nursing practice. When high-quality nursing leadership is enacted, positive patient, provider and system outcomes are demonstrated (Cummings et al., 2010; National Expert Commission, 2012; Wong, Cummings & Ducharme, 2013). Leadership begins in undergraduate education and continues throughout one’s career. Nursing leadership in this context is about looking beyond nursing, as a series of scientific acts of caring that can change individual lives, to include lifelong commitment to political action for system change. It requires nurses to lift their gaze from focusing only on individuals to populations, and from the local to global context (Canadian Nurses Association, 2009; Institute of Medicine, 2011; International Council of Nurses, 2016; National Expert Commission, 2012).

Comme les interventions infirmières ont tendance à être complexes et dynamiques, l’évaluation de leur incidence sur les soins et les systèmes de soins est un défi de taille bien connu. Les organisations infirmières qui cherchent à évaluer l’incidence de leurs efforts se trouvent souvent frustrées par le fossé existant entre l’idéal de la recherche en évaluation et les ressources dont elles disposent. Nous allons décrire, dans cet article, un processus à la fois pratique et maniable élaboré en vue de relever un tel défi en matière d’évaluation. Nous avons utilisé une approche d’investigation en trois phases ainsi qu’un modeste financement organisationnel et un apport de temps réaliste de la part de membres bénévoles pour parvenir à une compréhension significative des résultats croisés issus de la mise en œuvre de la Stratégie nationale d’administration de la chimiothérapie de l’ACIO/CANO. Notre expérience nous amène à voir bien du mérite au niveau du processus et des résultats de cette forme d’évaluation ciblée.

Because nursing interventions are typically complex and dynamic, evaluating their impact upon care and care systems is a notoriously daunting challenge. Nursing organizations seeking to evaluate the impact of their efforts are frequently frustrated by the gap between the evaluation research ideal and their available resources. In this paper, we describe a practical and manageable process developed to address such an evaluation challenge. Using a three-step inquiry approach, supported by modest organizational funding and a realistic level of voluntary member time, we were able to generate a meaningful understanding of intersecting outcomes arising from the implementation of CANO/ACIO’s National Strategy for Chemotherapy Administration. On the basis of our experience, we see considerable merit in both process and outcomes of this form of targeted evaluation.

Le thème de la conférence 2017 de l’ACIO/CANO, En route vers le changement : les infirmières en oncologie prennent les devants, a ouvert la voie à la réflexion sur les façons dont les infirmières en oncologie travaillent à transformer le système de soins contre le cancer pour le rendre plus équitable et davantage axé sur la personne, ses besoins et ceux de sa famille. Notre cheminement en tant qu’association et le positionnement des infirmières en oncologie en tant que contributrices clés dans la prestation de soins de qualité au Canada ont été solidement étayés par le leadership exemplaire dont nous avons fait preuve au cours des trois dernières décennies.

The CANO/ACIO 2017 conference theme, The Path to Change: Oncology Nurses Leading the Way, set the stage to consider the ways in which oncology nurses lead to transform our cancer care system to be more equitable, person-focused and responsive to patient and family needs. Our path, as an association, and the positioning of oncology nurses, as key contributors to high-quality cancer care in Canada, has been solidly forged by outstanding oncology nursing leadership over the past three decades.

Every year, in the lead-up to our annual conference, I am in awe of how CANO/ACIO members come together to get things done. It is astounding what takes place behind the scenes to prepare for and host a high-quality conference every year. Yet, there is no fuss, no drama (well, maybe a little bit of drama), and very little hesitation to step up to contribute. Each year we have a collaborative, pragmatic, articulate, wise and enthusiastic community of volunteers coming together with a common purpose—to create a forum for networking, celebrating and sharing the best of oncology nursing practice, education, research and leadership with our peers. From the Recognition of Excellence committee who ensures our peers are appropriately nominated for awards, to the scientific and planning committees, to those presenting at and/or attending the conference (and many more not mentioned), it takes a village of committed oncology nurses to make the conference a success. In every sense and every way, each of these nurses are leaders. And, it seems, we are very good at coming together for some things that are “inward facing” to our profession, such as focusing on our collective professional development and pride.

Radiation oncology registered nurses have post-basic radiation oncology nursing knowledge, skills and judgment.Radiation oncology registered nurses work within an environment that supports quality practice.

