BACKGROUND Animal-assisted interventions (AAI) show promising results but they suffer from a lack studies and contradictory results. OBJECTIVE Our study deals with dog intervention and agitation in older patients with dementia. METHODS An epidemiological quasi-experimental study was carried out in patients living in a Special Care Unit (SCU) of an Ehpad in the French Region of Haut-Rhin. Agitation was assessed by the Cohen-Mansfield Agitation Inventory at three consecutive periods of one month after the instauration, discontinuation and resumption of a dog mediation. RESULTS 26 patients were included in the study (76,92% female); mean age was 85,35 years and the mean score on MMSE was 6,85. Stopping AAI leads to a 11,9% relative increase in agitation (p = 0,03) whereas reintroduction of AAI leads to a 29,9% relative decrease in agitation (p

Purpose To describe the occurrence of potential statin misuse, its characteristics and associated factors in older people. Methods A retrospective cross-sectional study was conducted in Alsace and Lorraine (France) using French health insurance databases. Study population comprised community-dwelling patients and nursing homes residents aged 80 and over, who received at least one statin prescription between January 1, 2017 to April 30, 2017 (N = 38 268). Potential statin misuse was identified considering off-label uses, high-intensity doses, drug contraindications and statin-drug interactions. Results At least one potential statin misuse was detected for 19 468 patients (50.9%). Off-label prescription was the most frequent misuse observed (53.6%), followed by high-intensity statin dose (30.5%). Polypharmacy (five to nine drugs) and excessive polypharmacy (10 or more drugs) were the main risk factors associated with potential statin misuse (adjusted OR = 2.6 and 4.7 respectively). Nursing home, multi-morbidity and the presence of multiple prescriptions from different doctors for the same patient were other risk factors significantly associated. Two sensitivity analyzes (reconsiderations of off-label and high statin dose definitions) revealed the same statistical trend. Conclusion Potential statin misuse is frequent in people aged 80 and over. These findings should serve as a warning to health care professionals and hopefully will contribute to ensure an appropriate and safe use of statin in aged population.

The aim of the 'physical aptitude for health consultation' is to offer a validated physical reconditioning programme to adults with a stabilised chronic condition. It notably enables the absence of any contraindications to be established. A first of its kind in France, the programme has been implemented at Strasbourg university hospital.

Seniors represent the population where sedentary is the highest.To evaluate of the exact health benefits of regular aerobic training (AT) in seniors aged 70 years or older.Systematic review in CINAHL Plus, Embase, Medline, PubMed Central, ScienceDirect, Scopus, Sport Discus and Web of Science with a keyword search.Two independent readers have selected randomized controlled and quasi-controlled studies and observational cohort studies published in English.Of 3515 articles identified, 87 studies were included in the systematic review and categorized according to the analysed outcomes. The benefits of AT are clearly demonstrated on total mortality, coronary and neurovascular disease, glucose metabolism and type 2 diabetes, blood lipid profile, body composition, blood pressure, cardiorespiratory performances, muscle strength and functional capacity, and quality of life among senior aged 70 years or more. More recently, it has shown benefits for primary and tertiary prevention of cancer and primary and secondary prevention of cognitive decline. The benefits on bone health and the risk of falling are yet to confirm.The data of benefits result from studies published in English only.The AT is an important determinant of health and quality of life in seniors. Its promotion in this population should be part of the continuity of efforts undertaken among younger population. Thus, seniors should be more actively encouraged to engage in training programs.

