Les préparations radiopharmaceutiques sont réalisées sous la responsabilité du radiopharmacien qui procède à la libération des lots avant administration au patient. Cette libération pharmaceutique est conditionnée par un ensemble de critères dont la conformité des contrôles de qualité (CQ). Lors de la reconstitution des trousses la valeur de la pureté radiochimique (PRC) est le reflet du radiomarquage de la trousse par le technétium présent dans l’éluat. Le fabricant des trousses met à disposition des radiopharmaciens une méthode d’évaluation de la PRC. En pratique, certaines méthodes semblent peu adaptées ou ne permettent pas de détecter toutes les impuretés ayant parfois une incidence sur la qualité des images. Pour se faire, des méthodes alternatives existent ou peuvent être mises au point par le radiopharmacien. La difficulté réside dans le choix de la méthode selon différents paramètres à prendre en compte. Dans le cadre de ce travail nous avons proposé un score construit sur des critères simples et objectifs. Nous avons aussi élaboré un outil de comparaison et de simulation des méthodes alternatives permettant d’aider les radiopharmaciens dans leurs choix de méthodes alternatives en fonction du matériel dont ils disposent. Par ailleurs, nous nous sommes intéressés à la méthodologie permettant aux radiopharmaciens de valider les méthodes alternatives développées en interne. Ce travail a été proposé au groupe de travail de la Société Française de Radiopharmacie s’intéressant à cette thématique afin qu’il le valide et le diffuse à la communauté des radiopharmaciens.

The 2-step French process of nuclear waste treatment, which consists in calcining and vitrifying, enable integrating the radio-isotopes into a glassy matrix. The high temperatures involved (above 1000°C) provoke the volatilization of the fission product Cs, in the form of a solid aerosol. Its volatility is increased by the presence of the fission product Tc (simulated by Re). The goal of the work was to characterize the volatilization mechanisms and the retention of the main volatile chemicals in the vitrification process, in order to monitor the volatility from its origin. The first approach was to understand the behavior of the volatile elements in the glass melt, then the transport phenomena to the gas phase, and finally the formation of aerosols. This approach consisted in spiking simplified glass matrices elaborated in a mock-up, with Cs and Re in order to evaluate the effect on volatility. Data treatment was also performed on a series of glass elaborations in a pilot-scale process with diverse elemental concentrations, in order to evaluate the scale-up effects. That led to the conclusion that the Cs-Re volatility comes from the furnace, and to hypothesize about a CsReO4 volatilization mechanism. A second approach was to characterize the evolution of aerosol size distribution throughout the process, and to evaluate the particle capture by a typical dust scrubber. A measurement campaign was performed using an ELPI+ cascade impactor during a glass elaboration on the pilot-scale process. It showed that the aerosol size distribution in the furnace is 1-µm centered. Moreover, MEB-EDS analyses on the particles sampled showed that they are made of solid alkali perrhenates coupled with Mo, that confirm the volatilization mechanism hypothesis. Meanwhile, the characterization of the dust scrubber, with a 1-µm centered simulated alumina aerosol, revealed that the retention is maximized for micronic particles above 2.5 µm., Le procédé français de traitement des déchets nucléaires en deux étapes, calcination et vitrification, permet d’immobiliser les radio-isotopes dans une matrice vitreuse. Les hautes températures mises en œuvre, supérieures à 1000°C, génèrent la volatilisation du produit de fission Cs radioactif, sous la forme d’un aérosol solide. Cette volatilité est accrue en présence du produit de fission Tc (simulé par le Re). L’objectif de ces travaux a été de caractériser les mécanismes de volatilité et la rétention des éléments volatils dans le procédé, dans le but de maitriser la volatilité directement à sa source. Une première approche a visé à comprendre le comportement des éléments chimiques dans les fontes verrières, leur mode de transport jusqu’à la phase vapeur, puis la condensation des aérosols. Cette approche est passée par le dopage de verres simplifiés, élaborés dans une maquette, avec du Cs et du Re dans le but d’en évaluer l’effet sur les volatilités. Elle a été couplée au traitement de données d’un panel d’essais de vitrification sur prototype, mettant en jeu des conditions de concentrations élémentaires variées, dans le but d’évaluer l’extrapolation des phénomènes. Cela a permis de déterminer que l’essentiel de la volatilité Cs-Re provient du four de vitrification, et d’émettre une hypothèse sur le mécanisme de volatilité du CsReO4. Une deuxième approche a été de caractériser l’évolution de la granulométrie des aérosols tout au long du procédé, et d’évaluer la capture des particules par un dépoussiéreur « type » de prototype. Une campagne de mesures a été conduite à l’aide d’un impacteur cascade ELPI+ sur le prototype pendant une élaboration verrière, et a indiqué que la granulométrie dans le four est centrée à 1 µm. En outre, des analyses MEB-EDS des particules prélevées ont révélé qu’il s’agissait de perrhénates d’alcalins solides couplés avec du Mo, ce qui renforce la proposition de mécanisme de volatilité présentée. En parallèle, la caractérisation du dépoussiéreur, à partir d’un aérosol simulé d’alumine centré à 1 µm, a révélé que la rétention est maximale pour des particules microniques au-dessus de 2,5 µm.

Mémoire de Diplôme d'Etudes Spécialisées (DES) tenant lieu de thèse d'exercice.; Technetium-99m is the most widely use radioisotope for diagnosis in nuclear medicine. Siderophors such as trenox and trensox are iron chelating agents. Due to the 8-hydroxyquinoline groups of these two siderophors, they could chelate technetium in similar way as iron. Without heating and without pH change, using 50 µg of pyrophosphate as a reducing agent and 150 nmol of chelate and a fresh pertechnetate eluate, we can obtain very easily and quickly the 99mTc-trenox complex with radiochemical purity >98% and the 99mTc-trensox complex with purity >95%. Both complexes are stable in vitro as well as after injection in mice. The first complex is lipophilic and neutral while the second is hydrophilic and negatively charged. The biodistribution study in mice shows a 99mTc-trenox candidate for hepatobiliary imaging with a high accumulation in both tissues due to its lipophilicity. The addition of sulfonate groups to trensox makes it more hydrophilic, hence its predominantly renal biodistribution. These encouraging preliminary results allow us to continue to consider O-trensox and O-trenox/99mTc complexes as serious candidates for SPECT imaging. This study will have to be completed by an evaluation of the complexation of these siderophors with 188Re, an emitting isotope β- having coordination chemistry very close to that of technetium-99m thus allowing an alternative theranostic use to the classical pair (68Ga/177Lu).; Le technétium-99m est le radio-isotope le plus utilisé pour le diagnostic en médecine nucléaire. Les sidérophores tels que le trenox et le trensox sont des agents chélatant du fer. Grâce aux groupements 8-hydroxyquinoline de ces deux sidérophores, il est possible de chélater le technétium sur le même modèle que celui du fer. Sans chauffage et sans modifier le pH, en utilisant 50 µg de pyrophosphate comme réducteur et 150 nmol de chélate et un éluât de pertechnétate frais on obtient très facilement et rapidement le complexe 99mTc-trenox avec une pureté radiochimique >98% et le complexe 99mTc-trensox avec une pureté >95%. Ces deux complexes sont stables in vitro ainsi qu’après injection chez la souris. Le premier complexe est neutre, lipophile et candidat à l’imagerie hépatobiliaire grâce à une forte accumulation dans ces deux tissus chez la souris. Le second est chargé négativement et hydrophile grâce à l’ajout de groupement sulfonate sur le trensox d’où sa biodistribution majoritairement rénale chez la souris. Ces résultats préliminaires encourageants nous permettent de continuer d’envisager les complexes O-trensox et O-trenox/99mTc comme des candidats sérieux en imagerie TEMP. Cette étude devra être complétée par une évaluation de la complexation de ces sidérophores avec le 188Re, un isotope émetteur β- ayant une chimie de coordination très proche de celle du technetium-99m permettant ainsi une utilisation théranostique alternative à la paire classique (68Ga/177Lu).

