Le lupus érythémateux systémique (LES) est une maladie auto-immune rare ayant un impact significatif sur la morbi-mortalité et la qualité de vie. À ce jour, peu d'études ont évalué l'épidémiologie du LES à l'échelle nationale. L'objectif de cette étude était d'actualiser les données de prévalence et d'incidence du LES en France, à partir des données médico-administratives du Système national des données de santé (SNDS). Cette étude a utilisé le SNDS, qui contient les données pseudonymisées de plus de 66 millions de personnes. Les patients inclus dans l'étude étaient atteints d'un LES, identifiés par le code CIM-10 M32 (LES) documenté dans le cadre d'une affection de longue durée (ALD) et/ou au cours d'une hospitalisation. Deux sous-populations ont été considérées : la population prévalente constituée des patients vivants au 01/01/2020 et une population incidente constituée des patients nouvellement diagnostiqués en 2017 et 2018. Au 01/01/2020, un total de 31 852 cas prévalents de LES a été identifié, soit un taux de prévalence brut de 47,4 cas pour 100 000. La majorité des patients étaient des femmes (86,3 %). L'âge moyen était de 49,2 (±15,8) ans chez les femmes et de 52,5 (±15,9) ans chez les hommes. L'ancienneté moyenne de la maladie était de 7,2 (±6,3) ans chez les femmes et de 6,3 (±5,6) ans chez les hommes. La prévalence maximale était observée chez les femmes âgées de 40 à 49 ans (142,3/100 000) et chez les hommes âgés de 60 à 69 ans (23,7/100 000) (Tableau 1). Les taux de prévalence bruts par région en France métropolitaine étaient compris entre 32,4/100 000 en Pays de la Loire et 75,8/100 000 en Corse. Ces taux étaient plus élevés en Martinique (108,4/100 000) et en Guadeloupe (104,9/100 000). Parmi l'ensemble des cas prévalents, 10,2 % et 0,3 % bénéficiaient de la CMUc et de l'AME, respectivement. Un total de 2971 et 2695 cas incidents de SLE ont été identifiés respectivement en 2017 et 2018, soit des taux d'incidence bruts de 4,4/100 000 en 2017 et 4,0/100 000 en 2018. Parmi les patients incidents en 2018, la majorité était des femmes (83,9 %). Le pic d'incidence en 2018 a été observé chez les femmes âgées de 30 à 39 ans (11,6/100 000) et chez les hommes âgés de 50 à 59 ans (2,5/100 000). Les taux d'incidence bruts par région en France métropolitaine en 2018 étaient compris entre 2,6/100 000 en Pays de la Loire et 5,6/100 000 en Corse. Ces taux étaient plus élevés en Guadeloupe (8,5/100 000) et Martinique (8,4/100 000). Parmi l'ensemble des cas incidents en 2018, un total de 12,7 % et 0,5 % bénéficiaient de la CMUc et de l'AME, respectivement (Tableau 1). La prévalence a été estimée à 47,4 cas/100 000 et l'incidence annuelle à 4,0/100 000 en 2018, le LES reste donc considéré comme une maladie auto-immune rare en France. [ABSTRACT FROM AUTHOR]

