2. Estimating the completeness of physician billing claims for diabetes case ascertainment: a multiprovince investigation

IntroductionPrevious research has suggested that how physicians are paid may affect the completeness of billing claims for estimating chronic disease. The purpose of this study is to estimate the completeness of physician billings for diabetes case ascertainment. MethodsWe used administrative data from eight Canadian provinces covering the period 1 April 2014 to 31 March 2016. The patient cohort was stratified into two mutually exclusive groups based on their physician remuneration type: fee-for-service (FFS), for those paid only on that basis; and non-fee-for-service (NFFS). Using diabetes prescription drug data as our reference data source, we evaluated whether completeness of disease case ascertainment varied with payment type. Diabetes incidence rates were then adjusted for completeness of ascertainment. ResultsThe cohort comprised 86 110 patients. Overall, equal proportions received their diabetes medications from FFS and NFFS physicians. Overall, physician payment method had little impact upon the percentage of missed diabetes cases (FFS, 14.8%; NFFS, 12.2%). However, the difference in missed cases between FFS and NFFS varied widely by province, ranging from −1.0% in Nova Scotia to 29.9% in Newfoundland and Labrador. The difference between the observed and adjusted disease incidence rates also varied by province, ranging from 22% in Prince Edward Island to 4% in Nova Scotia. ConclusionThe difference in the loss of cases by physician remuneration method varied across jurisdictions. This loss may contribute to an underestimation of disease incidence. The method we used could be applied to other chronic diseases for which drug therapy could serve as reference data source.

IntroductionDes recherches antérieures ont suggéré que le mode de rémunération des médecins peut avoir une influence sur l’exhaustivité des demandes de remboursement lors de l’estimation des maladies chroniques. Le but de cette étude est d’estimer l’exhaustivité des données sur la facturation des médecins dans l’optique de déterminer des cas de diabète. MéthodologieNous avons utilisé les données administratives de huit provinces canadiennes pour la période du 1er avril 2014 au 31 mars 2016. Nous avons divisé la cohorte de patients en deux groupes mutuellement exclusifs en fonction du mode de rémunération de leur médecin : médecins rémunérés à l’acte pour ceux payés uniquement de cette façon et médecins non rémunérés à l’acte pour les autres. À l’aide des données sur les ordonnances de médicaments hypoglycémiants (source de nos données de référence), nous avons évalué si les cas de maladie avaient été déterminés avec la même exactitude en fonction du mode de rémunération. Nous avons ensuite corrigé les taux d’incidence du diabète afin d’optimiser l’exhaustivité des cas déterminés. RésultatsLa cohorte comprenait 86 110 patients. Dans l’ensemble, des proportions égales de patients ont reçu leurs médicaments hypoglycémiants de la part de médecins rémunérés à l’acte et de médecins non rémunérés à l’acte. Globalement, le mode de rémunération des médecins a eu peu d’effet sur le pourcentage de cas de diabète omis (14,8 % chez les médecins rémunérés à l’acte contre 12,2 % chez les médecins non rémunérés à l’acte). Toutefois, la différence de cas omis entre les médecins rémunérés à l’acte et ceux non rémunérés à l’acte était très variable d’une province à l’autre, allant de -1,0 % en Nouvelle-Écosse à 29,9 % à Terre-Neuve-et-Labrador. La différence entre les taux observés d’incidence de la maladie et les taux corrigés était également variable d’une province à l’autre, allant de 22 % à l’Île-du-Prince-Édouard à 4 % en Nouvelle-Écosse. ConclusionLa différence entre le nombre de cas omis selon le mode de rémunération des médecins est variable d’une province à l’autre. Cette perte de données contribue probablement à une sous-estimation de l’incidence de la maladie. Il serait possible d’appliquer la méthode que nous avons utilisée à d’autres maladies chroniques pour lesquelles des données de traitement pharmacologique pourraient servir de données de référence.

Claude-Henri Watelet’s 1774 Essai sur les jardins (Essay on Gardens) was the first French garden treatise to enter the picturesque garden debate, set into motion in England with the 1748 publication of William Gilpin’s A Dialogue Upon the Gardens […] at Stow, a dialogic garden tour which delineated the aesthetic principles of the picturesque, advancing a formalist approach to the visual apprehension of the landscape. Watelet’s Essay on Gardens, however, exemplified the affective development of the garden treatise in the second half of the eighteenth century, which featured a textual, oftentimes sentimental traverse of the picturesque landscape, evoking a sensation-imbued garden walk, or in this case, ferry crossing. Watelet’s Essay describes the new domain of landscape architecture as inhabited by artists, poets, and designers, or décorateurs, who conceived gardens as pictures, and the garden walk as a series of volatile, shifting tableaux. The picturesque garden ramble, vivified in Watelet’s ekphrastic prose, could thereafter be traversed and re-traversed by the reader regardless of their location in space-time. In Essai sur les jardins, Watelet crosses the Seine by boat, en route to Paris, when he serendipitously discovers his future garden isle, the Moulin Joly. Watelet’s gaze errs along the otherworldly pastorale, seizing upon what would become his future ferme ornée, or embellished farm, catching sight of its flowing waters and verdant groves, fortuitously up for sale. This fleeting glimpse, or coup d’œil, in which the fugitive tableau is instantaneously imprinted onto the retina, enabled the garden visitor an immediate entrée into the terrain of the subconscious, embarking upon an ever-changing traverse of the emotions suggested by the imagery, symbolism, and vocabulary of the landscape garden.