This thesis is anchored in the framework of the spent nuclear fuel reprocessing by a liquid liquid extraction process with the TBP molecule or a monoamide as an extractant molecule. The aim of this process is to recover uranium and plutonium in the organic phase. However, some fission products, like ruthenium and technetium, are also extracted. The studied way in this work is to add a masking agent in the aqueous phase to avoid the extraction of ruthenium and technetium, or to back extract these elements during a scrubbing stage. For the ruthenium, two different molecules were studied with OH group or nitrogen donor atoms. However, the ruthenium speciation in nitric acid medium is changing with the experimental conditions, so finding a selective masking agent is difficult. The best results obtained in this work is to heat the scrubbing stage as it allows the selective bask extraction of ruthenium.For technetium, some polyammonium cage ligands were tested to maintain this element in the aqueous phase during the extraction step or during the back extraction. The first studied molecule, whose solubility is weak in nitric acid medium, was optimized by adding hydrophilic groups. For the most soluble ligand, some outstanding results were obtained for technetium back extraction: it is selective towards uranium and plutonium and the equilibrium is reached very quickly. Masking agent stability regarding γ radiolysis has also been studied and results show similar complexation strength than before irradiation., Ces travaux de thèse ont été effectués dans le cadre des études sur le retraitement des combustibles nucléaires usés par un procédé d’extraction liquide liquide mettant en jeu une molécule extractante de TBP ou de type monoamide. L’objectif de ce recyclage est la récupération de l’uranium et du plutonium en phase organique. Cependant, certains produits de fission, tels que le ruthénium et le technétium, sont également extraits, ce qui diminue les performances du procédé. La solution envisagée au cours de ces travaux est l’ajout de molécules complexantes en phase aqueuse afin d’empêcher l’extraction de ces deux produits de fission en phase organique, ou de les désextraire sélectivement lors d’une étape de lavage. Ces deux éléments ont donc été traités séparement.Pour le ruthénium, deux types de molécules avec des groupements OH ou des azotes ont été étudiés en extraction ou en lavage. La spéciation versatile du ruthénium en milieu acide nitrique rend difficile la formation de liaison Ru L. La solution envisagée grâce à ces travaux et envisageable industriellement, est de chauffer l’étape de lavage, ce qui permet la désextraction sélective du ruthénium en phase aqueuse.Pour le technétium, des ligands de type polyammonium cage ont été testés en extraction et en lavage. La structure de la première molécule testée, faiblement soluble en milieu acide nitrique, a été optimisée en greffant des fonctions hydrosolubles. Le dérivé le plus soluble montre des propriétés remarquables pour une utilisation en lavage dans le procédé : il est sélectif de l’uranium et du plutonium avec une cinétique de désextraction rapide et il est résistant vis à vis de la radiolyse. La caractérisation des complexes formés montre qu’un anion pertechnétate vient se placer à l’intérieur de la cavité et il est stabilisé par un réseau de liaison hydrogène.

En octobre 2018, le laboratoire GE-Healthcare® a annoncé l’arrêt de commercialisation du chrome-51 sous forme de chromate (51CrO4 -) de sodium et d’édétate de chrome (EDTA-51Cr). Ce radiopharmaceutique est utilisé en médecine nucléaire puisqu’il possède différentes indications diagnostiques notamment en néphrologie (Débit de filtration glomérulaire) et en hématologie (Volume globulaire). Devant l’importance de maintenir les examens, il a été nécessaire de proposer aux cliniciens de nouvelles méthodes de marquage. Le but de ce travail a donc été d’étudier le remplacement du chrome-51 par le technétium-99m et de mettre en lumière les différentes étapes de mise en place des techniques afin de maintenir une qualité d’examen et un service rendu au patient. 1ère partie : Cette première partie fait l’objet de publications dans des revues nationales et internationales avec comité de lecture. Elle présente la mise en œuvre des techniques de remplacements concernant l’estimation des débits de filtrations glomérulaires et des volumes globulaires. Ces procédures ont été évaluées et validées afin de démontrer leurs faisabilités, leurs reproductibilités, leurs précisions et leurs comparabilités. Elles utilisent toutes les deux un radionucléide ayant l’AMM dans les différentes indications citées précédemment : le Technetium-99m sous forme de pentétate de technétium (DTPA-99mTc) et de pertechnétate (99mTcO4-) de sodium. 2ème partie : La seconde partie présente un intérêt actuel et concret dans nos recherches et travaux concernant le service rendu aux patients de nos deux nouvelles techniques. Elle a pu être réalisée grâce à une étude récente menée au sein du service de soins de néphrologie à l’unité de transplantation rénale et grâce à un case report d’un patient venu consulter en Juin 2020. Ces deux études ont démontré la faisabilité et la robustesse de ces nouvelles procédures mais également l’importance du partage pluridisciplinaire qui permet une réévaluation des modes opératoires existants. Ces études pourront être publiées prochainement. Conclusion : L’évaluation de la fonction rénale et la détermination du volume globulaire sont deux examens importants et l’arrêt de commercialisation du chrome-51 a nécessité la révision rapide des procédures de marquage cellulaire et de clairance isotopique. Ces nouvelles procédures ont engendré de nombreux partages, discussions et débats au sein des services de médecine nucléaire français mais également dans la littérature scientifique internationale.