L’accès aux soins et leur équité dans l’ensemble du système de soins contre le cancer étant devenus des préoccupations grandissantes, on constate un enthousiasme toujours plus vif pour divers types de postes d’intervenants pivots afin de faciliter la coordination des soins. Bien que l’intention soit digne de louanges, plusieurs stratégies les plus populaires d’implantation risquent d’accentuer les pressions sur le système et de compliquer encore plus le problème de coordination. Les auteures de cet article affirment qu’il est possible de reformuler les motivations sous-tendant le mouvement en faveur de l’aide à la navigation des patients en reconnaissant la valeur du travail du personnel infirmier lorsque ce dernier est déployé de façon optimale pour offrir aux patients du soutien tout au long de leur expérience du système de soins contre le cancer. Cet article incite les infirmières en oncologie du Canada à faire la critique des stratégies d’aide à la navigation, à les remettre en question et à adopter uniquement celles qui correspondent aux importantes réformes nécessaires pour rendre le système de soins contre le cancer si efficace qu’il n’aura plus besoin de faire appel à des intervenants pivots externes.

With increasing concern for equity and access across the cancer care system, we have seen expanding enthusiasm for various forms of designated patient “navigators” to facilitate coordination. While the intention is laudable, many of the popular implementation strategies risk accentuating strain upon the system and further complicating the coordination problem. These authors claim the motivation underlying the navigator movement can be reframed as an emerging recognition of the value of nursing work when it is optimally positioned to support patients, as they experience the cancer care system. This paper calls on Canadian oncology nurses to critically challenge navigation strategies, and adopt only those consistent with the significant reforms required to ensure a cancer care system so effective that external navigators are no longer necessary.

Post-basic oncology nursing education programs assist nurses to acquire the specialized knowledge and competencies required to provide care in cancer settings.Lifelong learning enhances continuing competence and contributes to both high quality patient outcomes and evidence-informed practice.Cancer care settings and organizations play a fundamental part in supporting nurses with professional development by providing equitable and accessible resources to promote lifelong learning. Such resources may include, but are not limited to, the development of mentorship programs and various activities to promote learning (e.g., space, time and resources).

Tous les ans au moment de préparer notre conférence annuelle, je n’en reviens pas de la façon dont les membres de l’ACIO/CANO parviennent ensemble à tout mener à bien. Je suis ébahie par toute l’activité en coulisse qui vise à préparer et à animer annuellement cette excellente conférence. Sans chichis ni mélodrames (mais peut-être un peu de suspense par contre!), les membres se proposent sans trop d’hésitation pour apporter leur contribution. Tous les ans, une armée de bénévoles enthousiastes, brillants, articulés, pragmatiques et coopératifs unissent leurs efforts pour atteindre un objectif commun : créer un espace pour réseauter, pour célébrer avec nos pairs ce qui se fait de mieux dans la pratique, l’éducation, la recherche et l’innovation en oncologie infirmière et pour échanger sur ces sujets. Que ce soit au sein du comité de reconnaissance de l’excellence qui veille à la présentation des candidatures prometteuses pour les prix, ou des comités scientifiques ou de planification, en passant par les conférenciers et les participants à la conférence (sans compter tous les autres), il faut une multitude d’infirmières et infirmiers engagés pour faire de la conférence un succès. Dans tous les sens et de toutes les façons possibles, chacun et chacune d’entre eux est un leader. Il semble que nous ayons du talent pour faire ensemble des choses qui concernent directement notre profession, « entre nous », comme nous concentrer sur notre fierté et notre perfectionnement professionnels collectifs.

Au cours des derniers mois, plusieurs événements historiques ont retenu notre attention :Un nouveau gouvernement canadien qui met l’accent sur la santé en promettant d’améliorer l’accès aux soins à domicile et aux services de santé mentale, de mettre sur pied un plan national d’assurance-médicaments et d’élaborer une stratégie pour un vieillissement en santé.De nouvelles dispositions législatives fédérales qui accordent aux Canadiens le droit d’opter pour un suicide médicalement assisté, bien que des inquiétudes persistent à l’égard de notre système de soins palliatifs, lequel serait inadéquat et marginaliserait davantage certaines personnes déjà vulnérables.Un regain d’attention envers les recommandations faites par la Commission de vérité et réconciliation du Canada (2015). Celles-ci soulignent les disparités sociales et en matière de santé, étroitement interreliées, qui touchent les peuples autochtones, et les facteurs qui ont mené à leur vulnérabilité, tels la montée du racisme, la colonisation et la pauvreté, pour en nommer quelques-uns.Une rencontre entre les ministres de la Santé pour envisager et élaborer un plan permettant d’abolir les structures politiques traditionnelles qui freinent la santé optimale des Canadiens.