Developing as a scholar is a critical aspect of graduate school. In this article, we use an autoethnographic approach to explore our experiences in a newly redeveloped interdisciplinary graduate course. The course was designed to emphasize interdisciplinary scholarship and to encourage students’ awareness of their own developing epistemological, ontological, and axiological commitments. In this paper, we identify themes that arose from the final class paper, in which the students/researchers reflected on our development as emerging scholars. Through this course we developed an understanding of the value of interdisciplinarity, the cultural embeddedness of knowledge, and our own responsibilities as researchers in relation to social structures of power and marginalization. Our analyses of our experiences suggests that reflections on our own positionality and axiology are important for developing our identities as emerging scholars and that becoming a scholar may be more about the process than an end goal. We conclude with a discussion of how our learning in this course may be relevant for other instructors and emerging scholars.

6. Does lack of CO2 control in environmental chambers influence the strength and stiffness of hydraulic lime mortars?

This study investigates the influence of carbon dioxide (CO2) control in environmental chambers, where hydraulic lime mortar specimens are typically cured before strength testing according to EN1015-11:2019. To do this, three sets of identical mortar samples were cast. Two sets were cured in an environmental chamber that offered the same temperature and relative humidity but different CO2 conditions. In the first chamber, carbonation reactions used the ambient CO2 trapped in the hermetically sealed chamber air. The second chamber was connected to an external carbon dioxide supply, which maintained a constant CO2 concentration, above ambient laboratory levels, during curing. A third set of samples was cured under laboratory conditions to serve as a reference. Following the 35-day curing period, the dynamic elastic modulus and flexural and compressive strengths of sample sets were obtained. Subsequent X-ray diffraction analysis gives additional insights into the material composition for each curing regime. Results indicate that controlling carbon dioxide to above ambient levels leads to noticeably higher stiffness and strength in samples. This suggests that implementing CO2 control as a part of standard curing conditions may enable more reliable estimates of strength and stiffness development in lime mortars, with respect to laboratory conditions.

7. Examining the Use of Twitter in Online Classes: Can Twitter Improve Interaction and Engagement?

Student engagement promotes communication and knowledge acquisition, a concept that is challenged in the online environment as few opportunities exist to physically connect instructors and learners. Limited research suggests that social media is a tool that can positively impact student engagement in the online classroom, which is especially relevant in the case of the COVID-19 pandemic and learning formats transitioning online. Specifically, Twitter, a favoured format for sharing news, entertainment, and professional networking, may provide a platform and an opportunity for engagement between students and the instructor outside of the traditional, formal classroom setting. This research explores how postsecondary students enrolled in two introductory online self-directed asynchronous courses used social media tools for personal, professional, and academic purposes and how Twitter, as a course evaluation requirement, contributed to interaction and engagement. Relying on 104 pre- and 34 post-semester surveys, our analysis revealed that while Twitter was not used as widely as other social media platforms, a notable proportion of students shared positive perceptions about Twitter’s use. Further analysis revealed some polarizing results with recommendations for successfully implementing Twitter in online learning. La participation des étudiants et des étudiantes favorise la communication et l’acquisition des connaissances, concept qui est mis au défi dans l’environnement en ligne du fait qu’il existe peu de possibilités de connecter physiquement les enseignants et les enseignantes avec les apprenants et les apprenantes. Des recherches limitées suggèrent que les médias sociaux sont un outil qui peut affecter positivement la participation des étudiants dans les salles de classe en ligne, ce qui est particulièrement pertinent dans le cas de la pandémie de la COVID-19 et des formats d’apprentissage qui sont passés en ligne. En particulier, Twitter, un format privilégié pour partager les nouvelles, les distractions et le réseautage professionnel, pourrait présenter une plate-forme et une occasion de participation entre les étudiants et étudiantes et les instructeurs et instructrices hors du cadre traditionnel formel de la salle d classe. This article explores the role of place-based philanthropy in rural community development through a case study of the Skeena watershed. The Skeena is an ecologically significant region in Northwest British Columbia that is confronting the complex and layered forces of change being experienced by many rural regions in Canada. Through qualitative interviews and document analyses, the article illustrates how a robust ecosystem of environmental community-based organizations (CBOs), funded by philanthropic capital, is extending beyond traditional environmental advocacy to fill important structural gaps in community development. Though pressed by capacity issues, the sector is shifting towards highly integrated and collaborative responses to development pressures and charting alternative pathways for development in the region. The complexity and scope of pressures in the Skeena offer insights for other rural regions and the dynamic potential and challenges associated with place-based philanthropy in community and regional development processes.