L’HTP est une maladie caractérisée par une élévation de la pression artérielle pulmonaire moyenne au-delà de 25 mmHg au repos. Dans le but de trouver un meilleur outil diagnostique pour cette affection, notre laboratoire a synthétisé puis expérimenté un radiotraceur nommé PulmoBind. Celui-ci s’avère être un dérivé synthétique de l’AM : un peptide vasodilatateur endogène de 52 acides aminés sécrété par de nombreux organes. Une fois couplé au 99mTechnétium, le PulmoBind permet par méthode scintigraphique la visualisation de la circulation pulmonaire métaboliquement active. Suite à notre étude de phase I, le PulmoBind s’est avéré sécuritaire et efficace. De plus, des analyses plus approfondies de la distribution spatiale ont démontré la capacité du PulmoBind à imager les divers gradients de la perfusion pulmonaire. À ce jour, aucun produit sur le marché n’emploie des données de captation et de distribution spatiale pour effectuer l’évaluation de la circulation respiratoire. Par conséquent, cette nouvelle méthodologie constitue une approche inédite et prometteuse pour le diagnostic de l’HTP et de son suivi., Background. The pulmonary circulation is submitted to large physiologic hemodynamic variations and possesses numerous important metabolic functions mediated through its vast endothelial surface. Unfortunately, no test is currently available that can directly evaluate endothelial integrity and pulmonary perfusion gradients at the same time. We developed PulmoBind, a chelated derivative of human adrenomedullin labeled with 99m-Tc for nuclear medicine SPECT imaging. By specifically binding to its endothelial receptors, abundantly expressed in human alveolar capillaries, PulmoBind can provide a non-invasive evaluation of pulmonary function that can help in the diagnosis of disorders affecting the pulmonary circulation. This thesis represents my doctoral work to determine the safety of PulmoBind in humans and its capacity to generate good quality imaging for diagnosis purposes. Methods. Twenty healthy participants were included into escalating doses groups of 5 mCi (n=5), 10 mCi (n=5) or 15 mCi (n=10) 99mTc-PulmoBind. Vital signs were closely monitored and safety biochemistry and hematology parameters obtained. SPECT imaging was serially performed and 99mTc-PulmoBind dosimetric and spatial distribution analysis accomplished. Imaging quality of the lungs was assessed serially. Results. Radiochemical purity of 99mTc-PulmoBind was greater than 95%. There were no safety concerns at the 3 dosages studied. Imaging revealed a predominant and prolonged lung uptake with mean peak pulmonary extraction of 58% ± 7% (mean ± SD) of the injected dose. PulmoBind was well tolerated and resulted in no clinically significant adverse events. The highest dose of 15 mCi provided a favorable dosimetric profile and excellent quality tomographic imaging of the lungs. The postural lung perfusion gradient was detectable. Indeed, dorsal activity was 18.1 ± 2.1% greater than ventral activity, and caudal activity was 25.7 ± 1.6% greater than cranial activity. The intensity of PB activity followed a normal distribution while macro aggregated albumin (MAA) activity was importantly skewed and bimodal. Conclusion. 99mTc-PulmoBind up to a dose of 15 mCi is safe and provides good quality lung perfusion imaging that can reveal the heterogeneity of the pulmonary perfusion. Therefore, the safety and efficacy of this agent could be tested in disorders of the pulmonary circulation such as pulmonary arterial hypertension.

Axillary sentinel lymph node (SN) biopsy by using indocyanine green (ICG) fluorescence for breast cancer is a recent technique. However, compared to Technetium-99m (Tc), which is the reference technique, its efficiency has received little testing.Between December 2013 and January 2014, 40 patients with node-negative breast cancer underwent SN biopsy by injecting sub areolar Tc in preoperative stage and injecting sub areolar ICG in intraoperative stage. SN were previously identified and resected by using ICG coupled with infrared camera. After resection of fluorescent SN, we check its radioactivity with a gamma probe (isotopic method). In case of residual radioactive labeling in the axillary crease, we remove the remaining SN. We have retrospectively analyzed the SN detection concordance rates of these two methods.In total we resected 53 SN, among which 48 (90.6%) were indocyanine green positive and 53 (100%) Tc positive. The remaining 5 SN were all ICG negative and Tc positive. Using ICG has not caused any side effect.SN detection for breast cancer by using ICG fluorescence is a promising, reliable technique which nonetheless requires a degree of expertise before reaching similar results as the Tc technique.

This project is part of the fundamental study of technetium speciation in highly acidic medium. The behaviour of technetium in HTFMS was carried out in the absence then in the presence of α irradiation. Given these two different conditions, spectrophotometric results of Tc(VII) reduction are similar. XAS analysis indicates the formation of a cyclic dimer of Tc(IV) complexed to triflate ligands and formulated asTc₂O₂(CF₃SO₃)₄(H₂O)₄. This compound is linearized to TcIV-O-TcIV with the increase of HTFMS concentration. At high concentration of HTFMS +98% (11.15 M), the protonated species TcO₃(OH)(H₂O)₂ which is formed in the absence of external ionizing radiations, is reduced to the V oxidation state under α irradiation. Structural characterization by EXAFS spectroscopy and DFT calculations suggests the formation of monomer species of Tc(V)-triflate complexes where [OTc(F₃CSO₃)₂(H₂O)₂]⁺ and [OTc(F₃CSO₃)₂(OH)₂]⁻ compounds were proposed. In concentrated H₂SO₄ (CH₂SO₄ ≥ 12 M), α-radiolysis experiments of Tc(VII) were performed in order to compare the radiolytic behaviour of Tc(VII) in both comparable media HTFMS and H₂SO₄. XANES studies show that radiolytic reduction of Tc(VII) leads to the formation of Tc(V)-Tc(VII) mixture in H₂SO₄ 13 M and just Tc(V) in 18 M of H₂SO₄. The analysis of EXAFS spectra is consistent with the formation of [TcO(HSO₄)₃(H₂O)₂] and [TcO(HSO₄)₃(H2O)(OH)]⁻ monomer complexes in H₂SO₄ 13 M and [Tc(HSO₄)₃(SO₄)(H₂O)] and [Tc(HSO₄)₃(SO₄)(OH)]⁻ species at 18 M of H₂SO₄.; Ce projet s’inscrit dans le cadre d’une étude fondamentale sur la spéciation du technétium en milieu fortement acide. Le comportement de Tc(VII) en milieu HTFMS a été étudié en absence et en présence d’irradiation α. Dans ces deux conditions différentes, les résultats spectrophotométriques de réduction de Tc (VII) obtenus pour les mélanges HTFMS-xH₂O sont similaires. L’analyse par SAX indique la formation d’un dimère cyclique de Tc(IV) complexé aux ligands triflates et formulé Tc₂O₂(CF₃SO₃)₄(H₂O)₄. Ce composé est linéarisé en TcIV-O-TcIV quand la concentration de HTFMS augmente. Dans HTFMS à +98% (CHTFMS=11,15 M), le Tc(VII) protoné, de formule TcO₃(OH)(H₂O)₂, stabilisé en absence de rayonnements ionisants externes est réduit en Tc(V) sous irradiation α. La caractérisation structurale par la spectroscopie EXAFS sur la base des calculs DFT suggère la formation d’un complexe monomère de Tc(V) avec les ligands triflates. Les deux composés [OTc(F₃CSO₃)₂(H₂O)₂]⁺ et [OTc(F₃CSO₃)₂(OH)₂]⁻ ont été proposées. En milieu H₂SO₄ concentré (CH₂SO₄ ≥ 12 M), des expériences de radiolyse α de Tc(VII) ont été menées afin de pouvoir comparer le comportement radiolytique de Tc(VII) dans les deux milieux apparentés HTFMS et H₂SO₄. Les études XANES montrent que la réduction radiolytique de Tc(VII) contribue à la formation d’un mélange de Tc(V) et Tc(VII) dans H₂SO₄ 13 M et à Tc(V) à 18 M en H₂SO₄. L’analyse des spectres EXAFS indique la formation de complexes monomères [TcO(HSO₄)₃(H₂O)₂] et [TcO(HSO₄)₃(H₂O)(OH)]⁻ en milieu H₂SO₄ 13 M et des espèces [Tc(HSO₄)₃(SO₄)(H₂O)] et [Tc(HSO₄)₃(SO₄)(OH)]⁻.