ObjectiveTo measure the impact of a short-term variable-workload endurance-training programme (PEP’C) on cardiorespiratory parameters and endurance, and also on quality of life for seniors. The respective roles of the perception of physical activity and the financial factor involved in joining such a program were also investigated.PopulationTwo hundred and thirty-two volunteers aged 50 or over voluntarily participated in the “Physical Fitness for Health” assessment (CAPS). Subjects presenting the common contra-indications to physical activity were previously excluded.MethodAfter an initial assessment of aerobic capacity using a maximum cardiorespiratory test, a special variable-workload endurance-training programme on ergometric bicycles was run, comprising 18 sessions of 30 minutes each for 9 weeks. A final assessment of aerobic capacity completed the program. Quality of life was measured before and after the PEP’C using the MOS-SF-36.ResultsThe volunteers’ mean age was 66.0 ± 6.5 years ; 77.5% were aged 60 to 74 years and 11.6% were 75 years or older. Among the volunteers, 15.9% had never practiced any sport or physical activity. One out of two said that maintaining their physical fitness was their main motivation. The programme adherence rate was 100%. Over 18 sessions, maximum cardiorespiratory parameters (MTP VO2peak and MMV) and aerobic endurance parameters (VT1, HR, VT1, and lactate at MTP) improved (P > 0.05). An effect of gender was observed whereby improvement in performance was significantly greater in women. For aerobic endurance abilities, a significant gain was measured for 92.2% of the participants. Before starting PEP’C, scores on the MOS-SF-36 were high in all dimensions. At the end, all quality of life dimensions were significantly improved and the increase in global score was 10.0% ± 20.0%. The mean scores in the dimensions of “perceived health” and “vitality” were those that increased most. In men, physical activity particularly improved the “vitality” dimension, while for women it was rather the “mental health” dimension (P < 0.01). On average, 134 € was the amount considered affordable for a program of this sort, this amount being strongly dependent on the level of satisfaction.ConclusionThis study confirms the feasibility and benefits of this “individually tailored” PEP’C on maximum cardiorespiratory and endurance parameters, and on quality of life, among seniors. It also shows that adherence to a programme of this type is more influenced by the satisfaction derived from practicing physical activity and sport than by affordability., ObjectifsMesurer l’impact d’un programme d’entraînement à l’effort personnalisé par créneaux (PEP’C) sur les paramètres cardiorespiratoires et d’endurance, mais également la qualité de vie des seniors. Les rôles respectifs de la perception de l’activité physique et du facteur financier sur l’adhésion à un tel programme étaient également investigués.PopulationDeux cent trente-deux volontaires âgés de 50 ans ou plus ayant volontairement participé à la consultation de l’aptitude physique pour la santé (CAPS). Parmi les volontaires, 15,9 % n’avaient jamais pratiqué d’activité physique et sportive. Un sur deux déclarait que le maintien ou la remise en forme physique était leur motivation pour faire du sport. Le taux d’adhésion était de 100 %. En 18 séances, une amélioration des paramètres cardiorespiratoires maximaux (PMT, VO2pic, VMM) et des paramètres d’endurance aérobie (SV1, FC au SV1, taux de lactate à la PMT) était observée (p < 0,05). Il a été observé un effet du sexe avec une augmentation des performances significativement supérieure chez les femmes. Pour les aptitudes d’endurance aérobie, il était mesuré un gain significatif pour 92,2 % des participants. Avant d’initier le PEP’C, les scores au MOS-SF-36 étaient élevés dans tous les domaines. À la fin du PEP’C, tous les domaines de qualité de vie étaient significativement augmentés et l’augmentation du score global était de 10,0 ± 20,0 %. Les scores moyens dans les dimensions de « santé perçue » et de « vitalité » étaient les plus augmentés. Chez les hommes, la pratique d’une activité physique améliorait particulièrement la dimension « vitalité » alors que pour les femmes c’était plutôt la « santé psychique » (p < 0,01). En moyenne, 134 € était la somme qui pourrait être dédiée à un tel programme, le montant étant fortement dépendant du niveau de satisfaction.ConclusionCette étude confirme la faisabilité et les bénéfices du programme personnalisé PEP’C sur les principaux paramètres cardiorespiratoires maximaux, l’endurance et la qualité de vie des seniors. Elle montre aussi que l’adhésion à un tel dispositif est peu influencée par la satisfaction induite par la pratique d’une activité physique et sportive que par les coûts engendrés.

Cardiovascular disease is a leading cause of morbidity and mortality in the elderly population. We evaluated the adequacy of prescribing (miss and under used) with respect to STOPP-START criteria.A sample of 100 patients hospitalized in cardiovascular specialty divisions (medicine or surgery) or in the different sectors making up the geriatric network (day-care hospital, short or rehabilitation ward, nursing home) has been considered. Drug prescriptions at the admission time were analysed.Eight hundred and seventy-four prescriptions were analysed. In 65% of patients, from 5 to 10 medications were prescribed and in 28% over 10. Fifty-four percent of patients had, at least, one potentially inappropriate prescription (PIP) by STOPP. Among them, 48% of PIP prescriptions contained 1, 41% 2 and 11% 3 or more. The omission of one medication according to START criteria concerned 57% of the sample. Among them, 46% had one omission, 44% 2 to 3 and 10% 4 omissions or over. The cardiovascular system is the one most concerned by the PIP. Whether 28.1% of the PIP by STOPP criteria concerned cardiovascular drugs, the omission of prescription, according to START criteria, was 41.8%. There was no significant difference between the different settings studied. There was no effect of age or sex on the impact of PIP (P0.20) or being polymédiqué (P=0.44). According to the criteria STOPP-A, the prescription of antiplatelet (indication and dose) was highlighted. Prescribing omission also concerned antiplatelet agents but also statins in patients with atherosclerosis as well as antiplatelet and anticoagulant in patients with permanent atrial fibrillation and inhibitor of angiotensin converting enzyme (ACE) after myocardial infarction or with chronic heart failure.Potentially inappropriate prescribing medications were very common in elderly patients with cardiovascular conditions. They concerned as much as underusing of important drugs with potential benefits and prescribing commission of treatment that did not fit with patients' comorbidities and/or characteristics.