9. Multisystem inflammatory syndrome in children in Canada

This article provides a summary of the epidemiology of multisystem inflammatory syndrome in children (MIS-C) cases reported nationally in Canada by provincial and territorial health authorities. Multisystem inflammatory syndrome in children is a post-viral inflammatory syndrome that temporally follows coronavirus disease 2019 (COVID-19). Symptoms may include fever, abdominal pain, vomiting, diarrhea, skin rash and other signs of inflammation. In Canada, MIS-C is rare, with 269 cases reported to the Public Health Agency of Canada between March 11, 2020 and October 2, 2021. One hundred forty-two (53%) of these cases were lab-confirmed COVID-19 cases or epidemiologically-linked with COVID-19 cases. Cases have been reported in infants as young as one week to youth as old as 18 years, with a median age of six years. Cases were more likely to occur in males than females (58% vs 42%, respectively; p=0.006). Almost all MIS-C cases (99%) required hospitalization and 36% required intensive care unit admission. No deaths have been reported to date. The time trend of MIS-C aligns with the incidence rate time trend of COVID-19 reported in children, with a two to six-week lag.

Cet article présente un résumé de l’épidémiologie des cas de syndrome inflammatoire multisystémique chez les enfants (MIS-C) déclarés à l’échelle nationale au Canada par les autorités sanitaires provinciales et territoriales. Le syndrome inflammatoire multisystémique chez les enfants est un syndrome inflammatoire post-viral qui suit la maladie à coronavirus 2019 (COVID-19). Les symptômes peuvent comprendre de la fièvre, des douleurs abdominales, des vomissements, de la diarrhée, des éruptions cutanées et d’autres signes d’inflammation. Au Canada, le MIS-C est rare, avec 269 cas signalés à l’Agence de la santé publique du Canada entre le 11 mars 2020 et le 2 octobre 2021. Cent quarante-deux (53 %) de ces cas étaient des cas de COVID-19 confirmés en laboratoire ou liés épidémiologiquement à des cas de COVID-19. Des cas ont été signalés chez des nourrissons d’à peine une semaine et des jeunes de 18 ans, l’âge médian étant de six ans. Les cas étaient plus susceptibles de se produire chez les garçons que chez les filles (58 % contre 42 %, respectivement; p = 0,006). Presque tous les cas de MIS-C (99 %) ont nécessité une hospitalisation et 36 % une admission en unité de soins intensifs. Aucun décès n’a été signalé à ce jour. La tendance temporelle du MIS-C s’aligne sur la tendance temporelle du taux d’incidence du COVID-19 rapporté chez les enfants, avec un décalage de deux à six semaines.

Contexte : Le programme de surveillance circumpolaire internationale assure la surveillance de cinq maladies bactériennes invasives : les pneumococcies invasives, les infections invasives à streptocoque du groupe A, l’Haemophilus influenzae, les infections invasives à méningocoque et les streptocoques du groupe B. Les maladies bactériennes invasives ont une charge de morbidité plus élevée dans les populations nordiques que dans le reste du Canada. Méthodologies : Afin de décrire l’épidémiologie des maladies bactériennes invasives dans le nord du Canada de 1999 à 2018, les données relatives aux pneumococcies invasives, aux infections invasives à streptocoque du groupe A, à l’Haemophilus influenzae, aux infections invasives à méningocoque et aux streptocoques du groupe B ont été extraites du programme de surveillance circumpolaire internationale et du Système canadien de surveillance des maladies à déclaration obligatoire (SSMDO) et analysées. Résultats : Les taux d’incidence annualisés des pneumococcies invasives, des infections invasives à streptocoque du groupe A, de l’Haemophilus influenzae, des streptocoques du groupe B et des infections invasives à méningocoque étaient respectivement de 23,3, 10,5, 8,9, 1,9 et 1,1 pour 100 000 habitants. L’incidence de la pneumococcie invasive, de l’infection invasive à streptocoque du groupe A et de l’Haemophilus influenzae de type b (Hib) était respectivement 2,8, 3,2 et 8,8 fois plus élevée dans le Nord du Canada que dans le reste du pays. Les taux de maladie ont diminué de manière statistiquement significative pour la pneumococcie invasive (β = -0,02) et ont augmenté de manière statistique pour l’infection invasive à streptocoque du groupe A (β = 0,08) et l’Haemophilus influenzae sérotype a (β = 0,04) pendant la période d’étude. Dans le Nord du Canada, les taux d’incidence annualisés des pneumococcies invasives, des infections invasives à streptocoque du groupe A et de l’Haemophilus influenzae étaient statistiquement plus élevés chez les résidents autochtones que chez les résidents non autochtones. Les taux d’incidence les plus élevés ont été enregistrés dans les groupes d’âge très jeunes et plus âgés. Conclusion : Les maladies bactériennes invasives représentent une charge de morbidité importante pour les populations nordiques du Canada. Les peuples autochtones, les enfants et les personnes âgées sont particulièrement exposés.