Anifrolumab est un anticorps monoclonal humain de type immunoglobuline G1 kappa qui se lie à la sous-unité 1 du récepteur à l'interféron de type I (IFNAR1). Il est indiqué en traitement additionnel chez les patients adultes atteints d'un lupus érythémateux systémique (LES) modéré à sévère, actif avec présence d'auto-anticorps malgré un traitement standard. L'objectif de l'analyse présentée est de décrire les caractéristiques et l'évolution des patients ayant bénéficié d'anifrolumab 300 mg, (solution à diluer pour perfusion ; une injection toutes les 4 semaines) dans le cadre d'une autorisation temporaire d'utilisation de cohorte (ATUc). Anifrolumab a été utilisé dans le cadre d'une ATUc, octroyée par l'ANSM en juin 2021 dans l'indication suivante : « en traitement de fond additionnel chez les patients adultes atteints d'un LES insuffisamment contrôlé (ou intolérant) malgré un traitement optimal avec les biothérapies actuellement disponibles ». Cette ATUc a pris fin en mai 2022. Les données présentées sont issues des fiches de collecte de données dans le cadre du protocole d'utilisation thérapeutique et de recueil d'information. Au total, 26 patients ont été inclus dans cette ATUc. Selon les définitions de l'ANSM, 23 d'entre-deux sont considérés comme « traités » (traitement fourni), et 7 comme « exposés » (réception d'au moins une fiche de suivi contenant une date d'administration). Pour les autres patients « traités », aucune fiche de suivi n'a été complétée. L'âge moyen des 23 patients traités est de 40,2 ans (écart-type [ET] 9,9 ans). Il s'agit de 21 femmes et 2 hommes, avec un poids moyen de 66,3 kg (ET 18,0 kg). L'ancienneté du diagnostic de LES est en moyenne de 13,9 ans (ET 8,2 ans). Il s'agit pour les 23 patients d'un LES modéré à sévère actif, avec un score SELENA–SLEDAI moyen de 9,7 (ET 5,3 ; n = 21) et un score PGA moyen de 2,3 (ET 0,7 ; n = 15). En moyenne, à l'inclusion, les patients présentent 6,1 articulations douloureuses (ET 5,9 ; n = 16) et 1,9 articulations gonflées (ET 2,9 ; n = 15). Le score CLASI-A moyen est de 11,9 (ET 8,2 ; n = 14). La durée préalable moyenne de traitement par bélimumab était de 13,4 mois (ET 13,8). Les caractéristiques des 7 patients exposés sont similaires à celles des patients traités. La durée moyenne d'exposition au traitement est de 5,7 mois (ET 3,3). Après un mois de traitement, une amélioration clinique des 7 patients exposés a été observée. Le nombre d'articulations douloureuses chez les patients exposés est passé de 6,0 (ET 7,7) en moyenne à 1,2 (ET 1,3) ; de même le nombre d'articulations gonflées est passé de 1,8 (ET 3,5) en moyenne à 0,3 (ET 0,8). Le score d'activité de la maladie (SELENA-SLEDAI) s'est amélioré en passant de 10,3 (ET 5,9) à 5,4 (ET 3,6). L'activité globale de la maladie évaluée par le clinicien à l'aide du PGA a diminué de moitié en passant de 2,0 (ET 0,4) à 1,0 (ET 0,7). Concernant les atteintes cutanées (CLASI-A), une amélioration a été observée ; le score est passé de 20,3 (ET 4,6) à 12,0 (ET 7,4). Ces bénéfices cliniques se maintiennent au cours du temps. Sur l'ensemble des patients traités (n = 23), 6 cas comprenant 16 suspicions d'effets/évènements indésirables, n'ayant pas entraîné l'arrêt du traitement, ont été déclarés pour 6 patients. Sur l'ensemble des cas rapportés, 2 ont été considérés comme graves et non reliés à anifrolumab : coxalgie droite aiguë et suspicion d'herpès. Aucun cas ne comprenait des évènements d'évolution fatale ou avec mise en jeu du pronostic vital. Du fait du faible de nombre de patients, des conclusions limitées peuvent être tirées de ces données de première utilisation d'anifrolumab 300 mg, solution à diluer pour perfusion. Introduction: The definition of the optimal timing of referral for lung transplantation in cases of chronic obstructive pulmonary disease or idiopathic pulmonary fibrosis remains a difficult question., Background: The decision is based on the balance between the risk of death associated with the transplant procedure on one hand and, on the other, the risk of death associated with the natural course of the disease and/or the severity of the disability. It is not surprising, therefore, that the selection criteria take into account the main prognostic factors identified in both diseases. Recommendations for the selection criteria based on these prognostic indices have been published recently by a panel of international experts. In case of chronic obstructive pulmonary disease, the main indication for lung transplantation worldwide, the decision is not simple. The course of the individual patient is not easy to determine as, on an individual basis, some patients have a prolonged survival. In the case of idiopathic pulmonary fibrosis, the survival benefit provided by lung transplantation is well documented and it is therefore recommended to refer the patient to a transplantation centre once the diagnosis is established. This evaluation for lung transplantation does not prevent the subsequent inclusion of the patients in therapeutic protocols., Conclusion: International guidelines assist the pulmonologist to define the optimal moment of referral but these guidelines are not absolute and therefore, in case of doubt, physicians should not hesitate to refer the patient to a transplant centre.