Vancomycin-resistant enterococci (VRE) are major nosocomial pathogens in many countries. VRE can spread rapidly, mostly by cross-transmission through hands of healthcare workers, leading to outbreaks. Moreover, VRE have the possibility to transfer vancomycin resistance genes to other Gram-positive organisms.We conducted a 9-month prospective study to demonstrate the effectiveness of "contact" precautions to prevent the spread of VRE in a long-term care facility.Six patients with VRE colonisation were admitted in an 80-bed long-term care facility. The following interventions were implemented to prevent the spread of VRE: gathering patients with VRE colonisation in the same unit, reinforcement of hand hygiene practices, "contact" precautions, reduction of some antibiotics and extensive screening of VRE carriers and contact patients by rectal swabs.There was no secondary case of VRE colonisation. Screening tests converted from positive to negative in four of the six patients.Compliance with hand hygiene recommendations and "contact" precautions can prevent the spread of VRE in a long-term care facility.

Physical activity and sports are considered as one of the determinants of health. The aim of this study is to review the rationale for the formulation of this public health issue and its integration in national action plans. The study shows that fourteen national programmes were drafted and implemented between 2001 and 2006 by seven institutions. The research methodology was based on crossing data obtained from semi-directed interviews and documents regarding the design, implementation and follow-up of these programmes. For the conditions of the success, the fourteen actions scored an average of 175.0 +/- 66.9 out of 300%. Public health actors and professionals must be given more opportunities to involve themselves and engage in developing stronger relationships and linkages, in particular with the institutional and community settings. In general, the most invested parts of a programme are the structural and operational aspects of activities. Six significant points surfaced from the study: consideration of drug use as an addictive behaviour; recognition of the psychological stress of professional athletes; acknowledgment of youth as being at high risk for doping behaviour; integration of the concept that physical activity and sports must take the benefit/risk perspective into account; and the necessity to promote health. Through the exchange of numerous local and regional experiences, an optimisation of their synergistic connections was made possible on a continuum extending from "health promotion through physical activity and sports" to "prevention of drug-use and doping behaviours". Professionals have been able to develop actions in the above-mentioned domains across this continuum that have, to date, remained isolated. Proposals are made to strengthen these dynamics. Other health determinants and public health priorities could be investigated with the same methodology.

This study aims to produce a review of the logical foundations at work that support the identification of a public health problem for this determinant and how to address it through national action. The research is based on cross-analysis of data from semi-directed interviews and data extracted from documents on the development, implementation and follow-up of such actions and programmes. The study shows that fourteen national programmes were created and implemented by seven different agencies. These fourteen activities scored an average of 175 +/- 66.9 out of 300%. Actors and professionals in the field must be given more capacity to be implicated and involved while simultaneously encouraging the strengthening of relationships with their environment, in particular the institutional and organizational settings as well as the community components. In general, the structural and operational aspects of action are those which receive the most investment in such activities. Six main points arose as important: doping considered as a addictive mechanism, acknowledgement of the psychological suffering of professional athletes, youth at high risk of doping, and that the concepts of physical activity and sports should take into account a risk/ benefit analysis and the necessity to first and foremost promote health. The act of sharing and exchanging a number of experiences, at the local and regional level, resulted in the identification of synergies between these experiences on a continuum from "health promotion through physical activity and sports" to "prevention of doping". Within this framework, professionals were able to develop activities in the aforementioned domains, which until now had remained isolated and marginal. Recommendations were made to reinforce and strengthen this dynamic. Other determinants of health and public health priorities could be explored using the same methodology.