Background: The International Circumpolar Surveillance (ICS) program conducts surveillance on five invasive bacterial diseases: pneumococcal disease (IPD), group A streptococcus (iGAS), Haemophilus influenzae (Hi), meningococcal disease (IMD) and group B streptococcus (GBS). Invasive bacterial diseases have a higher burden of disease in northern populations than the rest of Canada. Methods: To describe the epidemiology of invasive bacterial diseases in northern Canada from 1999 to 2018, data for IPD, iGAS, Hi, IMD and GBS were extracted from the ICS program and the Canadian Notifiable Diseases Surveillance System (CNDSS) and analyzed. Results: The annualized incidence rates for IPD, iGAS, Hi, GBS and IMD were 23.3, 10.5, 8.9, 1.9 and 1.1 per 100,000 population, respectively. The incidence of IPD, iGAS and Hi serotype b were 2.8, 3.2 and 8.8 times higher, respectively, in northern Canada than in the rest of Canada. Rates of disease decreased statistically significantly for IPD (β=−0.02) and increased statistically for iGAS (β=0.08) and Hi serotype a (β=0.04) during the study period. In Northern Canada, the annualized incidence rates for IPD, iGAS and Hi were statistically higher for Indigenous residents than for non-Indigenous residents. The highest incidence rates were among the very young and older age groups. Conclusion: Invasive bacterial diseases represent a high burden of disease in Canada’s northern populations. Indigenous peoples, children and seniors are particularly at risk.

13. Describing the burden of diphtheria in Canada from 2006 to 2017, using hospital administrative data and reportable disease data

Background: Canada has maintained a low incidence of toxigenic diphtheria since the 1990s, supported by continued commitment to publicly funded vaccination programs. Objective: To determine whether hospitalization data, complemented with notifiable disease data, can describe the toxigenic respiratory and cutaneous diphtheria burden in Canada, and to assess if Canada is meeting its diphtheria vaccine–preventable disease-reduction target of zero annual cases of locally transmitted respiratory diphtheria. Methods: Diphtheria-related hospital discharge data from 2006 to 2017 were extracted from the Discharge Abstract Database (DAD), and diphtheria case counts for the same period were retrieved from the Canadian Notifiable Disease Surveillance System (CNDSS), for descriptive analyses. As data from the province of Québec are not included in the DAD, CNDSS cases from Québec were excluded. Results: A total of 233 diphtheria-related hospitalizations were recorded in the DAD. Of these, diphtheria was the most responsible diagnosis in 23. Half the patients were male (52%), and 57% were 60 years and older. Central region (Ontario) accounted for the most discharge records (61%), followed by Prairie region (Alberta, Manitoba and Saskatchewan; 23%). Cutaneous diphtheria accounted for 43% of records, and respiratory diphtheria accounted for 3%, with the remainder being other diphtheria complications or site unspecified. Two records with diphtheria as the most responsible diagnosis resulted in inpatient deaths. Eighteen cases of diphtheria were reported through CNDSS. Cases occurred in all age groups, with the largest proportions among those aged 20 to 59 years (39%) and those aged 19 years and younger (33%). Cases were only reported in the Prairie (89%) and West Coast (British Columbia; 11%) regions. Conclusion: Hospital administrative data are consistent with the low incidence of diphtheria reported in CNDSS, and a low burden of respiratory diphtheria in Canada. Although Canada appears to be on track to meet its disease-reduction target, information on endemic transmission is not available.

Contexte : Le Canada a maintenu une faible incidence de la diphtérie toxigène depuis les années 1990 grâce à un engagement continu envers les programmes de vaccination financés par l’État. Objectif : Déterminer si les données d’hospitalisation, complétées par les données sur les maladies à déclaration obligatoire, peuvent décrire le fardeau de la diphtérie respiratoire et cutanée toxigène au Canada, ainsi qu’estimer si le Canada est en voie d’atteindre sa cible de réduction des maladies évitables par la vaccination pour la diphtérie, qui est de zéro cas par année de diphtérie respiratoire transmise localement. Méthodes : Les données sur les congés des personnes hospitalisées liés à la diphtérie de 2006 à 2017 ont été extraites de la base de données sur les congés des patients (BDCP), et les données sur les cas de diphtérie pour la même période ont été extraites du Système canadien de surveillance des maladies à déclaration obligatoire (SCSMDO), afin de procéder à des analyses descriptives. Étant donné que les données de la province du Québec ne sont pas incluses dans la BDCP, les cas du SCSMDO du Québec ont été exclus de l’analyse. Résultats : Au total, 233 hospitalisations liées à la diphtérie ont été enregistrées dans la BDCP. De ce nombre, la diphtérie était le diagnostic principal dans 23 cas. La moitié des patients étaient des hommes (52 %) et 57 % étaient âgés de 60 ans et plus. La région centrale (Ontario) comptait le plus grand nombre d’enregistrements de congés des patients (61 %), suivie de la région des Prairies (Alberta, Manitoba et Saskatchewan [23 %]). La diphtérie cutanée représentait 43 % des enregistrements, la diphtérie respiratoire 3 %, et le reste était d’autres complications liées à la diphtérie ou la diphtérie dont la localisation n’était pas spécifiée. Deux patients pour lesquels la diphtérie était le diagnostic principal ont entraîné le décès des patients hospitalisés. Dix-huit cas de diphtérie ont été déclarés par l’entremise du SCSMDO. Les cas étaient observés dans tous les groupes d’âge, dont la plus grande proportion de cas chez ceux de 20 à 59 ans (39 %) et ceux de 19 ans et moins (33 %). Les cas n’ont été rapportés que dans les Prairies (89 %) et les régions de la côte Ouest (Colombie-Britannique [11 %]). Conclusion : Les données administratives des hôpitaux sont conformes à la faible incidence de diphtérie déclarée dans le SCSMDO et le faible fardeau de la diphtérie respiratoire au Canada. Bien que le Canada semble en bonne voie pour atteindre sa cible de réduction des maladies, l’information sur la transmission endémique n’est pas disponible.