Molecular imaging is an essential non-invasive tool usable for diagnosis and characterisation of many diseases. Technetium-based tracers are the most popular ones due to disponibility, cost and radiochemical properties of 99mTc. Nevertheless, effective tracers development requires a long, expensive, and mainly empirical optimisation process. This context prompted us tu carry on the development of a new technetium structure which exhibits lots of potential functionalisation spots compatible with a combinatorial approach.We synthesised 12 N3X (X = N, O, S) different ligands. Each of them includes a triazole moiety, (formed via a click-chemistry reaction), which is involved in the metal complexation that implies one of its nitrogen atoms. Then we evaluated their ability to readily form oxotechnetium complexes in conditions that are compatible with medical use in hospital. One complex was formed in quantitative yields and its stability in mice plasma was investigated. A complex called TriaS-99mTc, stable to more than 90% after 6h incubation, was selected. In vivo study of TriaS-99mTc revealed an efficient blood clearance via the urinary excretion pathway with very low degradation.As an application, we used this structure for the development of tracers that target integrin αvβ3, a known biomarker of tumor neoangiogenesis. First, we synthesised functionnalised TriaS-based integrated complexes. Fonctionnal modification of TriaS by addition of side chains and substituents did not affect its ability to chelate oxotechnetium quantitatively. In addition, its stability in mice plasma was satisfactory. We also developped a bifonctionnal approach using c(RGDfK) peptide as the targeting biomolecule. In this way, a variable moiety (herein a PEG moiety) can be inserted in the structure through click-chemistry in order to modulate tracers solubility, biodistribution and excretion.; L’imagerie moléculaire est aujourd’hui un outil non-invasif essentiel pour le diagnostic de nombreuses pathologies. Les traceurs technétiés sont actuellement les plus répandus car le 99mTc est facilement disponible, abordable et présente des caractéristiques idéales pour l’imagerie. Néanmoins, le développement de traceurs efficaces nécessite un long et coûteux processus d’optimisation souvent empirique. Dans ce contexte, nous avons entrepris le développement d’une plateforme technétiée conçue pour présenter au sein de sa structure de nombreux sites potentiels de fonctionnalisation et compatible avec une approche combinatoire.Dans un premier temps, un ensemble de 12 ligands N3X (X = N, O, S) a été préparé. Chacun d’entre eux présente dans sa structure un motif triazole introduit par chimie-click et intervenant dans la complexation du métal par un de ses atomes d’azote. Nous avons ensuite évalué l’aptitude de ces ligands à chélater le cœur oxotechnétium dans des conditions douces (5 min, température ambiante) compatible avec une utilisation en milieu hospitalier. Le complexe TriaS-99mTc a été formé quantitativement et sa stabilité en plasma murin a été étudiée. Il s’est révélé stable à plus de 90% dans le plasma murin après 6h d’incubation. L’étude in vivo de ce complexe a par la suite révélé une élimination efficace du milieu circulant par la voie urinaire avec une dégradation minoritaire.A titre d’illustration, nous avons ensuite engagé la structure TriaS dans deux approches distinctes pour le développement de traceurs de la néoangiogenèse tumorale en ciblant l’intégrine αvβ3. D’une part, dans le cadre d’une approche intégrée, plusieurs complexes fonctionnalisés, mimes de RGD, ont été obtenus. Dans chaque cas, l’adjonction de groupements fonctionnels n’a pas affecté l’efficacité de la chélation. En outre la stabilité en plasma est maintenue à un niveau très correct. D’autre part, nous avons développé une approche bifonctionnelle dans laquelle le motif c(RGDfK) joue le rôle de molécule ciblante. Molecular imaging is an essential non-invasive tool usable for diagnosis and characterisation of many diseases. Technetium-based tracers are the most popular ones due to disponibility, cost and radiochemical properties of 99mTc. Nevertheless, effective tracers development requires a long, expensive, and mainly empirical optimisation process. This context prompted us tu carry on the development of a new technetium structure which exhibits lots of potential functionalisation spots compatible with a combinatorial approach.We synthesised 12 N3X (X = N, O, S) different ligands. Each of them includes a triazole moiety, (formed via a click-chemistry reaction), which is involved in the metal complexation that implies one of its nitrogen atoms. Then we evaluated their ability to readily form oxotechnetium complexes in conditions that are compatible with medical use in hospital. One complex was formed in quantitative yields and its stability in mice plasma was investigated. A complex called TriaS-99mTc, stable to more than 90% after 6h incubation, was selected. In vivo study of TriaS-99mTc revealed an efficient blood clearance via the urinary excretion pathway with very low degradation.As an application, we used this structure for the development of tracers that target integrin αvβ3, a known biomarker of tumor neoangiogenesis. First, we synthesised functionnalised TriaS-based integrated complexes. Fonctionnal modification of TriaS by addition of side chains and substituents did not affect its ability to chelate oxotechnetium quantitatively. In addition, its stability in mice plasma was satisfactory. We also developped a bifonctionnal approach using c(RGDfK) peptide as the targeting biomolecule. In this way, a variable moiety (herein a PEG moiety) can be inserted in the structure through click-chemistry in order to modulate tracers solubility, biodistribution and excretion.; L’imagerie moléculaire est aujourd’hui un outil non-invasif essentiel pour le diagnostic de nombreuses pathologies. Les traceurs technétiés sont actuellement les plus répandus car le 99mTc est facilement disponible, abordable et présente des caractéristiques idéales pour l’imagerie. Néanmoins, le développement de traceurs efficaces nécessite un long et coûteux processus d’optimisation souvent empirique. Dans ce contexte, nous avons entrepris le développement d’une plateforme technétiée conçue pour présenter au sein de sa structure de nombreux sites potentiels de fonctionnalisation et compatible avec une approche combinatoire.Dans un premier temps, un ensemble de 12 ligands N3X (X = N, O, S) a été préparé. Chacun d’entre eux présente dans sa structure un motif triazole introduit par chimie-click et intervenant dans la complexation du métal par un de ses atomes d’azote. Nous avons ensuite évalué l’aptitude de ces ligands à chélater le cœur oxotechnétium dans des conditions douces (5 min, température ambiante) compatible avec une utilisation en milieu hospitalier. Le complexe TriaS-99mTc a été formé quantitativement et sa stabilité en plasma murin a été étudiée. Il s’est révélé stable à plus de 90% dans le plasma murin après 6h d’incubation. L’étude in vivo de ce complexe a par la suite révélé une élimination efficace du milieu circulant par la voie urinaire avec une dégradation minoritaire.A titre d’illustration, nous avons ensuite engagé la structure TriaS dans deux approches distinctes pour le développement de traceurs de la néoangiogenèse tumorale en ciblant l’intégrine αvβ3. D’une part, dans le cadre d’une approche intégrée, plusieurs complexes fonctionnalisés, mimes de RGD, ont été obtenus. Dans chaque cas, l’adjonction de groupements fonctionnels n’a pas affecté l’efficacité de la chélation. En outre la stabilité en plasma est maintenue à un niveau très correct. D’autre part, nous avons développé une approche bifonctionnelle dans laquelle le motif c(RGDfK) joue le rôle de molécule ciblante. Molecular imaging has shown its interest in the diagnosis, staging and therapy monitoring of many diseases, especially in the field of cancer. This imaging modality can detect non-invasively early molecular changes specific to these diseases. Its expansion includes two aspects linked firstly with the advanced techniques of imaging modalities and secondly with the development of tracers as radio pharmaceuticals for imaging new molecular targets. Technetium-99m ((99m)Tc), because of its physical characteristics, its widespread availability and low cost, is the most used radionuclide in molecular imaging with the technique of single photon emission computed tomography (SPECT). Nevertheless, the current difficulty concerning the supply and the great interest of Positron Emission Tomography (PET), the "competitor" imaging modality-using molecules labelled with fluorine-18 ((18)F), legitimates the question about the future of (99m)Tc, its supremacy and the emergence of new tracer labelled with (99m)Tc. Focusing on the actual and future supply situation, the place of SPECT imaging in nuclear medicine, as well as the development of new molecules labelled with (99m)Tc is necessary to show that this radionuclide will remain essential for the speciality in the next years.