To administer a social handicap questionnaire associated with French DRGs (PMSI) to determine the social handicaps of a population hospitalized in a public health establishment and to measure the cost implications for the establishment due to increased length of stay (DMS).A prospective pilot study has been carried out in the Lariboisière-Fernand Widal Hospital Group in Paris targeting users 50 or more years old hospitalized for short stays in medicine, surgery and obstetrics. Data of the PMSI and answers to the questionnaire for hospitalisations longer than 24 hours have been exploited.Two hundred twenty-two stays from 8 to 23 November 2005 have been analyzed: 140 pertained to patients aged 50-69 years (27.8%) and 82 to patients aged 70 or more years (16.3%). Three-fourths of the persons aged 50-69 and 70 or over presented a social handicap: 45% showed a strong handicap and a third an average handicap. The three indicators "renter/owner", "interior comfort" and "family relations" were the major determinants of social handicap for those aged 70 or more, 50-69 and 50 or more years. For the patients 70 years and over and those 50-69 years, with an average handicap, the indicator was "income" with the domain "patrimony." For strong handicaps, it was the indicator "scolarisation" for the 70 or more years and the indicator "income" for the 50-69 years old. When all classes and populations were pooled, the DMS was significantly lower than that of the ENC (p0.001 for the 70 years and over; p0.05 for the 50-69 years). With again all classes and populations pooled, patients 70 and over stayed in hospital 6.50 days less on average compared to data published by the ENC; those 50-69 years stayed 3.57 days less. Persons aged 50 and over with a social handicap remained hospitalized on average more than 2.5 days: 2.2 days for the 70 and over and 3.1 days for the 50-69 years group. In terms of hospital days that produces an increase of 18%, corresponding to a supplementary expenditure attributable to social handicap of approximately 5.9 million euros.This pilot study with a questionnaire disability social PMSI proposing specific aid, but also reducing the DMS, provides several promising information but also indicates the limits of our approach. Among these, we note in particular: (a) its regular feasibility requires constant supervision, wider and well-trained, (b) that the measurement of our tool can only be affirmed after its use in many patients, and (c) the classification of disability in social class could even be discussed again. We still wanted to explore whether through this initiative without much logistical, markers of interest had emerged, which seems to be the case.

Food-cobalamin malabsorption is a new well-characterized syndrome. In association with pernicious anemia, it is the leading etiology of cobalamin deficiency in adult, especially in elderly patient. Currently, it is an exclusion diagnosis that requires a well-codified clinical strategy for diagnosis. There are several causes of food-cobalamin malabsorption, mainly gastric disorders and drugs (metformin and anti-acid drugs). Current treatment modality includes oral cobalamin administration with lower doses than in pernicious anemia. Studies are in the way to better characterize the food-cobalamin malabsorption in a clinical practice perspective and to validate the usefulness of oral cobalamin therapy.

Recent longitudinal studies have highlighted associations between Alzheimer's disease (AD) and several factors, especially some cardiovascular risk factors, including hypertension, diabetes, diet, obesity, and elevated levels of homocysteine and lipids in the blood. The strongest associations are with hypertension and diabetes. Moderate alcohol consumption also appears to be associated with a decreased risk of incident AD. Studies of the effect of interventions to control these risk factors on the onset and course of dementia report encouraging results about antihypertensive agents and statins. Benefits from other drug therapies such as nonsteroidal antiinflammatory drugs and antioxidants remain uncertain, and initial hopes for hormonal replacement therapy for postmenopausal women have not been confirmed. Physical, cognitive and leisure activities seem to provide protection against incident AD. Cautious interpretation is necessary in view of the possible biases in these studies (confounding factors as well as survival, regression dilution, and indication biases). These epidemiologic data raise questions about the diagnostic boundaries between AD and vascular dementia. Additional studies are needed to validate these concepts and to confirm the possible benefits of preventive measures.

Standard treatment of vitamin B12 deficiency involves regular intramuscular cobalamin administration. The aim of this study was to determine whether oral cobalamin treatment may be an effective therapy for treating older patients with cobalamin deficiency related to nutritional deficiency and food-cobalamin malabsorption.We prospectively studied 20 patients older than 80 years with established cobalamin deficiency related to food-cobalamin malabsorption (n=14) and nutritional deficiency (n=6) who received 1000 micro g of oral cyanocobalamin per day. Levels of serum cobalamin and blood counts were determined at baseline and after the first week of treatment.After an average of 8 days of treatment, 17 out of 20 patients normalized their serum cobalamin levels; the patients had increased their serum cobalamin level (mean increase of 0.23 micro g/L; p0.01 compared with baseline), reticulocyte count (mean increase of 27400/mm(3); p0.05), hemoglobin levels (mean increase of 0.7 g/dL; NS), and decreased the mean erythrocyte volume (mean decrease of 0.7 fL; NS).Our findings suggest that cyanocobalamin given orally during one week may be an effective treatment for cobalamin deficiency related to food-cobalamin malabsorption and nutritional deficiency and may avoid painful intra-muscular injections in older patients.