IntroductionHealth insurance registries, which capture insurance coverage and demographic information for entire populations, are a critical component of population health surveillance and research when using administrative data. Lack of standardization of registry information across Canada’s provinces and territories could affect the comparability of surveillance measures. We assessed the contents of health insurance registries across Canada to describe the populations covered and document registry similarities and differences. MethodsA survey about the data and population identifiers in health insurance registries was developed by the study team and representatives from the Public Health Agency of Canada. The survey was completed by key informants from most provinces and territories and then descriptively analyzed. ResultsResponses were received from all provinces; partial responses were received from the Northwest Territories. Demographic information in health insurance registries, such as primary address, date of birth and sex, were captured in all jurisdictions. Data captured on familial relationships, ethnicity and socioeconomic status varied among jurisdictions, as did start and end dates of coverage and frequency of registry updates. Identifiers for specific populations, such as First Nations individuals, were captured in some, but not all jurisdictions. ConclusionHealth insurance registries are a rich source of information about the insured populations of the provinces and territories. However, data heterogeneity may affect who is included and excluded in population surveillance estimates produced using administrative health data. Development of a harmonized data framework could support timely and comparable population health research and surveillance results from multi-jurisdiction studies.

MéthodologieL’équipe de l’étude et des représentants de l’Agence de la santé publique du Canada ont élaboré une enquête sur les caractéristiques individuelles et les données sur la population recueillies dans les registres d’assurance-maladie. Les réponses des informateurs clés des provinces et territoires qui ont répondu à l’enquête ont été analysées de façon descriptive. RésultatsToutes les provinces ont répondu à l’enquête et les Territoires du Nord-Ouest ont fourni des réponses partielles. Certaines caractéristiques individuelles figurant dans les registres d’assurance-maladie (l’adresse principale, la date de naissance et le sexe) sont saisies dans toutes les provinces et tous les territoires. Les données disponibles sur les liens de parenté, l’origine ethnique et le statut socioéconomique sont variables selon les provinces et territoires, tout comme les dates de début et de fin de couverture et la fréquence de mise à jour du registre. L’information sur des populations particulières, comme les membres des Premières Nations, est saisie dans certaines provinces et certains territoires seulement. Contexte : L'Agence de la santé publique du Canada (l'Agence) a amélioré la surveillance de la rougeole depuis 1998, année où la transmission de la rougeole a été éliminée au Canada. L'objectif de ce rapport national annuel de surveillance de la rougeole est de fournir un résumé épidémiologique de l'activité de la rougeole déclarée au Canada pour 2019 afin de fournir des données probantes pour appuyer la vérification continue du statut d'élimination de la rougeole au Canada. Méthodes : Les données de surveillance de la rougeole se trouvent dans la base de données du Système canadien de surveillance de la rougeole et de la rubéole (SCSRR). Des analyses descriptives de la démographie et des facteurs de risque ont été effectuées. Les caractéristiques de l'éclosion ont été résumées et des analyses génotypiques ont été effectuées. Les données de surveillance, de laboratoire et de couverture vaccinale de 2019 ont été utilisées pour évaluer la situation du Canada par rapport aux critères essentiels de l'Organisation panaméricaine de la santé (OPS) pour la vérification de l'élimination de la rougeole. Résultats : En 2019, 113 cas de rougeole ont été signalés au Canada (taux brut d'incidence de 3,0 cas pour 1 000 000 personnes). Parmi ces cas, 42 (37 %) ont été importés au Canada, et 12 (29 %) ont entraîné une transmission supplémentaire. Les nourrissons de moins d'un an présentaient le taux d'incidence par âge le plus élevé, soit 13,1 cas pour 1 000 000 personnes. Seulement 29 % des cas contenaient une ou plusieurs doses documentées de vaccin contre la rougeole. Un cinquième (19 %) des cas ont été hospitalisé; aucun décès n'a été signalé. Des renseignements sur le génotype étaient disponibles pour 100 % des éclosions signalées en 2019 et 90 % des cas de rougeole non liés aux éclosions. Parmi les cas pour lesquels des renseignements sur le génotype étaient disponibles, 27 % étaient de type B3 et 73 % étaient de type D8. Conclusion : Même si seulement trois des quatre critères essentiels de l'OPS pour l'élimination de la rougeole ont été respectés entièrement ou partiellement, les données ne démontrent pas le rétablissement de la transmission endémique du virus de la rougeole.