The lower gastrointestinal bleeding is common and somewhat difficult to diagnose. Scintigraphy with 99mTc labeled erythrocytes is a non invasive procedure which allows to demonstrate bleeding and sometimes to locate its origin. We report the case of a 54 year-old patient admitted for repeated digestive bleeding with impaired haemodynamics. Initial radiological assessment including a gastroscopy, a colonoscopy, an angiography and an angio-CT, did not found any active origin of the bleeding. The red blood cells scan helped to localize the site of active bleeding. This investigation technique has taken a prominent position as it is non-invasive, harmless, easy to perform and its ability to detect intermittent bleeding.

Integrins form a family of heterodimeric integral glycoproteins which play a central role in cell-cell adhesion and cell-matrix interactions. In particular, they are overexpressed during tumor neoangiogenesis. About 10 of them recognize a structured RGD (Arg-Gly-Asp) sequence. Analogs of this sequence can be used for the early detection of tumors and metastases. We developed new tracers, labeled with 99mTc, for the molecular imaging of alphaVbeta3 integrin. Until recently, there was no reliable ab initio structure prediction of complex molecules containing Re and Tc chelates. Therefore, we preferred a combinatorial approach to develop potential ligands of alphaVbeta3 integrin and we attempted to identify efficient tracers by in vivo screening. This method would account for biodistribution and pharmacokinetics properties in the early steps of the study. Tracers were obtained according two strategies: i) cyclization of linear RGD analogs; ii) combinatorial assembling of independent modules through metal core coordination by the well-known NS2+S motif. After synthesis and labeling, the stability of the tracers was investigated in presence of glutathione and in murine plasma. In vitro screening on purified integrin showed that a cyclic rhenium coordinate binds specifically alphaVbeta3. A tumor model (U87-MG tumor on nude mice) was validated in the laboratory and a method was developed to analyze in vivo experiments. During the past decade, radiolabeled peptides have emerged as an important class of radiopharmaceuticals for diagnosis and therapy in nuclear medicine. The concept of using radiolabeled peptides in vivo stimulates a large body of research for new peptidic vectors but also for radiochemical and radiopharmaceutical evaluations of these peptides after labeling.We studied and optimized the 99m-technetium, 125-iodine and 111-indium radiolabeling of a serie of peptides in order to allow the in vivo use of these tracers for imaging studies of three pathologies: tumoral angiogenesis, atherosclerosis and systemic lupus erythematosus. The radiolabeling with technetium was carried out using a tricarbonyle agent (IsoLink) and Indium was linked by DOTA macromolecule. Radioiodination was realized by electrophilic substitution. The reaction parameters were defined so that the labeling reaction presents a high radiochemical purity (>95%) and a high specific activity. The reactions were fast and reproducible. Various physicochemical and pharmacokinetic properties (lipophily, stability of labeling in vitro and in vivo, non-specific fixation, metabolization and organ biodistribution of peptides) were studied. These results permitted to select the best peptide for in vivo imaging., Au cours de la dernière décennie, les radiotraceurs peptidiques sont devenus importants en Médecine Nucléaire pour le diagnostic et/ou la thérapie. Le développement de ces radiopharmaceutiques nécessite la recherche de nouveaux vecteurs spécifiques mais aussi leur évaluation physico-chimique et pharmacologique après radiomarquage.Ces travaux concernent la mise au point et l'optimisation du radiomarquage au technetium-99m, à l'indium-111 et à l'iode-125 de nouveaux radiotraceurs peptidiques, destinés à l'imagerie in vivo de deux processus pathologiques : la néoangiogenèse tumorale et la plaque d'athérome vulnérable, et au suivi de la thérapie d'une maladie autoimmune : le lupus érythémateux disséminé. Le radiomarquage au technétium a été effectué par l'intermédiaire d'un petit synthon tricarbonyle (Isolink), l'indium a été fixé par une macromolécule DOTA et la radioiodation a été réalisée par substitution électrophile. Les paramètres de chaque réaction ont été définis afin que la réaction de marquage présente un haut rendement et une haute activité spécifique. Les réactions sont rapides et reproductibles et la mise en forme compatible avec une utilisation in vivo. Les propriétés physico-chimiques et pharmacologiques (lipophilie, stabilité de marquage in vitro et in vivo, fixation protéique, métabolisation et biodistribution) ont été mesurées. Ces études ont permis de sélectionner les radioligands présentant les meilleurs critères pour une utilisation in vivo en imagerie.