Introduction: Several methods have been reported to assess the survival benefit of lung transplantation. Incorrect use of these methods may lead to erroneous conclusions., Current Knowledge: In most cases, assessment of the survival benefit of lung transplantation relies on the modification of the baseline hazard by lung transplantation, using in most cases the Cox proportional hazard model. However, this method makes stringent assumptions on the relation between pre and post transplant survival. Other methods comparing expected survival without lung transplantation to observed survival after lung transplantation have been developed. We recently described an approach based on the comparison of expected pre and post transplant survival., Perspectives: Methods combining survival as well as functional and quality of life aspects would better describe the whole impact of lung transplantation., Conclusion: The assessment of the survival benefit of lung transplantation now relies on complex statistical methods. None of the proposed methods allow definitive conclusions to be reached on the survival benefit of lung transplantation.

Introduction: Alpha-1 antitrypsin deficiency is associated with the occurrence of pulmonary emphysema. The aim of this study is to describe the characteristics of patients with alpha-1 antitrypsin deficiency associated pulmonary emphysema., Methods: We describe a prospective cohort study including adult patients with alpha-1 antitrypsin deficiency associated pulmonary emphysema confirmed by CT scan living in France. Patients' clinical and functional characteristics, quality of life measures and management were recorded every 6 months during a five-year period., Results: 201 patients were included from 56 centres between 2005 and 2008. The characteristics of 110 patients have been analysed. Mean age was 50 years (SD:11.8), 62.7% were males, 90% were tobacco smokers. The main functional results (% predicted) were: FEV1: 42.8 (19.6), CPT: 128.3 (21.7), CRF: 167.0 (46.0), 6 minute walking distance (meters): 413 (130). 51 (46.4%) patients received augmentation therapy. Augmentation therapy was administered weekly (37.5%), twice a month (35.4%) or monthly (25.5%). Study centre was the only factor associated with the likelihood to received augmentation therapy., Conclusions: The clinical and functional characteristics as well as management of these patients varied markedly. There is a need for a standardization of the management of patients with alpha-1 antitrypsin deficiency associated pulmonary emphysema.

Background: The natural history and optimal therapeutic management of emphysema associated with alpha-1 antitrypsin deficiency are poorly understood. The purpose of this cohort of patients with emphysema associated with alpha-1 antitrypsin deficiency is to describe their characteristics and to study the factors associated with the evolution of their pathology., Materials and Methods: It is a multicentre, prospective, cohort study including all the patients in metropolitan France with alpha-1 antitrypsin deficiency associated with emphysema (diagnosed by CT scanning) and an obstructive ventilatory defect. All respiratory physicians will be approached. Patients meeting the inclusion criteria will be followed up every 6 months for 5 years. Clinical and spirometric data as well as those relating to quality of life will becollected. The principal criterion will be FEV1; the secondary criteria: quality of life, exacerbations requiring hospitalistion and mortality., Expected Results and Viewpoints: 300-500 patients are expected to be included. At the conclusion incorporation of this cohort into AIR (Alpha-antitrypsin International Registry) will increase the statistical power. This study could also provide the base for prospective trials and research projects.

Hemoptysis is a common respiratory symptom that always requires investigation. Its severity varies from bloodstained sputum to life-threatening hemoptysis. Blood originates most often from bronchial circulation. The most common causes of hemoptysis are tuberculosis, lung carcinoma, bronchiectasis but idiopathic forms are frequent. The diagnostic investigation is based mainly on history, chest radiograph, fibroscopy and CT scan of the thorax. The goal is to determine the cause as well as the site of bleeding. The management of life-threatening hemoptysis is mainly based on either surgical approach or bronchial artery embolization. The majority of centers favor this latter approach.

A POTENTIALLY FATAL CONDITION: Pulmonary embolism (PE) is a potentially fatal disorder for which heparin therapy improves the outcome. In spite of anticoagulation, mortality of PE remains high, especially when associated with shock or right ventricular dysfunction. THROMBOLYSIS: Indications of thrombolytic therapy in the treatment of PE remain relatively undefined. It is well established that thrombolytic therapy achieves a more rapid but not more complete dot lysis than heparin alone. RANDOMIZED STUDIES: Only one randomized add prospective study including 8 patients with massive PE associated with shock found a beneficial effect of thrombolysis treatment regarding mortality. The other studies which involved 453 patients did not find such a beneficial effect of thrombolysis on mortality. There is no convincing evidence suggesting beneficial effect of thrombolysis regarding PE recurrence or long term recovery. However, there is an increasing risk of major bleeding when using thrombolytic agents. In summary, thrombolytic therapy use should be restricted to patients who have hemodynamic instability in absence of absolute contraindications. A large-scale prospective randomized controlled trial, comparing heparin alone and thrombolysis therapy is needed to clarify the indications of these treatments.