Background: The Public Health Agency of Canada (PHAC) has conducted enhanced measles surveillance since 1998, the year endemic measles transmission was eliminated in Canada. The objective of this annual national measles surveillance report is to provide an epidemiologic summary of measles activity reported in Canada for 2019 in order to provide evidence to support the continued verification of Canada’s measles elimination status. Methods: Measles surveillance data are housed in the Canadian Measles and Rubella Surveillance System (CMRSS) database. Descriptive analyses of demographics and risk factors were performed. Outbreak characteristics were summarized and genotypic analyses conducted. Surveillance, laboratory and vaccine coverage data for 2019 were used to assess Canada’s status against the Pan American Health Organization (PAHO) essential criteria for the verification of measles elimination. Results: In 2019, 113 measles cases were reported in Canada (crude incidence rate of 3.0 cases per 1,000,000 population). Of these cases, 42 (37%) were imported into Canada, and of the imported cases, 12 (29%) resulted in further transmission. Infants younger than one year had the highest age-specific incidence rate at 13.1 cases per 1,000,000 population. Only 29% of cases had one or more documented doses of measles-containing vaccine. One-fifth (19%) of cases were hospitalized; no deaths were reported. Genotype information was available for 100% of outbreaks reported in 2019 and 90% of non-outbreak-related measles cases; of cases with genotype information available, 27% were B3 and 73% were D8. Conclusion: Despite meeting/partially meeting only three out of four of PAHO’s essential criteria for measles elimination status, there is no evidence that endemic measles transmission has been reestablished in Canada.

24. Voodoo Liability: Joint Enterprise Prosecution as an Aspect of Intensified Criminalisation

Following the collapse of a number of 'gang-related' prosecutions in England and Wales from the late 1990s, the police and Crown Prosecution Service revived a practice of 'joint enterprise' prosecution. Joint enterprise was a historic common law principle holding co-defendants equally responsible for offences which appeared to evince a common collective purpose. Unfortunately, over time, a combination of (apparently 'wayward') judicial interpretation, and police and prosecutorial practice contributed to a lowering of the threshold of 'joint liability' such that involvement in a gang, and 'bad character' evidence admitted at trial were taken to imply the 'foreseeability' of violent offences. The apparent tendency of the police to over-define criminal activity by young black males as 'gang-related' has led to the construction of a spurious and 'voodoo criminal liability' leading to the intensified criminalisation (over-prosecution and over-incarceration) of young black men. Between the first presentations of the paper and the written version which follows, the law was amended (R v Jogee 2016) but, as will be argued, not in a way which fundamentally changes the construction of 'joint liability' discussed here. Interactions between jets and the quark-gluon plasma produced in heavy ion collisions are studied via the angular distributions of summed charged-particle transverse momenta (p T) with respect to both the leading and subleading jet axes in high-p T dijet events. The contributions of charged particles in different momentum ranges to the overall event p T balance are decomposed into short-range jet peaks and a long-range azimuthal asymmetry in charged-particle p T. The results for PbPb collisions are compared to those in pp collisions using data collected in 2011 and 2013, at collision energy sNN−−−−√=2.76 TeV with integrated luminosities of 166 μb−1 and 5.3 pb−1, respectively, by the CMS experiment at the LHC. Measurements are presented as functions of PbPb collision centrality, charged-particle p T, relative azimuth, and radial distance from the jet axis for balanced and unbalanced dijets., Journal of High Energy Physics, 2016, ISSN:1126-6708, ISSN:1029-8479

International audience; The knowledge on death, birth, masculinity and feminity’s popular beliefs and traditions and customs is enlightened by notions such a as unconscious motives, Oedipus, incest prohibition, generation differences. We will examine magical thinking in anthropological and psychoanalytic studies about maternity and birth. We will question whether trauma transmission and unresolved mourning are better understood by psychoanalytical interpretation of magical thinking. Then we will point out how the due to stillbirth can lead a mother to anxiety and a psychic contagiousness state influencing the archaic psychic mechanism of her subsequent child.; La connaissance des croyances et coutumes d’autres traditions concernant la mort, la naissance, le masculin et le féminin est éclairée par la considération des pulsions, de l’œdipe, de l’interdit de l’inceste ou des différences de générations. Nous examinerons la pensée magique dans certains travaux anthropologiques et psychanalytiques sur la maternité et la naissance d’un enfant. Ensuite, nous nous demanderons si la transmission des traumatismes, des phénomènes de répétition, des deuils non élaborés à des enfants est éclairée par l’interprétation psychanalytique des mécanismes de pensée magique. Enfin, nous montrerons comment la coïncidence de la mort et de la naissance chez une mère ayant connu une mort in utero peut la conduire à se trouver, lors de la naissance d’un enfant suivant, dans un état d’angoisse et de contagiosité psychique conduisant à des mécanismes de pensée archaïque chez son enfant.