The onset of one or several diastema in adults with no link of etiology to tooth loss or to a periodontal problem can be the first evocative sign of an acromegaly or a benign or malignant tumoral process.A 40-year-old patient consulted for an implant to solve an esthetic problem caused by a diastema. The complementary exams revealed an osteolytic lesion marked by condensing areas. The biopsy proved the diagnosis of fibrous dysplasia.Etiologies of fibrous dysplasia are varied. They can be related to a congenital defect of osteo mesenchyma development or be the consequence of trauma, reaction to infection, or an endocrine or metabolic disorder.

To evaluate the accuracy of sentinel node biopsy for assessing the neck status for those patients with squamous cell carcinoma T1T2N0 of oral cavity.55 patients were included in a prospective study between 2000 and 2003. 53 underwent a sentinel node biopsy (SNB) followed by an elective neck dissection (END). Pathological examination with stepped serial sectioning and immunohistochemistry of sentinel node (SN) has been compared with routine pathology examination of remaining END nodes.12 patients had a positive SN. No false negative was found. Patient follow up on, at less of 3 years, did not show any node recurrence for those patients with negative SN. After that study, 44 patients had a SNB without END. 7 patients had a positive SN. Follow up showed a node recurrence for 3 patients. In two of these, pathological reexamination showed a micrometastase in SN. SN failure rate is less than 3% for those 99 patients.SNB is a liable procedure. Failure rate is the same as in END. We plan to use this procedure in orophyngeal tumors where it could be possible to reduce irradiation fields and treatment sequels for those patients with negative SN.

Radiation protection for innovative diagnostic and therapeutic approaches in nuclear medicine A real technological revolution has deeply modified the field of application and perspectives of nuclear medicine, and nuclear oncology in particular, during the past 5 years. Diagnostic applications such as positron emission tomography (PET) with 18F-fluorodeoxyglucose (FDG) have had a significant impact on the diagnostic strategy adopted by medical oncologists, with the addition of invaluable functional data to already available anatomical data provided by conventional imaging modalities. Numerous other 18F-labeled tracers currently under clinical evaluation have been developed to study various tumor functions (tumor proliferation, hypoxia, chemotherapy-induced apoptosis, etc.). These tracers may have a considerable impact on therapeutic strategies. Other positron-emitting radionuclides, such as copper-64, iodine-124, and yttrium-86 (whose respective half-lives are 12.7 hours, 4.2 days, and 14.7 hours) will soon be available for certain clinical indications, such as immuno-PET (with monoclonal antibodies or antibody fragments used as carriers) or pretreatment dosimetry, which cannot be performed with fluorine-18 due its short half-life. As far as therapeutic applications are concerned, the use of internal radiotherapy, which has been restricted to thyroid cancer for a long time, was recently extended to other cancers as new carriers, such as monoclonal antibodies (radioimmunotherapy) or peptides (radiopeptide therapy), new targeting methods (pretargeting), and new radionuclides, especially alpha particle emitters (alpha therapy), became available. These technological advances require that specific radiation safety regulations be implemented to protect nuclear medicine personnel, patients' close relatives, and the environment. Most current regulations concern diagnostic applications with technetium-99m and therapeutic applications with iodine-131. Regulations pertaining to the clinical use of 18F-FDG were recently enacted (2001). Regarding exposure nuclear medicine personnel, the amount of radioactivity used for therapeutic purposes is currently limited to 740 MBq, which requires that the patient be kept in a shielded room. Radiation exposure can be roughly estimated by measuring the exposure rate (in μSv/h) delivered at a distance of 1 meter from a source of one MBq. When considering both the exposure rate and half-life, it appears that similar doses of iron-52, yttrium-86, and iodine-124 deliver a high dose rate immediately after injection and during the following few hours. Current regulations on environmental exposure limit radioactivity levels in hospital sewage outfalls to 1000 Bq/L, for technetium 99m, and 100 Bq/L, for iodine-131. Forthcoming regulations for other radionuclides should not be established solely based on general rules such as the 100 Bq/L limit. Impact studies similar to those required in the nuclear industry, should be conducted. © 2006 EDP Sciences.; Une véritable révolution technologique a, au cours des 5 dernières années, profondément modifié le périmètre d’application et les perspectives de la médecine nucléaire, en particulier de la cancérologie nucléaire. Dans le cadre des applications diagnostiques, la tomographie par émission de positons (TEP) avec le 18F-fluoro déoxyglucose (FDG) a eu un impact déterminant sur la stratégie diagnostique des oncologues en ajoutant des informations fonctionnelles très précieuses aux informations morphologiques de l’imagerie conventionnelle. De nombreux autres traceurs fluorés, en cours d’évaluation clinique, sont destinés à étudier des fonctions tumorales diverses (prolifération tumorale, hypoxie, apoptose chimioinduite, etc.) pouvant avoir des répercussions thérapeutiques importantes. D’autres émetteurs de positons seront disponibles à terme, comme le 64Cu (période de 12 heures), le 124I (période de 4,1 jours) et le 86Y (période de 15 heures) dans des indications comme l’immuno-TEP ou la dosimétrie pré-thérapeutique qui ne peuvent être développées avec le 18F du fait de sa période trop courte. Dans le cadre des applications thérapeutiques, la radiothérapie interne, longtemps limitée au cancer thyroïdien différencié, s’est étendue à beaucoup d’autres types de cancers du fait de la disponibilité de nouveaux vecteurs de radionucléides comme les anticorps monoclonaux (radioimmunothérapie) ou des peptides (radiopeptide-thérapie), de nouvelles méthodes de ciblage tumoral et de nouveaux radionucléides, en particulier des émetteurs de particules alpha (alpha-thérapie). Toutes ces évolutions technologiques ont bien sûr une implication en radioprotection en ce qui concerne l’exposition du personnel de médecine nucléaire, de l’entourage familial proche et plus généralement de l’environnement. Actuellement l’essentiel des dispositions règlementaires s’applique au 99mTc pour les applications diagnostiques et au 131I en thérapie, elles ont été étendues en 2001 au 18F dans le cadre des contraintes de radioprotection liées à l’utilisation de FDG. Concernant l’exposition de l’entourage proche du malade, la règlementation est actuellement limitée au seuil d’activité de 740 MBq de 131I qui impose un confinement hospitalier du malade. Une indication indirecte de cette exposition peut être donnée par le débit d’exposition (en µSv/h) à 1 mètre pour une source de 1 MBq. Si on prend en compte la valeur du débit et la période physique, il apparaît, qu’à activité identique, le 52Fe, le 86Y et le 124I sont des radionucléides émetteurs de positons qui présentent un débit élevé à la sortie du service et pendant les heures qui suivent l’injection. Concernant l’exposition de l’environnement, la règlementation actuelle impose une concentration radioactive, à l’émissaire de l’établissement, de 1000 Bq/L pour le 99mTc et de 100 Bq/L pour le 131I. Pour les nouveaux radionucléides, d’autres hypothèses que celle d’une concentration permanente de 100 Bq/L doivent être retenues et des études d’impact, similaires à celles pratiquées dans le domaine du nucléaire devraient être effectuées.