Over the past few months, there have been a number of important historical events that have drawn our attention:A new Canadian government promoting a health agenda promising to improve access to home care, produce a national prescription drug plan, improve access to mental health services, and develop a strategy for healthy aging. Recent federal legislation that offers Canadians the right to choose medically assisted death, yet lingering concerns remain that our system of palliative care might not be optimal and may further marginalize already vulnerable individuals.Renewed attention on recommendations from the Truth and Reconciliation Commission (2015), which emphasizes the deeply interconnected health and social disparities of our Indigenous peoples and factors that have led to their vulnerability, such as racialization, colonization and poverty to name a few.Health ministers’ meeting to explore and make a plan to break down structural and political silos that prevent optimal health of all Canadians.

Cancer nurse leaders in Latin America, specifically Argentina, Brazil, Colombia, and Mexico, recently advocated for enhanced access to high-quality oral therapies (OT) nursing education and resources, as well as improved direct nursing care of people receiving OTs (Ayala de Calvo, 2010, 2014). Despite increasing numbers of cancer patients receiving OTs in those countries, few receive care by knowledgeable nurses who can effectively prevent and/or manage the associated toxicities and support patients to maintain adherence to the treatment plan (Barton, 2011; Schneider, Hess & Gosselin, 2011). This gap in OT nursing care may negatively impact patients’ quality and length of life (CANO/ACIO, 2010; Neuss et al., 2013; Oakley, Crowe & Johnson, 2010; Weingart et al., 2011).

With increasing numbers of individuals presenting with advanced cancer, strong recommendations for the integration of palliative care in oncology, and the availability and acceptance of medical assistance in dying, oncology nurses must be informed and prepared to provide patient-centred care in advanced cancer. This is part one of a two-part case report presenting an exploration of early palliative care and medical assistance in dying in the setting of advanced cancer. It focuses on the patient and family experience throughout the illness trajectory, from diagnosis to death, and incorporates some of the common challenges seen in supporting someone with advanced cancer. While highlighting the perspective of the patient and family, it demonstrates how palliative care and medical assistance in dying can work together to achieve a patient's goals. Preeclampsia is a pregnancy-specific condition characterized by gestational hypertension associated with proteinuria or organ dysfunction after 20 weeks of gestation. It complicates 2 to 8 % of pregnancies worldwide and represents the leading cause of maternal and fetal mortality in developed countries. The only definitive treatment remains termination of pregnancy and delivery of the placenta. Prompt assessment of maternal and fetal status should be held in search of severity criteria and adequate management of this condition according to gestational age. Foremost concerns for pregnant patients are impending eclampsia or placental abruption, while fetal complications arise from placental insufficiency and risks associated with premature pregnancy termination. The sole efficient prophylaxis of preeclampsia in current state of evidence is aspirin at a dosage of 160 mg per day in high risk patients. Preeclampsia is now recognized as a high-risk factor for cardiovascular, renal, and neurological diseases and should therefore be considered as an opportunity for screening and prevention.

Introduction: In Canada, people experiencing socioeconomic inequities have higher rates of late diagnosis and lower survival rates than the general population. Palliative care services focussed on this population are scarce. We developed a community-based nursing intervention to improve access to palliative care for people experiencing socioeconomic inequities and living with life-limiting illnesses in an urban Canadian setting., Methods: This community-based, qualitative research study combined critical and participatory research methodologies. The study was conducted in partnership with the Palliative Care Outreach Advocacy Team (PCOAT) based in Edmonton, Alberta, a team dedicated to serving populations experiencing socioeconomic inequities who require palliative care. Following an exploratory phase that served to delineate the intervention, we undertook a one-year pilot implementation during which a part-time registered nurse (RN) joined PCOAT. The RN engaged in trust building, resolution of health and practical needs and complex care coordination. Twenty-five patients participated in the intervention. Participants were interviewed at least once to explore their experiences with the intervention. Data were analyzed through thematic analysis., Results: Most participants were men, were Indigenous and had advanced cancer. Participants had significant financial concerns, lived or had lived in precarious housing situations and had previously faced serious challenges accessing health care. Participants reported social and health needs including housing, finances, transportation, symptom control, harm reduction and end-of-life care. Participants reported improved access to health and social services and expressed satisfaction with the study intervention., Conclusion: Study findings suggest the study intervention may have contributed to improved access to palliative care, improved experiences for participants and increased equity in the delivery of care., Competing Interests: The authors declare there are no conflicts of interest.

The so-called 4P medicine, preventive, predictive, participatory, and personalized, which places the patient at the center has influenced the latest recommendations for the management of common low back pain. The management of low back pain in the acute, subacute, and chronic phase is currently based on the profile of each patient with their risk factors, their prognosis, and the respect of their preferences, promoting an integrative approach. During the first consultation, it is important to identify factors of moderate to poor prognosis, including kinesiophobia and to search for false beliefs, through a detailed medical history. The non-pharmacological approaches are more effective and have less side effects than the medications.