International audience; An Essay on Gamete Donation.The question of lifting the anonymity of donors in medically assisted procreation procedures, involving artificial insemination with sperm donation or fertilisation of an oocyte donated by a third party, is at present the subject of heated debate in France. It is rooted in a train of thought underpinned by children trying to trace their paternal or maternal filiation and aims at making this easier, thereby promoting the child’s ‘right’ to gain knowledge of their own origins. Elective filiations such as these refuel the nature/nurture debate and the representations they go hand in hand with within the framework of reproductive medicine. Technical possibilities sometimes seem to far outdo the psychic capacity to cope with the consequences of infertility.; La possible levée de l’anonymat des donneurs dans le cadre des procréations médicalement assistées avec insémination des spermatozoïdes ou fécondation d’un ovocyte d’un tiers est débattue en France. Elle s’inscrit dans le courant initié par des recherches en paternité ou en maternité visant à la « traçabilité » des origines et du « droit » des enfants à connaître leurs origines. Ces filiations électives relancent le débat nature/culture et les représentations qui les accompagnent dans le domaine de la médecine de la reproduction. La technique semble parfois dépasser les possibilités psychiques de supporter les conséquences de l’infertilité.

International audience; Dans « vouloir un enfant » se conjuguent une constellation de désirs et de motions ambivalentes dont l’interprétation psychanalytique n’est ni aisée ni toujours souhaitable. Que faire de ces récits si singuliers des futurs mères et pères qui affleurent à la conscience ?2L’anticipation de l’enfant lors de la grossesse et son arrivée suscitent de nombreuses représentations individuelles et groupales à partir de fantasmes, de désirs, de croyances populaires, de peurs et de perceptions sélectives. Ces représentations mobilisées dès la conception remontent à l’enfance et sont retravaillées durant la grossesse dans les identifications avec les anciens, les pairs, le partenaire ou même des figures extérieures à la famille. Dans cette scène autour de la naissance, proche du sacré, se conjuguent l’infantile remanié des parents, l’actuel parfois mouvementé des circonstances de la vie mais aussi les traumatismes et les deuils non élaborés. L’avenir de l’enfant à naître est un ciel psychique, où se déploieront les variations de son passé et de ses liens intersubjectifs.

International audience; In scientific literature, the link between eating disorders and pregnancy has remained very little explored, despite the many questions it raises. Women suffering from eating disorders can become mothers without their disorders being taken into account by health care professionals. Moreover, the mental and physical adjustments and changes to eating patterns associated with pregnancy make it a particularly critical moment for this vulnerable group. The aim of this research, then, was to carry out a systematic study into the eating patterns of pregnant women in order to identify possible eating disorders and subclinical forms. The first results of the study revealed a major preoccupation with their food intake on the part of pregnant women, most of them reporting changes in their eating patterns and fears of losing control of the quantities ingested. The importance of early screening for these disorders is thus highlighted as is the need for a multidisciplinary team to take into account the psychological impact of all that surrounds giving birth.; En la literatura científica, la relación entre TCA y embarazo está poco investigada a pesar de su interés. Efectivamente las mujeres con TCA pueden llegar a ser madres sin que su trastorno sea tomado en consideración por parte de los profesionales de la salud. Por otra parte las transformaciones psíquicas y físicas y las modificaciones de la conducta alimenticia asociadas al embarazo hacen que este periodo sea particularmente crítico para estas personas vulnerables. Es la razón por la cual esta investigación tiene como objetivo estudiar sistemáticamente las conductas alimenticias en las mujeres embarazadas para diagnosticar las eventuales TCA y las formas sub clínicas. Los primeros resultados del estudio han evidenciado la gran preocupación de las mujeres embarazadas por su alimentación, la mayoría de ellas indican cambios en sus conductas alimenticias y el miedo de perder el control de las cantidades ingeridas. Queda pues demostrado el interés evidente del despistaje precoz de estos trastornos y la importancia de que un equipo pluridisciplinario tome en consideración los aspectos psicológicos durante el embarazo.; Dans la littérature scientifique, le lien entre TCA et grossesse semble particulièrement inexploité, bien que riche de questionnements. En effet, les femmes atteintes de TCA peuvent devenir mères, et ce, sans que leurs troubles soient pris en compte par les professionnels de santé. D’autre part, les remaniements psychiques et physiques et les modifications du comportement alimentaire associés à la grossesse en font un moment particulièrement critique pour cette population vulnérable. D’où l’ambition de cette recherche d’étudier systématiquement les conduites alimentaires des femmes enceintes en vue de repérer d’éventuels TCA et formes subcliniques. Les premiers résultats de l’étude ont mis en évidence la préoccupation majeure des femmes enceintes pour leur alimentation, la majorité rapportant des changements dans leur comportement alimentaire et des craintes de perdre le contrôle des quantités ingérées. Il est ainsi montré l’intérêt évident d’un dépistage précoce de ces troubles et d’une meilleure prise en compte psychologique de l’environnement de naissance par une équipe pluridisciplinaire.