In this context, we investigated the reduction of carbonyl functions from diketal dilactams to access both 6 diketal amino-lactams and cyclams homologues of 6 diketal diamines. Diketal dilactams were produced from hydroxyamidoketals, which are obtained from corresponding β-aminoalcools. We have shown that the efficiency of the reduction was dependent on both the nature of the substituant and the stereochemistry of the ketal OMe group. Diketal amino-lactames were obtained at low concentrations in diketal dilactams (3 to 14 × 10-3 M) whereas diketal diamines were obtained at high concentrations (15 to 30 × 10-3 M). Among the different molecules synthetised, diketal diamines led to the maximum complexation rate of 18% with technetium radioelement-99m (99mTc). We also optimised a new route for the synthesis of dimeric compounds; 5 bis-diketal amino-lactames and 2 bis-diketal diamines were isolated. These latter compounds are homologues of bicyclams which display anti-viral activity.; Nous avons etudié la réduction des fonctions carbonyles de dicétals dilactames qui sont engendrées à partir d'hydroxyamidoacétals. L'étude des propriétés complexantes des différentes molécules synthétisées vis-à-vis du radioélément technetium-99m (99mTc) montre que seuls les dicétals diamines se prêtent à une complexation avec des pourcentages maximum de 18%. Les études d'accès aux composés dimères ont permis d'isoler 5 bis-dicétals amino-lactames et 2 bis-dicétals diamines , homologues des bicyclames à activité anti-virale.

Nous avons montré que la facilité de la réduction dépend de la nature des substituant et de la stéréochimie cis ou trans des groupements OMe: les dérivés cis étant plus faciles à réduire que les dérivés trans. Les dicetals amino-lactames sont obtenus à des concentrations faibles( 3 à 14x10-3M) et les dicétals diamines à des concentrations plus élevées (15 à 30 x 10-3M). L'étude des propriétés complexantes des différentes molécules synthétisées vis-à-vis du radioélément technetium-99m (99mTc) montre que seuls les dicétals diamines se prêtent à une complexation avec des pourcentages maximum de 18%. Les études d'accès aux composés dimères ont permis d'isoler 5 bis-dicétals amino-lactames et 2 bis-dicétals diamines , homologues des bicyclames à activité anti-virale.