Aerobic training (AT) shows health benefits for older with Alzheimer disease, but the physiological mechanisms that explain these remain to be analyzed. The objectives of this work were to study the feasibility of continuous aerobic training (CAT) or interval aerobic training (IAT) over 9 weeks and to compare their effect on brain derived neurotrophic factor (BDNF) plasma level, aerobic fitness, cognitive capacities and quality of life (QoL) in older with AD. No significant changes in terms of BDNF plasma level and cognitive performance was measured but a significantly improvement of endurance parameters was found. QoL was improved after CAT. This work shown that CAT and IAT are both feasible and equally effective to improve aerobic fitness. CAT had a positive impact on QoL, but both aerobic training failed to induce significant BDNF responses and improve cognitive performances.; L’entrainement aérobie (EA) montre des bénéfices sur la santé des personnes âgées présentant la maladie d’Alzheimer (MA) mais les mécanismes physiologiques expliquant ceux-ci restent à analyser. Après EATC et EATI, aucun effet n’a été observé sur le taux de BDNFp et les performances cognitives. Une amélioration des paramètres d’endurance a été trouvée. La QdV était améliorée après EATC. Ce travail a montré que l’EATC et l’EATI sont faisables et efficaces indifféremment pour améliorer l’EC. L’EATC a eu un impact positif sur la QdV, mais les 2 types d’EA n’ont pas eu d’effet sur la synthèse de BDNFp et les capacités cognitives.

Consequently, avoiding an inactive lifestyle and encouraging regular physical activity remains one of the main measures that should be promoted for older adults. The main objective of our study was to determine the effects of a new lightweight protocol including recovery bouts called: “The Intermittent Aerobic Training Program with Recovery bouts” (IATP-R - PEP’C-R in French) for seniors over 70 on maximal cardiorespiratory and endurance parameters. The secondary objectives were to determine the effects of IATP-R on vascular function and cognitive performances. Results: Our results showed that this new “IATP-R” training protocol permits to achieve a significant improvement in both endurance parameters and maximal cardio-respiratory parameters. In addition, the IATP-R permits to improve the vascular function and cognitive performances. Conclusion: The results of this study could be useful to implement this new IATP-R protocol for healthy older adults over 70. This would also be within the framework of prescribing training programs.; Contexte : Les sujets âgés représentent un groupe d’âge dont la proportion ne cesse d’augmenter. Ainsi, la lutte contre la sédentarité et la pratique d’une activité physique régulière demeure l’une des principales mesures qui devrait être privilégiée chez les seniors. L’objectif principal de ce travail était de déterminer les effets d’un nouveau protocole de réentraînement en endurance sur ergocycle alternatif « le Programme d’Entraînement Personnalisé sur Cycle à charge constante avec Récupération intermittente » (PEP’C-R) sur les paramètres d’endurance et cardio-respiratoires maximaux chez des seniors de plus de 70 ans. Les objectifs secondaires étaient de déterminer les effets du PEP’C-R sur la fonction vasculaire et les performances cognitives. Résultats : le PEP’C-R permet une amélioration significative des paramètres d’endurance et cardio-respiratoires maximaux et permet aussi d’améliorer la fonction vasculaire et les performances cognitives. Conclusion : Les résultats apportés par ce travail permettront de diffuser le PEP’C-R chez les seniors en bonne santé et ceci dans le cadre de la prescription de programmes de réentraînement à l’effort.

Ainsi, la lutte contre la sédentarité et la pratique d’une activité physique régulière demeure l’une des principales mesures qui devrait être privilégiée chez les seniors. L’objectif principal de ce travail était de déterminer les effets d’un nouveau protocole de réentraînement en endurance sur ergocycle alternatif « le Programme d’Entraînement Personnalisé sur Cycle à charge constante avec Récupération intermittente » (PEP’C-R) sur les paramètres d’endurance et cardio-respiratoires maximaux chez des seniors de plus de 70 ans. Les objectifs secondaires étaient de déterminer les effets du PEP’C-R sur la fonction vasculaire et les performances cognitives. Résultats : le PEP’C-R permet une amélioration significative des paramètres d’endurance et cardio-respiratoires maximaux et permet aussi d’améliorer la fonction vasculaire et les performances cognitives. Conclusion : Les résultats apportés par ce travail permettront de diffuser le PEP’C-R chez les seniors en bonne santé et ceci dans le cadre de la prescription de programmes de réentraînement à l’effort.