However, the more the requests of assisted procreation emanate from extreme existential situations, the more they seem to reveal fragile people; indeed, the psychopathological factoften appears in extreme cases of the social, biological or legal standards... thus the teams of procreation medical assistance are led to be cautiousand submit these requests within a framework where the psychologist and the psychiatrist have their place.; A côté des indications médicales traditionnelles d’aide à la procréation, comme la stérilité tubaire ou les insuffisances spermatiques extrêmes, de nouvelles demandes d’enfants apparaissent dans des situations d’infertilité relatives, liées à l’âge ou bien aux marges des normes sociales admises jusque là. Efectivamente las mujeres con TCA pueden llegar a ser madres sin que su trastorno sea tomado en consideración por parte de los profesionales de la salud. Por otra parte las transformaciones psíquicas y físicas y las modificaciones de la conducta alimenticia asociadas al embarazo hacen que este periodo sea particularmente crítico para estas personas vulnerables. Es la razón por la cual esta investigación tiene como objetivo estudiar sistemáticamente las conductas alimenticias en las mujeres embarazadas para diagnosticar las eventuales TCA y las formas sub clínicas. Los primeros resultados del estudio han evidenciado la gran preocupación de las mujeres embarazadas por su alimentación, la mayoría de ellas indican cambios en sus conductas alimenticias y el miedo de perder el control de las cantidades ingeridas. Queda pues demostrado el interés evidente del despistaje precoz de estos trastornos y la importancia de que un equipo pluridisciplinario tome en consideración los aspectos psicológicos durante el embarazo.; Dans la littérature scientifique, le lien entre TCA et grossesse semble particulièrement inexploité, bien que riche de questionnements. En effet, les femmes atteintes de TCA peuvent devenir mères, et ce, sans que leurs troubles soient pris en compte par les professionnels de santé. D’autre part, les remaniements psychiques et physiques et les modifications du comportement alimentaire associés à la grossesse en font un moment particulièrement critique pour cette population vulnérable. D’où l’ambition de cette recherche d’étudier systématiquement les conduites alimentaires des femmes enceintes en vue de repérer d’éventuels TCA et formes subcliniques. Les premiers résultats de l’étude ont mis en évidence la préoccupation majeure des femmes enceintes pour leur alimentation, la majorité rapportant des changements dans leur comportement alimentaire et des craintes de perdre le contrôle des quantités ingérées. Il est ainsi montré l’intérêt évident d’un dépistage précoce de ces troubles et d’une meilleure prise en compte psychologique de l’environnement de naissance par une équipe pluridisciplinaire.

International audience; When the question of a woman’s desire to have a child later in life is raised, the first reasons that are put forward to explain this are sociological. But this female model of pregnancy after the age of 35 is having difficulty getting established. besides, these rational factors are often highlighted, whereas unconscious determiners are an important constituent of this desire.; Le désir tardif d’enfant d’une femme s’explique par des raisons sociologiques évidentes (travail précaire, fragilité des unions...), toutefois ce modèle féminin d’une grossesse après trente-cinq ans a du mal à s’imposer. Les techniques médicales permettent de mieux suivre ces grossesses,marquées par ne relative infertilité mais avec les procréations médicalement assistées, les situations fragiles sont émergentes. Il apparaît en suivant les histoires singulières que l’ambivalence à l’égard de la grossesse et ses effets inconscients agit pour une bonne part dans ce désir.

L’anticipation prenatale d’un enfant, dans le contexte d’une mort perinatale anterieure, risque d’etre figee. Inversement l’attente d’un autre enfant peut permettre un travail psychique reparateur. Le vecu psychologique des femmes enceintes a la suite d’une grossesse ayant abouti a une mort in utero a ete recueilli au cours d’entretiens cliniques et les premiers liens avec les enfants suivants ont ete observes. Il s’agit donc d’une recherche clinique en psychopathologie et particulierement d’un point de vue psychanalytique associee a une observation naturaliste et comparative des interactions parents-enfants, afin de prevenir l’impact de relations mal engagees avec l’enfant. Sur le plan metapsychologique, le modele freudien du deuil est en question au-dela du domaine de la perinatalite : comment penser le deuil a propos de l’enfant mort avec qui la relation est fortement investie mais non representable ? La mort in utero n’offrirait-elle pas une forme de figuration d’un objet narcissique de nature melancolique ? Quel est l’effet de la violence exercee sur le corps maternel sur le feminin ?

The 2000-2001 season was a relatively mild season worldwide. In Canada, lower than usual activity was reported for all national indicators of influenza activity, including the rate of influenza-like illness (ILI), the percentage of laboratory-confirmed cases of influenza and provincial/territorial influenza activity levels. However, there were a number of interesting characteristics of this year's influenza season. In contrast to the predominance of influenza A, and in particular the A/Sydney/5/97 (H3N2)-like virus over the past 3 years, influenza B predominated overall this season. Influenza A (H3N2) accounted for1% of all characterized isolates (H1N1 accounted for 49% of isolates). Increased laboratory-confirmed influenza activity began in the West (Yukon, prairie provinces and British Columbia) in mid-December, followed by the Atlantic provinces in mid- to late January and Ontario and Quebec in mid- February and March.