Diplôme : Dr. Ing.; For the safety assessment of underground radioactive waste disposal conducted by Andra, it is required to estimate the behaviour of radionuclides in soils for the next ten thousand years. Within this time scale, soils would probably evolve from their actual status, according to climate changes. It is then necessary to assess these changes in order to predict the behaviour of the radiochemicals that would enter the soils. We present a pedological forecast method for the assessment of a specific case (i.e. limestone plateau), undergoing periglacial conditions. Then we study the retention of mobile radionuclides, iodine (I) and technetium (Tc) on the forecast periglacial soils. The method relies on the study of analogous soils, sampled in regions with similar geochemical (calcareous) and topographical contexts but undergoing a cold climate which mimics future periglacial conditions of the reference region (steppe-tundra or boreal forest biomes). We have added to the cold and dry climatic series a cold and humid biome (montane strata). The organic matter compartment (OM), considered as an indicator of the effects of the climate on the soils, was thoroughly characterised and taken into account. The analogous soils sampled have developed under boreal forest, cold steppe and subalpine conditions from limestone or calcareous parent materials. The observed soils were eutric Leptosols under boreal forest, and Chernozems under cold steppe. In both biomes, under hydromorphic conditions, OM accumulated in the soil and was high in aromatic molecules in the humified fraction. In the montane strata, the leaching of carbonates was strong enough to allow a diversification of soil genesis: from humic Leptosols to dystric Luvisols. The OM of these soils was different from that of the soils from dry biomes. This study has confirmed that the main differences between soils were observed between the humid and cold climate (montane) on one side and the dry and cold (cold steppe and boreal) on the other. Evolution scenarios constructed from these results indicated that the periglacial conditions would probably favour the persistence of soils still similar to the present ones in the reference region, especially during the boreal stage. However these soils would display higher OM content. In this biome, soils would be mainly rendzic Leptosols and eutric (or calcaric) Cambisols as presently, but with a higher OM content in the surface horizon because of the forest cover. In the steppe-tundra biome, a pergelisol would favour hydromorphy in most soils (stagnic Cryosols). In both biomes, hydromorphic soils, developped on marly material could display very high OM content (fen). Maps summarising these scenarios have been edited for each biome. The consequences of these pedological changes on the behaviour of mobile radionuclides were studied through the retention of Tc and I in the analogous soils at field capacity in batch experiments. The experiment showed that the dehydration of soils could favour the retention of both elements under aerobic conditions, although the latter are not usually favourable to Tc and I retention (Kd > 1 l.kg-1 in analogous soils of boreal forest after one year of contact time). These results were used to establish maps of the retention of each element for both periglacial biomes. It is suggested that Tc would be retained in the soils in the cold steppe soils whereas I would be more retained in soils of the boreal biome. In both biomes, soils with the highest retention towards I and Tc would be peaty soils located in the bottom of marly valleys. Special attention was paid therefore to peaty soils : we studied the behaviour of I in situ in a peat bog. I had accumulated at the bog surface (Kd was multiplied by 6 within 15 years) and was found to be associated to polyphenolic substances from the humified peat. Carex sp was identified as the species that accumulated the highest contents of I amongst the vegetation of the bog. The proposed method underlain by analogy and laboratory retention studies should help to understand and forecast the behaviour of the long-lived radionuclides. It should help in the determination of the site specific parameters for the dose computation of safety assessment in a cold biosphere scenario.; Dans le cadre de l'étude de sûreté du stockage en couche profonde de déchets radioactifs à vie longue menée par l'Andra, il est nécessaire de prévoir le comportement des polluants dans les sols qui seront présents dans plusieurs dizaines de milliers d'années, en réponse aux changements climatiques. Une méthode de pédologie prospective est proposée pour répondre à cette problématique et est appliquée au cas particulier d'une région de plateau calcaire, le Barrois, pour un refroidissement climatique entraînant une glaciation et pour des radionucléides supposés mobiles (iode (I) et technétium (Tc)). Cette méthode repose sur l'étude de sols analogues qui proviennent de régions similaires à la région de référence, en termes géologiques et topographiques mais dont le climat actuel pourrait correspondre au climat périglaciaire futur de la région de référence. Deux biomes principaux de climats froids et secs, steppe-toundra et forêt boréale, ont été choisis, complétés par un biome froid et humide (étage montagnard) afin de couvrir une large gamme de refroidissements climatiques. Les sols analogues ont été prélevés en forêt boréale, en steppe froide et à l'étage montagnard sur des roches-mères calcaires. Des scénarios d'évolution ont été proposés et des cartes représentant ces scénarios d'évolution ont été éditées pour chaque biome. Compte tenu de son importance en tant que produit de la pédogenèse et facteur de rétention de certains éléments, le compartiment matière organique (MO) a été pris comme indicateur de l'impact du climat sur les sols. La rétention de I et de Tc dans les sols analogues a été caractérisée à partir d'incubations de terres à la capacité au champ. Enfin, le suivi de l'évolution in situ de la rétention de l'iode stable dans une tourbière a permis de préciser le devenir à moyen terme de cet élément en présence de matière organique en conditions froides. Les résultats montrent des sols brunifiés calcaires peu épais et peu évolués en forêt boréale et des chernozems en steppe froide. Dans ces deux biomes, les sols hydromorphes présentent une accumulation de MO aromatique dans la fraction humifiée. A l'étage montagnard, la décarbonatation est assez importante pour permettre le développement de sols plus variés, offrant une MO différente de celle des autres biomes. En biome froid, l'humidité du climat détermine donc les différences de fonctionnement entre les sols. Les scénarios d'évolution établis à partir de ces résultats indiquent que les conditions périglaciaires favoriseraient la présence de sols proches des sols actuels (Leptosols rendziques et des Cambisols eutriques ou calcariques), notamment en biome boréal. En biome de steppe-toundra, la présence d'un pergélisol pourrait favoriser l'hydromorphie. Dans les deux biomes, des sols hydromorphes formés sur matériau marneux seraient susceptibles de contenir des teneurs élevées de MO (tourbières alcalines ou peu acides). Nous avons montré que la dessiccation du sol pourrait favoriser la rétention de I et Tc sur la phase solide en conditions aérobies (Kd > 1 l kg-1 dans les sols de forêt boréale après un an d'incubation). Tc serait immobilisé dans les sols de steppe froide tandis que I serait davantage retenu dans les sols en forêt boréale. Dans ces deux biomes, les sols les plus favorables à la rétention de I et de Tc seraient des sols hydromorphes organiques situés dans les fonds des vallées sur matériaux marneux. Ces résultats ont été traduits en cartes de rétention pour chacun de ces éléments, pour les deux biomes périglaciaires. Enfin, dans la tourbière, l'accumulation de I à la surface (Kd multiplié par 6 en 15 ans) a été associée à la présence de composés polyphénoliques dans la tourbe en cours d'humification. Les cypéracées (Carex sp.) ont été identifiées parmi les espèces végétales présentes sur le site, comme étant celles qui accumulent les plus grandes quantités d'iode. En conclusion, la démarche prospective reposant sur l'analogie et des études en laboratoire et in situ a permis de prévoir l'évolution des sols sous l'influence d'un refroidissement climatique et de prévoir le devenir de radioéléments à vie longue en fournissant les valeurs des paramètres pour des biosphères froides devant être utilisés dans les codes de calculs de dose des études de sûreté.

A 21-year-old patient underwent surgery in 1987 for hydatid cyst of segment VIII of the liver. Six years later, the patient developed a cold thyroid nodule. During the thyroid operation, needle aspiration and cytology study confirmed the diagnosis of hydatid cyst. Resection of the prominent dome was performed. In 1996, the patient developed cervical recurrence. Pericytectomy was attempted and during the procedure a wound of the subclavian artery required sternoclavicular disarticulation. Hydatid cyst of the thyroid gland is rare, but should be considered in patients living in endemic zones who develop an anterior neck tumor. Cystectomy, isthmolobectomy or sub-total thyroidectomy may be indicated. Resection of the prominent dome may be required in the event of recurrence.

From a retrospective study carrying on 8 patients operated for sporadic parathyroid adenoma, we underline the place of medical imagery which allowed the detection of parathyroid adenoma in 7 cases. For all our patients the exploration of thyroid space was bilateral, seeking parathyroid glands. The authors discuss the stages and the various processes of the surgical strategy for parathyroid glands' exploration.

The goal of this study was to evaluate the technical feasibility of sentinel node biopsy in breast cancer and its capacity to predict axillary node status.Between February 2000 and June 2001, 70 patients with invasive breast carcinomas referred to the cancer center of Dijon, underwent lymphatic mapping (patent blue dye and lymphoscintigraphy with a gamma probe) and sentinel node biopsy followed by axillary clearance (Berg's level 1 and 2).A sentinel node was identified in 94.26% of the patients. We noted 2 false negative sentinel lymph nodes. The sentinel lymph node was involved in 32.8% of the patients and was the only node involved in 14.2% of the patients.This study confirms the feasibility of sentinal node mapping, emphasizing the advantage of using two methods (patent blue dye and gamma probe). A learning curve is required. This procedure is not adapted for tumors larger than 3 cm because of a greater risk of sentinel node involvement and the need for a complete procedure with an axillary clearance.

The detection of the sentinel lymph node is one of the most significant surgical advance in cancer research. This technique allow to decrease the morbidity of a surgical gesture which can be noxious in itself even without bringing a profit in survival. In the gynaecological cancers the detection of the sentinel lymph node is still at the stage of feasibility. An evaluation of the technique and the applicability in these cancers is mandatory in view of the prognostic value of the nodal involvement in these patients.
